The development of information interaction as a key element of state risk management and health protection

The article discusses ways to obtain information about risk factors and the health status of the population. The article describes a new information system «labor Medicine», which allows to organize the collection of a wide range of data for further analysis and application in the activities of various Executive authorities, public organizations, foundations, legal entities and citizens. It is proposed to improve this system by expanding the types of information collected, creating a passport for health promotion organizations, as well as integration with systems that are being implemented in the Russian Federation for managing the health of the working-age population in the context of state policy in the field of Informatization.

In the article studies modern communication technologies used in the Russian Federation for notarial acts. The main provisions of the issues for the development of modern information and telecommunication technologies for organizing notarial activities, within the framework of which the main stages of system construction, intermediate and final results, its targets, tasks, organizational structure and functions are determined. As a basis for providing all types of information interaction, the article presents the Unified Information System of the Notariat of Russia. The authors studying the novelties of the legislation on the application of information and telecommunication technologies for notarial acts, the peculiarities of electronic interaction with the state authorities of the Russian Federation. The conclusion is confirmed, that the Russian notary is ready to work in the new conditions, is focused on those requirements that dictate the modern information space.

The article discusses the issues of improving the examination system of competitive applications for state support in the form of grants of the President of the Russian Federation on the basis of an integrated information system that includes the information support system of the Grants Council of the President of the Russian Federation and the information system of the Federal Roster of Scientific and Technological Experts and containing information about experts, applications and expert examination results. In order to improve the principles of transparency and openness of support programs and competition winners, to ensure the objectivity of the competitive selection of projects, a number of organizational and technical solutions are proposed in the application examination system based on an integrated information system. The new and proposed new approaches to the organizational and technical support of the examination of competitive applications for state support in the form of grants of the President of the Russian Federation to young Russian scientists made it possible, by attracting a wide range of scientific and technological communities, to conduct examination of more than five thousand applications with high quality and deadlines set by the Ministry of Education and Science of Russia.

Introduction: In the context of a decreasing size of the working-age population, monitoring of the health status and disease incidence in this cohort shall be one of the most important tasks of public and occupational health professionals. Health risk management for the working population in the Russian Federation requires complete and reliable data on its morbidity, especially in view of the fact that its average age demonstrates a stable growth. It is, therefore, crucial to have precise and consistent information about the morbidity of the working-age population. Objective: The study aimed to assess incidence rates of diseases with temporary incapacity for work in the constituent entities of the Ural Federal District of the Russian Federation. Materials and methods: We reviewed data on disease incidence rates published by the Federal State Statistics Service in the Common Interdepartmental System of Statistical Information, Section 15.12, Causes of Temporary Disability, and Section 2.9.I.4, Federal Project for Public Health Promotion. The constituent entities under study were ranked according to the number of cases and days of temporary incapacity per 100 workers and E.L. Notkin scale was used to determine grade the incidence. The statistical analysis was performed using STATISTICA 10 software. Long-term average values of certain indicators, median values, standard deviation (σ) and coefficients of variation were estimated. The difference in the indices was assessed using the Mann-Whitney test. Results: Compared to 2010, incidence rates of diseases with temporary incapacity for work in the constituent entities of the Ural Federal District in 2019 demonstrated a significant decline. The sharp drop was observed in 2015. We also established that the Common Interdepartmental System of Statistical Information contains contradictory information on disease incidence. Conclusion: It is expedient to consider the issue of revising guidelines for organization of federal statistical monitoring of morbidity with temporary incapacity for work and to include this indicator in the system of public health monitoring.

Objective.To study the gender- and age-specific percentile distribution of carotid intima-media thickness (cIMT) in the unorganized urban working-age population.Material and Methods. Presented data were obtained in the ESSE-RF study in the city of Tomsk (1,412 participants, 25–64 years old without cardiovascular diseases, 59% women). All the surveyed signed voluntary informed consent form to participate in the study. We studied distributions of the mean and maximum cIMT obtained by the automatic and manual measurements, respectively. An error probability of less than 5% was considered statistically significant.Results. Both indicators of cIMT consistently increased with age in both gender groups. Maximum cIMT (max-cIMT) increased stronger than the mean cIMT (mean-cIMT). Compared with data obtained in other studies, the mean-cIMT estimates were distributed closer to the upper pole of the spectrum presented and increased stronger in 35–55-year-old men compared with those in the populations of Central and Southwestern Europe; the mean-cIMT estimates showed the most pronounced gender effect. Similar trends were identified in relation to the max-cIMT distribution.Conclusion. Obtained data allowed for specific assessment of the individual cIMT values by gender and age within the framework of risk stratification among people of working age without cardiovascular diseases. Further studies aimed at clarifying the prognostic role of high cIMT values in general population, taking into account the influence of traditional and new cardiovascular risk factors, can broaden the understanding of the significance of vascular state assessment as one of the key points, linking risk factors to clinical events, for primary prevention of cardiovascular diseases in population.

Syncope in a worker undertaking risky tasks may result in fatalities for the individual or for third parties. We aimed at assessing the rate of syncope recurrence and the risk factors underlying the likelihood of syncope relapse in a working-age population. A prospective cohort of all patients aged 18–65 years consecutively admitted to the Emergency Department for syncope was enrolled. Risk of syncope relapse was assessed at a six-month, 1-year, and 5-year follow-up. Predictors of syncope recurrence have been evaluated at six months and 1 year from the syncope index by a multivariable logistic regression analysis. 348 patients were enrolled. Risk of syncope relapse was 9.2% at 6 months, 11.8% at 1 year, and 23.4% at 5 years. At 6-month follow-up, predictor of syncope recurrence was ≥3 prior lifetime syncope episodes. At 1-year, ≥3 prior lifetime syncope episodes, diabetes mellitus, and anaemia were risk factors for syncope relapse. There was an exceeding risk of recurrence in the first 6 months and a reduced risk of 3.5% per year after the first year. Anaemia, diabetes mellitus, and prior lifetime syncope burden are of importance when giving advice about the resumption of “high risk” jobs following a syncope episode.

The article describes the concept of a new geographic information system for the provision of electronic state and municipal services. The problem of the lack of an open portal in the Internet for choosing unencumbered and suitable for use for the intended purpose land plots is considered. The importance of information systems in the development of the digital economy and information society in the Russian Federation is shown. The analysis of existing geographic information systems is given in a generalized and systematic way. As a result, it was found that the main drawback of the existing systems is the limitation of a certain territory and a set of services. It is concluded that it is necessary to display the free land. Automatic notifications should be sent to the competent authorities. The new information system should have direct and feedback links with all information and geographic information systems in operation and being developed in Russia. The results and conclusions are applicable to the creation of new geographic information systems.

Respiratory diseases (RD) represent one of the most urgent issues in Russian health care and have high socio-economic significance.The aim. To study the dynamics of total morbidity and mortality in the Russian Federation, as well as the mortality associated with RD in the working-age population in 2015 – 2019.Methods. The official statistical data of the Ministry of Health of the Russian Federation and the Federal State Statistics Service were analyzed.Results. In 2019, the total RD-associated morbidity increased by 5.4%, and the prevalence of pneumonia increased by 29.0% compared to 2015. In 5 federal districts (FD), the morbidity exceeded the average Russian morbidity in 2019 (40,694.7). The maximum level was observed in the North-Western FD (50,224.1). The prevalence of pneumonia (Russia – 524.4) in 4 FDs exceeded the average Russian prevalence. The maximum level was reported in the Far Eastern FD (749.2 cases per 100 thousand of the total population). The RD-associated mortality rate in Russia was 51.8 cases per 100 thousand in 2015 and 41.6 cases per 100 thousand in 2018 (–19.7%). In 2018, the highest RD-associated mortality was observed in the Siberian FD (68.0) and Far Eastern FD (57.8 per 100 thousand people). From January to December 2019, the highest mortality associated with pneumonia in the working-age population was observed in the Far Eastern FD (28.2 per 100 thousand people). The RD-associated mortality rate in the male population was 4.2 times higher than in the female population (26.7 and 6.3, respectively, per 100 thousand persons of matching age).Conclusion. The highest morbidity was found in 2018 and 2019 in the Northwestern FD and Far Eastern FD. The RD-associated mortality in the Siberian FD and Far Eastern FD exceeded the average Russian values. This last observation requires additional research to improve the quality of medical care.

The aim of this study was to evaluate the specific values of the indices that describe the spread of higher education institutions in all regions of Russia and the number of their students in the total working-age population living in these regions. The initial empirical data were the results of official statistical surveys conducted on information on the development of higher education, as well as the number of working -age population in eighty-two regions of the Russian Federation for 2020. In the course of the research, four mathematical models were developed. The study showed that on average, there are almost 14.8 higher education organizations per million working-age residents in the regions. It is proved that every twenty-fourth person of working age in 2020 studied under higher education programs. The conducted analysis showed the presence of a significant differentiation of the values of the considered indicators by region. The regions with the maximum and minimum values of the considered indicators were identified. It is shown that higher education has received significant development in Russia.
