In India more than 10 million cases of joint diseases are reported every year and amongst these joint diseases prevalence of osteoarthritis is high where patient ends up doing joint replacement. But this has various after effects and financial burden on society. Many patients approach Ayurved for the treatment of joint diseases. A proper differential diagnosis is important to provide accurate treatment so as to prevent further deterioration of patients .The approach of Ayurved and Modern science differs from diagnostic as well treatment point of view. The detail structural elucidation of each joint  is not mentioned in classical texts. And hence main emphasis is given on wholesome status of bodily condition rather than specific joint. In Ayurved the patient is diagnosed only on the basis of symptomatology. whereas in modern science diagnostic tools such as MRI, X ray ,USG, CT scan ,blood investigation are used to know the exact condition of the joint . A critical review of joint diseases is done here in describing the differential diagnosis according Ayurved and Modern science.

The article is devoted to the study of the worldviews and social contradictions in Russian society on the example of two different positions on the relationship between religion and science. According to one of these positions these relationships are defined as conflict. The second, opposing point of view says there never was and there cannot be any conflict between religion and science. In the publication such points are called “the paradigm of conflict” and “the paradigm of dialogue”. It shows, the first “paradigm” in the Soviet period of Russian history was determined by ideologization of science and was an important part of anti-religious propaganda. On the contrary, “the paradigm of dialogue” has always been represented primarily by religious thinkers. Today it is the official position of the Russian Orthodox Church of the Moscow Patriarchate. The official Church document “The Basis of the Social Concept” says religion and science are designed to complement each other, especially in solving ethical problems that inevitably arise in the face of modern science. However, secular scientists often see in such statements the Church’s claims to active participation in the public life, including the educational process. Representatives of the academic community often speak out against the introduction of the theological educational programs and the theological departments in secular Universities of the Russian Federation. Thus, in contemporary Russian society some continue to believe that there is a conflict between religion and science, while others insist on the need for dialogue.

Modern science and practice does not have a sufficient set of cost management tools, taking into account the duration of the production cycle, characteristic of agricultural activity. The implementation of a cycle-oriented approach to building a model of production costs was based on studying the existing options for classifying production costs, which were supplemented with features from the perspective of managing long production cycles. As a result of the study, a model of production costs was built from the point of view of a cycle-oriented approach, as well as a model of production costs from the standpoint of features of a long production cycle. The model can serve as the basis for the formation of the information space of cost management, control and cost analysis in the economy of agricultural enterprises.

Cystic Fibrosis—NCFRF. 16 mm., color, sound, showing time 32 minutes. Produced in 1961 by Samuel L. Schulman for the National Cystic Fibrosis Research Foundation, medical supervision by Giulio J. Barbero, M.D. Philadelphia. Procurable on purchase from National Cystic Fibrosis Research Foundation, 521 Fifth Avenue, New York City 17. Procurable on loan from American Medical Association, Motion Picture Library, 535 N. Dearborn Street, Chicago 10. This film has been prepared to aid physicians in making a correct diagnosis and to instruct them in the techniques currently being used to prolong life in cases of cystic fibrosis. Following the introductoy remarks there is a good discussion of the presenting symptoms, the differential diagnosis, the multiple system involvement, hereditary aspects, therapy, and prognosis. The film brings out well the panexocrine involvement, the clinical variability, and the fact that the disease is not an all or none phenomenon but rather a disease of all grades of severity and is a disease in which no single test is applicable to the exclusion of others. The diagrams and patient demonstrations are good. Perhaps too much review of older methods of therapy and diagnosis is given, but this serves as a background for the newer recommended procedures. More emphasis could have been given to the tremendous burden, both financial and emotional, this disease is on parents. This is an excellent instructive film and it emphasizes the many problems of cystic fibrosis as related to the diagnostic tools available and to forms of therapy. The photography and sound are satisfactory. It is recommended for pediatricians, general practitioners, house staff, and medical students.

Introduction. Most pelvic tumors originate from reproductive organs. Even using the up-to-date imaging techniques, radiologists experience difficulties in determining the source of the lesion since a wide range of tumors look similar to each other on the distorted backdrop of pelvic anatomy, large invasive formations, and an active inflammatory reaction of the pelvic peritoneum.Aim: to evaluate “pitfalls” in the preoperative noninvasive diagnosis of female pelvic tumors by applying the clinical diagnostic tools.Materials and methods. Four rare clinical cases were analyzed; all of them posed difficulties in interpreting the diagnostic examination due to their atypical characteristics. There were 2 cases of ovarian cancer, initially identified by an experienced team of radiologists as benign pelvic pathology. Also, there were fibroids with degeneration, marked proliferative activity, and a massive inflammatory reaction of the peritoneum – that was diagnosed as a malignant ovarian tumor. Tumor biopsies were examined using morphological and immunohistochemical methods (with the р16, Ki-67, p53, CD 117, S 100, CD 34 markers). Immunohistochemical (IHC) studies were performed with formalin-fixed paraffin materials using the avidin-biotin-peroxidase method. Antibodies to estrogen receptor (ER), progesterone receptor (PR), cytokeratin 7 (CK7), cytokeratin 20 (CK20) and Wilms tumor protein 1 (WT1) were also used.Results. A thorough analysis of the clinical picture and a joint multidisciplinary discussion (gynecologist, oncologist, radiologist, etc.) made it possible to avoid diagnostic errors.Conclusion. These observations demonstrate the difficulties of differential diagnosis between ovarian metastases of uterine cancer and primary multiple ovarian and uterine cancer, as well between leiomyosarcoma and uterine myoma with high mitotic activity. Obviously, the change in diagnosis calls for a change in the treatment strategy.

As the main title ‘COVID-19 revolution: a new challenge for the internist’ states, the global coronavirus infection disease 2019 (COVID-19) pandemic represented a new challenge for the internists. This paper is part of a series of articles written during the difficult period of the ongoing global pandemic and published all together in this fourth issue of the Italian Journal of Medicine, with the aim of sharing the direct experiences of those who were the first to face this severe emergency, expressing each point of view in the management of COVID-19 in relation to other diseases. Each article is therefore the result of many efforts and a joint collaboration between many colleagues from the Departments of Internal Medicine or Emergency Medicine of several Italian hospitals, engaged in the front line during the pandemic. These preliminary studies therefore cover diagnostic tools available to health care personnel, epidemiological reflections, possible new therapeutic approaches, discharge and reintegration procedures to daily life, the involvement of the disease not only in the lung, aspects related to various comorbidities, such as: coagulopathies, vasculitis, vitamin D deficiency, gender differences, etc.. The goal is to offer a perspective, as broad as possible, of everything that has been done to initially face the pandemic in its first phase and provide the tools for an increasingly better approach, in the hope of not arriving unprepared to a possible second wave. This paper in particular deals with hypovitaminosis D and COVID-19.

A conversation with the famous Russian biblical scholar Mikhail Seleznev is devoted to the modern humanitarian discipline of biblical studies. The conversation examines the origins of this scientific discipline, its formation and development, internal structure, its connection with other humanitarian disciplines, as well as with theology understood as a reflection of believers on the foundations of their faith. Examples of the achievements in biblical studies in the 19th and 20th centuries are provided, which as a result have changed our understanding of the origin of biblical texts and the context in which they were created. The article analyzes the attitude towards scientific study of the Bible in various Christian confessions, in particular, the perception of biblical studies in the Orthodox community and the related problems of modern church consciousness. The current state and the prospects of development of biblical studies in Russia are reported on in the article. The issue of the so-called “intertestamental studies” and its significance for comprehending early Christianity is discussed, as well as that of biblical exegesis from the point of view of its understanding in modern science and church tradition. Bibliographic recommendations are provided for those who wish to begin a more detailed acquaintance with modern biblical studies.

In accordance with GOST 20793-2009, the tractor and its components are subjected to resource diagnostics before maintenance. The technical condition of the components of the tractor or machine should be checked with the use of control and diagnostic equipment. Currently, the criteria for the limit state are significantly outdated and require revision from the point of view of tightening modern requirements for operational and economic characteristics and reliability indicators. (Research purpose) The research purpose is in analyzing the state of the issue and the current regulatory documentation and making proposals for remote monitoring of the criteria for the limit states of individual components and aggregates. (Materials and methods) The article presents an analysis of scientific and technical documentation, State standards of the Russian Federation and scientific papers on the problems of minimizing technological risks, diagnostics and control suitability for determining the maximum technical condition, and staged studies on the possibility of monitoring the operation of individual components and units online. The article notes the need to adjust the criteria for the maximum technical condition in accordance with the new designs of resource-determining units, aggregates and existing technical regulations. (Results and discussion). The article presents the justification of the diagnostic process and identified contradictions in the design of tractors and existing scientific and technical documentation and standards, and proposed option of using meters-identifiers when upgrading tractors as a system of built-in online diagnostic tools. (Conclusions) Timely, automated monitoring of the technical condition of tractors, which is based on comparing data with the criteria of the limit condition, serves as a justification for the effective operation of equipment with built-in devices for diagnostics, which allows minimizing agrotechnological risks.
