Introduction: Cervical cancer is the second most common gynaecological cancer worldwide and the third leading cause of cancer-related mortality in the less developed countries. Cervical cancer may be prevented or diagnosed at early stages due to the easy accessibility and clinical evaluation of the cervix. A pap test is a simple, easy, painless procedure which can be carried out as an outpatient procedure to detect specic, nonspecic inammations, precancerous and cancerous lesions In our study we have analysed the results of PAP smears of women in all age groups and have tried to evaluate the prevalence of cervical lesions and the importance of cervical cancer screening. Material And Methods: Our study was conducted between October 2020 to JULY 2021 and patients who have visited the Gynaec OP during this period were included for this study. We received Pap smears from the Department of Obstetrics and Gynecology, stained the slides with Pap stain and studied the smears according to the new Bethesda System for Reporting Cervical Cytology 2014. Results: In our study, majority of the patients were in the age group of 41-50 years. Smears showing ASCUS were 14 , LSIL reports were 5, HSIL reports were 4. No Smears showed squamous cell carcinoma in our study. Reports in our study like many other studies has shown the importance of Pap smear test in screening cervical cancer. By conducting health camps, increasing health awareness and performing Pap smear screening programmes the incidence of cervical carcinoma can be decreased.

The field of cervical cancer prevention is rapidly evolving because of identification of the cause of disease. Human Papilloma Virus (HPV) has been recognized as a necessary cause of cervical cancer, and the association applies equally to both squamous cell and adenocarcinoma and is consistent worldwide. HPV 16 and 18 together attribute 70% of world's cervical cancer. HPV is highly prevalent among sexually active women and traceable in its natural history with technology that can detect HPV DMA at all stages of infection and neoplastic process. HPV DMA testing represents the scientifically obvious next step after Pap test for secondary prevention. A clinically validated and FDA approved HPV test has proven a substantial gain in sensitivity with a limited loss in speciality as compared to the standard Pap smear. We are now in the fortunate position of having two highly promising HPV vaccines in the pipeline. These vaccines today represent new hope for protection against cervical cancer, pre-cancerous cervical lesions and other HPV related condition for the current generation of adolescents, for the young and middle aged women as well as for the future generations. <p><strong>Background</strong>: Fiji has a high rate of cervical cancer, which is the second most common cause of cancer deaths in women in the country. Less than 10% of women are screened for cervical cancer in Fiji.In this paper we report the result of a study on Knowledge, Attitude, Practice and Barriers (KAPB) to cervical cancer and its screening with Pap smears, conducted on 1505 rural women in Fiji.</p><p><strong>Objectives:</strong> To assess the knowledge, attitudes, practice and barriers to cervical cancer and it’s screening with Pap smears in rural women of Ba, Lautoka and Nadi, in Fiji.</p><p><strong>Methods</strong>: Structured questionnaires were administered to women presenting to a rural outreach Reproductive Health education and clinics, by trained health educators to ascertain their Knowledge, Attitudes, Practice and Barriers to cervical cancer and its screening with Pap smears.</p><p><strong>Results</strong>: Seventy two percent of rural women had no knowledge of cervical cancer and 80% had no knowledge of the risk factors of cervical cancer. Lack of knowledge was significantly different for age groups (p=0.006), education levels and employment status(p&lt;0.001) and ethnicity  (p=0.022). Those groups with lowest knowledge were teenagers (18 to 19 years); those with less education; and iTaukei respondents. Of the respondents who had at least some knowledge of the Pap smear, 75% had had a Pap test. Of those who had no knowledge of the Pap smear, only 45% had had the test (p&lt;0.001). Of those who did not have a Pap smear the commonest barrier was lack of knowledge at 46.3% and fear of procedure was 29.4%.</p><p><strong>Conclusion</strong>: More education is required to acquaint women in rural Fiji about cervical cancer, its associated risk factors as well as the benefits of cervical cancer screening programs and other prevention strategies. </p>

Cervical cancer is the second most frequent type of cancer and leading cause of mortality among women worldwide. More than eighty eight percent deaths from cervical cancer occur in developing countries and by 2030, it will be at least ninety eight percent. In developed countries, the cases and deaths have declined markedly due to their extensive screening programs. The present study was undertaken to assess precancerous and cancerous cervical lesion by cytology and their comparison with histopathology in VIA positive cases. This descriptive, cross-sectional type of observational study was carried out in the Department of Pathology, Mymensingh Medical College in collaboration with the Department of Gynecology and Obstetrics, Mymensingh Medical College Hospital, during the period of July 2012 to June 2013. Study was carried out among 160 VIA positive patients and selected by non-random judgment sampling from the colposcopy clinic. Histological findings revealed that the most common age group affected by different types of cervical lesions is 30 – 39 years. It also revealed that squamous cell carcinoma (SCC) was very much common in age group 40-49 years. The statistical value of accuracy, sensitivity and specificity of Pap smear cytology test and histopathology yielded some important directives. The sensitivity values of Pap smear cytology was found 87.50%. The accuracy of the Pap smears in this study was 88.13%. The present study show significant relationship between cytological test with histopathological diagnosis. However cytology testing is not suitable as a single test. In conclusion, it can be stated that combination of cytology (Pap smear) and histopathology would ultimately be more useful. CBMJ 2015 July: Vol. 04 No. 02 P: 35-40

193 Background: Cervical cancer is the most prevalent gynecologic cancer worldwide, but is third in the U.S. due to pap smear screening. However, American Society for Colposcopy and Cervical Pathology (ASCCP) guidelines are complex and frequently changed, resulting in guideline adherence as low as 33-44%. We describe a centralized pap smear tracking system (“pap hub”) and report preliminary results. Methods: On 1/1/13 we implemented the pap hub, routing all screening pap smear results to a dedicated OB/Gyn nursing staff with a new Epic workbench. Epic Health Maintenance Modifier updated to recommend pap smears for women age 21-65 years (not younger or older per guidelines). Screening Pap/HPV results auto routed to Pap Pool Epic inbasket (not to individual providers). Centralized nurse reviews results alongside past pap results on Pathology flowsheet in Epic If normal Normal history -> appropriate follow-up interval per screening guidelines If abnormal history -> repeat pap as determined by previous history and treatment If abnormal -> manage per ASCCP guidelines Document plan in Epic Result Note Update Epic problems list If Normal: document “Pap Hub Normal History” If Abnormal: document “Pap Hub Abnormal History” with specifics in Overview History Communicate results/plan to the patient. Update Health Maintenance Modifier with next step. Review abnormal results to ensure proper follow-up, with reminders as indicated. Results: Guideline recommendations against pap smears for patients <21 years and >65 years have not changed since 2009. Comparison of first-quarter data for 2010 (pre-Pap Hub) to 2013 (post-Pap Hub) have shown a 63.86% reduction in pap smears in women <21 years. Pap smears in women >65 years has always been low, but has decreased slightly from 2.4% of all pap smears in 2010 to 1.5% of all pap smears in 2013. The 2012 ASCCP guidelines have changed recommended pap smear frequency and abnormal pap smear follow-up, and compliance with these new recommendations is being collected. Conclusions: Pap smear screening has decreased the incidence of invasive cervical cancer in the United States. The Pap Hub, a centralized pap smear tracking system, improves compliance with pap smear screening guidelines.

Background: The case burden of cervical cancer has been increasing globally especially in developing countries without proper health system. Cervical cancer can be eliminated with timely vaccination and screening program as it usually takes years for pre-malignant lesions to develop into malignant lesion. Bhutan has committed to eliminate cervical cancer. Thus, it is important to understand the factors associated with abnormal Pap test findings. Methodology: A retrospective study was conducted using the Pap smear data for the year 2018. It was extracted from the records maintained in the cytology unit of Samtse General Hospital. Result: The abnormal slide rate in this study was 2.5%. The majority of women seeking Pap smear services were women in reproductive age group and housewife by occupation. There were significant differences between age groups and marital status among normal in the Pap test results. Conclusion: The slide abnormality of Pap smear in Samtse District is low. The abnormality is more common among married and older women. Therefore, additional screening efforts needs to be put into this group to detect pre-malignant lesions.

Background: "Preventable but not prevented", this is the reality of cervical cancer today, at least in developing countries. 80% of all the cases of cervical cancer occur in these developing countries. In India, cervical cancer is the leading cause of cancer related deaths in women. The Papanicolaou test is simple, quick, and painless. It is capable of detecting cervical cancer at an early stage and is used widely in developed countries, where it has decreased both the incidence and mortality of cervical cancer. Objective of the study was conducted to determine the importance of conventional Pap smears for the diagnosis of inflammatory, premalignant and malignant lesions of the cervix.Methods: This cross-sectional study was conducted from January 2017 to June 2018 on 2000 women coming for a Pap smear examination in Government Medical College attached to New Civil Hospital, Surat. After doing Pap stain, all cases were reported as per the 2014 Bethesda system.Results: A total of 2000 cases of Pap smears were received out of which 1914(95.7%) cases were satisfactory for evaluation, 86(4.3%) cases were unsatisfactory. The frequency of epithelial abnormalities was 3.66%. The age group of 51-60 years showed the highest epithelial abnormalities. The frequency of epithelial abnormalities in asymptomatic women was 2.3%. Out of the 1844 cases, 103(5.6%) showed changes of atrophy, 1011(54.8%) cases showed inflammatory/reactive changes, whereas organisms were seen in 558(30.3%) cases.Conclusions: Pap smear happens to be an economical, safe and pragmatic diagnostic tool for early detection of cervical cancer.

Background: Early detection of cervical cancer is possible with Pap smear tests. The proportion of women who undergo Pap smear testing ranges from 68% to 84% in developed countries as compared to India where the rates range from 2.6% to 6.9% among women in communities. This study is to assess the knowledge of women about cervical cancer, its screening, role of doctor and source of information.Methods: Cross-sectional study – observational study of Women attending gynecology camps/op clinics held in rural areas of Ernakulam and Thrissur who are between 25 and 65 years of age Non-random sampling with Sequential inclusion of the women who met the study criteria. A questionnaire devised collecting Basic sociodemographic profile like age, parity, educational qualification, Questions to assess knowledge about symptoms of cervical cancer Questions to assess the knowledge about cervical cancer screening and prevention doctor as Source of knowledge from the subjects. Correct response for questions carried score of 1 mark. So, the maximum was 6 and minimum was 0. The knowledge was graded as: <2 being poor knowledge; 2-5 being satisfactory knowledge and ≥5 being good knowledge.Results: 32.9% of women identified discharge per vaginum as the most common symptom. When 50.9% thought multiparity was a risk factor 30% identified early sexual activity. 2.3% women knew that pas smear screening has to be done repeatedly. 8.3% of participants already had screening done. Only 8.3%of doctors advised pap screening during their consultations. Uptake of pap smear after awareness program was 100%.Conclusions: Awareness classes are needed for increasing the coverage for pap smear screening. The women should be educated about cervical cancer, its screening methods and the facilities available for screening in the hospitals. An Information Leaflet has to be provided to these women in the local language. Training programs should be conducted for general practitioners and gynecologists to conduct awareness classes for the public and also to motivate women to undergo screening at every possible opportunity.

Background : Poor genital hygiene has been responsible for high prevalence of excessive vaginal discharge. Leucorrhoea is the clinical evidence of infection and can be treated satisfactorily whenever diagnosed. Occurrence of various cervical epithelial lesion associated with discharge can be easily prevented if detected early. Majority of the patients with various cervical epithelial lesion attending in the hospital present with varying degree of vaginal discharge and cytological cellular aberrations in the cervical epithelium.Objective : The aim of the present study is to observe the various patterns of cervical epithelial lesions in cytopathology and associated infections in patients with Leucorrhoea.Methodology : This observational study was done over a period of ten months in Popular Medical college hospitals of Bangladesh. A total of 230 cases were included in the study with women of age range 15-45 years, complaining of leucorrhoea. After a thorough vaginal examination Pap smears were taken and immediately fixed in absolute alcohol and stained according to the papanicolaous technique. The cytopathological changes observed in the cervical smears were graded according to the Bethesda system for reporting cervical cytology.Results : Out of 230 cases the cytological patterns were found Normal in 4 (1.7%), Inflammatory 204 (88.3%), Low grade squamous intraepithelial (LSIL)10 (4.8%), High grade squamous intraepithelial (HSIL) 6(2.6%), Atypical squamous cells of undetermined significance (ASCUS) 6 (2.6%) and the distribution of different types of pathogens detected were Trichomonas 33 (70.62%), Candida albicans 19 (40.66%).Conclusion : In this study significant numbers of Leucorrhoea patients were detected with cervical squamous intraepithelial lesions (SIL) in which LSIL (Low grade squamous intraepithelial) was higher. Associated infections were Trichomonas and Candida albicans. Therefore cytological screening for early detection of cervical squamous intraepithelial lesion is essential routine medical examination for Leucorrhoea patients in gynaecological practice.Northern International Medical College Journal Vol.9(1) July 2017: 252-254

Background: Limited information is provided on the quantitative cytomorphometric study of the cervical Pap test. The cervical Pap test is an important screening program for cervical cancer. A quantitative cytomorphometric examination of cervical Pap is used to accurately identify precancerous and cancerous lesions early and to reduce the occurrence and avoidance of invasive cancer. This study was aimed to assess the cytomorphological parameters (nuclear diameter [ND], cytoplasm diameter [CD], and nuclear-to-cytoplasmic ratio [N/C ratio]) of squamous epithelial cells from a cervical Pap smear. Methods and Results: A prospective study was performed on 142 consecutive cervical Pap smears from women with gynecological clinical complaints. The ND and CD were determined by the Optika optical microscope camera using a digitizer cursor in both axes. The final images were taken with an X40 magnification. The ND, CD, and the N/C ratio were then measured and expressed in micrometers. The women were classified into 5 age groups: 5(3.5%) in the age group of <19 years, 46(32%) in the 20-29 group, 67(47.2%) in the 30-39 group, 23(16.2%) in the 40-49 group, and 1(0.7%) woman was over age 50. There were no significant differences in the N/C ratio of superficial cells between age groups. The ND, CD, and the N/C ratio were significantly higher in women with clinical complaints than in women without clinical complaints Conclusion: Cytomorphometic analysis might assist in the identification of cellular alterations due to gynecological diseases and increase the sensitivity and accuracy of the Pap smear technique.

Medical genetics plays an important role in the screening and prevention of numerous diseases. Thus, it is important to develop effective screening and prevention programs and improve the assessment of the susceptibility of diseases. The development of screening and prevention programs depends on the identification of early biomarkers (including functional and behavioral) for the risk and onset of the disease, and such programs need to be designed according to internationally accepted criteria. Cervical cancer represents a very relevant disease from the health and social perspective; around 528,000 new cases are diagnosed every year globally, of which, 85% are from developing countries, representing almost 12% of all cancers in females. Substantial reductions in the incidence of and mortality from cervical cancer have been observed after the introduction of prevention campaigns with the implementation of cervical screening programs through Papanicolaou (Pap) tests and, in particular, following the introduction of organized programs which guarantee a high level of screening coverage, as well as, the quality and continuity of diagnostic-therapeutic procedures. It is estimated that Pap smear screening every 3-5 years provides 80% protection against the onset of cancer. Advances in diagnostic techniques, particularly the development of easy-to-use molecular genetic tests, are replacing the use of the established Pap smear as a screening tool. This is possible owing to the discovery in 1975 that some cellular morphological changes (koilocytosis) were related to the presence of a Human Papillomavirus (HPV) infection. The HPV test is performed on a small sample of cells taken from the cervix, similar to the Pap test; however, it is not a morphological exam but a molecular biology exam that detects the presence of HPV by identifying its deoxyribonucleic acid (DNA) or messenger ribonucleic acid (mRNA). The results of numerous experimental studies have demonstrated a greater sensitivity of this test compared to the sensitivity of the traditional Pap test. However, the HPV test has a lower specificity due to two main factors: 1) The HPV test is based on the search for the types of viruses that have a greater oncogenic potential, and 2) It does not discriminate between transient infections and persistent and productive infections. The most widely used molecular tests are based on the search for HPV sequences and genotyping using molecular biology techniques, such as direct hybridization, qualitative polymerase chain reaction (PCR), and viral nucleotide sequencing.
