La dinapenia pediátrica es una condición seria que ha visto incrementada su prevalencia entre los niños de hoy en día. Este fenómeno está caracterizado por bajos niveles de fuerza y potencia que no son consecuencia de una enfermedad y que generan limitaciones funcionales en el rendimiento de las habilidades motrices, así como alteraciones cardiometabólicas. Para evitar esta condición e incrementar la salud y el bienestar de los niños y adolescentes es necesario incluir, dentro de la práctica regular de actividad física, un programa de entrenamiento de fuerza. Se ha comprobado que el entrenamiento de fuerza tiene un papel fundamental a la hora de prevenir y controlar la dinapenia pediátrica. No obstante, requiere de una combinación específica entre investigadores, médicos y entrenadores de forma que permita diseñar e implementar de forma segura programas de entrenamiento de fuerza progresivos y adecuados a las necesidades y habilidades de los niños. Esta revisión recoge las principales aportaciones respecto al entrenamiento de fuerza pediátrico y proporciona datos basados en la evidencia para prevenir y controlar la dinapenia pediátrica en los niños.  La participación regular en programas de entrenamiento de fuerza contribuirá a prevenir la dinapenia pediátrica en las primeras etapas de la vida. Por tanto, los niños tendrán más posibilidad de ganar confianza y de adquirir las competencias necesarias para aumentar sus posibilidades físicas y de esta forma podrán cumplir en mayor y mejor medida la cantidad de ejercicio diario recomendado. La intervención se hace especialmente necesaria para mejorar los niveles de fuerza y potencia en los niños ya que, de no ser así, se dificultará en gran medida la posibilidad de prevenir una cascada de consecuencias adversas para la salud en etapas posteriores.Abstract. Pediatric dynapenia is a serious condition with a growing prevalence within youth nowadays. This phenomenon is characterized by low levels of strength and power not provoked by any disease, which produce functional limitations in motor skill performance and consequent cardio-metabolic abnormalities. In order to improve this condition and enhance the health and well-being of children and adolescents, regular participation in physical activities including resistance training is needed. The unique role of resistance training in preventing and managing pediatric dynapenia implies a joint effort from researchers, clinicians and practitioners in order to design, implement and safely progress developmentally appropriate resistance exercise programs consistent with each child’s needs and abilities. This review synthesizes the latest information on youth resistance training and provides evidence-based rationale for preventing and managing pediatric dynapenia in youth. Regular participation in resistance exercise programs in early ages prevent pediatric dynapenia. Consequently, youth will be more likely to gain confidence and competence on their physical abilities and accumulate the recommended amount of daily exercise. Interventions in schools and communities are needed to increase muscular strength and power in youth, with the aim to prevent inevitable cascade of adverse health consequences later in individuals’ life.

The good news is that a growing body of evidence recognizes resistance training as foundational to long-term physical development. Original research and reviews published in 2017 conclude that early exposure to developmentally appropriate resistance training can improve markers of health, increase muscular fitness, enhance physical literacy, and reduce the risk of injury in young athletes. Although the papers discussed in the commentary add to our understanding of the pleiotropic benefits of youth resistance training, they also raise concerns. As measures of muscular strength and power have been found to track from childhood to adulthood, the bad news is that youth with low levels of muscular fitness tend to become weak adults who are at increased risk for functional limitations and adverse health outcomes. Furthermore, global participation in youth resistance training is falling far short of public health recommendations, and these ugly trends will likely impact the health and well-being of future generations. A change in current attitudes and common practices is urgently needed to educate parents, practitioners, and clinicians about the potential benefits of resistance training for all children and adolescents, not only young athletes.

SummaryGlobal health reports indicate that a vast majority of children and adolescents are not accumulating the recommended amount of moderate to vigorous physical activity (MVPA) daily. This decline in MVPA has coincided with troubling temporal trends in muscular fitness in modern day youth. Since a prerequisite level of muscular strength is needed to move proficiently, developmentally appropriate interventions that target neuromuscular deficits are needed to prepare girls and boys for ongoing participation in active play, exercise and sport activities. Instead of focusing exclusively on aerobic activities, the promotion of youth physical activity should recognize the shared importance of strength, skill and aerobic activities for all youth regardless of body size or physical prowess. In addition to enhancing muscular fitness and improving motor skill performance, regular participation in resistance training has been found to have a favorable influence on musculoskeletal strength, cardiometabolic function, body composition, and mental health in children and adolescents. A new mindset that recognizes the critical importance of strength-building activities early in life is needed before this generation of girls and boys becomes resistant to exercise interventions later in life.

Androgen-deprivation therapy (ADT) for prostate cancer (PCa) has side effects that significantly impair health-related quality of life (HRQOL). Exercise ameliorates many side effects of ADT, but different modalities, particularly in the home-based setting, have not been well studied. In this study the authors randomly assigned 66 PCa survivors receiving ADT to 6 mo of home-based aerobic or resistance training. Psychosocial well-being and physical fitness were measured at baseline, 3 and 6 mo, and then 6 mo postintervention. Intention-to-treat analyses showed that fatigue and HRQOL were not significantly different between groups; however, in a per-protocol analysis the resistance-exercise training group demonstrated clinically significant improvements in HRQOL. Differential within-group effects on physical fitness were also observed at various time points. At all time points, the aerobic-training group engaged in significantly more physical activity than the resistance-training group, a finding that should be further examined given evidence-based guidelines for activity volume in cancer survivors.

AbstractPhysical activity at workplace can positively impact various wellbeing outcomes yet developing and implementing exercise programs that are straightforward, time-efficient and widely applicable remains a notable public health challenge. Sport4Health Network (SPORT4H) project co-funded by the European Union Erasmus+ programme unites health and sport professionals in an effort to encourage participation in physical activity among working population and reduce health risk factors for lifestyle diseases. A two-day SPORT4H scientific forum on non-traditional types of work-place exercise interventions was organized from 14th to 15th September 2020, to critically evaluate evidence on stretching and resistance exercise programs targeted to working population in aim to identify knowledge gaps and future areas of research and application. Evidence on traditional interventions (e.g., walking initiatives, active travel) appears more robust while only few studies evaluated the applicability of non-traditional PA programs in working population. However, we identified a moderate-to-strong link between non-traditional PA programs at the workplace and several health-related physical fitness indices, with resistance exercise turned out to be superior to other exercise interventions analyzed. It appears that low-volume high-repetition resistance exercise favorably affects musculoskeletal disorders, work performance and health-related quality of life in employees who exercised at least 3 times per week for over 8 weeks. In terms of safety, screening protocols should employ health-related questionnaires, adopting a progressive training load, and prescribing training programs to individual participants’ needs. Implementing non-traditional PA programs aimed to improve health-related physical fitness and counteract sedentary behavior at workplace might be therefore of utmost importance to contribute to health promotion in this sensible population.

Traditional fears and misinformed concerns regarding youth resistance training have been replaced by scientific evidence that indicates regular participation in well-designed resistance-training programmes can be safe and effective for both children and adolescents. In addition to increasing muscular strength and power, regular participation in a structured resistance training-programme can increase bone mineral density, improve cardiovascular risk factors, fuel metabolic health, facilitate weight control, enhance psychosocial well-being, and prepare youth for the demands of daily physical activity and sport. An integrative approach to training, grounded in resistance exercise and motor skill development, can optimize children’s fitness potential and maximize their athletic performance while reducing the risk of sports-related injury. Qualified professionals are needed to plan, implement and progress developmentally appropriate resistance training to attain a level of muscular fitness that facilitates long-term physical development.

The aim of this study was to carry out a literature review on the overall benefits of resistance training (RT) after stroke and undertake a critical analysis of the resistance exercise programs surveyed (rest interval between sets and exercises, number of sets, number of repetitions, intensity, duration of training, and weekly frequency). To obtain articles for the review, we searched PubMed, Google Scholar, and Physiotherapy Evidence Database (PEDro). Inclusion criteria were considered using the PICO (population, intervention, control/comparison, and outcome variables) model. The following characteristics were recorded for all articles: type of study, author, year of publication, participants (time after stroke, sample size, and age), benefits of RT, and structured resistance exercise programs. Positive effects of training were found on anxiety status, quality of life, muscle hypertrophy, cognitive function, strength, and muscle power. Only 5 studies described the main variables of RT in detail. Lack of control of some variables of RT may negatively affect the results of this practice. The findings of the present study may further inform health and physical conditioning professionals on the importance and necessity of using the main variables in the search for benefits for individuals with stroke.

IntroductionThere are 3.9 million people in the UK with diabetes. Sarcopenia, increased frailty and loss of independence are often unappreciated complications of diabetes. Resistance exercise shows promise in reducing these complications in older adult diabetes patients. The aim of this feasibility randomised controlled trial is to (1) characterise the physical function, cardiovascular health and the health and well-being of older adults with mild frailty with/without diabetes treated with insulin, (2) to understand the feasibility and acceptability of a 4-week resistance exercise training programme in improving these parameters for those with diabetes and (3) to test the feasibility of recruiting and randomising the diabetic participant group to a trial of resistance training.Methods and analysisThirty adults aged ≥60 years with insulin-treated diabetes mellitus (type 1 or 2), and 30 without, all with mild frailty (3–4 on the Rockwood Frailty Scale) will be recruited. All will complete blood, cardiovascular and physical function testing. Only the diabetic group will then proceed into the trial itself. They will be randomised 1:1 to a 4-week semisupervised resistance training programme, designed to increase muscle mass and strength, or to usual care, defined as their regular physical activity, for 4 weeks. This group will then repeat testing. Primary outcomes include recruitment rate, attrition rate, intervention fidelity and acceptability, and adherence to the training programme. A subset of participants will be interviewed before and after the training programme to understand experiences of resistance training, impact on health and living with diabetes (where relevant) as they have aged. Analyses will include descriptive statistics and qualitative thematic analysis.Ethics and disseminationThe North East-Newcastle and North Tyneside 2 Research Ethics Committee (20/NE/0178) approved the study. Outputs will include feasibility data to support funding applications for a future definitive trial, conference and patient and public involvement presentations, and peer-reviewed publications.Trial registration numberISRCTN13193281.

Functional fitness (FF) and gait ability in older populations have been associated with increased survival rates, fall prevention, and quality of life. One possible intervention for the improvement of FF is well-structured exercise programs. However, there are inconsistent findings regarding the effects of exercise interventions in the maintenance of gait parameters. The aim of this protocol is to develop a community-based exercise intervention targeting an older population. The intervention aim is the improvement of gait parameters and FF. A control trial with follow-up will be performed. The primary outcome variables will be plantar pressure gait parameters. The secondary outcome variables will be aerobic endurance, lower limb strength, agility, and balance. These variables will be recorded at baseline and after 12, 24, and 36 weeks, in the intervention and control groups. If effective, this protocol can be used by exercise professionals in improving community exercise programs.

Purpose To present an overview of exercise interventions in cancer patients during and after treatment and evaluate dose-training response considering type, frequency, volume, and intensity of training along with expected physiological outcomes. Methods The review is divided into studies that incorporated cardiovascular training, combination of cardiovascular, resistance, and flexibility training, and resistance training alone during and after cancer management. Criteria for inclusion were based on studies sourced from electronic and nonelectronic databases and that incorporated preintervention and postintervention assessment with statistical analysis of data. Results Twenty-six published studies were summarized. The majority of the studies demonstrate physiological and psychological benefits. However, most of these studies suffer limitations because they are not randomized controlled trials and/or use small sample sizes. Predominantly, studies have been conducted with breast cancer patients using cardiovascular training rather than resistance exercise as the exercise modality. Recent evidence supports use of resistance exercise or “anabolic exercise” during cancer management as an exercise mode to counteract side effects of the disease and treatment. Conclusion Evidence underlines the preliminary positive physiological and psychological benefits from exercise when undertaken during or after traditional cancer treatment. As such, other cancer groups, in addition to those with breast cancer, should also be included in clinical trials to address more specifically dose-response training for this population. Contemporary resistance training designs that provide strong anabolic effects for muscle and bone may have an impact on counteracting some of the side effects of cancer management assisting patients to improve physical function and quality of life.

Functional limitations—difficulty in carrying out activities of daily living—have been linked to poorer well-being in Western cultures. This might be partly due to the lower personal control associated with functional limitations. However, compared with the West, in Asian cultural contexts (e.g., Japan) where agency and control are based less predominantly on individual attributes, the link between functional limitations and well-being may be weaker. Using cross-sectional probability samples from the United States and Japan (Study 1), functional limitations were associated with lower well-being in both cultures, though the association was weaker in Japan than in the United States and personal control played a mediating role. Furthermore, analyses of longitudinal data (Study 2) showed the cross-cultural patterns generally consistent with the cross-sectional analyses of Study 1, though the cultural moderation was found for fewer well-being measures. Such findings enrich our understanding of how health status and well-being are related across cultures.
