Background: Vatarakta is a disease which is said to affect the population indulging in sedentary lifestyle. In today’s era the evolution of technology has reached a point where pretty much anything is available at the touch of a button. The lifestyle of today’s population is breeding ground for diseases like Vatarakta. Based on the causes, signs and symptoms, Vatarakta may be correlated to gouty arthritis in contemporary medicine. Gout affects about 2.1 million worldwide. Its prevalence is increasing; moreover it is a potential signal for unrecognized co-morbidities like obesity, diabetes mellitus, hypertension and renal diseases. In India the prevalence of gout is 2-6 per 1000. The purpose of this study is to explore and find out an effective, less expensive, easily available and well accepted drug with minimal or no complications for this dreadful condition. Materials and Methods: 40 patients diagnosed with Vatarakta w.s.r Gouty Arthritis were assigned randomly into 2 groups. Group A, were administered Bodhi Vruksha Twak Kashaya 50ml B.I.D with Madhu and Group B were administered Guduchi Kashaya 50ml B.I.D for a duration of 30 days. Assessment was done on day 0, day 15, day 30, and day 45. Observations and Results: In the present study Group A showed statistical significant results in all the parameters except for Sandhishotha and group B showed statistical significant result in Sandhishoola, Sandhidaha, Sparshasahaishnuta and uric acid levels. Conclusion: Overall result shows patients treated with Guduchi Kashaya showed better results than patients treated with Bodhi Vruksha Twak Kashaya with Madhu in Vatarakta with special reference to Gouty Arthritis.

INTRODUCTION: Otomycosis is the fungal infection of EAC (External Auditory Canal) that is very commonly seen in Ear , Nose and Throat clinics .Local debridement and antifungals , topical or systemic is the treatment of choice but recurrences are very common and resistant to treatment. METHODS:Total 220 patients, divided into two groups each 110 in 'A' and 'B' were subjected to betadine (povidone - iodine )10% solution and Clotrimazole (1 %) drops treatment and response of treatment assessed on 5th , 10th and 20th day and analyzed. RESULTS :Out of 110 patients in each group on 5th day 21.8 % patients in group 'A', and 23.6 % in group B' showed no response. 70.9 % in group 'A' and 72.7 % in group 'B' were showed good response at evaluation of 20th day of treatment. CONCLUSION :In our study, betadine (10% solution) and Clotrimazole (1 %) drops were equally effective in treatment of otomycosis. Betadine(10% solution) is low cost, non-ototoxic, nonresistant and equally effective in bacterial and fungal infection. That's why this study support betadine 10% solution as treatment of choice for otomycosis specially in developing countries .

Recent studies have focussed on the association between elevated homocysteine levels with megaloblastic changes and thromboembolic events, but the relationship between occult megaloblastosis (with normal haemoglobin levels) and ischaemic stroke has not been widely explored. The objective of this study is to establish a simple and economical marker for the detection of occult megaloblastosis at the community health care level in developing countries. A hundred patients who met the inclusion criteria were studied. At the 5% level of significance, the levels of cobalamin and folate were significantly lower, while the number of hypersegmented neutrophils on the peripheral smear was higher in patients from Group A (70 patients with high homocysteine) compared with the patients in Group B (30 patients with normal homocysteine). Forty-five (64.2%) of the 70 patients in Group A showed hypersegmentation of neutrophils in the peripheral smear. The high cost and difficulty in performing the vitamin assays limit their use as early markers of megaloblastosis. Hence, we conclude that in developing countries, the detection of hypersegmented neutrophils can be used at the primary healthcare level for early diagnosis of occult megaloblastosis, so that early therapeutic interventions with vitamins can prevent attacks of hyperhomocysteinemia-induced ischaemic stroke.

Protein-energy malnutrition is the most widespread nutritional deficiency disorder in India, commonly occurring in children aged 6 months-2 years,as per the National Family Health Survey, 2007. The dermatologic manifestations are more florid and characteristic in kwashiorkor than in marasmus. Here, we present a case of a 1½ years old male, with extensive skin lesions, owing to severe malnutrition. Malnourishment is one of the foremostconditions seen in the developing countries. It is essential to provide adequate nutritional support to the growing children and to impart proper education to the mothers with regard to weaning and care during the illness of children aged under 5 years.

Myopia is a refractive error which affects every age group person around the world. Specially in modern age when the use of mobile screen and computer is increased so much, this error has become horrible. The symptoms of myopia closely resembles with the disease Timir. It involves pratham and dwitiya patala of Netra. In the present study, 35 clinically diagnosed patients of Timira/Myopia were selected and randomly divided into two groups (Group-A-18 patients, Group-B-17patients) out of these, 30 patients completed the trial. Patients of group A were administrated Timirhar Lauha orally. While patients of group B were topically administrated Baladi Ghrita for Tarpana and Timirhar Lauha orally. The study shows that Timirhar Lauha (systemic drug) alone was effective in all symptoms of Timira /Simple Myopia but combination of the drug Timirhar Lauha (systemic drug) with Baladi Ghrita (Tarpana/topical application) had much greater potential to ameliorate the symptoms of Timira /Simple Myopia.

Wheat bread is the main foodstuff and supply of dietary energy/protein in most developing countries. Wheat based diets are poor regarding essential amino acid content especially lysine as the first limiting amino acid. Since human body is unable to build lysine, it is necessary to be taken via food and/or supplements. Recommended daily intake of lysine is estimated to be around 30-64 mg/kg body weight. Inadequate intake of lysine results in a syndrome called ‘protein energy malnutrition’ which is suspected to be more prevalent in developing countries. Since lysine is provided by different sources (especially meat), there is no published data about the lack of lysine, however it is estimated to be prevalent in developing countries where the lysine-rich sources are generally expensive and low accessible there. The lysine fortification of wheat bread is conducted mainly to provide an accessible lysine-rich source. Biofortification, using lysine rich sources (either as flour or protein concentrates) and directly addition of lysine amino acid and its derivative have been investigated in different studies. The aim of this article is to review the potential strategies to improve the lysine content of wheat bread from both nutritional and technological points of view.

We studied 23 patients (25 bones) with enchondromata of the hand which were treated with simple curettage without bone grafting. The cortical window was replaced in 18 bones (group A), whereas it was not replaced in six bones (group B). In one bone, only half of the cortical window was replaced. Local recurrence was not seen in any patient. Although bone grafting was not performed, new bone formation was observed in all the patients. Radiographic and functional results were excellent in most bones. Restoration of the continuity of the cortex was seen at 3 (range, 1.5–4) months in group A and 8 (range, 6–12) month s in group B. This restoration is important for the recovery of mechanical strength and we therefore consider that the cortical window should be replaced, unless this is impractical.

Background: Prevalence of the episiotomy varies around the world depending on whether it is used as a routine or a restricted procedure. The rates are still higher in developing countries, like ours, since the use of restricted episiotomy is not being practiced widely in primigravidas. Methods: In our randomized comparative study of 222 women who were admitted to the Department of Obstetrics and Gynaecology, SMS Medical College Jaipur, the enrolled women were divided into 2 groups. In Group-A: Vicrylrapide (polygalactin-910) suture material was used in 111 patients and in Group-B chromic catgut suture material was used in another 111 patients, for repair of episiotomies. Results: In our present study in vicryl rapide group at 24-48 hrs pain was present in 49.55% of patients, redness was found in 8.10% of patients, swelling was found in 3.60% patients and analgesic used by 72.97% of patients. In chromic group at 24-48 hrs pain was present in 69.36% of patients, redness was found in 21.62%, swelling was found in 7.20% and analgesic was used by 89.18% of patients. Conclusion: Our study recommends use of vicryl rapide for episiotomy repair in the care of parturient women. Keywords: Complication, Chromic Catgut, Vicryl Rapide, Episiotomy

Background: Warts are very common disorders. Plane warts are caused by Human Papilloma Virus (HPV) occurring mostly in children and young adults. Among the treatment modalities, topical application of trichloroacetic acid (TCA) is age old. Potassium hydroxide (KOH) has a keratolytic effect on virus-infected cells. Aims: To compare the safety and efficacy of topical 10% KOH solution with 50% TCA solution in the treatment of plane warts. Materials and Methods: Sixty-four consecutive patients with plane warts were randomly assigned into two groups of thirty-two patients. Group A received 10% KOH solution and group B received 50% TCA solution once weekly until the complete clearance of warts in a maximum period of 12 weeks. Results: In group under treatment with 10% KOH, 24 patients (75.0%) complete response, 5 patients (15.6%) moderate response, 2 patients (6.3%) mild response, and 1 patient (3.1%) had no response. In group under treatment with 50% TCA, 28 patients (87.5%) showed complete response, 2 patients (6.3%) moderate response, 2 patients (6.3%) mild response. Statistically no significant difference was found between the therapeutic response to 10% KOH and 50% TCA (P=0.41). Conclusion: 10% KOH was found to be equally effective in the treatment of plane warts compared to 50% TCA with the advantage of fewer side effects.
