ABSTRACTObjective: to identify the incidence of pressure ulcer (PU) and its risk factors in hospitalized patients in the medical-surgical units of a university hospital. Methodology: this is about a descriptive and quantitative study whose data were collected from November 2008 to February 2009 using a instrument containing demographic and clinical data, characterization of PU and Braden Scale after approved by the Ethics Committee of the State University of Londrina with protocol number 170/08. The results were organized in the database Epi info and analyzed using descriptive statistics Results: among 36 patients studied, the incidence of PU was 2.77%, nineteen patients were included already with PU, these were more frequent in the sacral region (50%) and stage III (37%). Hypertension was a statistically significant variable for the development of PU, as well as the patients with longer length of stay. Conclusion: for prevention of UP, it is the function of the nurse to know the possible risk factors, track changes and implement a clinical protocol and all available measures in health care practice. Background: Pressure ulcers are a major problem both in our context than in studies conducted around the world. In Tunisia, few studies have focused on estimating their extent. Aim: To estimate the incidence of pressure ulcers in some at-risk departments of the Hospital of Sahloul and to identify the associated risk factors. Methods: We carried out a longitudinal study in the departments of Anesthesia-Resuscitation, Internal Medicine, and Aesthetic and Restorative Surgery at the Hospital of Sahloul during 3 months from October the 1st until December 31st, 2015. Results: Overall, 150 patients were eligible. Their mean age was 46.1 ± 20.2 years. The sex ratio was 1.94. In our study, 29 patients had pressure ulcers, which represented a patient incidence of 19.3% (95% CI: [13-25.3]). In addition, 56 episodes of pressure ulcers were observed in these patients, resulting in a pressure ulcer incidence of 37% (95% CI: [29.3-44.7]) and incidence density of 2.3 episodes per 1000 days of hospitalization. The most frequent locations were the heels followed by the buttocks. In multivariate analysis, polytrauma (p=0.048) and disorders of higher functions (p<10-3) were found as independent risk factors for pressure ulcer occurrence. Regarding the evaluation of different pressure screening scores, Braden had the best sensitivity-specificity pair. Conclusion: Our results highlight the extent of the problem of pressure ulcers in our hospital. The fight against the occurrence of this complication requires a good management of traumatized and patients with a disorder of the higher functions based on the recommendations concerning the rehabilitation and the nutrition.

Objective: Pressure ulcers (PUs) are one of the most commonly occurring complications in hospitalised patients. Knowing the size of the problem and its risk factors will help in preventing it. The aim of this study is to measure the incidence of PUs in acute care settings in Jordan and to explore associated risk factors. Method: A prospective incidence study for hospitalised patients in Jordan according to the European Pressure Ulcer Advisory Panel methodology. All patients admitted into four Jordanian hospitals over a period of six months were included. Results: The cumulative incidence rate was 0.48%. Using multivariate analysis, a low albumin level, elevated white blood cells, incontinence and having more chronic illnesses were significantly associated with acquiring PUs. Conclusion: Incidence of PUs in Jordan is lower when compared with other parts of the world; this could be related to the relatively younger Jordanian population compared with other populations. Declaration of interest: The author has no conflicts of interest to declare.

Pressure injuries (PIs) are one of the most important and frequent complications in patients admitted to the intensive care unit (ICU) or those with traumatic brain injury (TBI). The purpose of this study was to determine the incidence and risk factors of PIs in patients with TBI admitted to the ICU. In this retrospective study, the medical records of 237 patients with TBI admitted to the trauma ICU of a university hospital were examined. Demographic, trauma-related, and treatment-related characteristics of all the patients were evaluated from their records. The incidence of PIs was 13.9%, while the main risk factors were a higher injury severity score, use of mechanical ventilation, vasopressor infusion, lower Braden Scale score, fever, and period of enteral feeding. This study advances the nursing practice in the ICU by predicting the development of PIs and their characteristics in patients with TBI.

BACKGROUND: Although it is well known that pressure ulcers are associated with negative patient outcomes and increased hospital cost, there is little research related to pressure ulcers in an intensive care unit population. OBJECTIVE: To determine the relative contribution of risk factors in the development of pressure ulcers in intensive care unit patients. METHOD: In an exploratory descriptive design, a convenience sample of 85 adults was used. Patients were enrolled in the study within 24 hours of admission to the intensive care unit; data were collected every other day until discharge from the intensive care unit. Instruments included a demographic data form, Braden Scale for Predicting Pressure Sore Risk, Skin Assessment Tool, and Decubitus Ulcer Potential Analyzer. RESULTS: The most common reasons for admission to the intensive care unit included multiple trauma from motor vehicle accidents, gunshot and stab wounds, and gastrointestinal bleeding. A pressure ulcer developed in 48 subjects. There were no significant differences in age, gender, history of diabetes or smoking, or medical diagnoses between patients in whom a pressure ulcer developed and those in whom it did not. Data analysis indicated that a Braden Scale score of 11, rather than the recommended score of 16, was statistically significant for predicting pressure ulcer risk. CONCLUSIONS: The results suggest that a cut-off score on the Braden Scale could be specific to an intensive care unit trauma population.

A 29-year-old woman presented to the Fondazione IRCCS “Cà Granda” Ospedale Maggiore, a tertiary care university hospital in Milan (Italy), with skin lesions, fever, myalgia, joint pain and swelling, and a one-week history of low back pain. The diagnosis wasStaphylococcus aureus(S. aureus) bacteraemia spreading to skin, bones, and joints and a lumbosacral epidural abscess L5-S2. Neither initial focus nor predisposing conditions were apparent. The antibiotic therapy was prolonged for six-weeks with the resolution of fever, skin lesions, articular inflammation, and the epidural abscess. Community-acquiredS. aureusinfections can affect patients without traditional healthcare-associated risk factors, and community acquisition is a risk-factor for the development of complications. Raised awareness ofS. aureusbacteraemia, also in patients without healthcare-associated risk factors, is important in the diagnosis, management, and control of this infection, because failure to recognise patients with serious infection and lack of understanding of empirical antimicrobial selection are associated with a high mortality rate in otherwise healthy people.

Abstract The aim of this study was to examine the prevalence and relationship between periodontal disease and preterm low birth weight (PLBW) among Saudi mothers at King Khalid University Hospital (KKUH) in Riyadh, Saudi Arabia. The periodontal status and the relative risk were also analyzed. The study consisted of 30 cases [infants <37 weeks and/or weighing ≥2.500 kilograms (kg)] and a daily random sample of 60 controls [≥37 weeks and/or weighing >2.500 kg]. Clinical periodontal indices were measured on the labor wards. Associated risk factors for periodontal disease and PLBW were ascertained by means of a structured questionnaire and maternal notes. The prevalence of the PLBW was found to be 11.3%, and the prevalence of periodontal disease was high among the study population. The risk of PLBW remained high with increasing periodontal disease (odds ratio [OR] 4.21, 95% confident interval [CI] 1.99-8.93) despite controlling the other risk factors such as age, smoking, and social class. In conclusion, there is a correlation between periodontal disease and PLBW in KKUH. Citation Mokeem SA, Molla GN, Al-Jewair TS. ABSTRACT Objectives: to assess the prevalence and associated risk factors for urinary tract infection in patients with chronic kidney disease under conservative treatment and identify the microorganisms isolated in the urine of these patients and the staging of chronic kidney disease. Methods: a cross-sectional, analytical study carried out at the Conservative Treatment Outpatient Clinic of a university hospital in the city of São Paulo. Results: the prevalence of urinary tract infection is 22%. The risk factors Diabetes Mellitus, hypertension, heart disease, neoplasms and thyroid and autoimmune diseases stand out in the infected group (p < 0.001). Most of the microorganisms found in urine cultures (87.9%) were Gram-negative, being Escherichia coli (50.70%), followed by Klebsiella pneumoniae (23.1%) and Enterococcus spp. (9.7%). Conclusions: the findings of this investigation reveal the intrinsic association between risk factors and microorganisms for the development of urinary tract infection.
