Heart failure is characterized as the lack of blood pumping capacity performed by the heart, which is considered a public health problem worldwide. Due to HF, the individual can develop clinical aspects that impact cognitive function and, consequently, self-care. Therefore, this study aimed to investigate cognitive changes and self-care in patients affected by HF and compare it with the cognitive and self-care changes of healthy participants. This is a quantitative, epidemiological, cross-sectional case-control study carried out at an institution in the city of Belém, Pará, Brazil. The following tests were used for data collection: Montreal Cognitive Assessment, Digit Symbol Substitution Test, European Heart Failure Self-care Behavior Scale (EHFScBS). Data were tabulated in Microsoft Excel 2010 and statistically treated by Epi Info version 3.5.2 with a 5% significance level and considering a 95% confidence interval in all analyzes. It was observed that patients with HF have slightly better self-care compared to patients without HF with scores obtained by EHFScBS equal to 29.7 ± 6.9 and 31.8 ± 8.2, respectively. Additionally, patients with HF showed impairments in the three cognitive domains, and women with HF demonstrated greater cognitive impairment compared to the other participants. The present study provides data to help build new approaches to interventions by the multidisciplinary team to promote better self-care and avoid cognitive impairments in patients with HF.

OBJECTIVE To analyze the self-care behaviors according to gender, the symptoms of depression and sense of coherence and compare the measurements of depression and sense of coherence according to gender. METHOD A correlational, cross-sectional study that investigated 132 patients with decompensated heart failure (HF). Data were collected through interviews and consultation to medical records, and analyzed using the chi-square and the Student's t tests with significance level of 0.05. Participants were 75 men and 57 women, aged 63.2 years on average (SD = 13.8). RESULTS No differences in self-care behavior by gender were found, except for rest after physical activity (p = 0.017). Patients who practiced physical activity showed fewer symptoms of depression (p<0.001). There were no differences in sense of coherence according to self-care behavior and gender. Women had more symptoms of depression than men (p = 0.002). CONCLUSION Special attention should be given to women with HF considering self-care and depressive symptoms.

Heart failure (HF) is a growing public health problem. Self-management (SM) of HF is an important component of chronic disease management. Guided by the Individual and Family Self-Management Theory (IFSMT), we examined the associations among complexity of condition, self-regulation, and self-efficacy mediation of SM behaviors in a population of HF outpatients. A cross-sectional design was used. Seventy-three outpatients with HF were enrolled. Simple and multiple linear regressions were run for each outcome variable. Only self-regulation was significantly associated with SM behavior. Complexity of condition was not significantly associated with SM behavior. There was no mediation by self-efficacy. Future nursing interventions should explore self-regulation in HF to provide a clearer understanding of the processes used to change health behavior. SM may be particularly useful in HF with preserved ejection fraction (EF), where there is no proven pharmacological treatment.

Background: Good knowledge of self-care related to heart failure (HF) is key to improve patient outcomes. However, information regarding how much patients know about self-care is lacking in developing countries, particularly in the study setting. Objective: The objectives of this study were to assess HF patients’ knowledge about self-care and to explore predictors among HF patients in Ayder comprehensive specialised referral hospital, Tigray, Ethiopia. Methods: This hospital-based cross-sectional study design was undertaken in Ayder comprehensive specialised referral hospital, Tigray, Ethiopia. A total of 408 HF patients participated, and the study was done between February and April 2018. An interviewer-assisted structured questionnaire was used to collect data. Bivariate logistic regression and multivariable logistic regression were conducted to identify the predictors associated with knowledge about self-care. Results: The mean age of the participants was 45.4 years (standard deviation=19 years). Around 255 (62.5%) of the participants had poor self-care knowledge. Age, New York Heart Association classes, duration of the disease and previous hospitalisation were the factors associated with knowledge about self-care. Conclusion: More than half of all participants had poor knowledge about self-care. Improving existing prevention strategies and strengthening patients’ knowledge are recommended to address this knowledge deficit.

Background: Heart failure (HF) is a major and growing public health problem worldwide. Across the world, heart failure is associated with high mortality, high hospitalization rates, and poor quality of life. Self-care is defined as a naturalistic decision-making process involving the choice of behaviors that maintain physiologic stability, the response to symptoms when they occur, and the ability to follow the treatment regimen and control symptoms. One instrument used to measure self-care is the Self Care of Heart Failure Index. Aim: The purpose of this study was to test the psychometric properties of the Spanish version of the Self Care of Heart Failure Index v.6.2 (SCHFI v.6.2). Methodology: Before testing its psychometric properties, the SCHFI v.6.2 was translated and adapted from its original English version into Spanish. Subsequently, we tested the instrument’s psychometric properties on a sample of 203 participants with HF. Descriptive statistics were used to analyze the sociodemographic and clinical variables, and to describe item responses. We tested the factorial validity of the SCHFI v.6.2 using confirmatory and exploratory factor analysis. Results: Confirmatory factor analysis (CFA) was performed using the our pre-existing models which resulted with poor fit indices. Thus, we performed exploratory factor analysis (EFA) on each of the SCHFI v.6.2 scales. Conclusion: The Spanish version of the SCHFI v.6.2. has good characteristics of factorial validity and can be used in clinical practice and research to measure self-care in patients with HF.

AimThe aim of this study was to critically analyse and describe gender differences related to self-care among patients with chronic heart failure (HF).Methods and resultsA monocentric real-world cohort of 346 patients with chronic HF in follow-up was used for this cross-sectional study. We report data related to the cohort’s demographic and clinical characteristics. Self-care was assessed using the Self-Care of Heart Failure Index before patients’ discharge. After bivariate analysis, logistical regression models were used to describe the relationship between gender, self-care behaviours and self-care confidence. While men were found to have more than quadruple the risk of poor self-care than women (OR 4.596; 95% CI 1.075 to 19.650), men were also found to be approximately 60% more likely to have adequate self-care confidence than women (OR 0.412; 95% CI 0.104 to 0.962). Considering that self-care confidence is described as a positive predictor of behaviours, our results suggest a paradox. It is possible that the patient–caregiver relationship mediates the effect of confidence on behaviours. Overall, adequate levels of self-care behaviours are a current issue, ranging 7.6%–18.0%.ConclusionThis study sets the stage for future research where elements of the patient–caregiver relationship ought to be considered to inform the planning of appropriate educational interventions. We recommend routinely measuring patients’ self-care behaviours to guide their follow-up and as a basis for any changes in their daily life behaviours.

ABSTRAKGagal jantung merupakan salah satu penyakit kardiovaskuler yang paling sering terjadi di seluruh dunia yang mengakibatkan tingginya angka mortalitas, morbiditas dan juga berdampak secara finansial terutama bagi lanjut usia. Rehospitalisasi merupakan masalah umum yang sering terjadi pada pasien gagal jantung yang sebagain besar disebabkan oleh keterlambatan dalam pengenalan gejala, pengobatan dan ketidakpatuhan diet serta kurangnya pengetahuan dan keterampilan dalam melakukan perawatan diri. Panduan penanganan gagal jantung menekankan pentingnya perilaku perawatan diri untuk menurunkan kekambuhan dan rehospitalisasi pada pasien gagal jantung. Penelitian ini bertujuan untuk menggambarkan perilaku perawatan diri pada pasien gagal jantung yang mengunjungi poliklinik jantung di RSUD Panembahan Senopati Bantul. Penelitian ini merupakan penelitian deskriptif quantitatif dengan menggunakan pendekatan cross sectional. Sampel berjumlah 74 responden yang merupakan pasien rawat jalan di poliklinik jantung pada bulan Juni hingga Juli 2018. Alat ukur yang digunakan adalah Self-Care of Heart Failure Index (SCHFI), (skor ≥70 poin=adekuat). Uji statistik chi-square digunakan untuk mengetahui hubungan antara factor sosiodemografi dengan perilaku perawatan diri pada pasien gagal jantung. Hasil penelitian ini menunjukkan bahwa perilaku perawatan diri pada sebagian besar responden masih belum adekuat dengan frekuensi perilaku perawatan diri adekuat yang masih rendah (pemeliharaan: 45±13.1 (7.7%), pengelolaan: 46±20.4 (13.0%) dan kepercayaan: 69±16.5 (38.5%)). Hanya satu faktor yaitu penyakit penyerta yang berhubungan dengan perilaku perawatan diri pada pasien gagal jantung pada dimensi kepercayaan diri. Hasil penelitian ini menunjukkan bahwa sebagian besar responden memiliki perilaku perawatan diri yang tidak adekuat seperti juga pada negara-negara lainnya sehingga diperlukan upaya untuk meningkatkan perilaku perawatan diri pada pasien gagal jantung. ABSTRACTHeart failure (HF) is one of the most prevalent cardiovascular diseases in the world which associated with significant mortality, morbidity, and huge financial burden, particularly among aging population. Rehospitalization remain a concern in the care of the heart failure patient which largerly associated with delay in symptoms recognition, treatment, diet non-compliance and also lack of knowledge and skills in heart failure self-care. Guideline on HF emphasize the important of self care in preventing symptoms and hospital readmission. This study aims to describe self-care behavior in a sample of heart failure patients. This is a descriptive cross-sectional study with non-probabilistic sample of 74 in an out-patient heart clinic from June-July 2018. Self-care was measured using the Self-Care of Heart Failure Index (SCHFI), (scores ≥70 points=adequate self-care). Chi-square test was conducted to test whether participant’s characteristics associated with self-care behavior among heart failure patients in three scales. The result shows that self-care was inadequate in most scales (self-care maintenance: 43.4±11.8, self-care management: 49.4±18.5 and self-care confidence: 68,6±14.5. Low frequencies of participants with adequate self-care also observed in the study (self-care maintenance: 5.4%, self-care management: 15.4%) and self-care confidence: 36.5%)). In self-care confidence subscale, higher scores of the SCHFI were associated with no comorbid conditions (p=0.01). None of the sociodemographic and clinical characteristics of the sample were associated with increased self-care in other 2 sub-scales. In conclusion, study shows that self-care in HF is inadequate, interventions aimed at improving self-care espescially in self-care maintenance sub-scale are greatly needed. Additional studies are needed to determine the best approach for this.

Introdução: Atualmente, a obesidade é considerada um grave problema de saúde pública, pois além de ser uma doença, é um fator de risco para progressão e desenvolvimento de outras doenças crônicas não transmissíveis. Objetivo: Compreender a percepção de indivíduos com obesidade quanto à participação em papeis ocupacionais, grau de importância atribuído a cada papel e padrão de desempenho de papeis ocupacionais ao longo da vida. Método: Configura-se como um estudo transversal do tipo exploratório de caráter descritivo, com análise quantitativa dos dados. Como instrumento, utilizou-se a Lista de Identificação de Papéis Ocupacionais. Resultados: A amostra foi composta por 65 indivíduos com obesidade, 87,7% (n=57) era do sexo feminino e 12,3% (n=8) do sexo masculino, tendo como média de idade 42 anos e 9 meses. Constatou-se que o papel mais comum no passado foi o de estudante e com relação ao presente e futuro, os papeis mais citados foram os de serviço doméstico e membro da família. Quanto ao grau de importância, identificou-se que os indivíduos valorizam os papeis ocupacionais, atribuindo maior valorização aos papeis de membro da família e trabalho, e menor proporção de valor ao papel de participação em organizações. Conclusão: A condição de obesidade pode interferir no modo como o indivíduo se envolve e desempenha suas ocupações, evidenciando a terapia ocupacional como uma estratégia importante no enfrentamento da obesidade. AbstractIntroduction: Currently, obesity is considered a serious public health problem, since it is also a disease, it is a risk factor for progression and development of other chronic non-communicable diseases. Objective: To understand the perception of individuals with obesity regarding the participation in occupational roles throughout life, about the degree of importance attributed to each role, and in a complementary way to identify the performance pattern of occupational roles in Long life. Method: It is configured as a cross-sectional exploratory study of descriptive character, with quantitative analysis of the data, as an instrument used the identification list of occupational roles. Results: The sample consisted of 65 obese individuals. Of these, 87.7% (n = 57) were females and 12.3% (n = 8) were males, with a mean age of 42 years 9 months. It was found that the most common role in the past was that of student and in relation to the present and future the most cited roles were those of domestic service and family member. As to the degree of importance, it was identified that the individuals value the occupational roles, attributing greater value to the roles of family members and work, and less appreciation of the role of participation in organizations. Conclusion: The condition of obesity can interfere in the way the individual engages and performs his occupations, thus placing the occupational therapy professional as an important member in coping with obesity.Key words: Obesity, Activities of Daily Living, Occupations, Occupational Therapy.ResumenIntroducción: Actualmente, la obesidad se considera un grave problema de salud pública, ya que es también una enfermedad, es un factor de riesgo para la progresión y el desarrollo de otras enfermedades crónicas no transmisibles. Método: se configura como un estudio exploratorio transversal de carácter descriptivo, con análisis cuantitativo de los datos, como instrumento utilizó la lista de identificación de roles ocupacionales. Resultados: la muestra consistió en 65 individuos obesos. De ellos, 87,7% (n = 57) fueron hembras y 12,3% (n = 8) fueron varones, con una edad media de 42 años 9 meses. Se descubrió que el papel más común en el pasado era el de los estudiantes y en relación con el presente y el futuro, los papeles más citados eran los del servicio doméstico y el miembro de la familia. En cuanto al grado de importancia se identificó que los individuos valoran los papeles ocupacionales, atribuyendo mayor valoración a los papeles de miembro de la familia y trabajo, y menor valorización al papel de participación en organizaciones. Conclusión: la condición de la obesidad puede interferir en la forma en que el individuo se involucra y realiza sus ocupaciones, colocando así al profesional de la terapia ocupacional como un miembro importante para hacer frente a la obesidad.Palabras clave: Obesidad, Actividades Cotidianas, Ocupaciones, Terapia Ocupacional.

Background: Patients with heart failure need to be engaged in adequate cardiac self-care behaviors helping to prevent the development of the disease and ameliorate their health status. However, the conceptual model of the present study has not been tested in previous studies among patients with heart failure. Objectives: The present study aimed to investigate the psychosocial determinants based on Pender’s health promotion model (HPM) affecting self-care behavior among outpatients suffering from heart failure. Methods: In this cross-sectional study, a total of 200 patients suffering from heart failure were selected from the outpatient clinics of Tabriz, Iran, using convenience sampling and assessed for self-self-care behaviors and major concepts of HPM via self-administered questionnaires. Path analysis was used in order to analyze the conceptual model Results: The present hypothetical model showed a good fit. Perceived benefits and activity-related effects directly affected self-care behaviors. Bootstrapping mediation analyses indicated that perceived self-efficacy, perceived barriers, perceived benefits, and activity-related effects indirectly affected self-care behaviors through commitment to action. Conclusions: The commitment to action can help to promote self-care behaviors among the subjects suffering from heart failure. The interventions should take the role of predictive variables of this study and commitment to action into account in these patients.

Assessing self-care is important aspects among patients with heart failure. However, few studies were conducted to assess self-care among patients with heart failure in Jordan. Therefore, this study aimed to assess the most and the least performed maintenance self-care behaviors and to examine the relationship between maintenance self-care behaviors and selected sociodemographics. A cross-sectional design utilizing a convenience sample of 226 patients with heart failure was used. The maintenance self-care mean was 53.89 and considered below the clinical target level (≥70). Asking for low salt item and performing physical exercises were the most performed self-care behaviors, while “trying to avoid getting sick” and “checking ankles for swelling” were the least performed self-care behaviors. Limited self-care behaviors indicated the need to implement cardiac education that may improve self-care behaviors. Cardiac education should target mainly patients with low income, low educational level, elderly, living alone, unemployed, and who are using traditional treatment.
