Poor sleep quality and sleep disorders are the most common problems in people, affecting health-related quality of life. Various studies show an association between sleep disorders and altered levels of stress hormones and inflammatory cytokines measured in saliva. The main objective of this article is to provide an analysis of the current evidence related to changes in inflammatory markers in the saliva and their associations with sleep quality measurement (both objective and subjective methods) in healthy subjects and in sleep-related disorders. To that end, a scoping review was carried out, following the PRISMA criteria in the bibliographic search in several databases: PubMed, EBSCO, and SCOPUS. Eleven of the articles are from the adult population and two from the child-youth population. They mainly measure the relationship between sleep and interleukin-1β (IL-1β), interleukin-6 (IL-6), and tumor necrosis factor alpha (TNFα) alpha, as well as other inflammatory markers such as myeloperoxidase (MPO) and prostaglandin-endoperoxide synthase 2. An analysis shows the relationship between these salivary biomarkers and sleep quality, especially in the case of IL-6 in both healthy subjects and several pathologies associated with sleep-disorders. The results for TNFα and IL-1β measurements are still inconclusive and the difference with IL-6 was assessed. Two studies reported interventions that result in sleep improvement and are accompanied by the normalization of inflammatory changes detected in the saliva. As it is an easy-to-apply and non-invasive method, the measurement of salivary cytokines can be very useful in chronobiology studies. Further studies are required to determine the sensitivity of salivary inflammatory markers in monitoring biological rhythms and acting as biomarkers in the detection of sleep disorders and sleep interventions.

Recent contributions have reported sleep disorders as one of the health impairment outcomes of workaholism. A possible factor affecting the sleep-wake cycle might be the intensive use of smartphones. The current study aimed to explore the role of intensive smartphone use in the relationship between workaholism and the sleep-wake cycle. Two serial multiple mediation models were tested on a sample of 418 employees, who filled self-report questionnaires measuring workaholism, use of smartphones, sleep quality and daytime sleepiness, using conditional process analysis for testing direct and indirect effects. Results supported our hypotheses regarding two serial multiple mediation models—that intensive smartphone use and poor sleep quality mediated the relationship between workaholism and daytime sleepiness, and that smartphone use and daytime sleepiness mediated the relationship between workaholism and poor quality of sleep. Although the use of a cross-sectional design and the snowball technique for collecting data can be considered as possible limitations, the current study is one of the first to document the potential detrimental role of the intensive smartphone use on the workaholism-sleep disorders relationship.

This study investigated the relationship between sleep quality and fibromyalgia symptoms in 326 patients. The Pittsburgh Sleep Quality Index was used to assess the presence of sleep disorders. Multivariate analysis of variance was performed to determine the influence of fibromyalgia symptoms on sleep quality. The prevalence of sleep disorders was 92.9 percent. Patients reported generalized pain (88.3%), memory failure (78.5%), moodiness (59%), excessive anxiety (77.5%), and concentration difficulties (69.1%). Patients with more symptoms reported poor sleep quality ( p < .05; d = .74), and the total Pittsburgh Sleep Quality Index score correlated with the number of symptoms ( p < .01). Sleep quality has an important association with fibromyalgia symptoms.

Sleep problems may have negative effects on work–life balance, overall health, and safety. We aimed to investigate the association between sleep disorders and absenteeism and delay to work (being late or tardy) among the working adult population. The study was conducted by using data from a large survey of working adults who participated in the Turkish Adult Population Epidemiology of Sleep Study (TAPES) managed by Turkish Sleep Medicine Society (TSMS). Secondary analyses was employed to examine absenteeism and delay to work and their associations with sleep problems, including sleepiness by Epworth Sleepiness Scale (ESS), parasomnias, sleep apnea (by Berlin Questionnaire), sleep quality (by Pittsburgh Sleep Quality Index), and restless leg. History of any absenteeism and delay to work was observed in 276 (18%) and 443 (29%) out of 1,533 working adults, respectively. In the multivariate analyses, absenteeism was associated with younger age, female gender and poor sleep quality, while delay to work was associated with younger age, poor sleep quality, parasomnia, and sleepiness. In the presence of absenteeism and delay to work, sleep disorders including sleepiness, poor sleep quality, and parasomnia should be considered. Such evaluation may improve worker well-being and provide some additional benefits in terms of increasing productivity and lowering work-related costs.

Abstract Introduction Pregnant women are more likely to develop sleep disorders due to poor sleep quality. Sleep hygiene has been reported as a method for dealing with sleep disorders. However, studies on the relationship between sleep hygiene and sleep disorders in Japanese pregnant women remain insufficient. This study aimed to clarify the actual state of sleep disorders during pregnancy and the relationship between sleep disorders and sleep hygiene. Methods A cross-sectional study was conducted on 147 Japanese pregnant women. We used the Japanese versions of the Pittsburgh Sleep Quality Index (PSQI) and the Epworth Sleepiness Scale (JESS) for evaluation of sleep. We used 32 items of habitual behavior important for sleep improvement for evaluation of sleep hygiene; these items were self-selected. Results The average PSQIG scores of women in their first, second, and third trimesters were 5.6 ± 4.3, 5.4 ± 2.4, and 6.6 ± 2.8, respectively; 22.2%, 46.4%, and 62.2% of women in their first, second, and third trimesters, respectively, exceeded the cut-off score. The average JESS scores were 12.2 ± 4.5, 9.9 ± 4.1, and 10.0 ± 4.6 for first, second, and third trimester women, respectively; 55.6%, 35.7%, and 41.9% of first, second, and third trimester women, respectively, slept excessively during the day (JESS score≥11). Following factor analysis, sleep hygiene was categorized into four factors: ”thinking positively,” ”adding rhythm to the day,” “avoiding going out and caffeine before bedtime,” and “avoiding eye irritation.” Pregnant women who “added rhythm to their day” had lower PSQI scores, and sleep hygiene was not related to JESS scores in the multiple regression analysis of sleep hygiene and sleep disorders. Conclusion Sleep disorders in pregnancy are related to habitual behavior such as being active during the day, eating and sleeping regularly, and basking in the sunlight every morning. Support

Abstract Introduction Despite positive secondary prevention strategies post myocardial infarction (MI), including statin use and lifestyle changes, 32% of the annual MIs are recurrent (MIR). As coronary heart disease is related to atherosclerosis, a chronic inflammatory process, and sleep is associated with cardiovascular disease and innate immunity, understanding the role of sleep and inflammation and MIR is important in developing interventions to improve sleep, reduce inflammation, and delay or prevent MIR. This study aimed to explore the role of sleep quality and inflammatory markers on MIRs. Methods We conducted a secondary analysis of cross-sectional data of individuals (N=156) having at least one or more MIs within the last 3 to 7 years. Using the hypothalamus-pituitary-adrenal axis model (Irwin, 2019), we tested sleep quality (Pittsburgh Sleep Quality Index [PSQI]) predicting MIR, using inflammatory markers (hs C-Reactor Protein [CRP], Interleukin-1ß [IL-1ß] and Tumor Necrosis Factor alpha [TNFα]) as the simultaneous indirect paths. Race, sex and body mass index (BMI) were also examined using moderated mediation. Results The sample ranged in age from 34 to 92 (M = 65.37, SD = 12.13), BMI averaged 31.11 (SD = 7.34), and was comprised of mostly male (57.1%) and White adults (67.9%). PSQI predicted only IL-1ß (ß= .02; p &lt; .01). IL-1ß predicted MIR (ß = .80, p = .05). The direct effect of PSQI to MIR was not significant (p =.12), the indirect path via IL-1ß was. This relationship was not moderated by race, sex, nor BMI. Conclusion IL-1ß is an inflammatory marker elevated after acute MI which does not reflect our selected sample. Inflammation may be an important marker of risk for MIR in those with poor sleep quality. Future studies should examine other markers of inflammation and sleep in those with MIR. Support (if any):

The increasing use of smartphone among young people is creating negative effects and is an important public health problem in many countries. Smartphone abuse and addiction may cause physical and psychological disorders among users. However, the awarenes on this issue has been inadequate due to lacking of evidence. Objectives: To describe the current situation of smartphone using among students at highschools and universities in Hue city and to examine the relationship between smartphone using and sleep disturbances and psychological disstress among participants. Methods: A cross-sectional study with a randomly selected sample of 1,150 students at highschools and universities in Hue city. SAS-SV scale was used to evaluate phone addiction status, K10 scale was used for psychological distress assessment and PSQI scale was used to examine the sleep quality. Results: The proportion of students at highschools and universities having smartphones was 78.0%. The rate of smartphone addiction among high school students was 49.1% and that among university students was 43.7%. There was 57.3% of high school students had poor sleep quality, and that of university students was 51.6%. There was a statistically significant association between smartphone addiction and sleep disturbances and psychological disstress among participants (p <0.05). Conclusion: The prevalence of smartphone addiction among students at highschools and universities is alarming and is related to sleep disturbances and psychological disstress among participants. There is a strong call to develop intervention to help students to aware and manage the use of smartphone effectively.

Background:Rheumatoid arthritis (RA) is a chronic, autoimmune inflammatory condition that affects 0.5% of the adult population worldwide (1). Sedentary behavior (SB) is any waking behavior characterized by an energy expenditure of ≤1.5 METs (metabolic equivalent) and a sitting or reclining posture, e.g. computer use (2) and has a negative impact on health in the RA population (3). Sleep is an important health behavior, but sleep quality is an issue for people living with RA (4, 5). Poor sleep quality is associated with low levels of physical activity in RA (4) however the association between SB and sleep in people who have RA has not been examined previously.Objectives:The aim of this study was to investigate the relationship between SB and sleep in people who have RA.Methods:A cross-sectional study was conducted. Patients were recruited from rheumatology clinics in a large acute public hospital serving a mix of urban and rural populations. Inclusion criteria were diagnosis of RA by a rheumatologist according to the American College of Rheumatology criteria age ≥ 18 and ≤ 80 years; ability to mobilize independently or aided by a stick; and to understand written and spoken English. Demographic data on age, gender, disease duration and medication were recorded. Pain and fatigue were measured by the Visual Analogue Scale (VAS), anxiety and depression were assessed using the Hospital Anxiety and Depression Scale (HADS), and sleep quality was assessed using the Pittsburgh Sleep Quality Index. SB was measured using the ActivPAL4™ activity monitor, over a 7-day wear period. Descriptive statistics were calculated to describe participant characteristics. Relationships between clinical characteristics and SB were examined using Pearson’s correlation coefficients and regression analyses.Results:N=76 participants enrolled in the study with valid data provided by N=72 participants. Mean age of participants was 61.5years (SD10.6) and the majority 63% (n = 47) were female. Participant mean disease duration was 17.8years (SD10.9). Mean SB time was 533.7 (SD100.1) minutes (8.9 hours per day/59.9% of waking hours). Mean sleep quality score was 7.2 (SD5.0) (Table 1). Correlation analysis and regression analysis found no significant correlation between sleep quality and SB variables. Regression analysis demonstrated positive statistical associations for SB time and body mass index (p-value=0.03846, R2 = 0.05143), SB time and pain VAS (p-value=0.009261, R2 = 0.07987), SB time and HADS (p-value = 0.009721, R2 = 0.08097) and SB time and HADSD (p-value = 0.01932, R2 = 0.0643).Conclusion:We found high levels of sedentary behavior and poor sleep quality in people who have RA, however no statistically significant relationship was found in this study. Fatigue and poor sleep quality are the most common clinical complaints of people with diabetes mellitus (DM). These complaints are early signs of DM and are closely related to diabetic control and the presence of complications, which lead to a decline in the quality of life. Therefore, an accurate measurement of the relationship between fatigue, sleep status, and the complication of DM nephropathy could lead to a specific definition of fatigue and an appropriate medical treatment. This study recruited 307 people with Type 2 diabetes from two medical centers in Northern Taiwan through a questionnaire survey and a retrospective investigation of medical records. In an attempt to identify the related factors and accurately predict diabetic nephropathy, we applied hybrid research methods, integrated biostatistics, and feature selection methods in data mining and machine learning to compare and verify the results. Consequently, the results demonstrated that patients with diabetic nephropathy have a higher fatigue level and Charlson comorbidity index (CCI) score than without neuropathy, the presence of neuropathy leads to poor sleep quality, lower quality of life, and poor metabolism. Furthermore, by considering feature selection in selecting representative features or variables, we achieved consistence results with a support vector machine (SVM) classifier and merely ten representative factors and a prediction accuracy as high as 74% in predicting the presence of diabetic nephropathy.

Background/Objective. Sleep apnea is associated with elevated inflammatory markers. A subgroup of patients never report sleep disturbances to their physician. The inflammatory status of this subgroup is not known. The present study aims to evaluate two inflammatory markers, C-reactive protein (CRP) and red cell distribution width (RDW), in those with unreported sleep disturbances and compares these findings to those with and without reported sleep disorders. We also investigate the utility of RDW as an inflammatory marker in sleep disorders. Methods. Sample includes 9,901 noninstitutionalized, civilian, nonpregnant adults from the 2005-2008 National Health and Nutrition Examination Survey, a nationally representative, cross-sectional U.S. study. Sleep questionnaire and laboratory data were used to compare inflammatory markers (CRP and RDW) in five subgroups of individuals: reporting physician-diagnosed sleep apnea, reporting another physician-diagnosed sleep disorder, reported sleep disturbance to physician with no resulting diagnosis, unreported sleep disturbance (poor sleep quality not reported to physician), and no diagnosed sleep disorder or sleep disturbance. Results. Individuals with unreported sleep disturbance had significantly higher odds of elevated RDW (>13.6%) when compared to those without a sleep disturbance in adjusted models (OR=1.33). Those with unreported sleep disturbance had significantly higher odds of elevated CRP levels (>1 mg/L) than those without sleep disturbances (OR 1.34), although the association was not significant when adjusted for obesity and other controls. Conclusion. Self-identified unreported sleep disturbances are associated with significantly higher odds of elevated RDW than those without sleep disturbances. RDW may serve as a valuable indicator in identifying individuals at higher risk for sleep apnea and other sleep disorders.

Both sleep–wake disturbance and malnutrition are common in cirrhosis and might be associated with similar adverse outcomes, such as impaired health-related quality of life, hepatic encephalopathy, and sarcopenia, but there is no study investigating the relationship between these two. We aimed to explore the relationship between sleep–wake disturbance [estimated by the Pittsburgh Sleep Quality Index (PSQI)] and malnutrition risk [estimated by the Royal Free Hospital-Nutritional Prioritizing Tool (RFH-NPT)]. About 150 patients with cirrhosis were prospectively recruited. The nutritional risk is classified as low (0 points), moderate (1 point), and high (2–7 points) according to the RFH-NPT score. A global PSQI &gt;5 indicated poor sleepers. Furthermore, multivariate linear regression analyses were performed to determine the relationship between sleep–wake disturbance and malnutrition. The median PSQI was seven, and RFH-NPT was two in the entire cohort, with 60.67 and 56.67% rated as poor sleep quality and high malnutrition risk, respectively. Patients with cirrhosis with poor sleep quality had significantly higher RFH-NPT score (3 vs. 1, P = 0.007). Our multivariate analyses indicated that male patients (β = 0.279, P &lt; 0.001), ascites (β = 0.210, P = 0.016), and PSQI (β = 0.262, P = 0.001) were independent predictors of malnutrition. In addition, the differences regarding PSQI score were more significant in male patients, as well as those &gt;65 years or with Child-Turcotte-Pugh class A/B (CTP-A/B) or the median model for end-stage liver disease (MELD) &lt;15. Taken together, the sleep–wake disturbance is strongly correlated with high malnutrition risk in patients with cirrhosis. Given sleep–wake disturbance is remediable, it is tempting to incorporate therapies to reverse poor sleep quality for improving nutritional status in patients with cirrhosis.
