Vitamin D deficiency has been associated with significant changes in dental structures. In children, it can induce enamel and dentin defects, which have been identified as risk factors for caries. This study aimed to assess the association between low serum 25-hydroxyvitamin D (25(OH) D) levels (<30 ng/mL) and the prevalence of caries in the permanent teeth and mixed dentition of 7-year-old children. A sample of 335 children from the population-based birth cohort Generation XXI (Porto, Portugal) was included. Data on children’s demographic and social conditions, health status, dental health behaviours, dental examination including erupted permanent first molars, and blood samples available for vitamin D analysis were collected. Dental outcomes included the presence of caries, including non-cavitated lesions (d1–6mft/D1–6MFT > 0), and advanced caries (d3–6mft/D3–6MF > 0). Serum 25(OH) D was measured using a competitive electrochemiluminescence immunoassay protein-binding assay. Bivariate analysis and multivariate logistic regression were used. Advanced caries in permanent teeth was significantly associated with children’s vitamin D levels <30 ng/mL, gastrointestinal disorders, higher daily intake of cariogenic food, and having had a dental appointment at ≤7 years old. Optimal childhood levels of vitamin D may be considered an additional preventive measure for dental caries in the permanent dentition.

The association between modifiable risk factors and caries in children has been documented; however, most studies have been cross-sectional and have not considered the complexity of dietary factors and oral health habits. This study aimed to investigate the prospective association between dietary patterns and oral health behaviours at 4 years of age and the development of new decayed, missing, and filled teeth (d3-6mft/D3-6MFT) over a period of three years. Participants were children from the Generation XXI population-based birth cohort. At 4 years of age, diet patterns were assessed using a food frequency questionnaire, with three dietary patterns being identified. For the purpose of capturing the new development of caries between 4 and 7 years of age, two dental outcomes were defined in the mixed dentition: “dental caries development” and “severe dental caries development” in the mixed dentition. Bivariate analysis and multivariate logistic regression were used. From 4 to 7 years of age, 51.2% of the children had at least one new d3-6mft/D3-6MFT and 27.4% had more than two new d3‑6mft/D3‑6MFT. Children belonging to the “energy-dense foods” (OR = 2.19; 95% CI: 1.20–4.00) and “snacking” (OR = 2.19; 95% CI: 1.41–3.41) dietary patterns at 4 years old were associated with severe dental caries development three years later. Preventive strategies should be implemented in an attempt to reduce snacking and the consumption of energy-dense, micronutrient-poor foods to promote children’s oral health.

Though the prevalence and the severity of dental caries is decreased in past few years, still children from low socioeconomical status in many industrialized or developing countries are still suffering from ill-effects of dental caries. The antimicrobial effect of silver compounds has been proven by the 100-year-old application of silver compounds. Silver diamine fluoride (SDF) has been used to arrest dental caries during 1970s in Japan, but it was not exposed much to other parts of the world. Today in many countries a 38% (44,800 ppm fluoride) SDF solution is commonly used to arrest caries and also to reduce hypersensitivity in primary and permanent teeth. Application of SDF to arrest dental caries is non-invasive procedure and is quick and simple to use. Reports of available studies showed that there is no severe pulpal damage after SDF application. However, it has some drawbacks like black discoloration of the carious teeth and an unpleasant metallic taste. But, low cost of SDF and its simplicity in application suggest that it is an appropriate agent for use in community dental health programme. Thus, SDF appears to meet the criteria of both the WHO Millennium Goals and the US Institute of Medicine’s criteria for 21st century medical care, that is, it is a safe, effective, efficient, and equitable caries preventive agent.

The Community Partnership Program (PKM) partners are SDN 96 Mannanti, Tellu Limpoe, Sinjai. The problem is: (1) The exchange rate of the rupiah against the US dollar is weakening which affects health costs (2) The high prevalence of dental caries which ranks fifth in the 10 most diseases (Data from the Mannanti Puskesmas Tellu Limpoe in 2017) (3). There is only one dentist in Mannanti village who only practices at the puskesmas. (4) Medical equipment especially dentistry tools at Mannanti puskesmas is limited. (5) Access to public transportation goes to Sinjai Hospital which is difficult and far enough (± 30 km distance and 60 minutes). (6) Public perception that dental examinations are expensive is also one of the reasons why someone is lazy to go to the dentist. The solutions offered from the PKM activities are: (a) Changing people's mindset from curative treatment shifting to promotive and preventive treatment so that the funds used are more minimum; (b) Implementation of SIKAPAMASE (SIkat gigi PAgi bersaMA di SEkolah) as an economical dental caries prevention program; (c) Application of topical fluor to students as an additional preventive measure to prevent cavities; (d) Guidance of teachers and parents as mediators of dental and oral health information to the community; (e) Healthy smile tooth ambassador competition for elementary students. The expected targets of the PKM activities are: (1) The caries status of SDN 96 Mannanti students reduced by 50% measured through the DMF-T index (def-t) measured through the results of pre-test and post-test activities; (2) Increasing students' knowledge about how to maintain dental health is measured through the results of pre-test and post-test activities; (3) Our program products are in the form of human resources, it is expected that SDN 96 students can be mediators of dental and oral health information in the family, school and home environment; (4) Caries preventive guidance booklet (5) Ambassador of healthy teeth. (6) SIKAPAMASE (SIkat gigi PAgi bersaMA di SEkolah) as an innovative program at the Mannanti health center that is carried out continuously.

Human saliva is a complex mixture of fluids considered to have a real potential in maintaining the dental health, carrying out important functions in the local modulation of inflammatory reactions and immune response. The biologic properties of salivary alpha-amylase are a subject of intense debate in the scientific literature, regarding its use as a biomarker for local and systemic diseases, including caries lesions. The purpose of our study was to measure the correlation between the salivary level of alpha-amylase and the incidence of caries in a group of children with young permanent teeth. A total of 128 participants were included in four study groups according to presence or absence of dental caries, based on specific inclusion/exclusion criteria and a scoring system between 0-5, depending on gender and the number of caries lesions. From each patient, 10 mL of saliva was collected and examined with a spectrophotometer in order to determine the level of salivary alpha-amylase. The collected data were statistically analyzed with the GraphPad Prism 7.03 and Mann-Whitneytest, a value of p [0.05 being considered statistically significant. Our results showed that caries active children had higher levels of salivary enzyme compared to caries free groups (p=0.001). Therefore, we concluded that salivary alpha-amylase can be considered a biomarker for the prognosis of dental caries development, offering new perspectives for preventive dentistry.

Objective: The purpose of this study was to assess the prevalence of dental caries, developmental defects of enamel, and related factors in children with clefts. Design: This cross-sectional prevalence study used standard dental indices for assessment. Setting: Children underwent a dental examination under standard conditions of seating and lighting in the outpatient department of a dental hospital as part of an ongoing audit to monitor clinical outcomes. Participants: Ninety-one children aged 4, 8, and 12 years were included in the study. Outcome Measurements Dental caries were assessed by use of the decayed, missing, and filled index for primary teeth (dmft); Decayed, Missing, and Filled index for permanent teeth (DMFT) according to the criteria as used in the national survey of children's dental health in the United Kingdom (O'Brien, 1994). Developmental defects were assessed using the modified Developmental Defects of Enamel Index (Clarkson and O'Mullane, 1989). Dental erosion was assessed using the criteria derived for the national survey of children's dental health (O'Brien, 1994). Results: Caries prevalence increased with age; 63% of patients at 4 years and 34% at 12 years were caries free. The mean dmft for the 4-year-olds was 1.3 with a mean DMFT for the 12-year-olds of 1.8. All the 4-year-olds had evidence of erosion of enamel in the primary teeth (incisors and first molars) and 56% of the 12-year-olds had erosion of permanent teeth (incisors and first permanent molars). Developmental defects of enamel became more prevalent with age, with at least one opacity in 56% of 4-year-olds and 100% of 12-year-olds. Hypoplasia was not found in the primary dentition but affected permanent teeth in 38% of 8-year-olds and 23% of the 12-year-olds. Conclusion: This study has shown that dental disease is prevalent in these patients. These assessments not only provide a baseline on oral health parameters in young people with clefts but underline the need for a more aggressive approach to prevention of oral disease to optimize clinical outcome.

The aim: A study of the impact of drinking water quality on the state of dental health in schoolchildren who permanently live in areas of biogeochemical deficiency of fluorine and iodine. Material and methods: The composition of the drinking water of the city of Uzhhorod and the city of Rakhiv has been investigated. The prevalence and intensity of dental caries in children is determined, drinking water is taken in accordance with Uzhhorod and Rakhiv. Results: The regional features of the composition of drinking water of the mountain Rakhivsky district of the Transcarpathian region are the high variety of combinations of mineral components and the frequent excess of the normative content of dry residue components, namely nitrates, sulfates, chlorides, magnesium and calcium.Water oxidation, total stiffness and total alkalinity were significantly different. The established drinking water quality indicators have a direct dependence on the prevalence and intensity of dental caries in children, that is, a significant increase in the number of carious temporary, carious and distant permanent teeth and a decrease in filled temporary and permanent teeth (p < 0. 05) were diagnosed. Conclusions: One of the most pressing problems in water hygiene is the urgent need to review and improve regulations, State sanitary regulations, standards and other regulations in the field of drinking water, including through the development of regional drinking water quality standards.

Small for gestational age (SGA) infants is one of the unfavorable outcomes of pregnancy that needs to be prevented. There are not many studies available on the association of maternal vitamin D levels during pregnancy and SGA infants. This study aimed to discover the association between maternal vitamin D level and SGA infants in West Java. A cohort study was performed from February 2018 to January 2019 by recruiting 304 pregnant women in Bandung, Cimahi, Waled, and Sukabumi, West Java Province, Indonesia. Of that number, 203 women were followed until the delivery date. Data were collected from the results of serial ultrasonography examinations and maternal vitamin D level measurements. Bivariate analysis and logistic regression were then performed to determine the relationship between variables. This study showed that 30 of 203 (14.78%) infants were born as SGA infants. The mothers of these SGA infants had a lower maternal vitamin D level at the beginning of the second trimester and smaller anthropometric measurements at the beginning of the third trimester. However, no significant difference was found between normal infants and SGA infants when the overall values for all trimesters were compared. It can be inferred that the maternal vitamin D level has a negative relationship with SGA infants in West Java, Indonesia. Further studies are required to prove the relationship between maternal vitamin Dl level and SGA infants.

The main problem of oral health of children is dental caries. Caries is a dental health problem incidence which is increasing. Factors that cause dental caries among gender, consumption of cariogenic foods/sweet, brushing teeth, dental care doctor. This study was conducted to determine the factors associated with the incidence of dental caries on toddlers in the region of Betunganhealth center. This study was an analytical study using case control design. The study population were all toddlers who came to visit the health center for dental treatment, with the sample size of 96 people, and the sampling technique used an accidental sampling. Data collection was done by direct interview using a questionnaire. Data was analyzed using univariate and bivariate analysis with chi-square test at α 5%. The results showed that most respondents (51.0%) frequently consume cariogenic foods, most respondents (55.2%) did not brush their teeth well, most respondents (58.3%) carried out dental care to the doctor was not effective, almost half respondents (47.9%) were female sex, and half of the respondents (50.0%) experienced dental caries. There was a relationship between the consumption of foods cariogenic, brushing and dental care to the doctor with the incidence of dental caries in BetunganHealth Center in Bengkulu(ρ = 0.000), and there was no relationship between gender and the incidence of dental caries in Betunganhealth center in the city of Bengkulu (ρ = 0.540). It is expected that health workers in the health center of Betungan can be a channel of information and give more intensive counseling and health promotion of the factors associated with the incidence of dental caries, as well as convey the prevention of dental caries.
