Memandangkan kemampuan internet mewujudkan prasarana yang sesuai untuk membina hubungan, pengamal kaunseling telah mengambil inisiatif memaksimumkan ruang internet ini. Satu perkhidmatan alternatif menolong melalui interaksi maya ini diwujudkan. E-Kaunseling telah mula mendapat perhatian kaunselor sebagai satu perkhidmatan alternatif yang ditawarkan kepada klien.Kajian ini bertujuan untuk meneroka sejauh mana keberkesanan perkhidmatan E-Kaunseling terhadap jantina. Kajian eksperimental-kuasi ini menggunakan reka bentuk Kumpulan Kawalan Ujian Pra Dan Ujian Pasca. Data kuantitatif diperolehi menggunakan soal selidik Client’s Satisfaction Inventory Short-Form (CSI-SF). Seramai 60 subjek kajian terlibat dan dijalankan dalam dua kumpulan kajian iaitu kumpulan kawalan menggunakan kaedah Kaunseling Bersemuka dan kumpulan eksperimen pula menggunakan kaedah E-Kaunseling.Dapatan menunjukkan tiada perbezaan antara jantina dan Kepuasan Klien dalam kedua-dua kaedah dan hasil analisis Anova Dua Hala menunjukkan tiada perbezaan utama (main interaction) antara Jantina dan Kepuasan klien. Hasil analisis lanjut menggunakan Anova Sehala menunjukkan kesan interaksi (interaction effect) bahawa klien lelaki mendapat lebih kepuasan dalam kaedah E-kaunseling manakala klien perempuan mendapat lebih kepuasan dalam kaedah Kaunseling bersemuka. Implikasi kajian ialah kepada kaunselor sekolah dalam menawarkan perkhidmatan e-kaunseling bagi membantu pelajar yang ingin mendapatkan perkhidmatan kaunseling di luar waktu persekolahan. In view of the ability of the internet to create an appropriate relationship-building infrastructure, counselling practitioners have taken the initiative to optimize this internet space. An alternative service aims to establish this virtual interaction. E-Counselling has begun to catch the eye of counsellors as an alternative service offered to clients.This study aims to explore the effectiveness of E-Counselling services on gender.This quasi-experimental study uses the design of Pre-Test and Post-Test Control Groups. Quantitative data were obtained using the Client’s Satisfaction Inventory Short-Form (CSI-SF) questionnaire. A total of 60 participants were involved and conducted in two study groups, namely the control group using the Face-to-Face Counselling method and the experimental group using the E-Counselling method. Findings show no difference between gender and Client Satisfaction in both methods and the results of the Two-Way Anova analysis show no significant difference between Gender and Client Satisfaction. The results of further analysis using One-Way Anova show the interaction effect that male clients get more satisfaction in the E-counselling method while female clients get more satisfaction in the face-to-face counseling method. The implication of the study is for school counselors to provide e-counseling programs to support students who want to access counseling after school hours.

Background: Access to precise information about sexual behaviors is an essential need for a favorable marital life. In the context of Afghan society culture, unawareness about sex issues in offspring on the verge of marriage could lead to an undesirable sexual function. The present study aimed to determine the effect of face-to-face sex education on the sexual function of adolescent female Afghan immigrants. Methods: This was a quasi-experimental study with a Pre-test-Post-test and a control group design. This research was conducted in the immigrant neighborhoods of Mashhad City, east of Iran, in 2018. Two charity centers with the largest numbers of Afghan immigrants were randomly selected as the case and control centers. A continuous sampling technique was applied to select the study participants. Besides, women who met the study inclusion criteria completed a demographic questionnaire and the Female Sexual Function Index (FSFI) before and at 4 and 8 weeks after the intervention. In addition to the center’s routine programs, the experimental group received face-to-face education through PowerPoint slides, while the control group only received routine training of the center. The collected data were analyzed by SPSS using descriptive and inferential statistics. Results: There was no significant difference in the sexual function scores between the two study groups before the intervention (P=0.850), while there was a significant difference between them after 4 (P=0.003) and 8 weeks (P<0.001) of intervention provision. Conclusion: Face-to-face sexual training improved sexual function in adolescent female Afghan migrants; therefore, this training method could be used for sexual education in young female immigrants.

During the current Covid-19 pandemic, the social relationship between humans has changed. As noticed, lockdowns and social distancing have become new norms. Unavoidably, counselling services were also affected with social distancing rules, especially in the school setting. Thus, online counselling or e-counselling has begun to gain in popularity, allowing counsellors to offer their services in an alternative way. In order to evaluate the suitability of e-counselling during the current pandemic, a client’s satisfaction towards e-counselling and face-to-face counselling needs to be investigated. This study aims to explore the effectiveness of these two counselling approaches using the level of the client’s satisfaction and its implication towards counselling practises during the outbreak. Quantitative data were obtained using the Client’s Satisfaction Inventory Short-Form (CSI-SF) questionnaire. The experimental design consisted of a total of 60 study subjects in two groups, namely the control group using the face-to-face Counselling method and the experimental group using the e-Counselling method. Based on the results, the difference in mean score showed that the mean difference in the experimental group was slightly higher than the control group. This finding indicates that e-counselling clients have a higher satisfaction compared to the face-to-face approach. The implications of this data are discussed on the role of school, school counsellor, and the government in enhancing the service of e-counselling during the current pandemic.

OBJECT Donor-side morbidity associated with contralateral C-7 (CC7) nerve transfer remains controversial. The purpose of this study was to evaluate functional deficits in the donor limb resulting from prespinal route CC7 nerve transfer. METHODS A total of 63 patients were included. Forty-one patients had undergone CC7 nerve transfer surgery at least 6 months previously and were assigned to one of 2 groups based on the duration of postoperative follow-up. Group 1 (n = 21) consisted of patients who had undergone surgery between 6 months and 2 years previously, and Group 2 (n = 20) consisted of patients who had undergone surgery more than 2 years previously. An additional 22 patients who underwent CC7 nerve transfer surgery later than those in Groups 1 and 2 were included as a control group (Group 3). Results of preoperative testing in these patients and postoperative testing in Groups 1 and 2 were compared. Testing included subjective assessments and objective examinations. An additional 3 patients had undergone surgery more than 6 months previously but had severe motor weakness and were therefore evaluated separately; these 3 patients were not included in any of the study groups. RESULTS The revised Short-Form McGill Pain Questionnaire (SF-MPQ-2) was the only subjective test that showed a significant difference between Group 3 and the other 2 groups, while no significant differences were found in objective sensory, motor, or dexterity outcomes. The interval from injury to surgery for patients with a normal SF-MPQ-2 score in Groups 1 and 2 was significantly less than for those with abnormal SF-MFQ-2 scores (2.4 ± 1.1 months vs 4.6 ± 2.9 months, p = 0.002). The 3 patients with obvious motor weakness showed a tendency to gradually recover. CONCLUSIONS Although some patients suffered from long-term sensory disturbances, resection of the C-7 nerve had little effect on the function of the donor limb. Shortening preoperative delay time can improve sensory recovery of the donor limb.

Objective: To compare the success rate of different obturation procedures in primary mandibular second molars clinically and also by digital radiovisiography. Methods: A total of 40 children aged between 4-8 years with deeply carious mandibular second primary molars indicated for single session pulpectomy were selected. Canals were obturated with Metapex. The 3 study groups (Endodontic plugger, Handheld lentulospiral, Navi Tip syringe) were compared with the control group (reamer) both clinically and radiovisiographically. The data collected were statistically analyzed using Pearson’s Chi-square and Fisher’s exact test. Results: The use of Navi tip syringe led to the least number of voids followed by Endodontic plugger and Reamer and the highest number of voids was reported with Lentulospiral. Navitip presented maximum number of optimally filled cases followed by Endodontic plugger and Lentulospiral and least number of optimally filled cases with reamer. However, there was no statistically significant difference (p>0.05) in any of the groups with clinical (pain and tenderness to percussion) and radiographic parameters (presence or absence of voids and length of obturation). Conclusion: Within the limitations of the present study, though the clinical outcome was statistically insignificant, Navitip syringe exhibited encouraging results and is a promising option for obturation in primary teeth.

Background: The aim of this study was to compare consequences in single and triple testicular biopsy by biopty gun in pubertal rats using histological and immunohistochemical analysis. Methods: Thirty-two Sprague-Dawley male rats were used as the experimental model. The rats were randomly divided into three study groups. The rats from the first group (n = 12) received a single-biopsy of upper pole of the left testis, while the rats from the second group (n = 10) received triple-biopsy of upper and lower poles and lateral surface of left testis. The third group (n = 10) was a control group. On the eightieth day after the biopsy in all rats bilateral orchiectomy and funiculectomy were performed to obtain testicular tissue and sperm for analysis. The consequences of the puncture were observed by pathohistology, immunohistochemistry and semen analysis. Results: The results of the study showed lower percentage of sperm count (14.5 mill/mL vs. 16 mill/mL, p = 0.130), sperm motility (24.6% vs. 32.7%, p > 0.05), abnormal sperm (30% vs. 27%, p > 0.05), atrophic tubules (21% vs. 6%, p < 0.001), volume (1.7 mL vs. 2.28 mL, p < 0.01) and apoptotic index (1.56 vs. 1.19, p = 0.650) in the testes with a triple-biopsy compared to the testes with a single-biopsy. Semen analysis showed a borderline significant difference between the group with triple-biopsy where sperm count was lower than it in the control group (14.5 mill/mL vs. 17.5 mill/mL, p = 0.05). A single-biopsy has little effect on the testis, especially on overall fertility. A triple-biopsy showed higher degree of the testicular damage but without a significant impact on overall fertility. Semen analysis showed that single- and triple-biopsies did not have a significant effect on sperm count, motility and morphology. Conclusion: Biopty gun procedure is a cheap, simple and reliable method for testicular biopsy in rats without a significant effect on sperm count, motility and morphology.

Abstract INTRODUCTION Controversy exists as to whether telephone clinics are appropriate in neurosurgical-oncology. The COVID-19 pandemic forced neuro-oncology services worldwide to re-design and at the University Hospitals Birmingham UK, telephone clinics were quickly implemented in select patients to limit numbers of patients attending hospital. It was important to determine how these changes were perceived by patients. METHODS A 20-question patient satisfaction questionnaire was distributed to patients who attended neuro-oncology clinic in person (“face-to-face”), or via the telephone. Fisher’s exact test was used to determine significance, which was set at p&lt; 0.05. RESULTS Eighty questionnaires were distributed between June 2020 and August 2020. Overall, 50% (n=40) of patients returned the questionnaire, 50% (n=23) of face-to-face and 50% (n=17) telephone patients. Of those who received telephone consultations, 88% (n=15) felt the consultation was convenient, 88% (n=15) were satisfied with their consultation and 18% (n=3) felt they would have preferred to have a face-to-face appointment. Of those who attended clinic in person, 96% (n=22) felt their consultation was convenient, 100% (n=23) were satisfied with their consultation and 13% (n=3) would have preferred a telephone consultation. Within the face-to-face clinic attendees, only 13% (n=3) were concerned regarding the COVID risk associated with attending hospital. There was no significant difference in patient convenience or satisfaction (p=0.565 and p=0.174 respectively) between face-to-face and telephone clinics. There was no significant difference in whether patients would’ve preferred the alternative method of consultation (p &gt; 0.999). CONCLUSION Our study suggests that careful patient selection for neuro-oncology telephone clinic is not inferior to face-to-face clinic. Telephone clinic during COVID-19 pandemic proved to be convenient, safe and effective. This global health crisis has transformed telephone neuro-oncology consultations from an experimental innovation into established practice and should be continued beyond the pandemic in select cases.

The use of videoconferencing in psychotherapy remains largely unexplored. Videoconferencing compromises the range and quality of interactional information and thus might be expected to affect the working alliance (WA) between client and therapist, and consequently the process and outcome of therapy. A single case study exploring the effect of videoconferencing on the development of the WA in the psychological treatment of a female–male transsexual is described. The self-rated Working Alliance Inventory (WAI) was used to measure client and therapist perceptions of the WA after each session over 10 sessions of eclectic therapy conducted over a videolink. The serial WAI measurements charting the development of the WA in 4 cases of 10-session, face-to-face therapy by Horvath and Marx1 were used as a quasi-control. Therapist and client impressions of teletherapy are described. WAI scores were essentially similar to the face-to-face control group except for lower client-rated bond subscale scores. It is suggested that client personality factors accounted for this difference and that videoconferencing did not impair the development of an adequate working alliance or successful therapeutic outcome.

SummaryObjective: The aim of the study was to investigate the use of somatic cell count (SCC) and aerobic bacterial counts as possible tools to choose animals for selective dry cow treatment (DCT) on cow level. Blanket antibiotic DCT is an approved and common method in Germany aimed at increasing the cure rate of existing intramammary infections (IMI) at the time of drying off and decreasing the risk of new infections during the dry period. However, not all animals benefit equally from an antibiotic DCT. Moreover, dairies, consumers and politicians demand a reduction in the use of antibiotics in farm animals. Several criteria for identifying animals which would benefit from antibiotic DCT have been described in the literature. Material and methods: Animals chosen for this investigation were divided into three study groups: Aerobic Count Petrifilm® group A (153 cows), SCC group S (160 cows) and control group C (169 cows). All quarters were treated with an internal teat sealant (ITS) no matter which group the cows belonged to. Results: Group C, in which all cows received an antibiotic DCT, achieved the best results referred to udder health, particularly regarding bacteriological dry period cure. However, the results of the two study groups A and S, in which the animals were dried off selectively, were only marginally worse. A significant difference regarding bacterial dry period cure was revealed between group S and group C (p = 0.039), whereas no significant difference concerning new IMI risk could be detected between the three study groups. Compared to a blanket antibiotic DCT of all cows in the corresponding study group, a reduction of the usage of antibiotics of 23.5 % (A) and 55 % (S) was noticed, respectively. Furthermore, it could be shown that the lactation number as well as the microorganisms present at the time of drying off influence udder health. Conclusion: Selective DCT is an alternative method to the widespread blanket antibiotic DCT. However, it has to be mentioned that selective DCT involves the described marginal decline in udder health and the additional amount of time, which might arise from the procedure of choosing the right animals and performing the applied rapid test methods.

Objectives: Proton pump inhibitors (PPI) provide a long-lasting anti-acidic effect by inhibiting the proton pump, and they are one of the most commonly prescribed drugs worldwide. PPIs adversely affect the bone structure via deficiency of vitamins and minerals. The aim of this study was to investigate the possible PPI-induced bone changes in the mandible on panoramic radiographs with the methods of fractal analysis and panoramic morphometric indices. Methods: Panoramic radiographs of 402 patients were used (201 PPI users, 201 control group). Fractal analysis was performed on 4 regions of interests (ROI): 1- upper part of the ramus, 2- angulus, 3- anterior of the mental foramen, 4- distal of the middle ramus. Also, the panoramic mandibular index (PMI), mandibular cortical width (MCW), and Klemetti index (KI) were performed on radiographs. Results: There were significant differences in terms of ROI3, MCW, and KI between the control and study groups (p < 0.05) while there was no significant difference for ROI1, ROI2, ROI4, and PMI (p > 0.05). Males were severely affected than females. Conclusions: Osteoporotic changes were detected in the trabecular and cortical bone in the mental foramen region in PPI users with fractal analysis and morphometric indices, while there were no differences for mandibular ramus and angulus regions according to fractal analysis.

AbstractNeopterin is a novel predictor for coronary events especially in diabetic patients and also an indicator for the effectiveness of the periodontal treatment. In this study, we assessed whether salivary neopterin can be used as a potential biomarker in evaluating the risk of cardiovascular disease in type 2 diabetic patients with chronic periodontitis. Forty subjects between 25 and 75 years of age and who matched the criteria were selected and divided into four groups. Their periodontal status was evaluated. Stimulated whole saliva and blood were collected for analysis of salivary neopterin and fibrinogen and HbA1c levels, respectively. Nonsurgical periodontal therapy was carried out. Patients were recalled after 3 months, and the same procedure was repeated. A reduction in all the parameters was seen after treatment in all the four groups. Salivary neopterin levels showed significant difference (p<0.001) in the values between the study groups and the control group before treatment. After 3 months of treatment, salivary neopterin levels showed a statistical significant reduction (p<0.001) in all the study groups. Neopterin could serve as an effective tool to assess the inflammatory process related to periodontitis and diabetes mellitus and also predict future cardiovascular events in diabetic patients.
