Background: Global estimates suggest there are almost 19 million visually impaired children worldwide, the major cause being uncorrected refractive error (RE).Aim: To assess the prevalence of visual impairment (VI) and RE among Sudanese school-aged children.Setting: Eight randomly selected primary schools from 21 districts in South Darfur State of Sudan were involved.Methods: A school-based cross-sectional study of RE and VI in primary schoolchildren from grades 1 to 8 (children aged 6–15 years) was investigated. A Refractive Error Study in Children (RESC) protocol was implemented to determine the prevalence of RE and VI in these schoolaged children. Participants were enrolled through stratified multistage cluster sampling of four all-male and four all-female primary schools from South Darfur State of Sudan. Examination procedures followed the RESC protocol, which included visual acuity (VA) measurements, binocular vision assessments, retinoscopy and autorefraction under cycloplegia, as well as examination of the external eye, anterior-segment, media and fundus.Results: A total of 1775 children were invited to participate in the study and 1678 were examined resulting in a participation rate of 94.5%. The findings indicated that the prevalence of uncorrected, presenting and best-corrected VA of 6/12 or worse was 6.4% (95% confidence interval [CI], 4.9–7.9), 4.4% (95% CI, 2.9–5.9) and 1.2% (95% CI, 0.3–2.7) respectively. RE was the cause of VI in 57% of participants, retinal disorders in 13.1%, amblyopia in 5.6%, corneal opacity in 0.9%, cataract in 3.7%, with the causes of reduced vision undetermined in 10.3% and various other causes contributed 9.3%. External and anterior-segment abnormalities were observed in 10.2% of children. This was mainly allergic conjunctivitis (5.3%) followed by bacterial and viral conjunctivitis (4.2%). The prevalence of myopia (≥|-0.50 D|) in one or both eyes was 6.8% (95% CI, 5.3–8.3), hypermetropia (≥ 2 D) was noted in 1.9% (95% CI, 0.4–3.4) and astigmatism (≥|-0.75 D|) prevailed in 2.5% (95% CI, 1.0–4.0). Prevalence of VI among schoolaged children were associated with the children’s age and grade levels (p = 0.00) but was not correlated with gender (p = 0.224). Prevalence of RE among school-aged children was significantly correlated with age and grade levels (p = 0.00). No significant correlation was found between gender and prevalence of RE (p = 0.833). The prevalence of VI because of myopia was associated with increasing grade levels and also the childhood age (p = 0.023), but there was no significant difference in the prevalence of VI because of myopia between male and female children.Conclusion: Uncorrected RE was a major cause of VI among children in the South Darfur State. There is thus a critical need for developing a comprehensive child eye care plan focusing on the reduction of uncorrected RE through collaboration between key stakeholders and government.

Objectives: The most distinctive group of people with sensory disabilities is those with visual impairment. This study aimed to evaluate the relationship between behavioral problems and demographic variables in students with visual impairment in the academic year of 2018-2019 in Tehran City, Iran. Methods: This cross-sectional, descriptive, and analytical study was conducted on students with visual impairment studying in four schools of exceptional children at the elementary and high school levels. The research population was 400, and the sample size was estimated at 200 using Cochran’s formula. The sampling was carried out using the random cluster sampling method, where the floors of the school were considered and visually-impaired students were selected randomly from each school based on the number of students. Demographic characteristics questionnaire and Rutter behavior evaluation questionnaire (teacher form) were used to collect data. Results: The Mean±SD behavioral problems score was 12.9±10.7. According to the results, there was a negative and significant correlation between the subscales of behavioral problems with age and duration of the impairment. A significant difference was found in the mean of behavioral problems of visually-impaired or blind children caused by accident with those with congenital and hereditary problems. The regression model showed a significant difference between male and female subjects in terms of behavioral problems in a short duration after the injury (≤5 years). Discussion: Given the relationship between behavioral problems with gender and duration of injury, it is recommended that interventions be carried out during the first few years of injury, especially for female individuals, to prevent or reduce the severity of behavioral problems.

ObjectivesThis study reports the prevalence of persistent amblyopia (post-traditional treatment age) in schoolchildren in the Republic of Ireland (henceforth Ireland) and Northern Ireland (NI), UK; populations with broadly similar refractive and genetic profiles but different eye-care systems.DesignThis is a population-based observational study of amblyopia and refractive error.SettingRecruitment and testing in primary and post-primary schools in Ireland and NI.ParticipantsTwo groups identified through random cluster sampling to represent the underlying population; Ireland 898 participants (12–13 years old) and NI 723 participants (295 aged 9–10 years old, 428 aged 15–16 years old).Main outcome measuresMonocular logMAR visual acuity (presenting and pinhole), refractive error (cycloplegic autorefraction), ocular alignment (cover test) and history of previous eye care. These metrics were used to determine prevalence and type of amblyopia and treatment histories.ResultsChildren examined in NI between 2009 and 2011 had a significantly lower amblyopia prevalence than children examined in Ireland between 2016 and 2018 (two-sample test of proportions, p<0.001). Using a criteria of pinhole acuity 0.2logMAR (6/9.5 Snellen) plus an amblyogenic factor, 4 of 295 participants aged 9–10 years old (1.3%, 95% CIs 0.4 to 3.6) and 3 of 428 participants aged 15–16 years old (0.7%, 95%CIs 0.2 to 2.2) were identified in NI. The corresponding numbers in Ireland were 40 of 898 participants aged 12–13 years old (4.5%, 95% CI 3.2 to 6.1). In NI strabismic amblyopia was the most prevalent type of persistent amblyopia, whereas anisometropic was predominant in Ireland. In Ireland, amblyopia was associated with socioeconomic disadvantage (OR=2.2, 95%CIs 1.4 to 3.6, p=0.002) and poor spectacle compliance (OR 2.5, 95% CIs 2.0 to 3.2, p<0.001).ConclusionsAmblyopia prevalence persisting beyond traditional treatment ages was significantly lower among NI children compared with Ireland. Uncorrected anisometropia, compliance with spectacle wear and socioeconomic disadvantage were contributing factors in Ireland. Children without obvious visible eye defects were less likely to access eye care in Ireland, resulting in missed opportunities for intervention where necessary.

Abstract Background Early and effective ocular screening may help to eliminate treatable eye disorders. The Lhasa Childhood Eye Study (LCES) revealed the particular prevalence of refractive error and visual impairment in grade one schoolchildren (starting age of 6 years old) in Lhasa. Methods This is a cross-sectional part of school-based cohort study. One thousand nine hundred forty-three children were enrolled (median age, 6.78 years, range, 5.89 to 10.32). Each child underwent general and ocular examinations, including logarithm of the minimum angle of resolution (logMAR) visual acuity, cycloplegic autorefraction, and slit-lamp biomicroscopy evaluation. Multivariate and correlation analyses were performed to evaluate the association between refractive error with gender and ethnics. Results The prevalence of visual impairment (logMAR visual acuity ≥0.3 in the better-seeing eye) of uncorrected, presenting and best-corrected visual acuity (BCVA) was 12.2, 11.7 and 2.7%, respectively. Refractive error presented in 177 (78.0%) out of 227 children with bilateral visual impairment. Myopia (spherical equivalent refractor [SER] ≤ − 0.50 diopter [D] in either eye) was present in 4.7% children when measured after cycloplegic autorefraction. Hyperopia (SER ≥ + 2.00 D) affected 12.1% children. Hyperopia was significantly associated with female gender (P<0.001). Astigmatism (cylinder value ≤ − 0.75 D) was present in 44.8% children. In multivariate regression and correlation analysis, SER had no significant difference between ethnic groups. Conclusion The Lhasa Childhood Eye Study is the first school-based cohort study to reveal the prevalence and pattern of refractive error and visual impairment in Lhasa. Effective strategies such as corrective spectacles should be considered to alleviate treatable visual impairment.

Abstract Objective: To estimate the prevalence of thinness, overweight and obesity among Tibetan adolescents aged 12-17 years. Design: Cross-sectional survey. Setting: Shigatse city of Tibet municipality, with an average altitude of more than 4000 m. Subjects: Study participants included 2,642 adolescents aged 12-17 years selected from 6 schools using a convenient cluster sampling method. Results: The prevalence of thinness/overweight/obesity among Tibetan adolescents was 9.4%/5.4%/1.4% (China definition), 14.7%/4.4%/0.7% (IOTF definition), and 2.8%/5.7%/0.9% (WHO definition). The prevalence of thinness and overweight was significantly different between both sexes based on each of three BMI classification criteria (P <0.001). There was no significant difference in the prevalence of obesity between both sexes according to each of three BMI criteria. There was no clear trend in the prevalence of thinness across ages according to the China or IOTF definition (both P >0.05), whereas an upward trend was observed for thinness in boys according to the IOTF definition (P for trend <0.05). In contrast, the prevalence of thinness tended to decrease with increasing age in girls according to the IOTF definition, and in total sample according to the WHO definition (P for trend <0.05). Conclusions: Among Tibetan adolescents, the prevalence of overweight and obesity is relatively low, while the prevalence of thinness is high, especially in boys. These data suggest urgent attention is needed to control adolescent thinness in Tibet.

Background: Visual impairment is disproportionately distributed between rural and urban dwellers. Rural dwellers have limited access to the eye care services that are available in urban cities.Aim: The aim of this study was to determine the prevalence and causes of visual impairment in Dariyah, a rural community in the Qassim region, Saudi Arabia.Setting: This research comprised a population-based assessment of the vision and visual status of the inhabitants of Dariyah community.Methods: A cross-sectional descriptive study was conducted amongst the inhabitants of the community of Dariyah. A comprehensive eye examination consisting of visual acuity, ocular health examination, objective and subjective refraction was performed. The examinations were conducted by registered ophthalmologists and optometrists from the Qassim University. Vision impairments were categorised according to the International Classification of Diseases (ICD), Section 9D90, ‘Vision impairment including blindness’ (2018).Results: In all, 68 (24.5%) participants had vision impairment (using the better-seeing eye), with refractive errors being the main cause of vision impairment. Other causes of vision impairment included cataract (20, 7.2%), trichiasis (5, 1.8%) and glaucoma (2, 0.7%). Refractive errors were present in 193 (69.7%), with astigmatism being the commonest refractive error. About 60% of those with refractive errors presented without spectacles for correction, and 20% reported that they had never had an eye examination. Refractive errors could not be determined in 19 (6.9%) of the subjects because of conditions such as matured cataract and other ocular abnormalities.Conclusion: Uncorrected refractive error was found to be the leading cause of vision impairment in this rural community in Saudi Arabia.

<p>Hydroxychloroquine (HCQ) is an antimalarial drug used extensively in treatment of autoimmune diseases such as rheumatoid arthritis. Retinal toxicity is the most important side effects of this drug. Even after the drug is discontinued, retinal degeneration from HCQ can continue to progress. Consequently, multiple ophthalmic screening tests have been developed to detect early retinopathy. The aim of the current study was to evaluate the value of central 2-10 perimetry method in early detection of retinal toxicity. This prospective cross-sectional investigation was carried out on 60 rheumatoid arthritis patients, who had been receiving HCQ for at least 6 months and still were on their medication (HCQ intake) at the time of enrollment. An ophthalmologist examined participants using direct and indirect ophthalmoscopy. Visual field testing with automated perimetry technique (central 2-10 perimetry with red target) was performed on all included subjects twice in 6 months interval: The first one at the time of enrollment and the second one 6 months later. Males and females did not show any significant difference in terms of age, duration of therapy, daily and cumulative HCQ dose, anterior or posterior segment abnormalities, hypertension, body mass index, and best corrected visual acuity. Anterior segment was abnormal in 9 individuals including 3 subjects with macular pigmentary changes, 4 individuals with cataract and 2 cases with dry eyes. Moreover, 12 subjects had retinal pigmented epithelium (RPE) in their posterior segments. After 6 months, depressive changes appeared in 12 subjects. Additionally, HCQ therapy worsened significantly the perimetric results of 5 (55.6%) patients with abnormal anterior segment. A same trend was observed in perimetric results of 6 (50.0%) subjects with abnormal posterior segments (P=0.009). The daily dose of HCQ (P=0.035) as well as the cumulative dose of hydroxychloroquine (P=0.021) displayed statistically significant associations with perimetric results. Central 2-10 perimetry is a useful method for early detection of HCQ retinal toxicity, but more comprehensive studies, with larger sample size, longer-term follow-up and more precise techniques are mandatory to confirm HCQ retinal toxicity.</p>

Background: Postural stability is quite mandatory when practicing high-performance sports. Investigations of postural stability and related variables in judokas with visual impairment can lead to new training plans targeting the improvement of postural stability and ultimately to enhanced performance. Objectives: To investigate postural stability and its relationship with anthropometric measurements, body composition, and experience in judokas with visual impairment. Methods: Seventeen judokas (70.6% men) with visual impairment participated in this cross-sectional study. The athletes were grouped based on the functional classification of partial (B2/B3, n = 10) and total visual impairment (B1, n = 7). Postural stability was assessed using the elliptical area of the 95% confidence interval (Area) and the average displacement velocity (Vavg) while remaining in a bipedal stance with eyes closed and blindfolded. Body mass, height, circumferences, skinfold thickness and diameters were measured and used to estimate body composition. Between-group comparisons were evaluated using the Mann-Whitney test. Bivariate correlations were determined with Spearman’s correlation coefficient with bootstrap analysis and 95% confidence interval (95% CI) from 500 resamplings. Results: No significant difference was observed between the B1 and B2/B3 groups in relation to postural stability (Area; P = 1.00; Vavg; P = 0.85). Postural stability (Area but not Vavg) correlated positively and moderately (P < 0.05) with anthropometric measurements and negatively with judo experience (practice time). Conclusions: The postural stability of judokas was unrelated to the degree of visual impairment. Postural instability was correlated with anthropometric measurements, mainly body fat and height, and judo experience.

Aim: To study the prevalence of myopia among school children in Aba, Nigeria.Methods: This cross-sectional study was conducted in public and private (primary and secondary) schools. A multi-stage random sampling technique was used for selecting participants aged between 8 and 15 years from 12 schools in Aba, Nigeria. Data were analysed for 1197 children who underwent a comprehensive eye examination. The children were divided according to the following criteria: age groups (group 1 [8–11 years] or group 2 [12–15 years]), gender (male or female), level of education (primary or secondary) and type of school (public or private). Myopia was defined as spherical equivalent refraction (SER) ≤ -0.50 D in the poorer eye.Results: The prevalence of myopia was estimated to be 2.7%. Of the 96 children with refractive error, 78.1% were uncorrected. In using logistic regression analysis, risk of developing myopia was associated with older age groups (odds ratio [OR]: 1.20; 95% confidence interval [CI], 0.16–9.11; p < 0.010) and higher level of education (OR: 1.73; 95% CI, 1.05–2.86; p < 0.030). There was no significant difference in myopia prevalence between male and female children (p = 0.89).Conclusion: Although the prevalence of myopia and overall prevalence of refractive error in school children in Aba were low, the high prevalence of uncorrected refractive error is a significant public health problem. An effective and sustainable children’s vision screening programme is needed to prevent visual impairment and blindness.

This study aimed to investigate whether mass drug administration (MDA) intervention has an equivalent effect on reducing the prevalence and intensity of Schistosoma haematobium infection regardless of the baseline values. A repeated cross-sectional survey was performed targeting students of 12 primary schools in Al Jabalain and El Salam districts of White Nile State, Sudan, at both 1 week before and 8 months after the MDA. Prior to the baseline survey, school-aged children in Al Jabalain had received MDA interventions twice in 4 years, while those in El Salam had not. The baseline prevalence was 9.1% in Al Jabalain and 35.2% in El Salam, which were reduced to 1.8% and 5.5% at 8 months after the MDA, respectively. The corresponding reduction rates were 80.3% and 84.4%, not significant difference between both districts. However, changes in the geometric mean intensity (GMI) of egg counts were significantly different between both districts. The baseline GMIs were 14.5 eggs per 10 ml of urine (EP10) in Al Jabalain and 18.5 EP10 in El Salam, which were reduced to 7.1 and 11.2 EP10 after treatment, respectively. The corresponding reduction rates were 51.0% and 39.5%. In conclusion, MDA interventions were found to bring about similar relative reduction in prevalence regardless of the baseline value; however, the relative reduction in infection intensity was more salient in the district with a low baseline value for both prevalence and intensity. This clearly points to the importance of repeated MDA interventions in endemic areas, which will eventually contribute to schistosomiasis elimination.

 Refractive error is a defect in the optical system of the eye which prevents light from being brought to a single point focus on the retina, thus reducing normal vision. This optical defect is the second most common cause of visual impairment globally as well in Nepal. At present, there are 285 million visually impaired people in the world. An estimated 4 out of 5 visual impairment (80%) can be prevented or cured, uncorrected refractive errors are the leading cause (42%) followed by cataract (33%). A descriptive cross sectional study was conducted to determine the proportion of undergraduate medical students with refractive errors in Nepal Medical College, and to identify factors associated with it. A total of 210 medical students volunteered for this study, with 100 males (47.6%) and 110 females (52.4%). The age of these students were between 18 to 26 years, with an average of 20.5 years. The proportion of medical students with refractive error was slightly more than half (51.4%), with simple myopia being the leading type (42.9%), followed by astigmatism (7.1%) and simple hypermetropia (1.4%). Parental history of refractive error was observed to be significantly associated with that of the medical students. There was also a significant association between refractive error and the daily use of mobile phones and laptops. However, years spent in medical education were not observed to be significant.
