Objective: To study the role of Indomethacin in patients with Idiopathic Polyhydramnios who were symptomatic. Methods: A prospective study was conducted in Prathima Institute of Medical Sciences, Karimnagar from September 2017 to May 2019. Eighteen patients with Idiopathic Polyhydramnios who presented with respiratory embarrassment, premature opening of os or presence of uterine activity were given Indomethacin orally at a dose of 25mg 6th hourly. The significance of changes in AFI was tested by paired t test. Results : There was significant improvement in the symptoms in all the eighteen patients. The mean age of the patients in our study was 26.4±5.5 years. The majority 14(77.7%) of women were multigravida. Mean gestational age at presentation was 30.78±1.56 weeks. Mean AFI at presentation was 31.56±3.68 cm. Indomethacin was given orally at a dose of 25mg, every 6th hourly for a mean duration of 22.5±6.38 days. Mean AFI at the end of treatment was 20.28±5.64 cm. Pregnancy was prolonged by an average of 5.5±1.89 weeks. The difference in AFI at the end of treatment was statistically significant (p<0.05). Oligohydramnios was seen in one patient. Mean gestational age at delivery was 36.28±1.41 weeks. 17(94.4%) of them had good neonatal outcome. There was one neonatal death at< 7 days of birth. Conclusion: Indomethacin significantly decreases amniotic fluid volume in patients with Idiopathic Polyhydramnios and relieves symptoms but close surveillance is necessary.

Objective: Not only has the prevalence of enteric fever due to Salmonella paratyphi A increased over the last decade, there has also been a change in common presenting symptoms and antibiotic sensitivity pattern of these organisms. Knowledge of the existing epidemiology of the disease is essential for a rational approach to treat the same. Hence the present study was done to establish the existing epidemiology of enteric fever due to Salmonella paratyphi A-their common presenting symptoms and antibiotic sensitivity pattern- at our setting. Material & Methods: This prospective study was conducted in MS Ramaiah Hospitals, Bangalore between January 2008 and December 2008. Patients admitted to the hospital with clinical suspicion of enteric fever and whose blood culture grew paratyphi A were included in the study. Common presenting symptoms and signs were recorded in these patients and the sensitivity patterns of the causative organisms were studied. The difference in their response to Ciprofloxacin and Ceftriaxone was studied. The analysis was done using the statistical software package-SPSS Version 16. The difference in the defervescence period was calculated by 2-way Independent two-sample t-test. Results: A total 32 patients were included in the study. Fever and hepatomegaly were the most common symptom and sign among the study subjects. All isolates were susceptible to commonly used antibiotics except for Nalidixic acid. The mean defervescence period was shorter in patients treated with Ceftriaxone as compared to those treated with Ciprofloxacin (p<0.002). Conclusion: We report a change in the presentation and antibiotic sensitivity of paratyphi A infection as compared to existing literature. Ceftriaxone is a better drug as choice as it has shorter defervescence time. AbstractBody part have an important role in various activities, excess activity can cause disturbance. Musculoskeletas disorder that commonly occur due to continuous activity are tendinitis, bursitis and tenosinovitys. De Quervain Syndrome is a tendinitis disorder caused by inflammation of the tendon sheath of the muscle in the m.abductor policis longus and m. Extensor policis brevis due to thickening of the extensor retinaculum, an important role of physiotherapy in the rehabilitation of movement disorder, namely use of modalities. The purpose of tis study is te see the description of the provision of exercise terapy in improving the functional ability of the hands in the case of De Quervain Syndrome. The method in this research design uses a literature review analysis wit the PICO method with a database seachr method through PUBMED, Z-Library and Google schoolar, five articles are reviewed. The test results of increasing functional ability in patients with De Quervain Syndrome showed that there was a significant effect of exercise therapy intervention with a mean value before the action of 50,5 and the mean value after of 26,8 the difference in ability improvement was 23,7. Health woekers can improve physiotherapy in patients with De Quervain Syndrome to improve te functional ability of the hand.Keywords: De Quervain syndrome, Exercise therapy, Disabilities Of The Arm, Shoulder And Hand (DASH) AbstrakBagian tubuh memiliki peran penting dalam berbagai aktivitas, aktivitas berlebih dapat menyebabkan gangguan. De quervain Syndrome merupakan salah satu gangguan tendinitis yang disebabkan oleh peradangan selubung tendon otot pada m. abduktor policis longus dan m. Ekstensor policis brevis akibat penebalan pada ekstensor retinaculum, peran penting fisioterapi dalam rehabilitasi gangguan fungsi gerak tubuh yaitu penggunaan modalitas. Tujuan penelitian ini melihat gambaran pemberian terapi latihan dalam meningkatkan kemampuan fungsional tangan pada kasus De Quervain Syndrome. Metode dalam desain penelitian ini menggunakan analisis literatur review dengan metode PICO dengan metode penelusuran database melalui PUBMED, Z-Library dan Google Scholar diperoleh lima artikel yang direview. Hasil uji peningkatan kemampuan fungsional pada pasien De quervain Syndrome menunjukan ada pengaruh signifikan intervensi terapi latihan dengan nilai mean sebelum tindakan 50,5 dan nilai mean setelah dilakukan 26,8 selisih peningkatan kemampuan 23,7. Tenaga Kesehatan dapat meningkatkan Fisioterapi pada penderita De Quervain Syndrome untuk meningkatkan kemampuan fungsional tangan.Kata kunci: De quervain syndrome, Terapi latihan, Disabilities Of The Arm, Shoulder And Hand (DASH)

Seasonal variations in the stable isotopic composition of snow and meltwater were investigated in a sub-arctic, mountainous, but non-glacial, catchment at Okstindan in northern Norway based on analyses of δ18O and δD. Samples were collected during four field periods (August 1998; April 1999; June 1999 and August 1999) at three sites lying on an altitudinal transect (740–970 m a.s.l.). Snowpack data display an increase in the mean values of δ18O (increasing from a mean value of −13.51 to −11.49‰ between April and August), as well as a decrease in variability through the melt period. Comparison with a regional meteoric water line indicates that the slope of the δ18O–δD line for the snowpacks decreases over the same period, dropping from 7.49 to approximately 6.2.This change points to the role of evaporation in snowpack ablation and is confirmed by the vertical profile of deuterium excess. Snowpack seepage data, although limited, also suggest reduced values of δD, as might be associated with local evaporation during meltwater generation. In general, meltwaters were depleted in δ18O relative to the source snowpack at the peak of the melt (June), but later in the year (August) the difference between the two was not statistically significant. The diurnal pattern of isotopic composition indicates that the most depleted meltwaters coincide with the peak in temperature and, hence, meltwater production.

Abstract Background The aim of the study was to compare the amount of interproximal enamel reduction (IPR) provided on ClinCheck software with the amount of IPR carried out by the orthodontist during treatment with clear aligners. Methods 30 subjects (14 males, 16 females; mean age of 24.53 ± 13.41 years) randomly recruited from the Invisalign account of the Department of Orthodontics at the University of Rome “Tor Vergata” from November 2018 to October 2019, were collected according to the following inclusion criteria: mild to moderate dento-alveolar discrepancy (1.5–6.5 mm); Class I canine and molar relationship; full permanent dentition (excluding third molars); both arches treated only using Comprehensive Package by Invisalign system; treatment plan including IPR. Pre- (T0) and post-treatment (T1) digital models (.stl files), created from an iTero scan, were collected from all selected patients. The OrthoCAD digital software was used to measure tooth mesiodistal width in upper and lower arches before (T0) and at the end of treatment (T1) before any refinement. The widest mesio-distal diameter was measured for each tooth excluding molars by “Diagnostic” OrthoCAD tool. The total amount of IPR performed during treatment was obtained comparing the sum of mesio-distal widths of all measured teeth at T0 and T1. Significant T1–T0 differences were tested with dependent sample t-test (P < 0.05). Results In the upper arch, IPR was digitally planned on average for 0.62 mm while in the lower arch was on average for 1.92 mm. As for the amount of enamel actually removed after IPR performing, it was on average 0.62 mm in the maxillary arch. In the mandibular arch, the mean of IPR carried out was 1.93 mm. The difference between planned IPR and performed IPR is described: this difference was on average 0.00 mm in the upper arch and 0.01 in the lower arch. Conclusions The amount of enamel removed in vivo corresponded with the amount of IPR planned by the Orthodontist using ClinCheck software.

AbstractIntroductionSmoking in pregnancy is a substantial public health issue, but, apart from nicotine replacement therapy (NRT), pharmacological therapies are not generally used to promote cessation. Bupropion and varenicline are effective cessation methods in nonpregnant smokers and this systematic review investigates their safety in pregnancy.MethodsWe searched MEDLINE, EMBASE, CINAHL, and PsychINFO databases for studies of any design reporting pregnancy outcomes after bupropion or varenicline exposure. We included studies of bupropion used for smoking cessation, depression, or where the indication was unspecified. Depending on study design, quality was assessed using the Newcastle-Ottawa Scale or Cochrane Risk of Bias Tool. Most findings are reported narratively but meta-analyses were used to produce pooled estimates for the proportion of live births with congenital malformations and of the mean birthweight and gestational age at delivery following bupropion exposure.ResultsIn total, 18 studies were included: 2 randomized controlled trials, 11 cohorts, 2 case– control studies, and 3 case reports. Study quality was variable. Gestational safety outcomes were reported in 14 bupropion and 4 varenicline studies. Meaningful meta-analysis was only possible for bupropion exposure, for which the pooled estimated proportion of congenital malformations amongst live-born infants was 1.0% (95% CI = 0.0%–3.0%, I2 = 80.9%, 4 studies) and the mean birthweight and mean gestational age at delivery was 3305.9 g (95% CI = 3173.2–3438.7 g, I2 = 77.6%, 5 studies) and 39.2 weeks (95% CI = 38.8–39.6 weeks, I2 = 69.9%, 5 studies), respectively.ConclusionsThere was no strong evidence that either major positive or negative outcomes were associated with gestational use of bupropion or varenicline. PROSPERO registration number CRD42017067064.ImplicationsWe believe this to be the first systematic review investigating the safety of bupropion and varenicline in pregnancy. Meta-analysis of outcomes following bupropion exposure in pregnancy suggests that there are no major positive or negative impacts on the rate of congenital abnormalities, birthweight, or premature birth. Overall, we found no evidence that either of these treatments might be harmful in pregnancy, and no strong evidence to suggest safety, but available evidence is of poor quality.

<b><i>Objective:</i></b> This study aimed to assess reduced fetal growth between 35 weeks of gestation and birth in non-small for gestational age fetuses associated with adverse perinatal outcomes (APOs). <b><i>Material and Method:</i></b> It is a retrospective cohort study of 9,164 non-small for gestational age fetuses estimated by ultrasound at 35 weeks. The difference between the birth weight percentile and the estimated percentile weight (EPW) at 35 weeks of gestation was calculated, and we studied the relationship of this difference with the appearance of APO. APOs were defined as cesarean or instrumental delivery rates for nonreassuring fetal status, 5-min Apgar score &#x3c;7, arterial cord blood pH &#x3c;7.10, and stillbirth. Fetuses that exhibited a percentile decrease between both moments were classified into 6 categories according to the amount of percentile decrease (0.01–10.0, 10.01–20.0, 20.01–30.0, 30.01–40.0, 40.01–50.0, and &#x3e;50.0 percentiles). It was evaluated whether the appearance of APO was related to the amount of this percentile decrease. Relative risk (RR) was calculated in these subgroups to predict APOs in general and for each APO in particular. Receiver operating characteristic and area under curves (AUC) for the difference in the percentile was calculated, used as a continuous parameter in the entire study population. <b><i>Results:</i></b> The median gestational age at delivery in uncomplicated pregnancies was 40.0 (39.1–40.7) and in pregnancies with APOs 40.3 (49.4–41.0), <i>p</i> &#x3c; 0.001. The prevalence of APOs was greater in the group of fetuses with a decrease in percentile (7.6%) compared to those with increased percentile (4.8%) (<i>p</i> &#x3c; 0.001). The RR was 1.63 (95% CI: 1.365–1.944, <i>p</i> &#x3c; 0.001). Although the differences were significant in all decreased percentile groups, RRs were significantly higher when decreased growth values were &#x3e;40 points (RR: 2.036, 95% CI: 1.581–2.623, <i>p</i> &#x3c; 0.001). The estimated value of the AUC for percentile decrease was 0.58 (0.56–0.61, <i>p</i> &#x3c; 0.001). <b><i>Conclusion:</i></b> Fetuses with a decrease in the EPW between the ultrasound at 35 weeks of gestation and birth have a higher risk of APOs, being double in fetuses with a decrease of &#x3e;40 percentile points.

Introduction & Objective: Nausea and vomiting during pregnancy is one of the most common gastrointestinal disorders that more than 85% of pregnant women experience. However, controlling and treating this complication is still one of the most important issues in antenatal care. Therefore, the aim of this study was to investigate the effect of ear acupressure at Shen Men point on relieving nausea, vomiting and retching during pregnancy. Materials and Methods: In this quasi-experimental study, 100 pregnant women with a gestational age of less than 16 weeks attending the health clinics of Zabol city during 2019-2020 were studied in two control and intervention groups (n = 50 in each group). The samples in the intervention group were trained to apply pressure on their ears’ Shen Men point with the thumb for three minutes three times a day (morning, noon, and night), for a duration of one month. At the end of second and fourth weeks, the data were collected using the Rhodes index form and then, were analyzed by SPSS software version 22. Results: According to the results, there was no significant difference between the two groups in terms of age, gestational age, occupation and education. The difference in the mean scores of nausea, vomiting and retching was not statistically significant between the control and intervention groups before the study. But four weeks after the study, a significant difference was observed in the mean scores of vomiting, nausea and retching between the two groups, so that the mean scores of vomiting, nausea and retching were significantly lower in the intervention group than in the control group. Conclusion: The ear acupressure medicine at the Shen men point can be used as a non-invasive, safe and inexpensive method to relieve nausea, vomiting and retching during pregnancy. AbstractObjectiveTo evaluate the prevalence of severe acute respiratory syndrome coronavirus-2 infection in patients presenting with epistaxis to a tertiary otolaryngology unit.MethodsA prospective study was conducted of 40 consecutive patients presenting with epistaxis referred to our tertiary otolaryngology unit. A group of 40 age-matched controls were also included. All patients underwent real-time reverse transcriptase polymerase chain reaction testing for severe acute respiratory syndrome coronavirus-2. Symptoms of fever, cough and anosmia were noted in the study group.ResultsThe mean age was 66.5 ± 22.4 years in the study group. There were 22 males (55 per cent) and 18 females (45 per cent). The mean age in the control group was 66.3 ± 22.4 years (p = 0.935). There were six positive cases for severe acute respiratory syndrome coronavirus-2 (15 per cent) in the epistaxis group and one case (2.5 per cent) in the control group. The difference was statistically significant (p = 0.05).ConclusionEpistaxis may represent a presenting symptom of severe acute respiratory syndrome coronavirus-2 infection. This may serve as a useful additional criterion for screening patients.

Norovirus outbreaks frequently occur in closed or semiclosed institutions. Recent studies in Catalonia and various countries indicate that, during outbreaks in these institutions, norovirus is detected in between 23% and 60% of workers, and the prevalence of infection in asymptomatic workers involved in outbreaks ranges from 17% to 40%. In this work, we carried out a prospective study to investigate the involvement of workers in closed and semiclosed institutions during outbreaks. The attack rates (ARs) and the rate ratios (RRs) were calculated according to the type of transmission and occupational category. The RRs and 95% confidence intervals (CIs) between workers and users were calculated. The mean cycle of quantification (Cq) values were compared according to the genogroup and the presence of symptoms. ARs were higher in person-to-person transmission than in common vehicle outbreaks, and 38.8% of workers were symptomatic. The RR between workers and users was 0.46 (95% CI 0.41–0.52). The ARs in workers were high, particularly in workers with closer contact with users. The mean Cq was lower in patients than in asymptomatic infected persons, although the difference was only significant for genogroup I (GI). The frequency of asymptomatic infected persons suggests that personal hygiene measures should be followed by all workers in the centers affected.

Objectives This study aimed to determine leptin levels in term newborns who were born in the north of Jordan. We also aimed to investigate the relationships of leptin levels with fetal growth parameters, and to assess the difference in leptin levels according to sex and gestational age. Methods A cross-sectional descriptive study that involved 170 term newborns was conducted. A working sheet for data collection was created for each newborn and included sex, weight, length, head circumference, gestational age, and Apgar score. Blood samples were obtained from the umbilical cord vein of newborns after delivery to measure serum leptin levels. Data are shown as frequency, percentages, means, and standard deviations. Results We found that the mean leptin level was 1.17 ± 0.48 ng/mL. The independent t-test showed that the mean leptin level in boys (0.93 ± 0. 34 ng/mL) was significantly lower than that in girls (1.38 ± 0.47 ng/mL). Pearson’s correlations showed that leptin levels of newborns were positively and significantly correlated with weight, length, and head circumference. Conclusion In Jordanian healthy term newborns, leptin levels correlate with sex and intrauterine growth parameters.
