Summary. The tibia non-unions after fractures require complex and durable treatment. One of the most common methods of variable osteosynthesis is the Ilizarov method. The long period of treatment causes the development of bone reaction to the immersion parts of the ring fixation (RF), osteoporosis, and as a result - reduction of the rigidity of the "fixator - bone fragments" system. All this is the basis for the application of the final stage of low-traumatic methods of fragments fixation with external bandages. Objective. The task of the study is to improve the results of tibial bone non-unions treatment after changeable osteosynthesis with ring fixators, based on the application of external individual fixation with semi-rigid bandages at the final stage of treatment. Material and methods. In our study, we had 36 patients with tibial bone non-unions under observation which have been treated with changeable osteosynthesis with RF. For the sake of prevention of displacement of the fragments, deformation of the regenerate, treatment of tissues inflammation in the area of pins implantation after disassembly of the Ilizarov device; and for the early rehabilitation of the calf functions - used individual semi-rigid fixation system Scotchcast/Softcast. The system allowed to create atraumatic conditions for regeneration and consolidation, full loading of the limb, its full usage during walking. The combination of rigidity (Sotshstast) and elasticity (Softstast) in the system of semi-rigid fixation with polymeric materials allowed the circular rounds of Softstast to change the shape of the bandage. In point of this, we were offered a method of pneumatic massage, which provided massage during the immobilization stage of treatment. Conducting kinesiotherapy and massage therapy was supplemented by medicament therapy. Results. The assessment also took into consideration the structure of non-unions with the Non-Union Scoring System - NUSS scale. The distribution of patients according to the total NUSS score indicated a very complex contingent of victims with a low probability of obtaining good results. Patients who needed standard methods of treatment, with a high probability of obtaining a positive result, were not observed; all required specialized treatments with the likelihood of achieving a positive result. In 20 cases, there was a need for complex specialized medical care with a possible positive result. 2 observation cases showed that even complex specialized medical care did not guarantee a positive result. Evaluation of the results of treatment of the victims with tibial bone non-unions after fractures were performed using the Modified Functional Evaluation System by Karlstrom-Olerud. Good results were obtained in 20 patients (30.45 ± 0.88 points, p <0.001) - 55.6%. Satisfactory results were observed in 7 patients (28.0 ± 0.125 points, p <0.001) - 19.4%. Moderate function impairment (4 patients - 13.9%) was 22.0 ± 1.323 p p <0.001. Poor, unsatisfactory results were observed in 1 case (2.8%); excellent functional status - 3 (8.3%). Conclusions. The usage of semi-rigid fixation systems at the final stage of treatment of tibial non-unions after osteosynthesis with ring fixators should be considered reasonable and safe. The results of the treatment of 36 patients with tibial bone non-unions with preliminary fixation by ring fixators according to the Ilizarov method, using semi-rigid fixation systems at the final stage of treatment -indicate high efficiency.

In this review, the unique properties of intrinsically conducting polymer (ICP) in biomedical engineering fields are summarized. Polythiophene and its valuable derivatives are known as potent materials that can broadly be applied in biosensors, DNA, and gene delivery applications. Moreover, this material plays a basic role in curing and promoting anti-HIV drugs. Some of the thiophene’s derivatives were chosen for different experiments and investigations to study their behavior and effects while binding with different materials and establishing new compounds. Many methods were considered for electrode coating and the conversion of thiophene to different monomers to improve their functions and to use them for a new generation of novel medical usages. It is believed that polythiophenes and their derivatives can be used in the future as a substitute for many old-fashioned ways of creating chemical biosensors polymeric materials and also drugs with lower side effects yet having a more effective response. It can be noted that syncing biochemistry with biomedical engineering will lead to a new generation of science, especially one that involves high-efficiency polymers. Therefore, since polythiophene can be customized with many derivatives, some of the novel combinations are covered in this review.

Simplicity and high efficiency of a wheeled mover as a mechanism for converting rotational motion into a translatory one have conditioned its wide application in overland machines including motor vehicles. However a wheel with a non-pneumatic tyre (NPT) has a sufficient drawback lying in termination of a wheeled machine movement at the excess air pressure loss. Moreover, the loss of excess air pressure in a pneumatic tyre of traditional design at high speed of movement of a motor vehicle can lead to a traffic accident with heavy consequences. The stop of a motor vehicle to change a wheel on a heavy traffic roadway or roadside also poses a threat. These reasons determine the necessity of both well-known design improvements and search for the new wheeled mover design solutions to enhance a motor vehicle safety, the use of wheels with non-pneumatic tyres of elastic polymeric materials being one of them. Safety enhancement by means of non-pneumatic tyre use along with keeping the high performance of wheeled machine operational properties, is an important scientific and technical task that determines the research urgency.

AbstractFree-form optoelectronic devices can provide hyper-connectivity over space and time. However, most conformable optoelectronic devices can only be fabricated on flat polymeric materials using low-temperature processes, limiting their application and forms. This paper presents free-form optoelectronic devices that are not dependent on the shape or material. For medical applications, the transferable OLED (10 μm) is formed in a sandwich structure with an ultra-thin transferable barrier (4.8 μm). The results showed that the fabricated sandwich-structure transferable OLED (STOLED) exhibit the same high-efficiency performance on cylindrical-shaped materials and on materials such as textile and paper. Because the neutral axis is freely adjustable using the sandwich structure, the textile-based OLED achieved both folding reliability and washing reliability, as well as a long operating life (>150 h). When keratinocytes were irradiated with red STOLED light, cell proliferation and cell migration increased by 26 and 32%, respectively. In the skin equivalent model, the epidermis thickness was increased by 39%; additionally, in organ culture, not only was the skin area increased by 14%, but also, re-epithelialization was highly induced. Based on the results, the STOLED is expected to be applicable in various wearable and disposable photomedical devices.

Advances in medical care has reduced the rate of mortality from strokes, but the incidence of stroke has remained stable while the incidence of ministrokes has increased. Most stroke victims require long-term care, imposing a heavy financial and emotional burden on families while incurring a heavy cost to society. Thus, strokes are a key issue in the context of health care in Taiwan. This paper proposes using VBA (Visual Basic for Applications) to build a system for assessing Brunnstrom stages based on the observation of several obvious rehabilitation features The system calculates features for accelerometer readings, which are then used as input parameters for a fuzzy algorithm to obtain the Brunnstrom action level. Experimental results show the proposed approach effectively assesses Brunnstrom level, and that the approach can be used to assist physical therapists in performing longitudinal assessments of stroke victim progress, thus improving evaluation efficiency.

We present a surgical technique of closed tympanoplasty for chronic otitis, together with an intraoperative functional evaluation system with the OssiMon LAIOM software. The technique can be used in one or two steps for an intraoperative evaluation of the functional effect during ear operation. Using OssiMon LAIOM, we were able to simultaneously measure the auditory steady-state response (ASSR), as well as to perform laser dopler vibrometry (LDV). For electrophysiologic measurements, OssiMon LAIOM uses the Intelligent Hearing System platform, and the Polytec single-point laser to evaluate the ossicular mobility. The measurements can be conducted using both methods at the same time or separately, applying each method independently. The OssiMon LAIOM software records the ASSR response intraoperatively and marks it automatically on the audiogram with the preoperative hearing level. The ossicular vibration level is determined based on the measured LDV response. To the best of our knowledge, OssiMon LAIOM is the first solution allowing to objectively measure the effectiveness of tympanoplasty using two methods simultaneously, i.e. ASSR and LDV. The system could be widely applied in the functional evaluation of the middle ear and in clinical practice.

AbstractIn recent years, a great deal of interest has been directed toward the use of organic materials in the development of high-efficiency optoelectronic and photonic devices. There is a myriad of possibilities among organic materials, which allows flexibility in the design of unique structures with a variety of functional groups. The use of nonlinear optical (NLO) organic materials as thin-film waveguides allows full exploitation of their desirable qualities by permitting long interaction lengths and large susceptibilities allowing modest power input. There are several methods in use to prepare thin films such as Langmuir-Blodgett (LB) and self-assembly techniques,2-4 vapor deposition.5-7 growth from sheared solution or melt,8,9 and melt growth between glass plates.10 Organic-based materials have many features that make them desirable for use in optical devices, such as high second- and third-order nonlinearity, flexibility of molecular design, and damage resistance to optical radiation. However, processing difficulties for crystals and thin films has hindered their use in devices. We discuss the potential role of microgravity processing of a few organic and polymeric materials. It is of interest to note how materials with second- and third-order NLO behavior may be improved in a diffusion-limited environment and ways in which convection may be detrimental to these materials. We focus our discussion on third-order materials for all-optical switching, and second-order materials for frequency conversion and electro-optics. The goal of minimizing optical loss obviously depends on processing methods. For solution-based processes, such as solution crystal growth and solution photopolymerization, it is well known that thermal- and solutal-density gradients can initiate buoyancy-driven convection. Resultant fluid flows can affect transport of material to and from growth interfaces and become manifest in the morphology and homogeneity of the growing film or crystal. Likewise, buoyancy-driven convection can hinder production of defect-free, high-quality crystals or films during crystal and film growth by vapor deposition.
