The ongoing pandemic of COVID-19 acute respiratory illness has affected the world in unimaginable manner. The treatment of moderate to severe COVID-19 Acute Respiratory Distress Syndrome (ARDS) includes use of steroids as proven therapy to reduce mortality.(1) The observed severity and mortality of COVID-19 is more in patients with co-morbidities and elderly age. Diabetes mellitus being the one of the most commonly associated comorbidities with poor outcome.(2) The stress response to the disease process as well as the use of steroids is causing a state of hyperglycemia due to insulin resistance, for which vigilant monitoring and insulin is needed.(3)

<p>Clinical judgement and suspicion of influenza based on signs and symptoms of influenza-like illness and severe acute respiratory illness are critical for better patient outcome. Whether clinical characteristics of patients are associated with the development of acute respiratory distress syndrome and PCR positivity of samples was the aim of this study. We included all patients (n=37) presenting with influenza like illness (ILI) or severe acute respiratory illness (SARI) to a tertiary care hospital in northwest Pakistan during December 2015 until the end of January 2016. Each patient was assessed for signs and symptoms, clinical features, treatment, complications and outcome of ILI and SARI. Throat or nasopharyngeal swabs were obtained from 36 patient and analyzed for the presence of Influenza virus by quantitative PCR.<strong><em> </em></strong>Patients presenting with ILI or SARI were febrile (p&lt;0.001, one sample <em>t-</em>test), significantly tachypneic (p&lt;0.001) and had critically lower oxygen saturation (p&lt;0.001). Nasal congestion at presentation (p=0.006, chi-square test for association) and infiltrates on chest radiographs (p=0.025) were significantly associated with acute respiratory distress syndrome. Likelihood of the occurrence of ARDS was significantly increased with decrease in oxygen saturation (Odds ratio; 0.75, 95% CI; 0.46, 1.21, p=0.048) and marginally significantly increased in lower age (Odds ratio; 0.82, 95% CI; 0.58, 1.15, p=0.055) and higher white cell count (Odds ratio; 1.001, 95% CI; 0.99, 1.002, p=0.054). The presence of Influenza type A/H1N1pdm09 strains was confirmed in 7/11 patients. However no significant difference was observed in the clinical features and complications of PCR positive and negative patients. Clinical signs and symptoms of influenza-like illness or severe acute respiratory illness significantly predict the development of complications irrespective of the positivity or negativity of laboratory qPCR reports.</p>

AbstractIn late 2019, China reported cases of respiratory illness in humans, which involved a novel Coronavirus SARS-CoV-2 (also known as 2019-nCoV). The World Health Organization (WHO) termed the disease COVID-19 (i.e., Coronavirus disease 2019). Most of the morbidity and mortality from COVID-19 is largely due to acute viral pneumonitis that leads to acute respiratory distress syndrome (ARDS). This article will discuss the clinical features of the multiorgan involvement in COVID-19 as well as the management of patients who become critically ill due to COVID-19.

In March 2020, the World Health Organization (1) declared COVID-19 as a pandemic and a threat to global public health (2). The virus mainly affects the lungs and can cause acute respiratory distress syndrome (ARDS). In addition, coronavirus 2 severe acute respiratory syndrome (SARSCOV2) also has devastating effects on other important organs, including the circulatory system, brain, gastrointestinal tract, kidneys and liver


The outbreak of emerging severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) disease (COVID-19) in China has been brought to global attention and declared a pandemic by the World Health Organization (WHO) on March 11, 2020. In a recent study of Nanshan Chen et al., on patients of Wuhan Jinyintan Hospital, Wuhan, China, from the 99 patients with SARSCoV-2 infection, 51% had chronic diseases and they had symptoms of fever (83%), cough (82%) shortness of breath (31%), muscle ache (11%), fatigue (9%), headache (8%), sore throat (5%), rhinorrhea (4%), chest pain (2%), diarrhea (2%), and nausea and vomiting (1%) [1, 2]. The majority of patients can recover, however, about 25% of patients will progress into severe complications including acute respiratory distress syndrome (ARDS), which may worsen rapidly into respiratory failure, need an intensive care unit (ICU) and even cause multiple organ failure [3]. Depending on the pathophysiological mechanisms supposed to be involved in the development of the various clinical forms of the disease, various types of treatment have been tested with varying degrees of success. We have developed a nanotherapy to block the entry of the virus into the host cell, to reduce its potential for replication and to regulate the immune response against the microbial aggressor [4].

Coronavirus disease 2019 (COVID-19) was first declared in December 2019 from Wuhan, China [1, 2]. It then has been reported a pandemic in March 2020 by the World Health Organization [3]. Clinical features of COVID-19 are different from asymptomatic to mild to moderate symptoms, such as fever, headache, myalgia, sore throat, anosmia, cough, fatigue headache, hemoptysis, and dyspnea to the life-threatening complications, including shock, pneumonia, acute respiratory distress syndrome, myocarditis, myocardial infarction, acute kidney injury, multi-organ failure, and even death [1, 2].

Modern ventilatory strategies are based on the assumption that lung terminal airspaces act as isotropic balloons that progressively accommodate gas. Phase contrast synchrotron radiation computed tomography (PCSRCT) has recently challenged this concept, showing that in healthy lungs, deflation mechanisms are based on the sequential de-recruitment of airspaces. Using PCSRCT scans in an animal model of acute respiratory distress syndrome (ARDS), this study examined whether the numerosity (ASnum) and dimension (ASdim) of lung airspaces change during a deflation maneuver at decreasing levels of positive end-expiratory pressure (PEEP) at 12, 9, 6, 3, and 0 cmH2O. Deflation was associated with significant reduction of ASdim both in the whole lung section (passing from from 13.1 ± 2.0 at PEEP 12 to 7.6 ± 4.2 voxels at PEEP 0) and in single concentric regions of interest (ROIs). However, the regression between applied PEEP and ASnum was significant in the whole slice (ranging from 188 ± 52 at PEEP 12 to 146.4 ± 96.7 at PEEP 0) but not in the single ROIs. This mechanism of deflation in which reduction of ASdim is predominant, differs from the one observed in healthy conditions, suggesting that the peculiar alveolar micromechanics of ARDS might play a role in the deflation process.

Indonesia contributes to 45% of TB cases in the world. Currently, Diabetes mellitus (DM) case is increasing. DM may worsen TB. The risk of developing latent into active TB increases with the presence of DM. Treatment failure in TB patients with DM is more frequent than non-DM. The aim of this study was to analyze of TB DM collaboration program at the one hospital in Semarang City. This was a qualitative study with content analysis. Informants consisted of internists, nurses, patients. This study showed there was a gap between technical guidance from minister of health and implementation of DM screening from TB patient. Besides, human resources factors also contribute to that gap.

A better understanding of the pathogenesis and the resolution of the acute respiratory distress syndrome (ARDS) is needed. Although some progress has been made with the use of protein biomarkers and candidate gene studies in understanding the pathobiology of ARDS, we propose that new studies that measure the chemical breakdown products of cellular metabolism (metabolomics) may provide new insights into ARDS, in part because metabolomics targets a later point in the genomics cascade than is possible with studies of DNA, RNA, and protein biomarkers. Technological advances have made large-scale metabolomic profiling increasingly feasible. Metabolomic approaches have already achieved novel insights in nonpulmonary diseases such as diabetes mellitus and malignancy, as well as in sepsis, a major risk factor for developing ARDS. Metabolomic profiling is a promising approach to identify novel pathways in both patients at risk for developing ARDS as well as in the early phase of established ARDS.

Type 2 diabetes mellitus is a violation of carbohydrate metabolism caused by insulin resistance and relative insulin insufficiency or a violation of insulin secretion with or without insulin resistance. The number of people with diabetes increased from 108 million in 1980 to 422 million in 2014. The World Health Organization (WHO) predicts that by 2030 this number will increase to 439 million, which is almost 10% of the adult population. A dentist is often the first to encounter manifestations of diabetes mellitus in the oral cavity, such as caries, candidiasis, dryness, inflammatory periodontal diseases, etc. Therefore, timely diagnosis and close interaction of doctors of different specialties is crucial for the treatment of this disease.
