Introduction. Silicon is a unique element in its physicochemical properties, almost ubiquitous in water supply sources but is not normalized in drinking water in the world practice of ensuring the harmlessness of water use of the population. In our country, the half-century history of the development of hygienic silicon standards in the water began with the justification of the MPC of silicon-containing reagents used in water treatment. However, over time, doubts have arisen about the suitability of manufactured silicon standards for controlling natural silicon in drinking water. Proponents of the harmlessness of natural soluble forms of silicon advocated the elimination of silicon standards in water. In contrast, researchers of the action of silicon in silicon biogeochemical provinces have constantly revealed its adverse effects on the health of the population living in these regions, confirming this with animal experiments. methods. Literature search methods on Scopus, CyberLeninka, PubMed databases: selective, analytical-synthetic, typological. the main part. The review provides information on soluble forms of silicon, their stability and bioavailability, examines the retrospective and current state of hygienic rationing of silicon in water, discusses the shortcomings of the currently existing MPC of silicon, sodium and potassium silicates in drinking water. A detailed review of studies carried out in the silicon biogeochemical provinces of Chuvashia is given, where the inhabitants of the ecological disaster zone, who consumed drinking water with a profound imbalance of macro-and microelements and a high silicon content relative to the optimum area, observed various metabolic disorders (mineral, lipid, carbohydrate, peroxide), changes in microbiocenosis of the large intestine and immune status, an increase in the incidence rate of the adult population with chronic non-infectious diseases by 2-3 times against the national average, the highest incidence rates in children of all age groups. Conclusions. The review draws attention to the need for expanded studies of the effect on the body of natural silicon in drinking water with the usual balance of trace elements to resolve issues about the standards for natural silicon at the federal and regional levels and to establish differentiated standards for silicon-containing reagents in drinking water.

SUMMARYThe reactivation of dormant alpha-human herpesvirus (αHHV) has been attributed to various causes often referred to as stressors. However, no clinical study investigating the relationship between stressors and reactivation exists in humans at this time. Herpes simplex virus type-1 (HSV-1), an important αHHV, was shown to have its gene expression and replication regulated by thyroid hormone (TH) using molecular biology approaches. Varicella zoster virus (VZV) is categorized in αHHV superfamily and shares similar homology with HSV-1. We hypothesize that a history of TH imbalance may be associated with the incidence of shingles (VZV reactivation). This current pilot study, based on a hospital medical claims database, was conducted as a retrospective case-controlled investigation to determine if a putative link between TH imbalance and incidence of shingles is present. An odds ratio of 2·95 with a χ2 value of 51·74 was calculated for the total population diagnosed with TH disruption and shingles. Further analyses indicated that African American males exhibited a much higher chance of simultaneous diagnoses. These results show that a TH imbalance history may affect VZV reactivation at different incidence rates in different races and age groups.

Aim of the study. Analysis of the dynamics of the alimentary-dependent prevalence in children from various age groups in the industrial center. Materials and methods. The study was conducted retrospectively according to official statistical reporting. The total and primary incidence in children and adolescents in the city of Angarsk was estimated for 6 years (2013-2018). Age-specific features of the incidence in children were analyzed over 3 years (2016-2018). Relative indices of the incidence were calculated (per 1000 population of the corresponding age), characteristics of the time series: the average annual absolute increment; the rate of average annual growth (decrease); linear regression models were built using the Excel software module. Results. An analysis of the primary morbidity of alimentary-dependent pathology in children of different ages showed that its structure is characterized by a predominance of diseases of the digestive, endocrine systems, and blood. Moreover, in young children, blood diseases are most pronounced. Assessment of the dynamics of alimentary-dependent diseases in children aged 0-14 and 15-17 years revealed multidirectional trends. In children aged 0-14 and 15-17 years, the presence of pronounced trends to a decrease in the incidence of the pathology of the digestive system was revealed. The differences are that pronounced trends in the increase in the incidence are observed in children aged 0-14 years - in the frequency of obesity, in children 15-17 years old - in the frequency of endemic goiter. Discussion. The obtained values of the regression analysis allow predicting an increase in the frequency of diseases of the thyroid gland associated with iodine deficiency in children aged 0-14 years, obesity - in 15-17 year adolescents. Differences in the frequency of these nosological forms depending on the age of the children can be presumably due to changes in eating behavior, the influence of heredity, and the adaptive capabilities of the body. The incidence rates of alimentary-dependent pathology depend on the quality of medical care and the geochemical characteristics of the territory of residence. Conclusion. The study revealed the age-related characteristics of alimentary-dependent pathology and concretized the directions for improving medical and preventive activities.

e13088 Background: Venous thromboembolism (VTE) is a common complication in cancer patients, but contemporary data on the incidence and characteristics of cancer-associated VTE in different racial populations is limited. We sought to measure the incidence rate and characteristics of cancer-associated VTE in a racially diverse population. Methods: We performed a prospective, population–based cohort study of VTE in Oklahoma County, OK during April 1, 2012–March 31, 2014. Surveillance staff regularly visited all tertiary care and relevant outpatient facilities in the county and reviewed the text from all imaging results of chest computed tomography or magnetic resonance imaging, lung perfusion scans, and ultrasound of the extremities to identify cases of VTE. Data on patient characteristics were collected from the electronic medical record. VTE was considered cancer-associated if the patient had a history of cancer other than basal or squamous cell skin cancer. Cancer was defined as “active” if metastatic or diagnosed within 6 months prior to VTE. Associations and incidence were calculated using the chi-square test and Poisson regression, respectively. Results: Of 3,255 patients with ≥1 VTE event (deep-vein thrombosis and/or pulmonary embolism), 732 (23%) had a history of cancer, of whom 309 had active cancer. The types of cancer varied widely. The age-adjusted incidence of cancer-associated VTE was 648 per 100,000 adult population (age ≥ 18). Incidence increased with each decade of age. The racial/ethnic-specific incidence rates were as follows (each per 100,000): Whites non-hispanic (607), Blacks non-Hispanic (557), Native Americans (128), Asian/Pacific Islanders (113) and Hispanics (70). Risk factors for VTE among the 732 with cancer-associated VTE included previous VTE (19%), hospitalization (37%), surgery (33%), and central venous catheter (22%). Of those with a history of cancer ≥ 6 months previously, 32% had no other identifiable VTE risk factor. Conclusions: Cancer-associated VTE comprised about 20-25% of the total population burden of VTE. The incidence varied substantially by race/ethnicity. An appreciable proportion of those with a history of cancer ≥ 6 months previously subsequently developed VTE in the absence of other risk factors.

Abstract Background The reported incidence and prevalence of atrial fibrillation (AF) has been inconsistent among studies. Purpose We aimed to study time trends in incidence (first time) of AF hospitalizations or AF deaths in Norway in the period 2004–2014 by age and sex. Methods Nationwide hospital discharge diagnoses in the Cardiovascular Disease in Norway (CVDNOR) database and in the National Patient Registry were linked to the National Cause of Death Registry. All hospitalizations with AF as primary or secondary diagnosis and out-of-hospital deaths with AF as underlying cause (ICD-9: 427.3 or ICD 10: I48; AF or atrial flutter) in individuals ≥18 years were obtained during 1994–2014. Incident AF was defined as first hospitalization or out-of-hospital death due to AF with no previous hospitalization for AF the past 10 years. Age-standardized incidence rates with 95% confidence intervals (CIs) were calculated using direct standardization to the age-distribution in the Norwegian population per Jan 1st 2004. Age-adjusted average yearly incidence rate ratios (IRR) with 95% CIs were estimated by Poisson regression analyses. Accumulated prevalence during 1994–2014 was assessed in Norwegian residents 18 years and older per Dec 31st 2014. Results During 39,865,498 person years of follow up from 2004 to 2014 we identified 175,979 incident AF cases of which 30% were registered with AF as primary diagnosis, 69% as secondary diagnosis and 1% as out-of-hospital cause of death. The age-standardized incidence rate of AF hospitalization or out-of-hospital death per 100,000 person years was stable at 433 (426–440) in 2004 and 440 (433–447) in 2014. IRR were stable or declining across age groups of both sexes, except for the youngest age group 18–44 years, where incidence rates of AF hospitalization or out-of-hospital death increased by 2% per year, IRR 1.02 (1.01, 1.03). By 2014, the prevalence of AF assessed from hospital or death records was 2.9% in the adult population 18 years and older. Conclusion We found overall stable incidence rates of AF from 2004 to 2014 in the adult Norwegian population. BackgroundDepression is known to run in families, but the effects of parental history of other psychiatric diagnoses on depression rates are less well studied. Few studies have examined the impact of parental psychopathology on depression rates in older age groups.MethodWe established a population-based cohort including all individuals born in Denmark after 1954 and alive on their 10th birthday (N = 29 76 264). Exposure variables were maternal and paternal history of schizophrenia, bipolar disorder, depression, anxiety or ‘other’ psychiatric diagnoses. Incidence rate ratios (IRRs) were estimated using Poisson regressions.ResultsParental history of any psychiatric diagnosis increased incidence rates of outpatient (maternal: IRR 1.88, p < 0.0001; paternal: IRR 1.68, p < 0.0001) and inpatient (maternal: IRR 1.99, p < 0.0001; paternal: IRR 1.83, p < 0.0001) depression relative to no parental history. IRRs for parental history of non-affective disorders remained relatively stable across age groups, while IRRs for parental affective disorders (unipolar or bipolar) decreased with age from 2.29–3.96 in the youngest age group to 1.53–1.90 in the oldest group. IRR estimates for all parental diagnoses were similar among individuals aged ⩾41 years (IRR range 1.51–1.90).ConclusionsParental history of any psychiatric diagnosis is associated with increased incidence rates of unipolar depression. In younger age groups, parental history of affective diagnoses is more strongly associated with rates of unipolar depression than non-affective diagnoses; however, this distinction disappears after age 40, suggesting that parental psychopathology in general, rather than any one disorder, confers risk for depression in middle life.

This text focuses on the situation of suicide in Brazil, defines and quantifies information, and presents a description of the main risk factors, as well as a reflection on the phenomenon and the possibilities for prevention. Fatal suicide is a serious public health problem. In 2012, 172 member states of the World Health Organization registered 804,000 self-inflicted deaths, representing an annual rate of 11.4/100,000, of which 15/100,000 men and 8.0/100,000 women. Consummate suicide rates are unevenly distributed globally, within countries, according to sex and according to age groups. The mortality rate is highest in Asia (17.7/100 thousand inhabitants), followed in Europe (12/100 thousand inhabitants). The Americas have a mortality rate of 7.3/100 thousand inhabitants (WHO, 2014). In Brazil, with an unevenly distributions between the regions, gender and ages, the total rate is 4.5/100,000. In the country and everywhere, risk factors are classified as medical, psychiatric and psychological, micro social, social and environmental. The history of the occurrence of suicides shows that it is possible to prevent them and to reduce the incidence rates. This requires investment in local diagnostics and multidisciplinary action. Given the delicacy of the problem and the taboos that surround it, the protection network for people at risk for suicide needs to be constantly in the process of training and taking action. As national and international surveys show, at least two-thirds of the individuals who tried or committed suicide had somehow communicated to friends, family, acquaintances or health professionals their intention to kill themselves.

Introduction. University pedagogical activity and unfavorable factors typical for the educational process have a negative impact on the health and contribute to the morbidity of teachers Objective. To evaluate the influence of occupational activity on the resistance of the body of medical school teachers. Material and Methods. Objects of study: teachers of Karaganda Medical University of three age categories (up to 30 years, 30-49 years and 50 years or more). An analysis of the incidence was carried out on sheets of temporary disability. The sociological survey was conducted on the questionnaires to assess the preventive, medical activity and self-assessment of the teachers’ own health. Statistical processing was made with the use of Statistica 8.0. Results. The incidence rate of university professors with temporary disability due to underestimated cases of diseases was revealed to be hidden (latent) in nature. A questionnaire survey revealed that 66.6% ÷ 85.0% of respondents had health problems. Only half of the respondents (55.6 ÷ 45.7%) rated their state of health as good, but this rating decreased with age (21.9%). According to nosology, in the structure of diseases among teachers of medical schools in the second and third age groups (30-49 years and 50 years and more), diseases of the cardiovascular system (52.6% and 61.9%, respectively), the musculoskeletal system (55.3% and 47.6%), the digestive system (36.4% ÷ 42.9%). The main reasons for the low level of health and the high incidence of medical teachers were: insufficient physical activity, poor nutrition, insufficient attention to their own health, failure to comply with treatment-and-prophylactic recommendations, self-treatment. Conclusion. An assessment of the resistance of an organism of university teachers in terms of incidence rates with a temporary disability, based on disability records, cannot be a reliable criterion. Using sociological methods of a questionnaire survey allows getting reliable information about the true incidence rates, identifying the causes of their growth, and can also be a related tool in the development of medical and preventive measures aimed at preserving the health of university teachers.

Background: Avascular necrosis (AVN) or osteonecrosis is the death of bone tissues due to decreased blood flow of femoral head, it is a major orthopaedic disease with many risk factors including the chronic alcohol and tobacco consumption.  Aims and objectives: To study the relationship of alcohol and tobacco with AVN of femoral head in adult population. Materials and Methods: This study included 72 patients of AVN of femoral head (ONFH) who attending our hospital between Sept 2018 to Sept 2020. History of alcoholism and tobacco consumption, detailed information of the amount and duration of smoking and alcohol intake was recorded. Results: Majority of the AVN patients were working age groups males (72%) and 28% were females. Out of 72 patient 55% had history of alcohol consumption and 60% were smokers which indicates a positive correlation of cellular toxicity with AVN of femoral head. Conclusion: Present study conclude that the alcohol ingestion and tobacco smoking are the major cause of cellular toxicity and AVN of femoral heads. In order to assess the epidemic situation in the diagnosis of infection caused by the Epstein-Barr virus, a retrospective epidemiological analysis of the incidence of infectious mononucleosis for the period from 1990 to 2018 was conducted in the Moscow region using generally accepted methods of statistical data processing. The description of the clinical case was based on the results of dynamic observation of the patient reflected in the outpatient chart. It is shown that against the background of increasing morbidity rates, there is an epidemiological problem with infectious mononucleosis. Despite the fact that children 0–14 years old are traditionally at risk for infectious mononucleosis, the epidemic process involves the adult population, whose incidence rates also tend to increase. The growth rate of in dicators in the age groups 0–14 and 15 years and older is comparable. The active form of EBV infection in adults can occur in the form of reactivation, which is illustrated by the clinical case described in the article. This case of reactivation of EB infection in an adult clearly illustrates the need to assess the current epidemic situation in differential diagnosis and inform clinicians of different specialties on the epidemiology of EB infection.

Background: Numerous studies have demonstrated a steadily growing trend in the prevalence of obesity around the world. The objective of this study was to assess obesity incidence rates among various age groups of the population of the Voronezh Region in 2010–2017. Materials and methods: We used official data of the Voronezh Regional Information Fund, Registration Form No. 12 of the Russian Federal State Statistics Service, as well as outpatient cards and medical histories of 685 adults and 269 adolescents. Results: We established that the number of obese adults prevailed. The study period of 2010–2017 was characterized by a steady increase in the incidence and prevalence rates of obesity across all age groups of the population: we observed a 1.5, 2.6 and 4.5-fold rise of the incidence rates in children, adolescents and adults, respectively; the percent increase in the prevalence rates was slightly lower and amounted to 34.51% in children, 70.09% in adolescents, and 146.50% in adults. The highest obesity rates were registered among adolescents: 5.02% of them were obese and the study of a random sample confirmed the official statistics. At the same time, the results of the random obesity study in adults demonstrated a tenfold excess of the registered rate. Ranking of the territory of the Voronezh Region by obesity prevalence rates and the analysis of long-term average rates of increase in overall morbidity over the study period showed uneven distribution of obesity by administrative districts of the region in all population groups. Conclusions: The study demonstrated a pronounced upward trend in the incidence of obesity in all population groups of the Voronezh Region and underreporting of this pathology in the adult population of the region. Both findings indicate the need for immediate evidence-based actions aimed at managing the incidence based on data on the actual prevalence of the pathology, identifying the leading risk factors, adjusting diet and physical activity of various population groups, and assessing the efficiency of preventive measures.
