The impact of regular long term physical load on cardiovascular functional parameters in children and adolescent athletes

Introduction. Regular physical load determines increase in functional capability of cardiovascular system. On the other hand, the cardiovascular system often appears as a conditional factor, which restricts organism adaptive abilities and limits general organism adaptation to the load. During physical load, not only cardiovascular system is activated, but also complex changes take place in the whole body. Therefore, with the aim to evaluate the functions of various systems, interrelation between them and systemic response of the body to physical load, a complex research on distinctive features of not only functional indices of the cardiovascular system, but also parameters reflecting interrelationship among functional systems of the body and speed of their changes is carried out. The goal of such complex research is the evaluation of adaptive and reserve capabilities of the athlete’s body and individualization and optimization of physical load. The aim of this study was to evaluate the speed of changes of parameters that characterize the functional condition of human organism of children and adolescents athletes and non-athletes controls. Materials and methods. One hundred sixty seven male athletes aged 14.8 (SD1.6, range 12-17 years) participating in basketball, rowing and cycling and168 healthy sedentary controls matched for age, sex and body surface area performed a graded exercise test (Mc. Master) on a cycle ergo-meter. 12 ECG standard derivations were synchronously recorded every second minute. During cycle ergo-meter integrated functional parameters, which could integrally and simple evaluate organism reaction to physical load, were assessed.Results. The present study demonstrates that before reaching the maximum of physical load, the speed of changes JT/RR in athletes and all functional parameters (HR, JT interval, RR interval, systolic blood pressure (SBP), diastolic blood pressure (DBP), pulse blood pressure (SBP-DBP), JT/RR ratio) in the control group decreased and in the last step of physical load has increased. That suggests that during intensive exercise training limits of physiological changes can be exceeded in athletes. Conclusions. Analysis of speed of changes in functional parameters during physical load can be applied for the evaluation of functional state of the human body and the cardiovascular system and aiming to optimize and individualize physical load in athletic children and adolescents.

Introduction. Th e actual problem of modern occupational health is the study of the role of exogenous and endogenous factors in the development of occupational pathology of the bronchopulmonary system. То identify groups at increased risk of developing a dusty pathology of the bronchopulmonary system, it is necessary to conduct a comprehensive study of clinical and genetic factors, as well as to determine the most signifi cant diagnostic markers of the development of this pathology.The aim of the study was to study the genetic status of a set of biochemical and molecular genetic markers, as well as biochemical parameters of blood and respiratory function in coal industry workers with chronic dust bronchitis and persons of the control group.Materials and methods. 115 workers of coal mines from the South of Kuzbass aged from 39 to 58 years were examined in the Clinic of the Institute. Among them — 71 people with a previously established diagnosis of chronic dust bronchitis (the main group) and 44 people of the control group of persons working in the same sanitary conditions, but not having this pathology. A complex of clinical, biochemical and genetic methods of research was used in the study, and functional parameters of the bronchopulmonary system were evaluated. Statistical processing of the results was carried out using IBM SPSS Statistics 22 soft ware. Results. Statistically signifi cant diff erences between biochemical (increase in the concentration of ceruloplasmin and α–1antitrypsin) and immunological parameters (increase in the total number of leukocytes and ESR, increase in the concentration of IgG) in miners with chronic dust bronchitis and coal industry workers without this pathology were revealed. The dependence of the functional changes of the respiratory system with the development of professional pathology is determined. Th e persons of the main group showed a statistically signifi cant decrease in functional parameters (forced exhalation per second and lung capacity), increased respiratory failure. A predisposition to the development of dust bronchitis in the owners of the HP 1–1 genotype and resistance to the formation of this pathology in persons with the HP 2–2 genotype were found. Th e study of GSTT 1 deletion polymorphism revealed that carriers of the GSTT 1 «+» variant are most susceptible to the development of chronic dust bronchitis, and owners of the GSTT 1 variant are» resistant to its formation. Th ere was a positive аssociation with the development of dust bronchitis of the holders of the MM phenotype (MN).Conclusions. When working in similar conditions, some workers have a deviation of biochemical and immunological parameters fr om the norm, as well as a violation of the respiratory system, while others have no changes. Th e response of the body to the impact of certain external adverse factors may be due to genetic predisposition or resistance to the development of the disease.

Introduction: This study aimed to examine the effectiveness of the national school-based intervention on both obesity and high blood pressure in Chinese children and adolescents aged 6–18 years.Methods: The national school-based cluster non-randomized controlled trial was done in seven provinces from September 2013 to February 2014. A total of 23,175 children and adolescents in the control group and 25,702 in the intervention group were included in this trial with a mean follow-up of 6.7 ± 0.9 months. Mixed-effects regression models were used to evaluate the effect of the interventions on body weight and blood pressure (BP).Results: A significant upward in the body mass index (BMI) levels but downward in systolic BP (SBP), diastolic BP (DBP), BMI Z-scores, SBP Z-scores, and DBP Z-scores were witnessed in the intervention group compared to those in the control group (<0.001). Subgroup analyses presented significant intervention effects in children aged 6–12 years for BMI, SBP, DBP, and their standardized values Z-scores, but no effective results were found in adolescents aged 13–18 years. Stratification analyses based on the dynamic weight changes presented non-differential HBP, SHBP, and DHBP prevalence gaps between the control and intervention groups. Children aged 6–12 years with higher BMI percentiles at baseline presented obvious declines in SBP and DBP standardized values Z-scores.Conclusion: A mean 6-month multi-centered school-based comprehensive obesity intervention in China yields a small to null effect on obesity and hypertension with increasing age; the early age before 12 years may be the key period for interventions, and the younger, the better. Precise and high-intensity interventions targeting the population at different stages of childhood and adolescence are urgently needed to be developed.Clinical Trial Registration:https://www.clinicaltrials.gov/, identifier: NCT02343588

Abstract. In the present paper we investigate whether patients with a clinical diagnosis of burnout show physiological signs of burden across multiple physiological systems referred to as allostatic load (AL). Measures of the sympathetic-adrenergic-medullary (SAM) axis and the hypothalamic-pituitary-adrenal (HPA) axis were assessed. We examined patients who had been diagnosed with burnout by their physicians (n = 32) and were also identified as burnout patients based on their score in the Maslach Burnout Inventory-General Survey (MBI-GS) and compared them with a nonclinical control group (n = 19) with regard to indicators of allostatic load (i.e., ambulatory ECG, nocturnal urinary catecholamines, salivary morning cortisol secretion, blood pressure, and waist-to-hip ratio [WHR]). Contrary to expectations, a higher AL index suggesting elevated load in several of the parameters of the HPA and SAM axes was found in the control group but not in the burnout group. The control group showed higher norepinephrine values, higher blood pressure, higher WHR, higher sympathovagal balance, and lower percentage of cortisol increase within the first hour after awakening as compared to the patient group. Burnout was not associated with AL. Results seem to indicate a discrepancy between self-reported burnout symptoms and psychobiological load.

Summary. The disbalanced feeding and the uneven distribution of micro- and macroelements in the environment leads to a trace element, in particular hypomelanosis. To accelerate the growth and preservation of young farm animals include in the diet of various biological additives and drugs, which include selenium. For stimulation of weight gain in the livestock industry, as well as for the prevention and treatment of pathological processes in addition to micro - and macrouse amino acids, primarily methionine. The aim of this work was to study the influence of composition of DAFS-25+Polizon on morpho-biochemical parameters of blood and functional state of the liver in fattening bulls of black-motley breed in the conditions of the Republic of Bashkortostan. Experiments using were conducted on bull-calves of black-motley breed of the properties in the properties age from 6 to 15 months. The first experimental group during the experiment was additionally given the composition of DAFS-25+Polizon at a dose of 2 mg/kg, the animals of the control group received a standard diet. To assess the impact of the composition DAFS-25+Polizon on metabolism cattle studied morphological and biochemical indicators of blood and conducted histological examination of the liver. It is established that the use of the composition of DAFS-25+Polizon at a dose of 2 mg/kg increases the number of erythrocytes and hemoglobin in the experimental group and reduces the amount of white blood cells. The serum content of total protein, phosphorus and calcium increases in the group of experimental animals. Microscopic examination of the liver revealed no changes in the structure of the organ and hepatocytes in the experimental group, whereas in the control group hemodynamic disorders and dystrophic changes in liver cells were observed. Thus, the use of the composition DAFS-25+Polizon at a dose of 2 mg/kg of live weight in fattening bulls black-and-white breed contributes to the increase of redox processes in the body, stimulation of metabolism, prevent the development of liver disorders of cellular mechanisms of metabolism, optimizes the structure of the liver, which generally provides higher productivity.

Introduction:The high level of morbidity and mortality from cardiovascular disease is largely due toinsufficient influence on the main risk factors that contribute to the development of myocardial infarction.Therefore, a detailed study and assessment of risk factors is among the most important problems of medical and social importance. The aim: To study and evaluate the impact of biological, social and hygienic, social and economic, psychological, natural and climatic risk factors on the development of myocardial infarction. Materials and methods: A sociological survey was conducted in 500 people aged 34 to 85. They were divided into two groups. The main group consisted of 310 patients with myocardial infarction. The control group consisted of 190 practically healthy people, identical by age, gender and other parameters, without diseases of the cardiovascular system. Results: It was defined that 30 factors have a significant impact on the development of myocardial infarction.Data analysis revealed that the leading risk factors for myocardial infarction were biological and socio-hygienic. The main biological factors were: hypertension and hypercholesterolemia. The man socio-hygienic factor was smoking. Conclusions: Identification of risk factors provides new opportunities for the development of more effective approaches for the prevention and treatment of myocardial infarction.

The aim of this study was to examine and assess the impact of a series of 20 whole-body cryotherapy (WBC) treatments on the biochemical and rheological indices of blood in people with multiple sclerosis. In this prospective controlled study, the experimental group consisted of 15 women aged 34–55 (mean age, 41.53 ± 6.98 years) with diagnosed multiple sclerosis who underwent a series of whole-body cryotherapy treatments. The first control group consisted of 20 women with diagnosed multiple sclerosis. This group had no intervention in the form of whole-body cryotherapy. The second control group consisted of 15 women aged 30–49 years (mean age, 38.47 ± 6.0 years) without neurological diseases and other chronic diseases who also underwent the whole-body cryotherapy treatment. For the analysis of the blood indices, venous blood was taken twice (first, on the day of initiation of whole-body cryotherapy treatments and, second, after a series of 20 cryotherapy treatments). The blood counts were determined using an ABX MICROS 60 hematological analyzer (USA). The LORCA analyzer (Laser–Optical Rotational Cell Analyzer, RR Mechatronics, the Netherlands) was used to study the aggregation and deformability of erythrocytes. The total protein serum measurement was performed using a Cobas 6000 analyzer, Roche and a Proteinogram-Minicap Sebia analyzer. Fibrinogen determinations were made using a Bio-Ksel, Chrom-7 camera. Statistically significant differences and changes after WBC in the levels of red blood cells (RBC), hemoglobin (HGB), hematocrit (HCT), elongation index, total extend of aggregation (AMP), and proteins (including fibrinogen) were observed. However, there was no significant effect of a series of 20 WBC treatments on changes in blood counts, rheology, and biochemistry in women with multiple sclerosis. Our results show that the use of WBC has a positive effect on the rheological properties of the blood of healthy women.

Stress is an individual's adaptive response to various external pressures or demands and produces a variety of disorders including physical, emotional and behavioral disorders. Final year students often experience feelings of stress so that it can interfere with concentration to attend lectures and can even hamper completing education in a timely manner. There are several ways of managing stress, one of which is using aromatherapy. Aromatherapy is a non-pharmacological therapy, which is a type of therapy that applies direct body contact and has a therapeutic effect that combines physiological effects, caused by massage on the body, with psychological effects, which are derived from essential oils. The general purpose of the study is to analyze the effect of aromatherapy on blood pressure and stress levels by using the Kessler Psychological Distress Scale in final semester students. The research method used a quasi-experimental design with a pretest-posttest control group design approach, in 28 intervention groups, namely respondents who were given lavender aromatherapy massage and 28 respondents in the control group. The univariate analysis was conducted to show frequency distribution and bivariate analysis was conducted by independent sample T-test and dependent sample T-test. The results showed that there were significant differences in systolic blood pressure before and after the intervention in the intervention and the control group with a p-value of 0.01 < 0.05. Whereas the stress level variable found that there is no difference in stress levels in the intervention and the control group before and after the intervention with p-value 0.169> 0.05.

High blood pressure, diabetes, hyperlipidemia and obesity are risk factors for cardiovascular diseases. With regard to the significant role of a healthy diet in the prevention and even treatment of diseases together with the high cost and side effects of drugs, finding foods effective in the treatment of metabolic disorders has been widely considered. This study aimed to evaluate the effect of oxymel – an Iranian traditional syrup with vinegar base – on cardiovascular risk indicators in obese and overweight people. Candidates were selected based on a set of inclusion criteria and were divided into two groups of control and test. The control group received 250 cc of water, while the test group received 250 cc water containing 30 cc of the oxymel for 30 days. Anthropometric and biochemical indicators were measured at the beginning and end of the study. The results showed that there were no significant changes in the body mass index, waist circumference, hip circumference, waist to hip ratio, HDL, LDL, Triglycerides, systolic and diastolic blood pressure, and blood glucose level. However, weight (P = 0.053) and cholesterol (P = 0.083) decreased relatively significantly in the test group compared to the control group. This study shows that consumption of oxymel has positive cardiovascular effects such as lowering the blood cholesterol level and can contribute to weight loss; however, studies with a larger sample size are recommended.

Meal replacement plans are effective tools for weight loss and improvement of various clinical characteristics but not sustainable due to the severe energy restriction. The aim of the study was to evaluate the impact of meal replacement, specifically 388 kcal in total energy, on body composition and metabolic parameters in individuals with overweight and obesity from a Chinese population. A parallel, randomized controlled trial was performed with 174 participants (ChiCTR-OOC-17012000). The intervention group (N=86) was provided with a dinner meal replacement, and the control group (N=88) continued their routine diet as before. Body composition and blood parameters were assessed at 0, 4, 8, and 12 weeks. A post hoc analysis (least significant difference (LSD) test), repeated measurements, and pairedT-test were used to compare each variable within and between groups. Significant (p<0.001) improvements in body composition components were observed among the intervention group, including body weight (−4.3 ± 3.3%), body mass index (−4.3 ± 3.3%), waist circumference (−4.3 ± 4.4%), fat-free mass (−1.8 ± 2.9%), and body fat mass (−5.3 ± 8.8%). Body composition improvements corresponded with significant metabolic improvements of blood glucose (−4.7 ± 9.8%). Further improvements in visceral fat area (−7.7 ± 10.1%), accompanying with improvements in systolic (−3.7 ± 6.9%) and diastolic (−5.3 ± 7.7%) blood pressure, were only found in male subjects. To conclude, meal replacement intake with 388 kcal in total energy at dinner time for 12 weeks contributed to improvement in body composition and clinically significant metabolic parameters in both male and female participants with overweight/obesity. Additionally, glucose and blood pressure reduction were gender-specific highlighting the importance of gender stratification for design of nutritional intervention studies for improvement of health.

Abstract Objectives Community- and clinic-based fruit and vegetable “prescription” (FVRx) programs, which include nutrition education and subsidies to reduce the cost of fruits and vegetables have improved dietary intake and some clinical outcomes, such as hemoglobin A1c (HbA1c) and blood pressure. However, few if any studies have investigated the impact of an FVRx program in a worksite setting. The purpose of this study was to determine the effects of a worksite FVRx program on diet quality, body mass index (BMI), waist circumference, blood lipid concentrations, hemoglobin A1c (HbA1c), and blood pressure. Methods In the first of two planned cohorts, we randomized healthy adults employed at a community hospital in southern Connecticut to receive either the FVRx program for 10 weeks (n = 20) or standard worksite wellness offerings (n = 20). Each week, intervention participants received a 45-minute cooking and nutrition education session held during the workday and a voucher valuing $15–25, depending on household size, that could be redeemed for fruit and vegetable purchases at a local grocery store. Outcome measures were assessed at baseline and at the end of the intervention. Results Compared to the control group, participants in the FVRx group significantly increased their HEI-2010 score for vegetable intake (0.91 ± 1.41 vs. 0.02 ± 1.18, P < 0.05) and reduced their HE-2010 score for empty calories (−4.61 ± 4.82 vs. −0.75 ± 3.20, P < 0.01). There were no between-group differences in other HEI-2010 components, body composition, HbA1c blood lipids, or blood pressure. However, the FVRx group did improve their overall HEI-2010 score from baseline (7.85 ± 10.82, P < 0.05) whereas the control group did not (3.57 ± 9.51, P > 0.05). Conclusions In this interim analysis, we demonstrated potential benefits of a worksite FVRx program on intake of vegetables and empty calories. Changes in anthropometric or biochemical measures were not observed immediately post-intervention, but this may be due to enrollment of a low-risk population or length of time needed to influence those measures. That the intervention nevertheless improved dietary intake suggests that it may be valuable for prevention of diet-related disease in healthy adults.
