Enterococcus faecalis is known to cause severe acute endophthalmitis and often leads to poor visual outcomes in most ophthalmic infections. This retrospective study is to report the clinical settings, antimicrobial susceptibility patterns, and visual outcome of E. faecalis endophthalmitis at a tertiary referral institution in Taoyuan, Taiwan. E. faecalis endophthalmitis was diagnosed in 37 eyes of 37 patients. Post-cataract surgery was the most common cause (n = 27, 73%), followed by bleb-associated (n = 3, 8%), endogenous (n = 2, 5%), corneal ulcer-related (n = 2, 5%), post-vitrectomy (n = 1, 3%), post-pterygium excision (n = 1, 3%), and trauma (n = 1, 3%). Visual acuities upon presentation ranged from counting fingers to no light perception. Pars plana vitrectomy with intravitreal antibiotics were performed in 23 eyes (76%) as primary or secondary treatment. All isolates (37/37, 100%) were sensitive to vancomycin, penicillin, ampicillin, and teicoplanin. Six of 22 eyes (27%) were resistant to high-level gentamicin (minimum inhibitory concentration > 500 mg/L). Final visual acuities were better than 20/400 in 11 eyes (30%), 5/200 to hand motions in 4 eyes (11%), and light perception to no light perception in 22 eyes (59%). Three eyes were treated with evisceration. Compared with non-cataract subgroups, the post-cataract subgroup showed a significant difference of better visual prognosis (p = 0.016).

AbstractStreptococcus pneumoniae endophthalmitis is clinically more severe, more difficult to treat, and carry a higher risk of vision loss, evisceration, or enucleation. This study is to investigate the clinical settings, antibiotic susceptibility, and visual outcomes of S. pneumoniae endophthalmitis at a tertiary referral center in Taiwan. S. pneumoniae endophthalmitis was diagnosed in 38 eyes of 38 patients. The main clinical features were postcataract endophthalmitis (n = 13, 34%) and endophthalmitis associated with corneal ulcer (n = 12, 32%), trauma (n = 6, 16%), endogenous etiology (n = 4, 11%), trabeculectomy (n = 2, 5%), and pterygium excision-related scleral ulcer (n = 1, 3%). Presenting visual acuity ranged from counting fingers to no light perception. Pars plana vitrectomy with intravitreal antibiotics was performed in 17 eyes (39%) in primary or secondary treatments. S. pneumoniae isolates were susceptible to vancomycin (38/38, 100%), penicillin (37/38, 97%), ceftriaxone (37/38, 97%), cefuroxime (12/15, 80%), levofloxacin (13/15 ,87%), and moxifloxacin (15/17, 88%). Final visual acuity was better than 20/400 in 3 of 38 eyes (8%), 5/200 to hand motions in 3 eyes (8%), and light perception to no light perception in 32 eyes (84%). Ten eyes (26%) underwent evisceration or enucleation. Although S. pneumoniae isolates were susceptible to vancomycin, S. pneumoniae endophthalmitis had a very poor visual prognosis.

Power sports have always been very popular among servicemen. This is especially evident in recent times, when such power sports, as armsport, crossfit, kettlebell lifting, powerlifting have received some development. The article presents a comparative analysis of the power capabilities of cadets who specialize in various power sports. The study involved male cadets of the 4th year of various faculties (n=53) of the Military Academy (Odessa), who in the process of training were engaged in power sports: armsport (n=12), crossfit (n=16), kettlebell lifting (n=15), powerlifting (n = 10). In addition, the results of cadets-athletes were compared with the results of cadets of the faculty of training specialists of airborne assault troops and military intelligence and special purpose, who did not attend sports sections of the academy (n=60). Analysis of the power of the cadets was carried out on the following exercises: pull-up, lifting by coup, lifting by strength on the crossbar, 24 kg kettlebell snatch, push-up on the parallel bars, push-up. The conducted researches testified to the high level of development of power qualities of cadets of all sections of academy in power sports. There was no significant difference between the results of cadets of different power sports in most exercises. It is established that the power capabilities of cadets who are engaged in various power sports are significantly better than those of cadets who are not engaged in sports. This suggests that any of the studied power sports can be an effective means of developing and improving the power opportunities of future officers of the Armed Forces of Ukraine

Sodium hypochlorite (NaOCl), an effective endodontic irrigant against Enterococcus faecalis (EF), is harmful to periapical tissues. Natural pineapple-orange eco-enzymes (M-EE) and papaya eco-enzyme (P-EE) could be potential alternatives. This study aimed to assess the antimicrobial efficacy of M-EE and P-EE at different concentrations and fermentation periods against EF, compared to 2.5% NaOCl. Fermented M-EE and P-EE (3 and 6 months) at various concentrations were mixed with EF in a 96-well plate incubated for 24 h anaerobically. Minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) of M-EE and P-EE were determined via EF growth observation. EF inhibition was quantitatively measured and compared between different irrigants using the one-way analysis of variance (ANOVA), and different fermentation periods using the independent-samples T-test. M-EE and P-EE showed MIC at 50% and MBC at 100% concentrations. There was no significant difference in antimicrobial effect when comparing M-EE and P-EE at 50% and 100% to 2.5% NaOCl. P-EE at 6 months fermentation exhibited higher EF inhibition compared to 3 months at concentrations of 25% (p = 0.017) and 0.78% (p = 0.009). The antimicrobial properties of M-EE and P-EE, at both 100% and 50% concentrations, are comparable to 2.5% NaOCl. They could therefore be potential alternative endodontic irrigants, but further studies are required.

<span>This study investigates whether indirect corrective feedback is effective o<span>n students’ writing <span>accuracy and whether there is any interaction between corrective feedback and students’ levels of <span>grammatical sensitivity. A quasi-factorial design was adopted for this research. The subjects of the <span>study were fourth-semester students of English Department, at a State University in Malang, selected <span>randomly. The experimental group was treated with indirect corrective feedback and the control <span>group with direct corrective feedback. A parametric statistical test, ANCOVA, was used to test the <span>hypotheses. The findings show that there was no statistical difference on writing accuracy between <span>the experimental and control groups. Yet, among students with a high level of grammatical <span>sensitivity, there was significant difference in writing accuracy between those given indirect and <span>direct corrective feedback. Further, there was no interaction between corrective feedback on writing <span>accuracy and students’ levels of grammatical sensitivity. Abstract Background: Enterococci are environmental pathogens that can cause bovine mastitis and macrolides are widely used for the treatment of bovine mastitis caused by staphylococci and streptococci/enterococci. The aim of this study was performed to compare the phenotypic and genotypic characteristics of high-level erythromycin-resistant (HLER) Enterococcus faecalis (E. faecalis) collected from bulk tank milk of four dairy companies (A, B, C, and D) in Korea. Results: Although isolates from company D showed the highest prevalence of E. faecalis, the prevalence of HLER E. faecalis in company A (73.1%) and C (57.0%) was significantly higher than company D (33.9%) (P < 0.05). A total of 149 HLER E. faecalis isolates showed high rates of resistance to tetracycline (93.3%), followed by doxycycline (70.0%), and chloramphenicol (48.3%). In the distribution of macrolides resistance genes, 147 (98.7%) isolates carried ermB gene alone, and two isolates carried both ermA and ermB genes. No isolates carried ermC, msrA, msrC, or mef genes. In the distribution of other resistance genes, 72 (48.3%) and 60 (40.3%) isolates carried both tetM and tetL genes, and tetM gene alone, respectively, and 38 (25.5%) isolates carried optrA gene. For aminoglycosides resistance genes, the prevalence of both aac(6′)Ie-aph(2″)-la and ant(6′)-Ia genes (43.0%) was the highest. Moreover, 104 (70.0%) isolates harbored Int-Tn gene carrying the Tn916/1545-like transposon. Although the distribution of ermB gene showed no significant difference between the dairy companies, the prevalence of other resistance genes and transposons showed a significant difference between the dairy companies (P < 0.05). Virulence genes, such as ace (99.3%), cad1 and efaA (each 98.7%), and gelE (83.9%), were also highly conserved in the 149 HLER E. faecalis isolates. Conclusions: Our results indicated that HLER E. faecalis isolates from bulk tank milk showed significant differences in phenotypic and genotypic characteristics between the dairy companies. In addition, the prevalence of resistance genes and virulence factors was also high in HLER E. faecalis isolates.

Objectives: The hospital can be saturated with noxious smells. Anecdotally, medical staff apply products to surgical masks to lessen the impact of these smells. This study aimed to determine the odour-masking ability of 4 inexpensive and convenient products. Methods: A randomized, single-blinded crossover study was conducted in Vancouver, Canada. Participants, 19 to 30 years old, were invited to participate. Participants with active allergies, upper respiratory tract infection, alteration to sense of smell, or failure of olfactory screen were excluded from the study. An experimental odour was used in lieu of a noxious surgical odour. After smelling the experimental odour without barriers, participants were re-exposed to the odour using 5 surgical masks in randomized order. Each mask was lined with a test product (cherry lip balm, tincture of benzoin, Mastisol, mint toothpaste, and control [plain mask]). Participants rated the effectiveness of products at masking the experimental odour from 0 to 100 (0 = completely ineffective, 100 = completely effective). Participants also rated the pleasantness of the products, recorded if the products made them feel unwell, and identified their preferred product overall. Results: Eighty participants were included in the study (33 male, 47 female), averaging 24.2 years of age. Mean odour-masking effectiveness for cherry lip balm was 66.5 (±24.6), tincture of benzoin: 62.6 (±25.0), Mastisol: 61.3 (±23.9), mint toothpaste: 57.5 (±27.4), and control: 21.9 (±21.8). All products performed better than the control ( P < .001), but there was no significant difference in performance between products. Cherry lip balm was the most preferred odour-masking product (29 participants), followed by mint toothpaste (22), Mastisol (14), tincture of benzoin (10), and control (5). Conclusions: All tested products demonstrated equivalent odour-masking abilities. If health care professionals choose to use an odour-masking product, they should consider their own olfactory preferences.

The present study examines the level of aspiration of students belonging to different streams of education and its impact on academic performance. The sample of the study consisted of 717 students (358 males and 358 females). The age range of the sample was 18 to 19 years. Tool used for measuring level of aspiration was Level of Aspiration Measure developed by Dr. Mahesh Bhargava and Prof. M A Shah. Goal Discrepancy Score is the difference between aspiration and the achievement on the same trial. The percentiles of marks obtained in the last qualifying exam were treated as an indicator of academic performance. For inferential purpose the data was treated with ANOVA and Scheffe’s test of Multiple Comparison. The findings of the study revealed that no significant difference exists between Academic Performance of students with high and low achievers belonging to Engineering and Management streams of education; however there exists significant differences between academic performances of students with high and low Level of Aspiration belonging to Law fraternity; with students showing low Level of Aspiration performing better than students with high Level of Aspiration. There exists no significant difference between academic performance of boys and girls with respect to Level of Aspiration in engineering stream. However, there exists significant difference between academic performance of boys and girls with respect to Level of Aspiration in Management and Legal streams of education. In both the streams girls outshine boys.

Background. Route navigation is a high-level skill and requires intact executive functioning to successfully find one’s way while driving in unfamiliar environments. Purpose. Driving performances were compared while navigating using electronic devices and printed directions on unfamiliar driving routes as well as in an interactive driving simulator. Method. Twenty-four participants drove two on-road routes using GPS and printed directions, and navigated using printed directions in the simulator, using a point system to evaluate performance. The two unfamiliar routes, order of simulator and on-road driving, and use of GPS and printed directions were counterbalanced. Paired t test were used to compare both GPS versus printed directions and performance between on-road driving and the simulator. Findings. Participants’ performance using GPS on the road was significantly better than with printed directions. There was no significant difference between performance in the simulator and on the road. Implications. Using GPS may be an effective strategy for improving safety. Using a driving simulator may be an efficient means of evaluating the strategic level of driving, executive function, and readiness to drive.

Purpose. To evaluate the effectiveness and safety of perfluorohexyloctane (F6H8) for intraoperative flattening of the retina and of F6H8/silicone oil (SO) 1000 cSt as a postoperative tamponade for inferior retinal detachment with inferior proliferative vitreoretinopathy.Methods. This is a retrospective review of 22 patients who underwent pars plana vitrectomy using F6H8 as an intraoperative tool to flatten the retina. At the end of the surgery a direct partial exchange between F6H8 and SO 1000 cSt was performed, tamponing the eye with different ratios of F6H8/SO (70/30, 60/40, 50/50, 40/30, and 30/70). Anatomical and functional results and complications were evaluated over the follow-up period (mean 22.63 months).Results. F6H8 was efficacious for intraoperative flattening of the retina. Twenty-one of the 22 patients achieved a complete retinal reattachment. Postoperative visual acuity (VA) ranged from light perception to 20/70, with 72% of patients obtaining VA better than 20/400. No emulsification/inflammation was observed whatever the ratio of F6H8/SO used. With higher ratios of F6H8/SO (70/30 and 60/40) cloudiness of the tamponade was observed. A transparent mixture was present with all the other ratios.Conclusions. The surgical technique adopted is very simple and safe. The optimal F6H8/SO ratio seems to be between 50/50 and 30/70.

Extended-spectrum β lactamases (ESBLs) continue to be a major challenge in clinical setups world over, conferring resistance to the expanded-spectrum cephalosporins. An attempt was made to study the prevalence of ESBL-producing Klebsiella pneumoniae clinical isolates in a tertiary care hospital in Kurnool. A total of hundred collected isolates of Klebsiella pneumoniae was studied for their susceptibility patterns to various antibiotics and detection of ESBL producers by double disc synergy test (DDST) and phenotypic confirmatory disc diffusion test (PCDDT). Of the 100 isolates tested for their antibiogram, 61% isolates have shown susceptibility to 3rd-generation cepholosporins and 39% were resistant. Amoxycillin showed the highest percentage of resistance followed by tetracyclins and cotrimoxazole. Among 39 resistant isolates of Klebsiella pneumoniae, 17 were ESBL producers detected by DDST and PCDDT. ESBL producers were more in the hospital isolates (28%) compared to community isolates (6%). Maximum percentage of ESBL producers were noticed from blood sample with 57.14%. In the present study, a large number of isolates were found to be multidrug resistant and ESBL producers. PCDDT was found to be better than DDST in the detection of ESBLs. Continued monitoring of drug resistance is necessary in clinical settings for proper disease management.
