Family Needs of Patients Admitted to Critical Care Unit (CCU) Abstract The growing threat of patients’ death will impact on their family’s worry and stress which will negatively affect the role of the family as a part of the support system for the patient. The purpose of this study is to determine family needs of patients admitted to CCU of Arifin Achmad General Hospital of Riau Province. This is a quantitative study using descriptive design with 21 respondents chosen using consecutive sampling technique. The data were collected using questionnaire and then analyzed using univariate analysis. The study found that, regarding the need for service guarantees, 85.7% respondents stated that it was very important to understand the purpose of the medication and health treatment given to the patient; regarding the need for information, 66.7% respondents stated that it was very important to know the real condition of the patient; regarding the need for mental support from the family, 71.4% respondents stated that it was very important to have support from the family; regarding the need for convenience, 42.9% respondents stated that it was important and very important that they were provided with washroom close to the waiting room as well as the waiting room close to the intensive room; and regarding the need for closeness, 38.1% respondents stated that it was very important to be able to visit the patient at CCU with one member of the family. It can be concluded that the need for understanding the purpose of the medication and health treatment given to the patient is the one that patients’ family need the most. It is suggested that the nurses prioritize providing nursing care for the most needed family needs. Key words: CCU, Family needs.   Kebutuhan Keluarga Pasien Critical Care Unit (CCU) Abstrak Besarnya ancaman kematian pasien akan memiliki dampak kecemasan dan stres keluarga dan berdampak pada peran keluarga sebagai support system menjadi tidak maksimal. Penelitian ini bertujuan untuk menggambarkan kebutuhan keluarga pasien CCU di RSUD Arifin Achmad Provinsi Riau. Jenis penelitian ini adalah kuantitatif dengan desain deskriptif,  pengambilan sampel menggunakan teknik consecutive sampling, jumlah sampel dalam penelitian ini sebanyak 21 responden. Alat pengumpulan data menggunakan kuesioner dan analisis data dilakukan secara univariat. Hasil penelitian didapatkan pada kebutuhan jaminan pelayanan 85.7% responden menyatakan sangat penting untuk mengetahui tujuan pengobatan dan tindakan perawatan yang diberikan, pada kebutuhan informasi 66.7% responden menyatakan sangat penting mengetahui kondisi pasien yang sebenarnya, pada kebutuhan dukungan mental keluarga 71.4% responden menyatakan sangat penting mendapatkan dukungan antar keluarga, pada kebutuhan kenyamanan 42.9% responden menyatakan penting dan sangat penting untuk tersedia kamar mandi dekat ruang tunggu dan tersedia ruang tunggu didekat ruang intensif, serta pada kebutuhan kedekatan 38.1% responden menyatakan sangat penting untuk mengunjungi pasien diruang intensif (CCU) bersama salah satu kerabat. Dapat disimpulkan bahwa kebutuhan mengetahui tujuan pengobatan dan tindakan perawatan yang diberikan merupakan kebutuhan yang paling dibutuhkan keluarga. Kepada perawat disarankan untuk memprioritaskan asuhan keperawatan terhadap kebutuhan keluarga yang paling dibutuhkan.   Kata kunci : CCU , Kebutuhan keluarga.

ABSTRAKKeluarga dengan anak penderita penyakit kronis membutuhkan dukungan baik secara moril maupun spiritual. Dukungan akan kebutuhan spiritual tidak jarang dianggap hal yang kurang penting. Keluarga melaporkan belum terpenuhinya kebutuhan spiritual selama menunggu anak di rumah sakit. Tujuan dari penelitian ini adalah untuk mengetahui gambaran kebutuhan spiritual manakah yang paling dibutuhkan keluarga dengan anak penderita penyakit kronis di ruang rawat inap anak RS Al Islam Bandung. Penelitian ini menggunakan deskriptif kuantitatif. Jumlah sampel 39 responden dengan teknik purposive sampling dengan kuesioner yang kembangkan dari konsep kebutuhan spiritual keluarga menurut Ruth A. Tanyi dengan nilai uji validitas 0,33-1 dan nilai reliabilitas 0,93. Analisis menggunakan analisis statistic deskriptif yang menghasilkan distribusi frekuensi serta persentase masing-masing dimensi kebutuhan. Hasil penelitian ini menunjukan bahwa dimensi dengan kebutuhan tertinggi adalah kebutuhan terhadap keyakinan (57,4%), diikuti oleh kebutuhan terhadap kekuatan (57,1%), kebutuhan terhadap family’s preference (52,3%), kebutuhan terhadap spiritual anggota keluarga (41%), kebutuhan terhadap makna dan tujuan (39%), dan kebutuhan terhadap hubungan (37,8%). Penelitian ini menunjukan bahwa dimensi kebutuhan terhadap keyakinan merupakan dimensi kebutuhan spiritual keluarga yang dirasa paling utama oleh responden. Berdasarkan hasil penelitian tersebut disarankan agar perawat dapat meningkatkan pelayanan tentang asuhan keperawatan spiritual dengan pengembangan protap dengan memasukan enam dimensi ke dalam protab yang ada, disediakan ruang tunggu yang tenang untuk keluarga dalam beribadah, adanya konseling antara perawat dan keluarga, dan menyediakan bacaan-bacaan tentang kebutuhan spiritual keluarga. ABSTRACTFamilies with children with chronic illness need support both morally and spiritually. Support for spiritual needs is not uncommonly perceived as less important. The family reported not having fulfilled the spiritual needs while waiting for the child in the hospital. The aim of this research is to know the description of spiritual needs which is most needed family with children suffering from chronic illness in the inpatient room of RS Al Islam Hospital Bandung. This research used quantitative descriptive. Total sample 39 respondents with purposive sampling technique with a questionnaire developed from the concept of spiritual family needs according to Ruth A. Tanyi with validity test value 0,33-1 and reliability value 0,93. The analysis used descriptive statistic analysis which produces frequency distribution and percentage of each need dimension. The results of this study indicate that the dimension with the highest need is the need for confidence (57.4%), followed by the need for strength (57.1%), the need for family's preference (52.3%), the need for spiritual family members (41 %), Need for meaning and purpose (39%), and need for relationship (37.8%). This study showed that the dimension of need to belief is a dimension of the spiritual needs of families that are considered most important by the respondents. Based on the results of this study it is suggested that nurses can improve the service of spiritual nursing care with the development of protap by including six dimensions into the existing protab, provided a quiet waiting room for families in worship, counseling between nurses and families, and provide readings about The spiritual needs of the family.

Skizofrenia merupakan suatu gangguan mental yang menyebabkan timbulnya pikiran, persepsi, emosi, gerakan, perilaku yang aneh dan terganggu.Penderita skizofrenia 70% diantaranya mengalami halusinasi.Dampak terberat yang dirasakan oleh keluarga dalam merawat pasien dengan halusinasi adalah dampak pada psikologis, terutama stress atau beban yang dirasakan oleh keluarga. Tujuan penelitian ini adalah untuk mengetahui Hubungan Beban Keluarga dengan Kemampuan Keluarga Dalam Merawat Pasien Halusinasi di InstalasI Rawat Jalan Rumah Sakit Jiwa Provinsi Jawa Barat. Metode penelitian ini menggunakan deskriptif korelasional dengan tehnik purposive samplingterhadap 69 responden dari populasi 996 responden di Instalasi Rawat Jalan Rumah Sakit Jiwa Provinsi Jawa Barat. instrumen yang digunakan dalam penelitian ini adalah kuesioner beban keluarga dan kemampuan keluarga yang terdiri dari kognitif dan psikomotor. Analisa univariat dalam penelitian ini menggunakan distribusi frekuensi dan analisa bivariat pada penelitian ini menggunakan uji chi square  hasil penelitian menunjukan beban berat yaitu 35 responden (50,7 %), sedangkan untuk kemampuan keluarga baik 46 responden (42 %). kesimpulannya bahwa ada hubungan antara beban keluarga dengan kemampuan keluarga dalam merawat pasien halusinasi di Instalasi Rawat Jalan Rumah Sakit Jiwa Provinsi Jawa Barat dengan nilai p value (0,001) < (0,005). Kata kunci: halusinasi, beban keluarga, kemampuan keluarga FAMILY BURDEN CONNECTED WITH FAMILY ABILITY TO TREAT HALLUCINATORY PATIENTS ABSTRACTSchizophrenia is a mental disorder that causes the onset of thought, perception, emotion, movement, strange and disturbing behavior. Schizophrenia 70% of them have hallucinations.The toughest impact the family feels in treating patients with hallucinations is its impact on the psychological, especially the stress or burden felt by the family.The purpose of this research is to know the family's burden relationship with the ability of the family to treat hallucinative patients in the installation of hospital outpatient in West Java province.This method of study uses descriptive correlation with purposive sampling technique against 69 respondents from the population of 996 respondents in an outpatient installation of psychiatric hospitals in West Java province. The instruments used in this reseacrh used family load questionnaires and family skills questionnaires comprising cognitive and psychomotor. Univariate analysis in this research using frequency distribution and bivariat analysis of this study using chi square test. the results showed a heavy burden of 35 respondents (50.7%), while for family ability of either 46 respondents (42%).in conclusion that there is a connection between the burden of the family with the ability of families to treat hallucinatory patients in the hospital outpatient installation in the psychiatric province of West Java with a value of P (0.001) < (0.005). Keywords: hallucinations, family load, abillity family

Background and objective: Acute critical illness represents a crisis not just for the individual patient however conjointly for the members of the family. Moreover, the admission of the patient represents a sudden crisis allowing no time for its preparation. The responsibilities of critical care nurses extend beyond the patients in the intensive care unit (ICU) to incorporate the members of the family of these patients. Nurses are a primary resource for members of family of ICU patients and they are in a perfect position to assist patients’ members of the family in an applicable approach. For this reason, recognition of these needs by nursing personnel is very important for applying of holistic nursing care. The aims of this study were 1) Ranking the immediate needs of members of family of critically ill patients and nurses. 2) Comparing between nurses, and families, opinion regarding priorities of immediate patient's family needs using Critical Care Family Needs Inventory (CCFNI).Methods: This descriptive study was conducted at the general ICUs of Damanhour Medical Institute which has 15 beds, Damanhour chest hospital which has 7 beds and Itay Elbaroad General Hospital which has 11 beds. The three units have a total of 100 nurses. A questionnaire was used for data collection. It consisted of part one which comprised patient's condition whose families were studied participants, biosocial knowledge related to the members of family and part two which included CCFNI tool. The adapted CCFNI was translated into colloquial Arabic. Participant members of family were individually interviewed by the researcher. Members of family were required to answer each statement with strongly agree, agree, neutral, do not agree and strongly do not agree. The interview took approximately 45 to 60 minutes to be completed, and all interviews took place within the 72 hours' time frame of each patient's admission to the critical care unit. The nurses were approached and given the questionnaires to be filled in by themselves during handover, tea or lunch break.Results: Generally, families ranked their knowledge, proximity and comfort needs higher in importance than the nurses. Also, the knowledge and assurance needs were ranked above the needs for support, comfort, and proximity by our participating nurses and members of family. Moreover, the mean scores of knowledge, proximity, support and comfort needs for members of family were significantly higher than these for nurses. Out of 35 needs of the members of family in the CCFNI, there were no significant differences between the mean scores of the nurses and those of the members of family in 10 individual needs. Results show that the nurses were correct in 10 out of 35 members of family’ individual needs. The remaining mean scores from 25 individual needs showed significant differences between the nurses and actual family needs. Results show that did not meet all the specific family needs during the care of the critically ill patients. Results show that members of family scored significantly higher than nurses on 20 statements while, the nurses scored significantly higher than members of family on only 5 of the statements.Conclusions: Families considered that knowledge then assurance was the most important needs. While nurses considered that assurance then knowledge were the most important needs which indicate that nurses underestimated the needs of the family and family need may be inaccurately evaluated by heath care team and almost unmet. Furthermore, Members of family in this study considered the needs that bring comfort and support as less in priority needs.

<pre>Schizophrenia is a disease that affects the brain and causes strange, disturbing thoughts, perceptions, emotions, movements and behavior. Recurrence is a term that relatively reflects the worsening of symptoms or behaviors that endangers patients and or their environment. This study aims to analyse the characteristics of schizophrenia patients with recurrence at the Tampan Mental Hospital of Pekanbaru With quantitative descriptive research design. The research sample were 167 respondents taken based on inclusion criteria by using purposive sampling technique. The analysis method used is univariate analysis. The results showed clinical characteristics of 167 respondents, received the majority of the respondents lived with the family 85 respondents (50.9%). Medical diagnosis of most schizophrenia paranoid 121 respondents (72.5%). Diagnosed for 1-5 years 82 respondents (49.1). Based on the most recurrence factor caused by treatment 120 respondents (71.9%). Based on the most predisposition factor caused by genetic 74 respondents (44.3%). while the precipitation factors resulting from family and socioeconomic 51 respondents (30.5%). Based on the results of this study can be concluded that the recurrence in schizophrenia patients is caused by a variety of conditions, so that it takes appropriate treatment and according to the characteristics Patients.</pre>

The critical care unit environment has been observed as a leading stressor not only amongst the health care professionals but in patients also. Unfamiliar surroundings, dependency on others, financial problems, etc. are among factors that are responsible for the development of stress in the patients admitted to the critical care units. Stress and its factors are also responsible for the patient's deliberate recovery and depraved prognosis. In this study, we explored the association between selective factors and perceived stressors among adult patients admitted to critical care units. A quantitative, exploratory research design study was conducted, to identify the association between the perceived stressors and selected factors of adult patients admitted in Critical Care Units of the selected multispecialty hospitals at Vadodara. A total of sixty patients admitted to the critical care departments were selected through the convenience sampling technique. A demographic variables datasheet and hospitals stress rating interview scale was used after the expert’s content validation to collect the data. Analysis and interpretation of data were done by using descriptive and inferential statistics. Results: Study findings revealed that all the patients, who were interviewed through the hospital stress interview scale, expressed the perception of stressors as indicated by the total perceived stressors score of above (1%). The study also revealed that separation from family members or spouses and financial problems are the highly rated perceived stressors among the patients admitted to the critical care unit. Patients also reported the least perceived stressful area was the problem with medication. There was a significant association between the socioeconomic status and medical-surgical diagnosis of patients with perceived stressors. Conclusion: Therefore, the findings of the study concluded that the study, patients admitted in the critical care department also suffer from stress due to some stressors, and it affects their prognosis and early recovery. Nursing professionals need to focus on the subjective characteristics of patients so that perceived stressors and early detection of the related complication can be prevented.

Family-centered care (FCC) in intensive care as a family-based care approach has been developed for a long time, but this development in Indonesia is still not optimally created. The FCC needs basic data about the needs of the family who are caring for it, so the need for the family can be identified therefore nursing intervention can be done optimally. One measure that has been developed to assess family needs is Critical Care Family Needs Inventory (CCFNI). The purpose of this study was to analyze the CCFNI questionnaire in Indonesian version. This study tried to test the questionnaire through several steps translation into Indonesian, validity testing in the form of validity content documents (CVI) and construct validity and reliability testing. The results of the CVI test on 45 Indonesian language CCFNI questionnaire statements found that the CVI value was in the range 0.6-0.9 obtained 36 valid statement items, while the results of the construct validity test were found to be valid. Reliability test results obtained an alpha value of 0.97 which means it is very reliable. The conclusion is that the Indonesian version of the CCFNI questionnaire which is translated into Kebutuhan Keluarga Pasien di Ruang Perawatan Intensif (K3PI) is valid and reliable.

Peningkatan suhu tubuh pada pasien cedera kepala bisa menyebabkan peningkatan metabolisme yang dapat memperburuk kondisi pasien, meningkatkan lama hari rawat dan menambah resiko kematian. Metode pendinginan yang sering digunakan adalah kompres selimut basah dan cold-pack. Namun belum ada penelitian yang membuktikan efek kedua metoda tersebut terhadap suhu tubuh pasien cedera kepala. Penelitian ini menggunakan rancangan perbandingan tidak berpasangan. Teknik pengambilan sampel yang digunakan adalah consecutive sampling dengan jumlah 24 orang responden. Penelitian ini memberikan hasil tidak terdapat perbedaan yang signifikan antara penurunan suhu tubuh setelah kompres selimut basah dan setelah kompres cold-pack (p=0,371). Oleh karena itu, kompres cold-pack dapat dijadikan alternatif kompres selimut basah yang biasa digunakan.Kata kunci : Cold-pack, cedera kepala, selimut basah, suhu tubuh. Wet Blanket and Cold-pack Application to Reduce Body Temperature among Patients with Head Injury in Neurosurgical Critical Care UnitAbstractThe elevation of body temperature among patients with head injury may lead to increase total metabolism of the body. Such situation may worsen the patient condition, prolonged length of stay and increase risk of death. Cooling methods using wet blanket and cold-pack have been commonly adopted to reduce the body heat. However no empirical studies have proved these methods are effective to reduce high temperature of patients with head injury. This non-paired comparative study seeks to examine the difference of those two methods towards body temperature involving 24 patients with head injury that recruited using consecutive sampling technique. Results indicated that there is no significant difference of the temperature decrease after wet blanket and cold-pack application (p= 0,371). However, cold-pack still can be used as an alternative compress beside wet blanket application.Key words: Cold-pack, head injury, body temperature, wet blanket.
