Introduction: The part of purulent inflammatory processes (IP) of the maxillofacial area (MFA) as a result of insect bites are increased. The aim: To attract attention of the medical community to the problem of the complexity of the diagnosis and treatment of these dermatogenic forms of IP MFA. Materials and methods: The work is based on the results of 5 year observations of 42 patients with acute IP MFA arising after insect bites, their comprehensive examination and treatment. Conclusions:1. IP of MFA, arising as a result of insect bites, is a very urgent problem of pediatric surgical dentistry and require an individual approach in the diagnosis and treatment of patients. 2. A large role in preventing the occurrence of these nosological forms is given to medical workers, a sufficient organization level of sanitary and educational work, and thetreatment of such cases becomes a common task of doctors of different profiles. However, much depends on the parents, their level of responsibility and competency, ensuringof timely treatment and provision of qualified medical care to children, preventing the development of severe complications. 3. The issues related to the etiopathogenesis of this pathology require profound scientific research.

This review is addressing an urgent problem of diagnosis and treatment strategy of polycystic ovary syndrome in adolescent girls. We analysed data from modern literary publications on the epidemiology and classification of the disease, as well as the adaptation of general principles for the management of young female patients tailored to their age peculiarities. The effective treatment strategies for adolescent girls with PCOS depending on its type are presented. Today there are several contradictions associated with both the diagnosis and treatment of the disease in the medical community. Thus, the use of the well-known Rotterdam criteria makes it difficult to diagnose PCOS in adolescent girls, since the criteria do not take into account characteristic changes that occur during puberty. Such manifestations as acne, hirsutism, menstrual irregularities, high androgen levels and morphology of polycystic ovaries on pelvic ultrasound imaging should be assessed with due account for age, puberty, hormonal balance with an assessment of the ratio of LH (luteinizing hormone)/FSH (follicle-stimulating hormone) and Free Androgen Index. There is no consensus in the medical community regarding the treatment strategy for PCOS. International communities have identified two main goals in the management of patients: normalization of menstrual function and improvement of the patients’ quality of life by preventing clinical symptoms of hyperandrogenism, and metabolic disorders of hyperplastic processes in target organs.Our analysis of objective scientific data showed that modern monophasic combined oral contraceptives (COCs) supplemented by of a folate component should be used to treat young female patients with PCOS, hyperandrogenism and psychosomatic disorders.

TSH-secreting pituitary neuroendocrine tumor (PitNET) is one of the causes of central hyperthyroidism. The incidence of TSH PitNET is far lower than that of other PitNETs. The clinical manifestations of TSH PitNETs mainly include thyrotoxicosis or thyroid goiter, secretion disorders of other anterior pituitary hormones, and mass effect on the pituitary gland and its surrounding tissues. The application of high-sensitivity TSH detection methods contributes to the early diagnosis and timely treatment of TSH PitNETs. Improvements in magnetic resonance imaging (MRI) have advanced the noninvasive visualization of smaller PitNETs. Treatments for TSH PitNETs include surgery, drugs, and radiotherapy. This review focuses on the progress in pathogenesis, diagnosis, and treatment of TSH PitNETs to provide more information for the clinician.

Among the large and heterogeneous group of patients with irritable bowel syndrome (IBS) comorbidity is characteristic feature. Comorbidity is an urgent problem in real clinical practice. Gastrointestinal comorbidity of a patient with IBS is associated with the presence of common pathogenetic links and is represented by combined pathology with functional disorders and the formation of “overlap syndrome” with organic diseases. Often, with a comorbid course, the phenomenon of “mutual burdening” is observed, when the severity of the course of each of them increases. All this requires optimization of approaches to the diagnosis and treatment of this cohort of patients. Obviously, with a comorbid course of IBS, it is advisable to prescribe drugs that affect the general links of pathogenesis, rather than focusing on individual symptoms. This article presents modern information on the gastrointestinal comorbidity of a patient with IBS.

The influence of obesity on human health, as a multifactorial and multigenic disorder, is a rather complex, interdisciplinary and at the same time extremely urgent problem of modern society. Vitamin D deficiency is one of the consequences of obesity that negatively affects a person’s life expectancy. Vitamin D deficiency is rightfully considered a silent, non-infectious metabolic pandemic of the 21st century. Its significant role in the functioning of the human body is deep and multifaceted, since vitamin D is an integral regulator of the transcriptional activity of genes that control 3–5% of the human genome. There are ongoing discussions among experts in the medical community about the negative impact of obesity on 25 (OH) D levels, and the opposite hypothesis is also being discussed, where vitamin D deficiency is considered an independent risk factor for obesity. Both external causes of the formation of vitamin D deficiency against the background of excessive deposition of adipose tissue and internal metabolic processes underlying the pathogenetic association are analyzed two pathological conditions.

The article describes a case of diagnosis and treatment of an early stage of ankylosing spondylitis in a young man. This disease, which can potentially lead to disability, may manifest in the pathology of peripheral joints, often prevailing over the symptoms of axial damage. Ankylosing spondylitis is characterized by varied clinical symptoms, this, early diagnostics is a valuable prerequisite for timely treatment.

Most of the literature about hemophilia has been from a medical perspective and about men. This on-going project has been to document the lives and ‘voices’ of women who have a bleeding disorder, their experiences with the medical community, and the course of their diagnosis and treatment. The women in this study were predominantly Caucasian (92%), were currently married (66.7%), had (68.9%), and of those women who had children, the majority of their children (67.9%), both boys and girls, as well as other family members(63.3%) had some type of bleeding disorder. Findings revealed there was on average, a 14 year gap between the first bleeding episode and the diagnosis of a bleeding disorder. Women reported long, heavy menstrual cycles and having consulted a physician for menstrual bleeding as teenagers. Treatments for menstrual bleeding included D and C's (42.9%) and hysterectomies (34.5%), while referrals to Hemophilia Treatment Centers for a consultation were rare.

Annotation. This article highlights the urgent problem of combined diseases of women's reproductive system. On the basis of theoretical analysis, generalized statistics for recent years are presented. The modern concepts of pathogenesis of diseases of the uterus and mammary glands, their correlation, as well as models of diagnosis and treatment are considered. Indeed, a more in-depth understanding of the problems of the combined pathology of the woman’s reproductive system (diseases of the uterus, ovaries, mammary gland), namely etiopathogenesis, diagnostic and treatment algorithms, will reduce the incidence of women of reproductive and perimenopausal age, as well as improve their quality of life. Only having the fullness of information about the state of the reproductive system of women, the doctor is able to prescribe thoughtful and effective treatment.

Rheumatology is one of the areas of internal medicine that deals with the diagnosis and treatment of rheumatic diseases. The term rheumatology is based on the merger of the words «rheuma», which means «flowing current», and «logos», which means «studies». Thus, the subject of the rheumatology study is inflammatory and degenerative-dystrophic diseases affecting the joints, as well as systemic diseases of the connective tissues (the outdated name is collagenosis). Diversity and polymorphism of the clinical symptoms of connective tissue diseases often leads to the difficulties in diagnosis and timely treatment.

Chronic obstructive pulmonary disease (COPD) is a disease with increasing prevalence and burden for health systems worldwide. Every country collects its own epidemiological data regarding COPD prevalence, morbidity and mortality while taking steps to educate the population and medical community to improve early detection and treatment. The rising COPD prevalence creates a need for comprehensive guidelines. In 2012 and 2017–2018, the Romanian Society of Pneumology (SRP) organised national inquiries for COPD, while lung physicians in Romania began receiving education regarding the correct algorithms for COPD diagnosis and therapy. During 2019, a Romanian clinical guideline for diagnosis and treatment of COPD was published, and a condensed version of key points from this guideline are presented herein. COPD is diagnosed based on the presence of three major components: relevant exposure history, respiratory symptoms, and airway limitation that is not fully reversible. Clinical evaluation of patients diagnosed with COPD should include the level of symptoms, exacerbation rate, the presence of comorbidities and determination of phenotypes. The present abridged guideline is designed to be accessible and practical for assessing and managing patients with COPD. The application of up-to-date COPD guidelines may enhance the optimism of physicians and patients in managing this disease.
