Introdução: Pacientes em tratamento com bisfosfonatos ou mesmo com queixa principal de uma lessão bucal com diagnóstico provável de osteonecrose dos ossos maxilares por uso de bisfosfonatos podem ser frequentes na rotina diária de atendimento odontológico. Objetivos: A presente pesquisa objetiva revisar de forma sistematizada os estudos existentes sobre a relação entre procedimentos endodônticos e eventos de osteonecrose em ossos maxilares expostos a Bisfosfonatos. Materiais e Métodos: A estratégia de busca sistemática de artigos contemplou as principais fontes de evidência científica, bases de dados convencionais e também fontes de dados não publicados, além de busca manual das referências bibliográficas dos artigos pré-selecionados. A busca foi feita utilizando-se as palavras-chave obtidas por descritores relacionados com a pergunta a ser respondida. Resultados: A busca inicial resultou em 683 artigos. Apés a aplicação dos critetérios de eligibilidade, oito artigos foram selecionados para a tabulação e coleta de dados. Apenas dois estudos avaliaram causalidade entre procedimentos endodônticos e eventos de osteonecrose nos ossos maxilares, em pacientes usuários de bisfosfonatos. Dos procedimentos odontolôgicos avaliados, os menos frequentemente relacionados com os casos dessa patologia foram as terapias endodônticas (0,8% dos casos). Conclusão: A realização de procedimentos endodônticos em pacientes que fazem uso de bisfosfonatos pode resultar em casos de osteonecrose dos ossos maxilares. Entretanto, mais estudos são necessários para investigar a possiíel relação de causalidade entre o fator de exposição (procedimentos endodônticos) e o desfecho (osteonecrose dos maxilares, em pacientes usuários de bisfosfonatos) estudados. Objectives: The present study aims to systematically review existing studies on the relationship between endodontic procedures and events of osteonecrosis in maxillary bones exposed to bisphosphonates. Materials and Methods: The systematic search strategy of articles included the main sources of scientific evidence, conventional databases as well as unpublished data sources, and manual search of the bibliographic references of the pre- selected articles. The search was done using the keywords obtained by descriptors related to the question to be answered. Results: The initial search resulted in 683 articles. After applying the eligibility criteria, eight articles were selected for tabulation and data collection. Only two studies evaluated causality between endodontic procedures and events of osteonecrosis in maxillary bones in patients using bisphosphonates. Of the dental procedures evaluated, the least frequently related cases of this pathology were endodontic therapies (0.8% of cases). Conclusion: Performing endodontic procedures in patients who use bisphosphonates may result in cases of osteonecrosis of the maxillary bones. However, further studies are needed to investigate the possible causal relationship between the exposure factor (endodontic procedures) and the outcome (osteonecrosis of the jaws in patients using bisphosphonates).

Composite dust generation is most likely a continuous and daily procedure in dental practice settings. The aim of this systematic review was to identify, compile and evaluate existing evidence on interventions and composite material properties related to the production of aerosolized dust during routine dental procedures. Seven electronic databases were searched, with no limits, supplemented by a manual search, on 27 April 2020 for published and unpublished research. Eligibility criteria comprised of studies of any design, describing composite dust production related to the implementation of any procedure in dental practice. Study selection, data extraction and risk of bias (RoB) assessment was undertaken independently either in duplicate, or confirmed by a second reviewer. Random effects meta-analyses of standardized mean differences (SMD) with associated 95% confidence intervals (CIs) were employed where applicable. A total of 375 articles were initially identified, resulting in 13 articles being included in the qualitative synthesis, of which 5 contributed to meta-analyses overall. Risk of bias recordings ranged between low and high, pertaining to unclear/raising some concerns, in most cases. All types of composites, irrespective of the filler particles, released significant amounts of nano-sized particles after being ground, with potentially disruptive respiratory effects. Evidence supported increased % distribution of particles < 100 nm for nanocomposite Filtek Supreme XTE compared to both conventional hybrid Z100MP (SMD: 1.96, 95% CI: 0.85, 3.07; p-value; 0.001) and nano- hybrid Tetric EvoCeram (SMD: 1.62, 95% CI: 0.56, 2.68; p-value: 0.003). For cytotoxicity considerations of generated aerosolized particles, both nanocomposites Filtek Supreme XTE and nanohybrid GradiO revealed negative effects on bronchial epithelial cell viability, as represented by % formazan reduction at 330–400 μg/mL for 24 hours, with no recorded differences between them (SMD: 0.19; 95% CI: −0.17, 0.55; p-value: 0.30). Effective and more rigorous management of dental procedures potentially liable to the generation of considerable amounts of aerosolized composite dust should be prioritized in contemporary dental practice. In essence, protective measures for the clinician and the practices’ personnel should also be systematically promoted and additional interventions may be considered in view of the existing evidence.

SummaryThe role of extracorporeal membrane oxygenation (ECMO) in refractory cardiorespiratory failure is gaining momentum with recent advancements in technology. However, the need for dialysis modes such as continuous renal replacement therapy (CRRT) has also increased in the management for acute kidney injury. Establishing the exact timing of CRRT initiation in these patients from the electronic medical record is vital for automated data extraction for research and quality improvement efforts.We aimed to derive and validate an automated Electronic Health Records (EHR) search strategy for CRRT initiation in patients receiving ECMO.We screened 488 patients who received ECMO and a total of 213 patients underwent CRRT. We evaluated random 120 patients, 60 for derivation and 60 for validation cohorts. Following implementation of eligibility criteria, the algorithm was derived in 55 out of 120 ECMO/CRRT patients. The search algorithm was developed using first-time chart entry of ‘access pressure drop’ at CRRT initiation. The algorithm was then validated in an independent subset of 52 patients from the same time period. The overall agreement between electronic search algorithm and a comprehensive manual medical record review in the derivation and validation subsets, using ‘access pressure drop’ as the reference standard, was compared to assess CRRT initiation time.In the derivation subset (N=55), the automated electronic search strategy achieved an excellent agreement with manual search (D =0.99, 54 were identified electronically, and 55 upon manual review). There was no time difference observed in 49/54(89%) patients, while in the remaining 5 (9%) patients time difference was within 15 minutes. In the validation cohort (N=52), agreement was 100 % (D = 1.0, both methods identified 52 patients). Out of 52 patients, 47 (90%) had no time difference between the methods, for the remaining 5 (10%) patients, differences were within 15 minutes.The use of an electronic search strategy resulted in determining an accurate CRRT initiation time among ECMO patients.

Breast milk components contribute to the infant’s immune development and protection, and among other immune factors, immunoglobulins (Igs) are the most studied. The presence of IgA in milk has been known for a long time; however, less information is available about the presence of other Igs such as IgM, IgG, and their subtypes (IgG1, IgG2, IgG3, and IgG4) or even IgE or IgD. The total Ig concentration and profile will change during the course of lactation; however, there is a great variability among studies due to several variables that limit establishing a clear pattern. In this context, the aim of this review was firstly to shed light on the Ig concentration in breast milk based on scientific evidence and secondly to study the main factors contributing to such variability. A search strategy provided only 75 studies with the prespecified eligibility criteria. The concentrations and proportions found have been established based on the intrinsic factors of the study—such as the sampling time and quantification technique—as well as participant-dependent factors, such as lifestyle and environment. All these factors contribute to the variability of the immunoglobulinome described in the literature and should be carefully addressed for further well-designed studies and data interpretation.

Abstract Background/objective Craniofacial discrepancies have been associated with congenital dental anomalies. The aim of this study was to determine if there is any scientific evidence in the literature regarding the association between dental agenesis and craniofacial morphology. Materials and methods A systematic review and meta-analysis following the PRISMA Statement were conducted and registered in PROSPERO database. A broad search was conducted on databases (PubMed, Virtual Health Library, Web of Science, and Scopus) and grey literature. Articles that were selected based on predetermined eligibility criteria were assessed for quality and risk of bias according to the guidelines described by Folkes and Fulton. Those articles with similarities were submitted to meta-analysis using the RevMan 5.3 program. The certainty of the evidence was tested using the GRADE (Grading of Recommendations, Assessment, Development and Evaluation). Results An electronic search of 975 articles resulted in 12 articles. These and four articles identified through manual search were eligible to be assessed for methodological quality and the risk of bias. Eleven articles presented moderate to high methodological quality. The included articles observed a reduced mandibular plane, a smaller maxilla and more differences with the increase in the severity of dental agenesis. Seven articles were included in the meta-analyses. Dental agenesis presented smaller SNA angle (P &lt; 0.0001/CI −1.74 [−2.55, −0.92]) with moderate certainty of the evidence; and smaller ANB angle (P = 0.01/CI −0.80 [−1.44, −0.17]), with low certainty of the evidence. Conclusions This systematic review and meta-analysis demonstrated that dental agenesis may be associated with specific craniofacial morphology. Further studies are necessary due to the variation of the certainty of the evidence. Conflict of interest none declared. Registration PROSPERO (CRD42017055882).

Abstract We aimed to investigate relationships between Facebook problematic use (Facebook PU) and pathological traits of personality, by a systematic review. The literature search was proceeded based on a search strategy including the keywords “Facebook”, “problematic use” and “personality”. We found a total of 236 publications and selected 21 papers for full verification. All of them were excluded, characterizing an empty review. As an attempt to increase the probability of finding eligible publications, first, we have broadened the search of the systematic review from “Facebook” to “social network sites” (and variations); and then from “social network sites” to “internet”. Regarding this last change, the final set of publications after full reading based on the eligibility criteria was composed by 15 papers. We emphasize that there was no empirical evidence on the relationship between Facebook PU and pathological traits of personality. Given the proximity between Facebook PU and internet addiction, it is possible to hypothesize that there is a relationship between pathological traits (i.e., Neuroticism, Impulsivity, and Psychoticism) and Facebook PU.

The purpose of the research is to study the possibility of predicting the development of aerobic endurance of athletes through a comparative analysis of the outcomes of genetic and computerized testing. Methods and organization of the research. In our research, we used scientific and methodological literature, as well as the outcomes of competitive activity and genetic analysis of a particular athlete. The total number of survey respondents was 158 people including athletes (n = 85) and coaches (n = 73). Results and discussion. Some authors find the relationship between the ability to develop and manifest endurance and the presence of the appropriate alleles of genes: ACE I, ACNT X, ACNT3 (RX, XX), ADRA2A, AMPDI C, PGC1A Gly, NFATC4, UCP2. Most studies reveal the relationship between the ACE gene and endurance supported by the I allele. Genetic foresight of the possibility of developing aerobic endurance of athletes can become the framework for the application of an individual approach in sport training, and contribute to the development of techniques aimed at the refinement of physical qualities. The modern approach to the coaching and competitive activities of athletes should include both genetic and functional studies of the human body. Conclusion. Sport forecast cannot be based solely on genetic testing results. The presence of polymorphisms in one or more genes associated with sport activity is the platform for good performance in a particular sport, but the actual manifestation of genetic predisposition depends on many factors. These factors include nutrition, daily routine and competent organization of the training process, which requires a high level of theoretical and practical competency of coaches in the field of biomedical education.

In recent decades, there has been a very rapid increase in the prevalence of diabetes globally, with serious health and economic implications. Although today there are several therapeutic treatments for this disease, these do not address the causes of the disease and have serious side effects, so it is necessary to seek new treatments to replace or complement the existing ones. Among these complementary treatments, a strong link between the intestinal microbiota and diabetes has been demonstrated, which has focused attention on the use of biotherapy to regulate the function of the intestinal microbiota and, thus, treat diabetes. In this way, the main objective of this work is to provide a review of the latest scientific evidence on diabetes, gathering information about new trends in its management, and especially, the influence of the intestinal microbiota and microbiome on this pathology. It is possible to conclude that the relationship between the intestinal microbiota and diabetes is carried out through alterations in energy metabolism, the immune system, changes in intestinal permeability, and a state of low-intensity systemic inflammation. Although, currently, most of the experimental work, using probiotics for diabetes management, has been done on experimental animals, the results obtained are promising. Thus, the modification of the microbiota through biotherapy has shown to improve the symptoms and severity of diabetes through various mechanisms related to these alterations.

Abstract Introduction Few studies have examined circadian phase after job loss, an event that upends daily routine. It is common that a daily routine begins with the consumption of breakfast, and breakfast behavior may contribute to health status in adults. Therefore, we sought to examine whether a later midpoint of sleep was associated with breakfast skipping among adults whose schedules were no longer dictated by employment. Methods Data were obtained from the Assessing Daily Activity Patterns Through Occupational Transitions (ADAPT) study. The sample of 155 participants had involuntarily lost their jobs in the last 90 days. Both cross-sectional and 18-month longitudinal analyses assessed the relationship between sleep midpoint after job loss and current and later breakfast skipping. Assessment periods were 14 days. Sleep was measured via actigraphy, and breakfast skipping was measured via daily diary (1 = had breakfast; 0 = did not have breakfast). The midpoint of sleep was calculated as the circular center based on actigraphy sleep onset and offset times. Results The midpoint of sleep at baseline was negatively associated with breakfast consumption at baseline (B = -.09, SE = .02, p = .000). Also, a later midpoint was associated with breakfast skipping over the next 18 months (estimate = -.08; SE = .02; p = .000). Prospective findings remained significant when adjusting for gender, ethnicity, age, perceived stress, body mass index (BMI), education, and reemployment over time. Education (estimate = 14.26, SE = 6.23, p &lt; .05) and BMI (estimate = -.51, SE = .25, p &lt; .05) were the only significant covariates. No other sleep indices predicted breakfast behavior cross-sectionally or prospectively. Conclusion Consistent with research in adolescents, unemployed adults with a later circadian phase are more likely to skip breakfast more often. Breakfast skipping was also associated with higher BMI. Taken together, these findings provide support for the future testing of sleep/wake scheduling interventions to modify breakfast skipping and potentially mitigate weight gain after job loss. Lycopene is a well-known compound found commonly in tomatoes which brings wide range of health benefits against cardiovascular diseases and cancers. From an anti-cancer perspective, lycopene is often associated with reduced risk of prostate cancer and people often look for it as a dietary supplement which may help to prevent cancer. Previous scientific evidence exhibited that the anti-cancer activity of lycopene relies on its ability to suppress oncogene expressions and induce proapoptotic pathways. To further explore the real potential of lycopene in cancer prevention, this review discusses the new insights and perspectives on the anti-cancer activities of lycopene which could help to drive new direction for research. The relationship between inflammation and cancer is being highlighted, whereby lycopene suppresses cancer via resolution of inflammation are also discussed herein. The immune system was found to be a part of the anti-cancer system of lycopene as it modulates immune cells to suppress tumor growth and progression. Lycopene, which is under the family of carotenoids, was found to play special role in suppressing lung cancer.
