Medical records as evidence of health services have an important role in proving law including forms on medical records that have specific functions and meanings in each item. One of which is an operating report that is the contents of an everlasting or perpetuated medical record. The purpose of this research is to know a deeper overview of the completeness of the medical record on the Operation report form at the Sumber Waras Hospital of Cirebon Regency.  The type of research used is descriptive research. Data collection techniques using an observation method with the research instrument sheet Checklist. Simple random sampling techniques. The sample in this study is the medical record of the operating patients in the Cirebon district source Waras Hospital as much as 72 medical record documents.  The results of the medical record in the Operations report form there are 11 documents (15%) a fully stocked and 61 documents (85%.) that are not fully stocked.  The conclusion obtained in this study is that the minimum medical record service standard at Sumber Waras Hospital in Cirebon Regency is not in accordance with Kepmenkes number 129 of 2008.

Organisational culture plays an important role for enabling the process of knowledge sharing. Organisational culture is not only reflected in the visible aspects of organization such as structure, mission, and objectives, it is also embedded in the behaviour of people. The purpose of this chapter is to close research gaps present in knowledge sharing success by examining the linkages between employees’ knowledge-sharing through collaboration, perceived values of involvement, trustworthiness, and formal recognition. The research data was collected by using simple random sampling techniques from a population of knowledge workers in Malaysian IT organisations. The findings highlight the importance of organisational culture for successful knowledge sharing within organisations. The results of factor analysis show the emergence of four new cultural values extant in the Malaysian context. These values are involvement, trustworthiness, formal recognition, and independence. Successful knowledge sharing is significantly related to the perceived value of involvement, trustworthiness, and formal recognition. This chapter will be beneficial for researchers, practitioners, scholars, and organisations (leaders and employees); it will also be helpful for those interested in organisational structure and relationships across organisations in knowledge contexts.

The study was aimed at seeing how extracurricular Islamic spirituality affects student’s behavior at SMAN 10 Bengkulu. The kind of research that was used in this study was quantitative research with a quantitative descriptive approach. The data-collection techniques used were questionnaires, observation, interview, and documentation. The sample in this study was all of the students who have participated in extracurricular Islamic Spirituality at SMAN 10 Bengkulu. Researchers used probability sampling techniques of simple random sampling. Research data analysis used simple linier regression analysis, F-Test, t-test, and coefficient determinations (R2) with the help of SPSS 16. The finding of these studies' extracurricular activities of Islamic spirituality affects student’s behavior. This was evidenced by tcount of 3,120 whereas ttable of 2,037 at a significant level of 0.05. Based on these results tcount was greater than the expectation tchart. Thus, it may be concluded that Ha was accepted and Ho was rejected so that there was an influence of Islamic spirituality extracurricular activities on student behavior by 22.8% while the remaining 77.2% was influenced by other factors not studied in this study.

Abstract Internal patient transfer is the process of transferring patients from one room to another in a hospital while still being oriented towards quality and patient safety. The results of the observations showed that the filling of the internal patient transfer form was not optimal so that there were still incomplete forms due to the large number of patients and the weak coordination between health workers. To determine the effect of the completeness of filling out the internal patient transfer form on the quality of medical records. The research method used is quantitative with a descriptive approach. Data collection techniques used are observation, questionnaires and literature study. The sampling technique was simple random sampling technique so as to obtain a sample of 91 internal patient transfer forms. From the results of observations, 22% of the internal patient transfer forms were found that were not completely filled in, especially in the signature and clear name. The two variables have a strong relationship. The effect of the variable completeness of the internal patient transfer form on the medical record quality variable is 90.1% and the remaining 9.9% is influenced by other factors. Based on these studies, it can be concluded that the lack of accuracy and coordination of nurses, doctors and other officers in filling out internal patient transfer forms so that this affects the quality of medical records in the aspect of accuracy. Keyword : Completeness, Internal Patient Transfer Form, Medical Record Quality   Abstrak Transfer pasien internal merupakan proses pemindahan pasien dari satu ruangan ke ruangan yang lain di dalam satu rumah sakit dengan tetap berorientasi pada mutu dan keselamatan pasien. Hasil observasi menunjukkan bahwa belum optimalnya pengisian formulir transfer pasien internal sehingga masih terdapat formulir yang tidak lengkap disebabkan oleh faktor dari banyaknya pasien dan lemahnya koordinasi antara tenaga kesehatan. Untuk mengetahui pengaruh kelengkapan pengisian formulir transfer pasien internal terhadap mutu rekam medis. Metode penelitian yang digunakan yaitu kuantitatif dengan pendekatan deskriptif. Teknik pengumpulan data yang digunakan adalah observasi, kuesioner dan studi pustaka. Teknik pengambilan sampel adalah teknik simple random sampling sehingga memperoleh sampel sebanyak  91 formulir transfer pasien internal. Dari hasil observasi ditemukannya formulir transfer pasien internal yang belum terisi lengkap sebanyak 22% terutama pada tandatangan dan nama jelas. Kedua variabel memiliki hubungan yang kuat. Pengaruh variabel kelengkapan formulir transfer pasien internal terhadap variabel mutu rekam  medis sebesar 90,1% dan sisanya 9,9% dipengaruhi oleh faktor lain. Berdasarkan penelitian tersebut dapat disimpulkan bahwa kurangnya ketelitian dan koordinasi perawat, dokter dan petugas lain dalam pengisian formulir transfer pasien internal sehingga hal ini mempengaruhi mutu rekam medis pada aspek keakuratan. Background. Medical record service is a professional supporting activity oriented to health information needs organized according to minimum service standards based on regulation number 129 of the year 2008.  Purpose. The purpose of this study is to find out the minimum standard of medical record services at Sumber Waras Hospital, Cirebon Regency. Method. The research design used was descriptive research. Data collection techniques using the observation method with the research instrument used is a checklist sheet. Data collection procedures are carried out by determining inclusion and exclusion criteria. Sampling in this study uses quota sampling. Result. The results showed that completeness of filling in inpatient medical record documents had 22 documents (73%) complete and 8 documents (27%) incomplete. Completeness of inpatient informed consent was obtained as many as 22 documents (73%) complete and 8 documents (27%) were incomplete. The time to provide medical record documents for outpatient services is provided quickly as many as 13 documents and slow as many as 17 documents. Conclusions. The conclusions obtained in this study are, the minimum standard of medical record service at Sumber Waras Hospital, Cirebon Regency is not in accordance with the Decree of the Minister of Health number 129 of 2008.

Bullying is a behavior consciously done by someone to abuse and hurt others. Ideally, the guidance and counseling teachers (BK teachers)are demanded to give attentions in preventing bullying behavior. However, in fact, the BK teachers have not plan special programs yet to prevent the bullying behavior among students in school. This research aims to describe the understanding of BK teachers about the concept of bullying behavior. Then, it aims to describe the treatment of the BK given by the teachers to the students. Finally, it aims to describe the cooperation among the BK teachers within the school. This research is kind of descriptive research. The population of this research is all of the BK teachers of SMA Negeri (State school for secondary education) in Padang. Sample is taken by using area sampling and simple random sampling techniques. Generally, the result implicates that the BK teachers have done preventing efforts toward bullying behavior among students in the school. Thus, the teachers should increase the understanding in reference to bullying behavior among students to prevent the behavior in school.

This study aims to describe the onomatopoeic form in the Comic Nusa Lima volume 1 by Sweta Kartika. The theory used is the onomatopoeic form theory proposed by Sudaryanto (1989) and the onomatopoeic meaning theory proposed by Ulman (2011). This research is descriptive research. Data collection using the observation method. The technique used in the data collection stage is a non-participant technique. Researchers observed and recorded data in the form of words containing onomatopoeic elements contained in the comic Nusa Five by Sweta Kartika. The data analysis method used in this research is the equivalent method and the method of agih. The results of this study found that the onomatopoeic form of the root word was divided into one syllable and bisilabel. In the reduplication category, the researchers found full repetitions and three repetitions (trilingga). The results of this study concluded that the use of onomatopoeia in the form of reduplication was dominant in Nusa Five Comics.

The implementation of coding done in the medical record should be done very thoroughly, precisely and accurately according to the diagnostic code that exists in ICD-10. If there is an error in coding, it will have a bad impact on the patient, puskesmas or hospital. But in the fact found in the field there are still problems in the implementation of the accuracy of the encoding of disease diagnosis based on ICD-10. The purpose of this research is to describe the accuracy of disease code in the medical record in Puskesmas Plumbon. The population in this study was the entire quarterly medical record document of year 2020 with a total of 1,098 medical record documents with a total sample of 92 documents using simple random sampling technique. This study was conducted in Plumbon Puskesmas Medical record unit on 20 June 2020. The method used is descriptive with a quantitative approach. The data collection procedures used are by observation and checklist sheet. The population in this study was the entire quarterly medical record document of year 2020 with a total of 1,098 medical record documents with a total sample of 92 documents using simple random sampling technique. This study was conducted in Plumbon Puskesmas Medical record unit on 20 June 2020. The results of the study obtained from 92 samples of medical record documents, the appropriate disease code in accordance with the ICD-10 as much as 39 (42.39%) and improper code of 53 (57.61%). There is still an imprecision of unsuitable disease code due to coding officers that do not include the 4th character. It is best to need a fixed procedure in accordance with WHO provisions for coding the disease to make the koder more thorough in determining the disease code.

Food is the most essential requirement for humans to maintain life and living. One of the quality of food can be from nutrients contained in it. Efforts to diversify food can be done by finding new food ingredients or materials from existing food and developed into a diverse food material. The use of lamtoro seeds as the basis for making tempeh is one form of food diversification. Sampling in this study is Simple Random Sampling with a total sample of 2 soybean tempeh and 2 lamtoro tempeh. Research data collection using experiments. In this study, different results were obtained from each soybean tempeh and lamtoro tempeh.

This study investigated how beliefs in magical powers and related sacred activities, particularly those that accept human body parts for ritual sacrifices in return for fortunes, alongside poverty, unemployment, and quest for wealth as predictors responsible for the increase in money ritual–related killings in Ilorin Emirate of Kwara State, Nigeria. A total of 1736 respondents were selected using simple random sampling techniques. The instrument employed for data collection was a questionnaire. In addition, the information gathered was analyzed using the Statistical Package for Social Sciences. The results indicated that the increase in money ritual–related killings in Ilorin Emirate is a result of the general belief that ritual sacrifices enhance fortunes, the boundless desire for material wealth among Nigerians, unemployment, and poverty in Nigeria. Therefore, this paper suggests that there should be public enlightenment in all languages spoken in Nigeria about the dangers in associating individual success with spiritual influences.
