Urinary tract infections (UTIs) are among the commonest types of bacterial infections. The antibiotic treatment for UTIs is associated with important medical and economic implications. Many different microorganisms can cause UTIs though the most common pathogens are E. coli and members of family Enterobacteriaceae. The knowledge of etiology and antibiotic resistance pattern of the organisms causing urinary tract infection is essential. The present study was undertaken to evaluate trends of antibiotic susceptibility of commonly isolated uropathogens using newer antimicrobial agents, prulifloxacin, fosfomycin (FOM) and doripenem. We conclude that maintaining a record of culture results and the antibiogram may help clinicians to determine the empirical and/or specific treatment based on the antibiogram of the isolate for better therapeutic outcome.

Objective: Urinary tract infections (UTIs) are some of the most common bacterial infections encountered in community and cause of significant morbidity and high medical cost. Escherichia coli is the most common pathogen belongs to Enterobacteriaceae family responsible for majority of UTI infections. Antimicrobial drugs have been routinely prescribed for empirical treatment of UTIs which has led to a dramatic increase in antibiotic resistance pattern of E. coli. The aim of present study was to analyse the multidrug resistance patterns (MDR) of E. coli isolated from UTI patients.Methods: A total of 80 urine samples collected from the patients suspected of having UTI attending Maharishi Markandeshwar Institute of Medical Sciences and Research (MMIMSR), Mullana, Ambala were cultured using standard microbiological techniques. Antibiotic susceptibility testing of E.coli was done by using minimum inhibitory concentration (MIC). MIC of tetracycline, doxycycline, azithromycin, erythromycin, ciprofloxacin, levofloxacin, ampicillin, amoxicillin and amikacin was done by agar dilution method.Results: Of the total 46 isolates contributing 33 females and 13 males were confirmed as E. coli. About 51.34% of the female patients belonged to the age group 21-40 yr and 53.84% of the male population belonged to 41-80 yr were found to be more susceptible to UTI infection. All isolates confirmed as E.coli were found to be multidrug resistant. 80% of the isolates exhibited MICs higher than 1000mg/L against β-lactams. 20% of the E. coli isolates exhibited MICs higher than 1000mg/L against ciprofloxacin, amikacin and erythromycin. 23% and 95% of E. coli isolates exhibited MICs less than 128 mg/L against doxycycline and levofloxacin respectively.Conclusion: The present study revealed the decreased susceptibility of the E.coli to all drugs. E. coli resistance profile to beta lactams, quinolones, macrolides, tetracyclines and aminoglycosides were also found to be quite high in this study emphasizing the need to educate public about appropriate use of antibiotics.NA

Background: Urinary tract infections (UTIs) are the most common bacterial infection encountered in tertiary care settings UTI is one of the most common bacterial infections in humans and a major cause of morbidity. Its antibiotic sensitivity pattern varies with the widespread availability of antimicrobial agents; UTI has become difcult to treat because of appearance of pathogens with increasing resistance to antimicrobial agents. This prospec Methods: tive and observational Study was conducted on patients attending the outpatient and indoor admitted patients at North Bengal Medical College at Darjeeling district of West Bengal. From 2016 to 2018. A total 100 patients were taken who having clinical features of UTI. The study region covers the various districts of North Bengal. Template was generated in MS excel sheet and analysis was done on SPSS 20.0 software. A Results: mong 100 UTI patients, 34 (34%) were male and 66 (66%) were female. The majority of patients (30%) were 46-60 yrs of age. In the study group symptomatic presentation in order of frequency were, dysuria 85%, urgency74%, fever with chill 60%, hypogastric pain and tenderness 70%, ank pain 30%. E.coli & klebsiella were most common isolates among i.e 66% and11.% respectively. Proteus were sensitive to Netlimycine 90%, Imipenem 87%, Meropenem 79%, Amikacin 75% Gentamycin 68 %, levooxacin 82 % of patients. In the present study, females (66%) Conclusions: were mostly affected and the most common organisms were E.Coli and Klebsiella. These organisms were most sensitive to Amikacin, Nitrofurantoin etc. The sensitivity and resistance pattern of uropathogens to common antimicrobial agents must be taken into account when selecting treatment plans for UTI.

Objective: The research was carried out with isolate and determines the antimicrobial sensitivity in E. coli from urinary tract infections in special hospital in Denizli and recorded at specimens. Methods: Urine samples (n=21) were collected from patients with signs and symptoms of Urinary tract infections. Bacteria were isolated and identified by conventional biochemical profile. Antibiotic resistance pattern of E. coli against different antibiotic was determined by Kirby-Baur method. Results: The results revealed that sensitivity rate of antimicrobial agents were in the range of meropenem (100%), norfloxacin and ciprofloxacin (86%), cefotaxime (80%), aztreonam (76%). None of the samples showed no resistance to amikacin, ceftazidime, aztreonam, amoxicillin/clavulanic acid, and meropenem. Out of 21 isolates, 3(14%) isolates showed Multiple Antibiotic Resistance ten to thirteen antibiotics. Conclusion: It is concluded that most of the urinary tract infections in human are caused by E.coli exhibited highest resistance to meropenem (100%), followed by norfloxacin and ciprofloxacin  (86%).

Background: Urinary tract infection (UTI) is one of the most common bacterial infections encountered by clinicians particularly in developing countries. Current knowledge on antimicrobial resistance pattern is essential for appropriate therapy. The aim of the present study was to identify the causative organisms for UTI and to determine the antibiotic susceptibility pattern of organisms causing UTI. Method: This cross sectional study was carried out in the department of Microbiology, Sir Salimullah Medical College, Dhaka, from a period of January 2014 to December 2014. Results: Out of 2136 clinical sample of urine, 430 (20.1%) showed significant bacterial growth. Escherichia coli was the commonest urinary pathogen (76.3%), followed by Pseudomonas spp. (7.9%), Proteus spp. (7.2%), Klebsiella spp., Citrobacter spp. (1.9% each) and Staphylococcus aureus (1.6%). Isolated uropathogens showed highest resistance for Amoxycillin (86%-97%) and Cefradin (71%-100%), resistance rate for other commonly used antimicrobial agents was high; Cefixime (52%-85%), Ceftriaxone (50%-71%), Ciprofloxacin (50%-88%), Cotrimoxazole (50%-75%), Gentamicin (57%-75%) and Nitrofurantoin (43%-100%), while uropathogens were least resistant to Imipenem (0%-15%) and Amikacin (0%-29%). Conclusion: Due to high degree of resistance to commonly used antimicrobials to treat UTI, routine monitoring and evaluation studies should be conducted to update physicians’ knowledge about most effective antibiotics for treatment of UTI. Background and objective: Urinary tract infection (UTI) is one of the frequently seen infections both in the world and in our country as well. Escherichia coli (E. coli) aremost frequently isolated in complicated or uncomplicated, nosocomial or community acquired urinary tract infections. To guide the empirical therapy, the resistance pattern of E. coli responsible was evaluated throughout the period in this study.Material and Method: Urine samples from outpatient / inpatient department of Ibn Sina Medical College Hospital between 1st January, 2015 and 31st December, 2015 were retrospectively analyzed. Presence of ?105 colony forming units/ml in urine culture was considered as significant for UTI. Isolated bacteria were identified by standard laboratory techniques and antibiotic susceptibility testing was performed by Kirby-Bauer disk diffusion method using Clinical Laboratory Standard Institute (CLSI) criteria.Result: A total of 271(13.4%) uropathogens were isolated. Overall E. coli accounted for 180 (66.42%) of all isolates. Resistance rates of E. coli to antimicrobial agents was demonstrated to be as follows: cefuroxime 82%, nalidaxic acid 74%, azithromycin 56%, cefotaxime52%, ceftazidime50%, cefixime 47%, cotrimoxazole 43%, ceftriaxone 41%, ciprofloxacin38%, amoxicillin- clavulanic acid 31%, cefepime30%, and low resistance which ranges from 9 to 1% included gentamycin 9%, meropenem 3%, imipenem2%, nitrofurantoin2% and amikacin 1%.Conclusion: As resistance rates show regional differences, it is necessary to regularly monitor regional resistance pattern to determine the appropriate empiric antibiotic treatment. Objectives: To investigate clinical characteristics, bacterial characteristics, drug resistance status in patients with urinary tract infections treated at Department of Urology, Hue University Hospital. Materials and Method: The study was conducted in 474 patients with urological disease treated at Department of Urology, Hue Universiry Hospital from July 2017 to April 2018. Urine culture was done in the patients with urine > 25 Leu/ul who have symptoms of urinary tract disease or infection symptoms. Patients with positive urine cultures were analyzed for clinical and bacterial characteristics. Results: 187/474 (39.5%) patients had symptoms associated with urinary tract infections. 85/474 (17.9%) patients were diagnosed with urinary tract infection. The positive urine culture rate was 45.5%. Symptoms of UTI were varied, and no prominent symptoms. E. coli accounts for the highest proportion (46.67%), followed by, Staphycoccus aureus (10.67%), Pseudomonas aeruginsa (8,0%), Streptococcus faecali and Proteus (2.67%). ESBL - producing E. coli was 69.23%, ESBL producing Enterobacter spp was 33.33%. Gram-negative bacteria are susceptible to meropenem, imipenem, amikacin while gram positive are vancomycin-sensitive. Conclusions: Clinical manifestations of urinary tract infections varied and its typical symptoms are unclear. E.coli is a common bacterium (46.67%). Isolated bacteria have a high rate of resistance to some common antibiotics especially the third generation cephalosporins and quinolones. Most bacteria are resistant to multiple antibiotics at the same time. Gram (+) bacteria are susceptible to vancomycin, and gram (-) bacteria are susceptible to cefoxitin, amikacin, and carbapenem. ABSTRACTA mouse model of cystitis caused by uropathogenicEscherichia coliwas used to study the distribution of gallium in bladder tissue following oral administration of gallium maltolate during urinary tract infection. The median concentration of gallium in homogenized bladder tissue from infected mice was 1.93 μg/g after daily administration of gallium maltolate for 5 days. Synchrotron X-ray fluorescence imaging and X-ray absorption spectroscopy of bladder sections confirmed that gallium arrived at the transitional epithelium, a potential site of uropathogenicE. coliinfection. Gallium and iron were similarly but not identically distributed in the tissues, suggesting that at least some distribution mechanisms are not common between the two elements. The results of this study indicate that gallium maltolate may be a suitable candidate for further development as a novel antimicrobial therapy for urinary tract infections caused by uropathogenicE. coli.

SummaryStrains ofEscherichia coliisolated from adult females with symptomatic urinary tract infection were found to possess the following properties significantly more frequently than faecal strains: (i) high K-antigen titre; (ii) haemolysin; (iii) type 1 pili; (iv) mannose-resistant haemagglutination; (v) fermentation of dulcitol and salicin; (vi) O serotype 2, 6 and 75; (vii) H serotype 1.E. coliisolated from urine specimens containing significant numbers of antibody-coated bacteria were richer in these seven properties than strains from urines without detectable antibody-coated bacteria.The O and H serotypes ofE. coliobtained from patients with urinary tract infection in two New Zealand cities were compared with those reported in the world literature and found to be similar.

Background:Urinary tract infection is the most common bacterial infections in humans and serious health problem in many parts of the world. It has become more complicated in treatment due to different pathogens and increasing resistant to antimicrobial agents. This study aims to investigate the prevalence of urinary tract infection and antibiotic susceptibility patterns of pathogens among the patients attending in B & B hospital Nepal.Materials and Methods:A hospital based cross sectional study was conducted in between April 2010 to March 2011. Urine samples were collected from clinically suspected patients and tested bacteriologically using standard procedures. Antimicrobial susceptibility test was performed for isolated pathogen using the Kirby-Bauer disk diffusion method according to Clinical and Laboratory Standards Institute guidelines.Results:Out of 1260 examined specimens 25.24 % were positive and majority 61.64% were females.  The most common pathogens isolated were Escherichia coli (66.67%), Enterococcus (7.55%) and Staphylococcus (6.60 %). The drug resistant among the positive cases were reported. The highest resistant of positive cases was found with Cefexime (87.88%) and Enterococcus with Ampicillin (66.67%) and Staphyllococcus with Cloxacillin (66.67%). The highest susceptibility was for Vancomycin and Ampicillin i.e. 33.33% in each.Conclusion:The findings showed that E. coli isolates were the predominant pathogen and the presence of bacterial isolates with very high resistance to the commonly prescribed drugs. As drug resistance among bacterial pathogens is an evolving process and serious issue. Therefore, routine surveillance and monitoring studies should be conducted to provide physicians knowledge on the updated and most effective empirical treatment of UTIs. Asian Journal of Medical Sciences Vol.7(5) 2016 47-51

As a result of the conducted researches it is shown that 44.1% of urinary tract infections (UTIS) caused by E. coli are accounted for by producers of beta-lactamase of the extended spectrum of action (ESBL). Associated resistance to fluoroquinolones and co-trimoxazole was found in 93.3% of BLRS-producing E. coli strains. All studied strains regardless of ESBL production were sensitive to imipenem, the majority showed sensitivity to ertapenem, gentamicin and resistance to doxycycline. Not producing ESBL strains of E. coli were sensitive to fosfomycin. Comparison of data obtained during testing of isolated cultures on ESBL, study of their sensitivity and resistance to beta-lactams (amoxicillin/clavulanate, ceftazidime, ceftriaxone, cefotaxime, imipenem) indicates the need to test isolates for AmpC products. To this end, during the screening test for ESBL and the method of «double disks», along with cephalosporins of III generation, it is necessary to use a phenotypic test for sensitivity to cefepime. The use of test results of E. coli isolates isolated from patients with UTIS for the production of ESBL, ampC enzymes, carbapenemase and sensitivity to AMP will improve the effectiveness of antimicrobial therapy and will help to curb the formation and spread of antimicrobial-resistant strains.
