To address the low performance of health care service delivery in the half-market system, the Chinese government has begun to advocate the medical alliance (MA) recently. Instead of strict regulations on the procedure of diagnosis and treatment, flexible resource sharing among medical institutions of different grades inside each MA is encouraged. By now, many attempts have been made for MA establishment from different perspectives, but there is no effective model maturely developed. For the promotion of the spatial accessibility to medical services at different levels, it is important to organize the hierarchial medial services according to the distributions of different grades of health care facilities in a city. With the city proper of Tianjin as the study case, we explored the optimal establishment of MAs using the geographic information system (GIS). By means of the Voronoi Diagrams, the service regions of different medical institutions were precisely defined and the organizational structure of hierarchical medical services in MAs was determined. Through interpolation analysis, accessibility to different levels of medical services was measured, and on this basis, discussions were conducted on the service efficiency of the MAs. According to the results from Tianjin, (1) under the proposed organizational model for MAs, the fit of the service regions of the first grade and the other two higher grades of medical institutions was good. but the fit of the second and the third grade medical institutions was insufficient. (2) Although the overall service efficiency was excellent, there were still deficiencies in a number of the MAs. (3) Increasing the number of second and third grade medical institutions in specific regions near the city’s edge, as well as the number of first grade institutions, could further improve the performance of hierarchical medical services.

This article summarizes the arguments and counterarguments within the scientific discussion on improving the marketing policy of the clinic. The purpose of the study is to improve the marketing policy of the clinic. The urgency of solving the scientific problem lies in need to promote medical services, where marketing activities contribute to establishing relations between providers and consumers of medical services. The authors noted that patients have a low level of awareness in medical services, making it difficult to understand the market of medical services. In turn, public and municipal health care facilities have a passive marketing policy that is not competitive enough with private health care facilities. In most cases, state and municipal medical institutions do not use marketing tools to acquaint consumers with the list of available services, do not inform consumers about the expansion of medical services and the benefits of their medical institution. The object of the study was selected health care management. According to the results of empirical analysis of ways to improve the marketing policy of the clinic, it is established that the reform of the medical sector of Ukraine intensifies the processes of competition in the market. The marketing system of Ukrainian health care has no concept of medical care. Instead of that, there is a market element as medical service or health care service provided to the patient by a health care institution or an individual registered according to the law and possessing the medical business license. Besides, the medical services are paid for by the customer, such as patients, various organizations, local authorities, and the government. Thus, public medical institutions faced the need to fight (particularly with private medical institutions) for the consumer and state funding for the package of primary medical services. The study empirically confirms and theoretically proves the existence of competition between private and public medical institutions in the market of medical services. The results of this study can help improve the marketing policy of public and municipal clinics.

Two hundred and fifty five heads of households in Iyekuselu District, Bendel State Nigeria, were interviewed. Fifteen villages were randomly selected from the 107 villages that make up the district. There is high morbidity of infectious diseases identified in the study. Limited number of medical professionals and medical institutions present problems of availability of services. This is compounded by high cost of medical services and poor access to health care facilities. The self-perceived health care needs of the heads of households are disease prevention, availability of health services, improved accessibility to health care facilities and reduced cost of care. There is a strong need for health education programs in these villages. This should be attempted with the cooperation of community members, so as to attain the goal of promoting health and preventing diseases.

The article defines the scientific and terminological problems of researching services in the field of services, the basic problems of the evaluation of services in health care organizations. On the basis of a systematic approach to the category of quality of medical services the article investigates patient satisfaction with the perceived quality of service, shows the role of consumer expectations in the evaluation of the perceived quality of services and finds that the requirements for the service in medical institutions are underestimated. On the basis of studies the authors identify behaviors of consumers of budgetary medical services organizations, develop a model of consumer activities, which differs from the existing ones that along with the economic component the model is complemented with communication components. The approbation of the authors´ model for health care services shows a characteristic pattern of consumer activity of budgetary organizations. The article highlights the factors of subjective judgment of health care consumers in assessing perceived quality. Based on the study of patient satisfaction the authors develop a hierarchical model of the perceived quality of health services, as well as the place of services defined in the model.

Background: Social support refers to the assistance people receive from others, and it is divided into four types of support. Given the increasing mortality and morbidity rates of mothers and neonates postpartum, this study intended to determine whether the social support needs of the first-time mothers were met after early discharge from health care facilities.Objectives: The objective of the study was to explore the lived experiences and social support needs of the first-time mothers after an early discharge from health care facilities in the City of Tshwane, Gauteng.Method: A qualitative explorative study was conducted to explore the lived experiences and social support needs of the first-time mothers. The population were first-time mothers who had a vaginal delivery and were discharged within 6–12 hours of delivery from health care facilities. Purposive sampling was performed and 14 semi-structured interviews were conducted, with those mothers who came for the prescribed three postnatal check-ups at the three health care facilities identified according to maternity services provided. Saturation of data for the three health care facilities was reached at the 14th interview. Data analysis was performed using the hermeneutic interpretive approach.Results: Almost all participants had completed grades 11 or 12, but most were unemployed. The needs identified included the need for social support, lack of confidence, knowledge and skill to care for themselves and their newborn babies after early discharge.Conclusion: There is need to identify alternative types of social support for the first-time mothers, to ensure a normal adjustment to motherhood.

Medical service is a process of interaction between a doctor and a patient in order to improve health outcomes. The quality of medical services is influenced by various factors: a lack of equipment in the preventive health care facilities, pharmaceutical products supply, coordination of appointments of the patients, etc.). Accordingly, there appears a tendency to increase the demands for provision of medical services. The social significance of healthcare services renders the process of their provision as the category of mandatory measures for governmental control and regulation in all countries, with the purpose to provide access for all strata of the population and maintain the appropriate quality level. Therefore, the standards of medical services have become the main line of activity for many specialized international organizations and governments (personnel requirements, medical products and medicines, regulation of medical insurance, etc.). In turn, standardization creates significant entry barriers in the market of medical services. The need of implementing international and national standards for the provision of medical services and their convergence are stipulated by the specificity of the healthcare sector. International medical service trade, implementation of telemedicine technologies and innovative methods of treatment, standardization of production and consumption of medical services are modern trends that lead to the formation of the international market for medical services. The provision of medical services to patients of other countries in more than 50 countries is defined as one of the main directions of national development. Since the cost factor is an important determinant of competitiveness, patients from countries with highly developed health care system go to developing countries where they receive medical services of good quality at lower prices. In light of the above, the article analyzes the stages of development of the medical service market depending on the level of economic development, preconditions for developing competitive advantages and the global factors that shape the trends of development of the international market of medical services.

The mental state of the mentally ill is one of the most important factors in the treatment of the patient by a medical professional. But aggression, high-pitched communication, indifference of a doctor or health care worker can negatively affect the outcome of treatment and recovery of a patient with mental and behavioral disorders. The problem of modern medical deontology in Ukraine is the stigmatization of health workers to patients with mental disorders, which lead to self-stigmatization of both patients and their families. The purpose of the study is to enable the relatives of mentally ill people undergoing long-term inpatient treatment, to determine the quality of medical services in a psychoneurological hospital, to identify ethical and deontological shortcomings in the doctor’s relationship with relatives of mentally ill person, to determine the level of self-stigmatization in relatives of the patient by medical staff. The analysis of domestic and foreign scientific sources, bibliosemantic, analytical and statistical research methods were used in the work. A questionnaire was conducted (using a specially designed questionnaire) with elements of interviews of 92 respondents (relatives of mentally ill patients who were treated at a psychoneurological hospital in Vinnytsia). The results were processed using the licensed standardized package "Statistica 6.1". The need to comply with state building codes for health care facilities in accordance with European requirements has been proven. This will eliminate the feeling of hopelessness during hospitalization in public psychoneurological hospitals, which are a great trauma for relatives of patients. It has been shown that in order to prevent stigmatization of relatives of patients in psychoneurological hospitals, it is necessary to conduct trainings and interviews after training, which will determine the degree of stigmatization and subsequent preventive measures. It is shown that for relatives of mentally ill on the basis of psychoneurological hospitals it is necessary to equip rooms for psychodiagnostics and further psychocorrection. Development of affordable correctional programs for relatives of patients will reduce maladaptation and improve life satisfaction.

The graded treatment system refers to the gradation of diseases according to their priority and ease of treatment, with medical institutions at different levels undertaking the treatment of different diseases and gradually realizing the medical process from general practice to specialization. Since the “new medical reform”, China has been committed to promoting “primary care”, “two-way referral”, “separation of acute and slow treatment” The “new health care reform” has been implemented in China since the beginning of the reform. However, in the process of implementation, the effectiveness of the system has always been controversial due to the inadequate construction of primary medical institutions and the weak awareness of graded treatment among residents.

Purpose. Population assessment of the quality and effectiveness of medical services, as well as the activities of medical organizations working with the child population in comparison with 2016–2017.Materials and methods. Sociological research in polyclinics of four large health care institutions of Yakutsk of the com­pared groups is carried out: Polyclinic No. 1–100 respondents, City hospital No. 2–60 respondents, City hospital No. 3–80 respondents, medical center of Yakutsk — 100 respondents. The study was conducted in 2016–2017. during the study, 340 respondents were interviewed, including 87% of women and 13% of men (the survey was conducted among parents and other legal representatives of minor patients).Results. The article deals with the issues related to the implementation in the activities of public health institutions of the Republic of Sakha (Yakutia) quality standards and evaluation of the results of their implementation in terms of pa­tient awareness of the service (availability, openness of information), comfort conditions of service, waiting time for the service, the attitude of staff to patients (courtesy, goodwill), competence of staff and the overall level of satisfaction with the service provided. Despite the fact that more than 60% of respondents are aware of the functioning of the official websites of medical institutions, only 15.8% of respondents used information about the services and how to get them posted on them. At the same time, three-quarters of respondents are aware of the possibility of remote appointment by phone or Internet. However, only 18.6% of them took advantage of these opportunities, and 56% did not resort to them. The greatest criticism is caused by the lack of medical report on the patients’ state of health –60.8% of dissatisfied surveyed parents chose this criterion. More than 40% of complaints are due to the fact that pediatricians have not given recommendations for diagnosis and treatment. More than half of the survived participants (54.5%) at the recent visit to a medical institution, received a medical service on the day of making an appointment. The average waiting time to visit a paediatrician is 76 minutes. Two-thirds of respondents complain about the lack of available sitting chairs. Almost half of the respondents indicate the lack of drinking water in medical institutions, Almost 80% of respondents are generally satisfied with the medical services received in the clinics of Yakutsk. More than 70% of respondents are ready to recom­mend their clinic to friends, acquaintances and colleagues.Conclusion. According to the results of the study, it was concluded that the organizers of health care need to readjust appropriately some management procedures of the organizations, in particular: patients’ attendance by specialists, public relations, personnel management, facilities and resources, in connection with the visitor’s basic rebukes.
