Local content modification provided to subscribers of the terrestrial digital television signal is necessary to provide the technical possibility of organizing inserts of local content signals, such as TV programs of regional broadcasters, as well as information on emergency situations. Broadcast multiplexes of federal TV programs are designed for use within the corresponding time zone (A, B, C, D, M). In each time zone, there are a number of regions, in each of which there are local TV and radio companies that produce local content that must be delivered to the subscribers of the whole subject. The task of embedding/modifying content at each remote transmitting station is performed by an inserter or local content insertion device (ETSI TS 102773). The reliability parameters of the restorable system for organizing terrestrial television broadcasting at a remote station with the content modification were calculated in this article. Tables and a graph of the broadcasting system states are presented, on the basis of which, systems of Kolmogorov differential equations are compiled. It was found that additional redundancy organized by connecting the output stream from the RX1 receiver directly to the transmitting device allows for a 2.5-fold increase in the average operating time between failures, as well as an increase in the availability factor by 5.26 percent. All calculations were performed using the SimInTech software package. The influence of automatic redundancy of the local content inserter and the transmitter on the occurrence of errors in the stream that affect the quality of the output signal is considered. The relationship between the availability factor and the components of the quality of service parameter – SAE, SDE and SIE is determined.

Abstract Background According to United Nations, 19% of females in the world relied only on the permanent method of family planning, with 37% in India according to NFHS-4. Limited studies tried to measure the sterilization regret, and its correlated factors. The study tried to explore the trend of sterilization regret in India from 1992 to 2015 and to elicit the determining effects of various factors on sterilization regret, especially in context to perceived quality of care in the sterilization operations and type of providers. Data and methods The pooled data from NFHS-1, NFHS-3 and NFHS-4 was used to explore the regret by creating interaction between time and all the predictors. Predicted probabilities were calculated to show the trend of sterilization regret amounting to quality of care, type of health provider at the three time periods. Results The sterilization regret was increased from 5 % in NFHS-1 to 7 % in NFHS-4. According to NFHS-4, for those whose sterilization was performed in private health facility the regret was found to be less (OR-0.937; 95% CI- (0.882–0.996)) compared to public health facility. Also, the results show a two-fold increase in regret when women reported bad quality of care. The results from predicted probabilities provide enough evidence that the regret due to bad quality of care in sterilization operation had increased with each subsequent round of NFHS. Conclusion Many socio-economic and demographic factors have influenced the regret, but the poor quality of care contributed maximum to the regret from 1992 to 2015. The health facilities have seriously strayed from improving the health and well-being of women in providing the family planning methods. In addition, to public facilities, the regret amounting to private facilities have also increased from NFHS-1 to 4. The quality of care provided in the family planning operation should be standardized in every hospital to strengthen the health systems in the country. The couple should be motivated to adopt more of spacing methods.

Study Design: Retrospective cohort study. Objective: To compare the outcomes of patients with adult spinal deformity (ASD) following spinal fusion with the lowest instrumented vertebra (LIV) at L4/L5 versus S1/ilium. Methods: A multicenter ASD database was evaluated. Patients were categorized into 2 groups based on LIV levels—groups L (fusion to L4/L5) and S (fusion to S1/ilium). Both groups were propensity matched by age and preoperative radiographic alignments. Patient demographics, operative details, radiographic parameters, revision rates, and health-related quality of life (HRQOL) scores were compared. Results: Overall, 349 patients had complete data, with a mean follow-up of 46 months. Patients in group S (n = 311) were older and had larger sagittal and coronal plane deformities than those in group L (n = 38). After matching, 28 patients were allocated to each group with similar demographic, radiographic, and clinical parameters. Sagittal alignment restoration at postoperative week 6 was significantly better in group S than in group L, but it was similar in both groups at the 2-year follow-up. Fusion to S1/ilium involved a longer operating time, higher PJK rates, and greater PJK angles than that to L4/L5. There were no significant differences in the complication and revision rates between the groups. Both groups showed significant improvements in HRQOL scores. Conclusions: Fusion to S1/ilium had better sagittal alignment restoration at postoperative week 6 and involved higher PJK rates and greater PJK angles than that to L4/L5. The clinical outcomes and rates of revision surgery and complications were similar between the groups.

Assessment of plant height is an important factor for agronomic and breeder decisions; however, current field phenotyping, such as visual scoring or using a ruler, is time consuming, labour intensive, costly and subjective. For agronomists and plant breeders, the most common method used to measure plant height is still a meter stick. In a 3-year study, we have adopted a herbometre similar to a rising plate meter as a reference method to obtain the weighted plant height of barley cultivars and to evaluate vehicle-based ultrasonic and laser distance sensors. Sets of 30 spring barley cultivars and 14 and 60 winter barley cultivars were tested in 2013, 2014 and 2015, respectively. The herbometre was well suited as a reference method allowing for an increased area and was easy to handle. The herbometre measurements within a plot showed very low coefficients of variation. Good and close relationships (R2 = 0.59, 0.76, 0.80) between the herbometre and the ultrasonic distance sensor measurements were observed in the years 2013, 2014 and 2015, respectively, demonstrating also increased values of heritability. Hence, both sensors were able to differentiate among barley cultivars in standard breeding trials. For the sensors, we observed a 4-fold faster operating time and 6-fold increase of measurement points compared with the herbometre measurement. Based on these results, we conclude that distance sensors represent a powerful and economical high-throughput phenotyping tool for breeders and plant scientists to estimate plant height and to differentiate cultivars for agronomic decisions and breeding activities potentially being also applicable in other small grain cereals with dense crop stands. Particularly, ultrasonic distance sensors may reflect an agronomically and physiologically relevant plant height information.

Purpose. To evaluate the results of the rational use of ophthalmic medicines with the use of pharmacoeconomical analysis. Material and methods. With the help of ABC-VEN analysis, a comparative study of the quality of pharmacotherapy and drug provision of patients who were monitored and treated by an ophthalmologist of the polyclinic in 2015 and 2019 was conducted. Results. As a result, it was found that the share of costs for vital drugs increased by 10.1% and amounted to 71.1% in 2019, which corresponds to the standardized criterion (70-80%). There was a reduction in the cost of purchasing secondary drugs by 7.6%, which indicates a rational drug supply of ophthalmic drugs. The most commonly used drugs for the treatment of glaucoma were 27.8% in 2015 and 35.1% in 2019. The share of their acquisition costs was 54.9% and 67.9%, respectively. There was a significant increase in the range and number of drugs for the treatment of patients with glaucoma: a 2.8-fold increase in the number of purchased eye drops for monotherapy and a 12% increase in the number of combined drugs. Conclusion. Pharmacoeconomical analysis showed an optimization of spending money on the purchase of drugs used in ophthalmology: an increase of 10.1% in the share of vital drugs and a decrease of 7.6% in the cost of purchasing secondary drugs. There was an increase in the range and quantity of drugs for the treatment of patients with glaucoma. Key words: ABC-VEN-analysis, drugs, ophthalmology, pharmacoeconomics.

The benchmark for the reliability quality of networks depends mainly on the accuracy and comprehensiveness of the reliability parameters. Downtime prediction of a communication system is crucial for the quality of service (QoS) offered to the end-user. Markov model enables analytical calculation of average single figure cumulative downtime over one year. The single average approach, generally, does not adequately describe the wide range of service performance that is likely to be experienced in communications systems due to the random nature of the failure. Therefore, it would be more appropriate to add downtime distribution obtained from network availability models to predict the expected cumulative downtime and other performance parameters among a large number of system populations. The distribution approach provides more comprehensive information about the behavior of the individual systems. Laplace-Stieltjes transform enables analytical solutions for simple network architectures, i.e. the simplex system and the parallel system. This paper uses simulations to determine reliability parameters for complex architecture such as the Multiprotocol Label Switching (MPLS) backbone planned for next-generation Internet. In addition to the single figure downtime, simulations provide other reliability parameters such as probability of zero downtime. The paper also considers the downtime distribution among a population of equally designed systems.Key Words: Reliability, availability, downtime, Multiprotocol label switching

This article discusses methods of assessment of the trunk "Customers" for the ISUP re-engineering. We have used two tools: • Current state assessment of service quality using tree chart of indicators on four trunks: Customers, Process, Owners and the State. • Action plan development based on the Kobayashi PROF methodology. Evaluation of the trunk "Customers" (q1) is conducted in three branches: P1 - functionality, P2 - convenience, P3 - emotional background. Functionality is evaluated by type of work and sufficient level of Reliability, Responsibility, Availability and Sociability of each of them in relation to all applications. Convenience is estimated by means of survey of customers by means of questioning on the website by criteria (leaves): comfort, the interface, conflict situations, feedback, emergency situations and functionality Emotional background is assessed by analyzing changes in the emotional background of communication and is based on the content analysis of the text. All messages from the user are considered to be text. The proposed tree-like information model of structure construction of indicators makes it possible to assess the quality of services in the field of ISUP support at the enterprises of the radio-electronic industry.

Halyomorpha halys is an important invasive pest that causes severe damage to fruits and vegetables. Peppers are susceptible to infestation by H. halys, resulting in yield losses. Plants respond to the insect infestation with a metabolic response. With this study, we attempted to determine the intensity of the metabolic response of infested peppers, how pungent and non-pungent peppers react to the infestation, and how the H. halys affects the post-harvest quality of both cultivars. The shelf life of the infested peppers did not change compared to the control treatments. We observed a drastic decrease in metabolite levels after storage in all three treatments in both cultivars, especially capsaicinoids, with an approximate decrease of 30% in the pericarp and 95% in the placenta of the pungent ‘Eris F1’. In some cases, the accumulation of metabolites was not limited to the fruit exposed to the H. halys infestation, but to the entire plant. We observed a 15-fold increase in capsaicinoid content in the infested fruits of cultivar ‘Eris F1’ and a 4-fold increase in the pericarp of cultivar ‘Lombardo tago’, which could lead to a possible further study on the defensive function of capsaicinoids and their use against H. halys.

Abstract BACKGROUND: Midwifery care plays a vital role in the reduction of preventable maternal and newborn mortality and morbidity. There is a growing concern about the quality of care during facility based childbirth and the occurrence of disrespect and abuse (D&A) worldwide. While several studies have reported a high prevalence of D&A, evidence about the drivers of D&A is scarce. AIM: This study aims to explore midwives’ professional identity and perspectives on the occurrence of D&A in urban Mozambique. METHODS: A qualitative study took place in the central hospital of Maputo, Mozambique. Nine focus group discussions with midwives were conducted, interviewing 54 midwives. RQDA software was used for analysing the data by open coding and thematic analysis from a grounded theory perspective. RESULTS: Midwives felt proud of their profession but felt they were disrespected by the institution and wider society because of their inferior status compared to doctors. Furthermore, they felt blamed for poor health outcomes. The occurrence of D&A seemed more likely in emergency situations but midwives tended to blame this on women being “uncooperative”. The involvement of birth companions was a protective factor against D&A together with supervision. CONCLUSION: In order to improve quality of care and reduce the occurrence of D&A midwives will need to be treated with more respect within the health system. Furthermore, they should be trained in handling obstetric emergency situations with respect and dignity for the patient. Systematic and constructive supervision might be another promising strategy for preventing D&A.

This chapter discusses mainly on dynamic behavior of railway passengers by using twitter data during regular and emergency situations. Social network data is providing dynamic and realistic data in various fields. As per the current chapter theme, if the twitter data of railway field is considered then it can be used for enhancement of railway services. Using this data, a comprehensive framework for modeling passenger tweets data which incorporates passenger opinions towards facilities provided by railways are discussed. The major issues elaborated regarding dynamic data extraction, preparation of twitter text content and text processing for finding sentiment levels is presented by two case studies; which are sentiment analysis on passenger's opinions about quality of railway services and identification of passenger travel demands using geotagged twitter data. The sentiment analysis ascertains passenger opinions towards facilities provided by railways either positive or negative based on their journey experiences.
