The medical record is one of the most important parts of a health service which must be supported by an adequate number of human resources, so it is necessary to calculate the exact workforce requirements. Medical record activities at Ramdani Husada Inpatient Clinic are not done by officers with medical record education qualifications but are carried out by officers with qualifications of high school education, midwifery diploma, and midwifery diploma. The research objective was to describe the need for medical record personnel based on the WISN (Workload Indicator of Staffing Need) method at the Ramdani Husada Inpatient Clinic. The research design used descriptive quantitative with a cross-sectional approach. The population and sample of the study were all officers who carried out medical record activities as many as 3 people including the clinic manager, head of administration, and administrative staff. The results showed that the available work time was 1,953 hours/year or equivalent to 117,180 minutes/year, the workload standard was not in accordance with the number of available officers, the allowance standard was 1.70 workers. So that the need for medical record officers based on the WISN method is 9 workers. Based on the research results, there is a gap between theory and practice, where the Ramdani Husada Inpatient Clinic does not use the WISN method or other methods for planning its workforce. It is suggested that the Ramdani Husada Inpatient Clinic do deeper planning related to the workforce, especially for medical record officers to increase work productivity and reduce the risk of fatigue in officers.

Abstract Background: Human resources in health are limited, and a variety of healthcare services are increasing, especially in developing countries. This study aimed to determine the anesthesia and surgical technologist staff for operating room based on the Workload Indicators of Staffing Need (WISN).Methods: This cross-sectional descriptive study was conducted at Shahid Madani Medical and Training Hospital in 2019. The data collected through interview and hospital documents observation. The WISN method was used to analyze and determine staffing need.Results: The WISN ratio for anesthesia and surgical technologist staff were 1.60 and 4.94, respectively. In other words, in both categories, this ratio is >1.00. Conclusions: WISN results can used to assess staffing capacity and capability over the previous year and the policy and financial implications due to staff shortages or surplus. As the workload of staff is an important issue and the staff have a right to have a standard workload in their position, then determining this workload is an essential issue in healthcare organization. Also, WISN method work as a human resource management tools, it improvers health managers to make right decision about staffing needs and then better manage of organization’s resources.

ABSTRACT Hyperemesis Gravidarum marked excessive nausea and vomiting in pregnant women at a young age. WHO estimates that 536,000 women died from direct complications of pregnancy and childbirth. One complication of pregnancy is Hyperemesis Gravidarum. Hyperemesis Gravidarum in the world has been estimated to occur in 1-2% of pregnant. According to data from the Medical Record of Dr. Mohammad Hoesin Palembang Hospital, the incidence of Hyperemesis Gravidarum in 2011 there were 72 people of 661 pregnant women. The purpose of this reseach was to determine the relationship between maternal age and parity with Hyperemesis Gravidarum in Dr. Mohammad Hoesin Palembang Hospital 2011. The Design of this reseach used Cross Sectional by analytic approach survey. The population of this reseach are all of mother who gestational ≤ 16 weeks (four months) in the Installation of Obstetrics and Gynecology, Dr. Mohammad Hoesin Palembang Hospital in January to December of 2011 with the sample of 661 people which taken by systematic random sampling and the reseach was conducted from 17 April to 24 April 2012.  Each variable that was observed in tests using Chi-Square test with a (0.05).  The results of this study showed that 5.9% of mothers with hyperemesis gravidarum, 21.8% of mothers with high risk age, and 30.0% primigravida. Chi-Square test showed no significant relationship between age (p value = 0.000) and parity (p value = 0.000) with the incidence of hyperemesis gravidarum. Expected to the Hospital to be implemented properly instructed how to provide counseling to pregnant women about pregnancy and childbirth, as well as provide confidence that the nausea and vomiting is a symptom of physiology in pregnancy.   ABSTRAK Hiperemesis Gravidarum ditandai mual dan muntah yang berlebihan terjadi pada ibu hamil di usia muda. WHO memperkirakan 536.000 perempuan meninggal dunia akibat langsung dari komplikasi kehamilan dan persalinan. Salah satu komplikasi kehamilan adalah Hiperemesis Gravidarum. Insiden Hiperemesis Gravidarum di dunia telah diperkirakan terjadi pada 1-2% wanita hamil. Menurut data dari Medical Record Rumah Sakit Umum Pusat Dr. Mohammad Hoesin Palembang, angka kejadian Hiperemesis Gravidarum pada tahun 2011 terdapat 72 orang dari 661 ibu hamil. Tujuan penelitian ini adalah untuk mengetahui hubungan antara umur dan paritas ibu dengan kejadian Hiperemesis Gravidarum pada ibu di Rumah Sakit Umum Pusat Dr. Mohammad Hoesin Palembang tahun 2011. Desain Penelitian ini menggunakan survei analitik dengan pendekatan Cross Sectional. Populasi penelitian ini adalah dengan umur kehamilan ≤ 16 minggu (4 bulan) yang pernah dirawat inap di Instalasi Kebidanan dan Penyakit Kandungan Rumah Sakit Umum Pusat Dr. Mohammad Hoesin Palembang pada bulan Januari-Desember tahun 2011 dengan jumlah sample 661 orang yang diambil secara systematic Random Sampling (secara acak sistematis) dan penelitian ini dilakukan dari tanggal 17 April sampai dengan 24 April 2012. Masing-masing variabel yang diteliti di uji dengan menggunakan uji Chi-Square dengan a (0,05). Hasil penelitian ini menunjukkan bahwa 5,9% ibu mengalami hiperemesis gravidarum, 21,8% ibu dengan umur resiko tinggi, dan 30,0% ibu primigravida. Uji Chi-Square menunjukkan ada hubungan yang bermakna antara umur (p value = 0,000) dan paritas (p value = 0,000) dengan kejadian hiperemesis gravidarum. Diharapkan dapat menjadi masukan bagi pihak Rumah Sakit agar dilaksanakan penyuluhan dengan cara memberikan konseling terhadap ibu hamil tentang kehamilan dan persalinan, serta memberikan keyakinan bahwa mual muntah merupakan gejala fisiologi pada kehamilan.

Introduction Recent evidence reveals that diabetes and prediabetes (preDM) can be reversed to normal glucose regulation (NGR) through significant weight loss, but how physicians clinically identify the principles of partial and complete remission of diabetes is largely unknown. Methods As part of the cross-sectional omnibus survey conducted in March 2019 at a professional annual meeting in the United States, physician participants answered case scenario questions about the diagnosis and documentation of patients with preDM and type 2 diabetes (T2DM). Results Of the registered conference attendees, 387 (72.7%) responded. When presented with the initial case of preDM, 201 physicians (70.8%) selected R73.03 Prediabetes. In a follow-up encounter with improved lab results, 118 physicians (58.7%) indicated that they would not chart any diabetes-related code and 62 (30.8%) would chart preDM again. When presented with the case of T2DM, 256 physicians (90.1%) indicated E11.0–E11.9 Type 2 Diabetes. In the follow-up encounter, only 38 (14.8%) coded a diagnosis reflecting remission from T2DM to prediabetes and 211 (82.4%) charted T2DM. Conclusion Physicians may be reluctant to document diabetes regression as there is little evidence for long-term outcomes and “downgrading” the diagnosis in the medical record may cause screenings to be missed. Documenting this regression in the medical record should communicate the accurate point on the continuum of glucose intolerance with both the patient and the care team.

The purpose of this study was to describe the number and types of errors that adolescents and caregivers report making when using asthma controller medications. A total of 319 adolescents ages 11 to 17 with persistent asthma and their caregivers participated in this cross-sectional study. Adolescent and caregiver reports of asthma medication use were compared to the prescribed directions in the medical record. An error was defined as discrepancies between reported use and the prescribed directions. About 38% of adolescents reported 1 error in using asthma controller medications, 16% reported 2 errors, and 5% reported 3 or more errors. About 42% of caregivers reported 1 error in adolescents using asthma controller medications, 14% reported 2 errors, while 6% reported 3 or more errors. The type of error most frequently reported by both was not taking the medication at all. Providers should ask open-ended questions of adolescents with asthma during visits so they can detect and educate families on how to overcome errors in taking controller medication use.

Background:Limited data exist of work productivity loss in patients with Rheumatoid Arthritis (RA), Psoreatic Arthritis (PsA) and Spondyloarthritis (axSpA).Objectives:The objective of this research was to assess productivity loss and absenteeism in patients with RA, PsA and axSpA.Methods:The study was designed as a cross-sectional study aimed to collect patient-reported outcomes from patients with RA, PsA and axSpA in Denmark via a nurse administered questionnaires and patient journals. Patients ≥18 years with RA, PsA or axSpA were consecutively recruited for the study over a 6-month period via routine visits to outpatient rheumatology clinics. Descriptive statistics were analyzed using SAS.Results:Of 488 respondents, 62% were women and mean age was 53.5 years (RA:57.4; PsA:52.6; axSpA:43.6). Average time since diagnosis was 11-15 years, however, for PsA and axSpA most patients answered 6-10 and 0-5 years, respectively. 280 (57%) answered that they had a job and completed the WPAI questionnaire (RA: 149 (51%); PsA: 48 (56%); axSpA: 83 (75%)). Average work hours was 31.9 in the last week (RA:31.2; PsA:33; axSpA:32.4). Average missed work hours were 4.3 in the last 7 days ((RA:4.0; PsA:4.2; axSpA:4.8), of which 32% was missed due to their inflammatory arthritis (RA:30%; PsA:38%; axSpA:32%). Mean absenteeism was highest for patients with PsA (mean=6.8; SD=17.7) followed by patients with axSpA (mean=5.4; SD=15.1) and with RA (mean=3.4; SD=12.2). Mean productivity loss was 20.5 (SD=23.8) for patients with RA, 27.6 (SD=25.8) for PsA and 26.3 (SD=25.8) for axSpAConclusion:We found that patients with PsA or axSpA miss more hours of work compared with patients with RA and when they are at work they have a higher absenteeism/lower productivity. This article reports on a dyad model of lesson study aimed at scaffolding the theory and practice of learning to teach physical education. Participants were pre-service teachers (PSTs) completing a 38-week Master’s-level Postgraduate Certificate in Education in eastern England, training to teach the secondary age range (11–18 years). A total of 40 PSTs volunteered to participate in the study during their school-based training. A three-year cross-sectional case-study framework involving three distinct cohorts of PSTs allowed for a comparison of data, captured through computer-mediated communication. Dialogue through email communications and electronic evaluations was analysed inductively. Three substantive themes were identified as a result of the PSTs’ experiences: (a) developing confidence in the classroom through collaboration with a peer; (b) developing physical education pedagogies to support students’ individual learning needs; and (c) developing physical education pedagogies to support assessment of students’ progress. The dyad lesson study model provided a safe and non-hierarchical platform for collaboration between PSTs. Peer-to-peer reflection on aspects of their own practice instilled confidence and enhanced understanding, particularly in relation to understanding students’ individual learning needs to promote progress and assessing such progress. Dyad lesson study positively supported PSTs’ professional development against prescribed Teachers’ Standards beyond the formal hierarchical rules and structures associated with the school-based training process. Such collaborative conversations can help to minimise professional isolation for PSTs during their school-based training and address the juxtaposition of connecting the theory of learning to teach with a holistic view of student learning in practice.

Abstract Background Job satisfaction describes an employee’s motivation and/or feeling of satisfaction towards his/her work. Globally, healthcare professionals’ turnover and retention play a critical role in the delivery of essential health services. In Ghana, however, little has been done to ascertain job satisfaction levels among human resources for eye-health. The objective of this study therefore was to assess job satisfaction and its associated factors among optometrists in Ghana. Methods A cross-sectional survey was conducted among 304 registered and licensed optometrists of the Ghana Optometric Association between September 2018 and June 2019. A validated, well-structured questionnaire was used to elicit information on socio-demographic characteristics of participants and measures on job satisfaction. Scores from a five-point Likert scale was employed to examine job satisfaction and its associated factors. Linear regression analyses were used to evaluate the association between overall job satisfaction and its associated factors using Rasch logit scores. Results A total of 214 optometrists gave valid responses to the questionnaires used for the final analysis. The mean (± SD) score of the overall perception of job satisfaction among optometrists was 3.36 (± 1.00), with 74.3% of them being satisfied with their jobs. After statistical adjustment, Good work-life balance (Unstandardized co-efficient (β) = 0.288, p = 0.001), Salary (β = 0.222, p < 0.0005), Supervision (β = 0.117, p = 0.044), and Continuing Education Opportunities (β = 0.138, p = 0.017) were all significantly associated with higher levels of overall job satisfaction. Conclusions Most optometrists were satisfied with their jobs. Effective strategic planning and management of human resources for eye-health in Ghana are essential in the development of quality eye-health systems and the provision of high-quality eyecare services.

Background and Objective. The number of new entrants to higher medical schools of Kazakhstan increased by 1.6 times from 2007 to 2012. However, it is not known how it will affect the shortage of human resources for health. Additionally, human resources for health in rural areas of Kazakhstan are 4 times scarcer than in urban areas. The aim of the present study was to investigate the intentions of students toward their professional future and readiness to work in rural areas, as well as to determine the causes for dropping out from medical schools. Material and Methods. A cross-sectional survey was conducted in 2 medical universities in Almaty during the academic year 2011–2012. The study sample included medical students and interns. In total, 2388 students participated in the survey. The survey tool was an anonymous questionnaire. Results. The students of the first years of studies compared with those of later years of studies were more optimistic about the profession and had more intentions to work in the medical field. Only 8% of the students reported a wish to work in rural localities. On the other hand, 4% of the students did not plan to pursue the profession. On the average, every third medical student dropped out on his/her own request. Conclusions. Associations between intentions to work according to the profession and the year of studies, faculty, and residence area before enrolling in a medical school were documented. The majority of the students who came from rural areas preferred to stay and look for work in a city, which might contribute to an unequal distribution of physicians across the country.

The Ministry of Health established the Academia da Saúde Program (PAS) with the objective of contributing to the promotion of health, the implementation of policies for the conduct of physical activities, physical activity and leisure and the reduction of healthy lifestyles. The objective of this study was to identify managers of physical activity programs on the barriers to the operation of the PAS poles in Santa Catarina (SC). A cross-sectional study was carried out with 26 managers (61.5% women) of physical activity programs in Santa Catarina. As compensation payments were withdrawn from the base for updating health statistics. Descriptive statistics (mean, standard avoidance, absolute and relative frequency) were made for data analysis. The results triggered a lack of human resources, lack of leadership, lack of materials, facilities, equipment and a lack of standardized instruments to carry out an evaluation of actions. It was concluded that organizational, infrastructure and human resources barriers are present in the day-to-day actions of PAS in SC.

ObjectivesThe aim of this study was to investigate whether reported prevalence of experienced threats, real acts of violence and debilitating fear of violence among Norwegian doctors have increased over the last two decades.DesignRepeated cross-sectional survey.SettingAll healthcare levels and medical specialties in Norway.ParticipantsRepresentative samples of Norwegian doctors in 1993 (n=2628) and 2014 (n=1158).Main outcome measuresRelative risk (RR) of self-reported prevalence of work-time experiences of threats and real acts of violence, and of being physically or psychologically unfit during the last 12 months due to fear of violence, in 2014 compared with 1993, adjusted by age, gender and medical specialty.ResultsThere were no differences in self-reported threats (adjusted RR=1.01, 95% CI 0.95 to 1.08) or real acts (adjusted RR=0.90, 95% CI 0.80 to 1.03) of violence when comparing 2014 with 1993. The proportion of doctors who had felt unfit due to fear of violence decreased from 1993 to 2014 (adjusted RR=0.53, 95% CI 0.39 to 0.73). Although still above average, the proportion of doctors in psychiatry who reported real acts of violence decreased substantially from 1993 to 2014 (adjusted RR=0.75, 95% CI 0.60 to 0.95).ConclusionsA substantial proportion of doctors experience threats and real acts of violence during their work-time career, but there was no evidence that workplace violence has increased over the last two decades. Still, the issue needs to be addressed as part of the doctors’ education and within work settings.
