Background: Gestational diabetes mellitus (GDM) is a strong risk factor for type 2 diabetes mellitus (T2D) and the postpartum period is crucial for early treatment in at-risk women. However, despite recommendations, only a fraction of women undergo a postpartum screening for glucose intolerance (ppOGTT). The present study aims to verify the reason(s) for poor adherence in our population. Research design and methods: This retrospective study includes 451 women in which GDM was diagnosed between 2015–2016. During 2017, we verified by phone interview how many women underwent ppOGTT at 6–12 weeks postpartum, as recommended by the Italian guidelines. The non-compliant women were asked about the reason(s) for failing to screen. The non-parametric Mann-Whitney test and the 2-tailed Fisher exact test were used to compare continuous and categorical features, respectively, among women performing or non-performing ppOGTT. Results: Out of 451 women with GDM diagnosis, we recorded information from 327. Only 97 (29.7%) performed ppOGTT. The remaining 230 women (70.3%) provided the following explanation for non-compliance: (1) newborn care (30.4%); (2) misunderstood importance (28.3%); (3) oversight (13.0%); (4) unavailability of test reservation in the nearest centers (10.4%); (5) normal glycemic values at delivery (8.3%); (6) discouragement by primary care physician (5.6%). Conclusions: In our population, most women with recent GDM failed to perform ppOGTT. Our results indicated that the prominent barriers could potentially be overcome.

Abstract: Motivation of each individual is related to the self-efficacy that each individual has. The low level of client motivation affects client's self-efficacy, so that self-care management of DM patients cannot run well. The purpose of this study is to know the relationship between motivation and self-efficacy in patients with type 2 diabetes mellitus at the GMIM Hospital Pancaran Kasih Manado in 2018. The Design of this study was descriptive analytical with cross sectional approach. The study was conducted at the GMIM Hospital Pancaran Kasih Manado. The sample used was 34 patients. The instruments used were questionnaire demographic characteristics of respondents, motivational questionnaires, and self-efficacy questionnaires tested using the Fisher Exact test. The results of the Fisher Exact statistic test are the relationship between motivation and self-efficacy of patients with Type 2 Diabetes Mellitus at GMIM General Hospital Pancaran Kasih Manado obtained p = 0.026 (α = 0.05). The conclusion is that there is a relationship between motivation and self-efficacy of patients with Type 2 Diabetes Mellitus in RSU GMIM Pancaran Kasih Manado. Jenis penelitian yang digunakan adalah deskriptif analitik dengan pendekatan cross sectional. Penelitian dilaksanakan di RSU GMIM Pancaran Kasih Manado.. Sampel yang digunakan sebanyak 34 pasien. Instrumen yang digunakan kuesioner karakteristik demografi responden, kuesioner motivasi, dan kuesioner efikasi diri yang diuji menggunakan uji Fisher Exact. Hasil uji statistik Fisher Exact hubungan antara motivasi dengan efikasi diri pasien Diabetes Melitus Tipe 2 di Rumah Sakit Umum GMIM Pancaran Kasih Manado diperoleh nilai p = 0,026 (α = 0,05). Kesimpulan yaitu ada hubungan antara motivasi dengan efikasi diri pasien Diabetes Melitus Tipe 2 di RSU GMIM Pancaran Kasih Manado.Kata Kunci: Motivasi, Efikasi Diri

AbstractObjective: to evaluate the effect of foot reflexology on feet impairment of people with type 2 diabetes mellitus.Method: this is a randomized, controlled and blind clinical trial. The sample was comprised by people with type 2 diabetes mellitus who, after being randomized into Treated group (n = 21) and Control group (n = 24), received guidelines on foot self-care. To the Treated Group it was also provided 12 sessions of foot reflexology. The scores of impairment indicators related to skin and hair, blood circulation, tissue sensitivity and temperature were measured by means of the instrument for assessing tissue integrity of the feet of people with diabetes mellitus. Chi-square test, Fisher exact test, Mann-Whitney test and regression analyzes were applied to the data, considering a significance level of 5% (P value <0.05).Results: participants who received the therapy showed better scores in some impairment indicators related to skin and hair (hair growth, elasticity/turgor, hydration, perspiration, texture and integrity of the skin/ skin peeling).Conclusion: the foot reflexology had a beneficial effect on feet impairment of people with type 2 diabetes mellitus, which makes it a viable therapy, deserving investment. This study was registered in the Brazilian Registry of Clinical Trials - RBR-8zk8sz.

Objective. Type 2 deiodinase (Dio2) is an enzyme responsible for the conversion of T4 to T3. The Thr92Ala polymorphism has been shown related to an increased risk for developing type 2 diabetes mellitus (T2DM). The aim of this study is to assess the association between this polymorphism and glycemic control in T2DM patients as marked by the HbA1C levels.Design and Methods.The terms “rs225014,” “thr92ala,” “T92A,” or “dio2 a/g” were used to search for eligible studies in the PubMed, Embase, and Cochrane databases and Google Scholar. A systematic review and meta-analysis of studies including both polymorphism testing and glycated hemoglobin (HbA1C) assays were performed.Results. Four studies were selected, totaling 2190 subjects. The pooled mean difference of the studies was 0.48% (95% CI, 0.18–0.77%), indicating that type 2 diabetics homozygous for the Dio2 Thr92Ala polymorphism had higher HbA1C levels.Conclusions. Homozygosity for the Dio2 Thr92Ala polymorphism is associated with higher HbA1C levels in T2DM patients. To confirm this conclusion, more studies of larger populations are needed.

IntroductionTo describe the incidence and compare in-hospital outcomes of community-acquired pneumonia (CAP), ventilator-associated pneumonia (VAP) and non-ventilator hospital-acquired pneumonia (NV-HAP) among patients with or without type 2 diabetes mellitus (T2DM) using propensity score matching.Research design and methodsThis was a retrospective observational epidemiological study using the 2016–2017 Spanish Hospital Discharge Records.ResultsOf 245 221 admissions, CAP was identified in 227 524 (27.67% with T2DM), VAP was identified in 2752 (18.31% with T2DM) and NV-HAP was identified in 14 945 (25.75% with T2DM). The incidence of pneumonia was higher among patients with T2DM (CAP: incidence rate ratio (IRR) 1.44, 95% CI 1.42 to 1.45; VAP: IRR 1.24, 95% CI 1.12 to 1.37 and NV-HAP: IRR 1.38, 95% CI 1.33 to 1.44). In-hospital mortality (IHM) for CAP was 12.74% in patients with T2DM and 14.16% in matched controls (p<0.001); in patients with VAP and NV-HAP, IHM was not significantly different between those with and without T2DM (43.65% vs 41.87%, p=0.567, and 29.02% vs 29.75%, p=0.484, respectively). Among patients with T2DM, older age and dialysis were factors associated with IHM for all types of pneumonia. In patients with VAP, the risk of IHM was higher in females (OR 1.95, 95% CI 1.28 to 2.96).ConclusionThe incidence rates of all types of pneumonia were higher in patients with T2DM. Higher mortality rates in patients with T2DM with any type of pneumonia were associated with older age, comorbidities and dialysis.

Background and Aims. In the present study, we have investigated the activity of adenosine deaminase (ADA) as a diagnostic marker in type 2 (or II) diabetes mellitus (T2DM). Design and Methods. The deaminase activity of ADA1 and ADA2 was determined in serum from 33 patients with type 2 (or II) diabetes mellitus and 35 healthy controls. We also determined the proportion of glycated hemoglobin (HbA1c). Results. Our results showed significant differences between total serum ADA (tADA) and ADA2 activities in the diabetic groups with HbA1c < 8 (%) and HbA1c ≥ 8 (%) with respect to the values in healthy individuals (p<0.001). ADA2 activity in patients with high HbA1c was found to be much higher than that in patients with low HbA1c (p=0.0001). In addition, total ADA activity showed a significant correlation with HbA1c (r=0.6, p<0.0001). Conclusions. Total serum ADA activity, specially that due to ADA2, could be useful test for the diagnosis of type 2 (or II) diabetes mellitus.

Diabetic patients can suffer from hipokalemia because of prolonged osmotic diuretic and management diabetic mellitus with insulinif does not associate with kalium intake. The aimed of this study was to evaluate kalium serum level on type 2 DM, and comparisonof kalium serum level on type 2 DM patients with or without insulin therapy. A cross-sectional study was done on 33 patiens type 2diabetic mellitus that admission on Stella Maris Hospital who examined kalium serum level. The data were colected from Medical Recordperiode January to December 2007. Data were analyzed with Fisher Exact test using SPSS versi 11.5. From 33 samples found normalkalium level 9 samples (27.3%) and decreased 6 samples (18.2%) and without insulin therapy found decresed 3 samples (9.1%) andnormal Kalium level 15 samples (45.5%). Comparison of Kalium serum level of type 2 DM patiens with or without insulin therapydid not statistically significant with P = 0.135. Kalium serum level of type 2 DM patiens with or without insulin therapy commonly inreference value.

Penyakit Diabetes melitus yang tidak dikelola dengan baik dapat menyebabkan komplikasi dan membahayakan kehidupan pengidapnya. Salah satu komplikasi diabetik yang sering terjadi adalah neuropati (kerusakan syaraf) di kaki yang menyebabkan ulkus kaki, infeksi, dan bahkan keharusan untuk amputasi kaki. Penelitian ini bertujuan untuk mengidentifikasi persentase penggunaan antibiotika empiris yang rasional serta pengaruhnya terhadap outcome terapi pada pasien Diabetes melitus dengan ulkus dan gangren di RSUD Dr Moewardi Surakarta dan RSUD Bagas Waras Klaten. Penelitian ini merupakan penelitian observasional dengan pengambilan data pada penelitian dilakukan secara retrospektif dan prospektif selama periode Januari sampai Agustus 2018. Ketidakrasionalan penggunaan antibiotika empiris dievaluasi dengan metode Gyssens. Sebanyak 36 pasien yang memenuhi kriteria inklusi 75% pasien mendapatkan terapi antibiotika empiris yang rasional dan 25% pasien mendapatkan terapi antibiotika empiris yang tidak rasional. Uji Chi Square dengan taraf kepercayaan 5% (p < 0,05) digunakan untuk membandingkan rasionalitas pengunaan antibiotika terhadap outcome terapi. Hasil Fisher’s Exact Test (Two Tailed) menunjukkan bahwa ada hubungan/pengaruh antara rasionalitas penggunaan antibiotika empiris dengan outcome terapi tetapi pemberian antibiotika empiris tidak berpengaruh pada angka leukosit bagi pasien, baik di RSUD Bagas Waras Klaten maupun RSUD dr. Moewardi Surakarta.Diabetes mellitus that is not managed properly can cause complications to endanger life. One of the diabetic complications that often occurs is foot neuropathy (nerve damage) inducing foot ulcers, infections, and even the necessity for a leg amputation. This study aims to determine the percentage of rational use of empirical antibiotics and their effects on outcomes therapy of diabetes mellitus patient with ulcers and gangrene in RSUD dr. Moewardi Hospital Surakarta and Bagas Waras Hospital Klaten. This study is an observational study with data collection in the study carried out retrospectively and prospectively from January to August 2018. The irrationality of using empirical antibiotics evaluated by the Gyssens method. Thirty six patients were enrolled and 75% of them treated with rational and empirical antibiotic therapy,. Chi-Square Test with a confidence level of 5% (p <0.05) used to compare the rationality of antibiotic use to the therapeutic outcome, the results of the Fisher Exact Test (Two-Tailed) indicated that there were relationship/influence between the rationality of using empirical antibiotics and outcomes therapy, but there was no influence between the rationality of using empirical antibiotics and leucocyte both in RSUD Bagas Waras Klaten and RSUD dr. Tuberculosis (TB) is considered as the leading killer among infectious diseases because of bacteria resulted in a high number ofmorbidity and mortality world wide. One risk factor for tuberculosisis diabetes mellitus (DM). The prevalence of TB and DM jump together globally, especially in developing countries, including Indonesia. Early and accurate diagnosis of TB cases in the enforcement is sputum smear examination. Monitoring of TB seen through sputum conversion. The purpose of this studyis to determine the duration of sputum conversion difference new case smear positive pulmonary TB patients with and without DM in hospital Prof. Dr.Margono Soekarjo (RSMS) Purwokerto. Thestudy design is observational analytic with cohort retrospective study. Sample of this study was patients who checked their health in Lung Clinis of RSMS Purwokerto obtained total sample of 44 people, consisting of 22 patients clinically diagnosed new cases smear-positive pulmonary TB with DM and 22 patients without DM in 2009 until 2012.Results of this study were analyzed using Fisher Exact Test. pvalues obtainedof 0.000 (p <0.05) (95% CI = 0,288-0,718) with relative risk 0,455. The conclusion is there were significant differences in sputum conversion time between the group of patients suffering from pulmonary uberculosis with and without DM. The conversion of sputum in patients with pulmonary tuberculosis who suffered DM is longer than patients without DM. Relative risk value which is equal to 0.455 which means that patients with DM have pulmonary tuberculosis risk by 0.455 times more likely to experience delays in sputum conversion than TB patients without DM
