The COVID-19 pandemic (Corona Virus Disease 2019) is considered a global pandemic by WHO (World Health Organization) and its spread has reached Indonesia. The COVID-19 pandemic has an impact on national economic growth which causes a decline and also causes an economic downturn. This Pandemic also caused an economic downturn experienced by the Province of East Java which had a relatively large number of cases. The agricultural sector is present as a resilient sector that can survive the COVID-19 pandemic which can contribute to supporting the economic growth rate of East  Java. The agricultural sector consists of several sub-sectors, one of which is the horticultural crop sub-sector which has high potential. There are many obstacles experienced by horticultural farmers in their farming. The purpose of this study is to determine the problems experienced by horticultural farmers in East Java Province and to form an institutional model as a solution to the problem to improve the welfare of horticultural farmers in East Java Province through increasing NTP (Farmer Exchange Rate). Analytical descriptive method, case study method and literature study were used in this research. The results showed that (1) the problems experienced by farmers were (a) horticultural farmers generally have a relatively small capacity to provide capital; (b) Sales of horticultural crops are only made to middlemen; (c) Low accessibility to local financing institutions; (d) Agricultural financing credit system that is too burdensome for farmers; (e) There is a gap between farmers and financial institutions; (f) Credit by formal institutions is prioritized in the non-agricultural sector; (2) The existence of the "Horti Bank" institution which is implemented will help in overcoming the problems of horticultural farmers in East Java Province by providing facilities in the form of capital loans with the concept of investment, managing finances, and providing access to insurance for horticultural farmers in East Java Province; (3) The simulation results show that the existence of a Horti Bank can increase the exchange rate of farmers so that it is expected to reduce poverty in East Java.

AbstractIn January 2016, the World Health Organization warned that Zika virus is “spreading explosively” in the Americas and that up to 4 million infections could be present worldwide within a year. Soon thereafter, some politicians and authors publicly advocated for quarantine of travelers returning from regions where mosquitoes carrying Zika virus are prevalent. The public health tool of quarantine can be used to prevent the spread of infection by restricting the movement of persons who have been exposed to a deadly disease that can be transmitted from person to person before symptom onset. With 80% of Zika virus infections being asymptomatic, no rapid test being available to detect the virus, and primary transmission being via the bites of certain mosquitoes, application of quarantine in this setting is not scientifically sound or practically feasible. Rather, public health interventions should focus on preventing bites from infected mosquitoes, counseling pregnant women on the risks of fetal microcephaly and other birth defects, and identifying patients with signs and symptoms of Guillain-Barré syndrome. As was seen in the Ebola virus disease outbreak of 2014, non-evidence-based factors can influence policy decisions. Public health experts must ensure that policy makers are informed that quarantine is not a scientifically sound approach for the control of Zika virus. The spread of Covid-19 has become one of the people's concerns, starting in the city of Wuhan, China at the end of 2019 when this virus was discovered, the spread of the virus that the antidote has not yet been found is now out of control. More than 200 countries in the world have reported that their people have contracted the Covid-19 virus. Corona Virus Disease 19 has been declared a Global Public Health Emergency by the World Health Organization (WHO) on January 30, 2020. Conditions in Indonesia until Thursday, November 30, 2020, the number of people who tested positive for Covid-19 reached 538,883 cases, 450,518 people recovered and 16,945 of them died. This figure will continue to increase in line with the opinion of some epidemiologists and statistics that a pandemic outbreak will not end quickly. The purpose of this community service activity is to realize one of the contents of Presidential Instruction Number 4 of 2020, namely "Rrefocussing activities, reallocation of budgets and procurement of goods and services in order to accelerate the handling of Corona Virus Disease 2019 (COVID-19)", namely by making and distributing fluids. Disinfectant that is safe and environmentally friendly and recommended by the BPOM and the World Health Organization (WHO) to help people face the New Normal era. The result of this service activity is a disinfectant liquid that is safe and environmentally friendly and is able to anticipate the spread of covid-19 and increase public awareness of the Covid-19 Virus in the face of the New Normal era.

Breast milk contains many nutrients according to the needs of the baby for growth and development. Breast milk is very important for the health and well-being of infants. The world health organization recommends exclusive breastfeeding mothers for six months starting from the baby's first life and continuing until the age of two. Breastfeeding can provide economic benefits for the family and community. This study aimed to explore the barrier and facilitator on breastfeeding promotion and education. This study was literature study. This study finding that support from the baby's father is needed as a strong decision exclusively breastfeeding mothers. Pediatricians and midwives have an important role to support in providing exclusive breastfeeding from mothers and infants. Mothers who work full time find it very difficult to care for their babies exclusively. The workplace can be a barrier for mothers to care for and provide exclusive breastfeeding. Breastfeeding competency-based training is needed as a more optimal promotion of exclusive breastfeeding. Promotion of breastfeeding can increase the duration and exclusivity of breastfeeding. Knowledge promotes breastfeeding can increase the chances of successful breastfeeding and improve baby's health. An important role in the successful promotion of breastfeeding is the quality of knowledge and support for exclusive breastfeeding.

Latar Belakang : COVID-19 merupakan masalah kesehatan yang dinyatakan sebagai pandemi oleh WHO. Beberapa penelitian melaporkan tentang hubungan SARS-CoV-2 dengan kelainan pada mata. Pada era pandemi, pengelolaan pasien sakit mata harus hati-hati untuk mencegah penularan petugas medis. Laporan kasus ini menyajikan pengelolaan perforasi kornea pasien terkonfirmasi COVID-19. Metode : Diagnosis ditegakkan berdasarkan anamnesis, pemeriksaan fisik dan oftalmologi dengan pemeriksaan penunjang lainnya. Laporan Kasus : Perempuan berusia 54 tahun dirujuk dengan keluhan keluar darah pada mata kiri sejak 17 jam sebelum sampai RS. Keluhan batuk atau demam 14 hari sebelumnya disangkal. Pada pemeriksaan oftalmologi didapatkan perforasi kornea mata kiri disertai prolaps isi bola mata (iris, vitreus dan choroid). Pada pemeriksaan penunjang didapatkan foto rontgen thorax normal, rapid test covid-19 reaktif dan hasil positif pada PCR dari swab nasofaring. Seluruh staf medis menggunakan alat pelindung diri (APD) tingkat tiga dalam melakukan pemeriksaan fisik maupun penunjang. Dari hasil anamnesis pemeriksaan fisik dan penunjang, pasien didiagnosis dengan perforasi kornea spontan mata kiri dan terkonfirmasi COVID-19. Pasien direncanakan untuk eviserasi mata kiri dengan anestesi umum dan saat tindakan operator dan seluruh staf ruang operasi menggunakan alat pelindung diri (APD) tingkat tiga. Setelah operasi dilakukan, pasien kemudian dirawat di ruang isolasi khusus COVID-19. Swab nasofaring dilakukan ulang sehari setelah operasi dan keesokan harinya. Dua hari setelah operasi, pasien dipulangkan dan isolasi mandiri di rumah Kesimpulan: Laporan kasus ini menunjukkan manajemen perforasi kornea pada pasien terkonfirmasi COVID-19 yang dilakukan sesuai alur khusus termasuk pada pemeriksaan fisik dan penunjang, penegakkan diagnosis oftalmologi dan COVID-19, prosedur pembedahan dan perawatan pasca operasi. Kata Kunci : SARS-CoV-2, Perforasi Kornea, APD, PCR, Eviscerasi   Introduction: Corona-Virus-Disease-2019 (COVID-19) is an important health problem that was defined as a pandemic by the World Health Organization (WHO). There are few reports on the association of SARS-CoV-2 with ocular abnormalities. In the pandemic era, management of ocular patients should be more careful to prevent transmission to medical staff. This report presenting the management of corneal perforation in confirmed COVID-19 patients. Method: The diagnosis was established based on history taking, physical and ophthalmology examination with other ancillary tests. Case Report : 54-year-old female patient referred with complaints of left eye bleeding since 17 hours ago. This patient never complaint about cough or fever 14 days before admission. From the ancillary test (complete blood count and chest x-ray are within normal limit, reactive rapid test for COVID-19, positive result of PCR nasopharynx).  The patient was diagnosed with spontaneous corneal perforation on the left eye with COVID-19 confirmed. The evisceration procedure with general anesthesia was performed with exceptional precaution. Operator and all operating room staff wear the 3rd level of personal protective equipment (PPE). The surgery was successfully done and the patient then hospitalized in the COVID-19 isolated room. Nasopharynx swab performs the day after surgery and the next day after the first swab. Two days after surgery, the patient was discharged and followed by a self-isolation procedure. Conclusion: This case report presenting corneal perforation management in confirmed COVID-19 patients should be performed under a firm scheme including physical and ophthalmological examinations, ophthalmology and COVID-19 diagnosis enforcement, ancillary test, surgery procedure, post-operative care and also PPE utilization. Background: World Health Organization (WHO) and other Health officials alert clinicians about a rare but severe inflammatory condition seen in children and linked to Corona Virus Disease 2019(COVID-19). The WHO is describing the condition as a multisystem inflammatory syndrome in children (MIS-C) and is recommending clinicians to report those cases to get a better understanding of the disease and clinicians can learn more. Case Presentation: We are reporting the clinical course of the youngest case of COVID-19 related MIS-c; a two-week-old term neonate with COVID-19 infection and features suggestive of MIS-C, managed with intravenous immunoglobulin (IVIG), pulse steroid, and interleukin-1 inhibitor (Anakinra). By reviewing the literature, our baby is the first neonatal case who has been diagnosed with MIS-C. Conclusion: COVID-19 infections in pediatrics are likely to present with a mild course; however, some may develop a hyperinflammatory syndrome. Pediatricians should be aware of such presentation, the clinical course, the management modalities,and inform parents and caregivers about common signs and symptoms. Anakinra may consider as effective second agent in (IVIG and steroid-refractory pediatric cases).

Objective. In 2014, the member countries of the Pan American Health Organization signed the Strategy for Universal Access to Health and Universal Health Coverage. In it, they committed to increasing public expenditure on health until reaching the benchmark of 6% of gross domestic product (GDP). The objective of this paper is to determine, for each country in the Region, whether they can reach this target through economic growth alone and, if so, how long it would take. Methods. Using World Bank and World Health Organization data, elasticity of public health expenditure (PHE) with respect to GDP was estimated for each country. Real economic growth and International Monetary Fund projections for 2016–2021 were used to project the expenditure series and determine the year each country would reach 6% of GDP. Results. Six countries have already reached the 6% target. The Latin American and Caribbean countries that have achieved it are those that have single health systems, based on universal access and coverage. If current prioritization of PHE is maintained, three countries could reach the target in the next decade. Four more countries would reach it before mid-century, 10 in the second half of the century, and one would have to wait until the next century. Finally, 13 countries would never reach the proposed target. Conclusions. This analysis demonstrates the limitations of economic growth as a source of fiscal space. Other sources will need to be tapped, such as increased tax collection, specific health taxes, and greater efficiency in public spending, which will require social and political dialogue in the countries regarding their commitment to universal health principles.

The 2019-nCoV (new Corona Virus) outbreak was announced by the World Health Organization (WHO), the International Emergency Public Health Organization, on January 30, 2020, and WHO reported the 2019-nCoV pathogen to SARSCoV-2 and Corona virus Disease 2019 on 12 February. (COVID2019). COVID-19 was officially recognized as a Pandemic by WHO on March 11, 2020. Plants have been used all over the world as traditional medicine for centuries to treat many chronic infections, including viral diseases. In recent years, scientists have been trying to verify the potential of functional compounds to protect human health and cure diseases with their research on functional and nutraceutical foods. Traditional medicinal plants have a long history supported by many researches such as maintaining a healthy life, toxins taken in daily life, fighting and preventing diseases, and longevity. Studies on the antiviral, antiinflammatory and antioxidant properties of ethnomedical plants and natural phytochemicals can be considered as a great potential drug source against various ailments as well as Covid-19 treatment. Based on this study, plant extracts increase immunity with the increase in the number of white blood cells and lymphocytes in viral infections such as Covid-19, which can lead to fatal consequences, regulating the production and release of proinflammatory cytokines, showing an anti-inflammatory effect with a decrease in the C-reactive protein and erythrocyte sedimentation rate, It appears to have a positive effect such as interfering with the development and potential antiviral agent activity. In this study, phytochemicals and effects associated with COVID-19 infection were reviewed.
