Background: Recurrent pregnancy loss is an important reproductive health issue, affecting 2%–5% of couples. An unsupportive endometrium, leading to abnormal implantation, is considered to be one of the key factors contributing to idiopathic recurrent spontaneous miscarriage (IRSM). The aim of this work was to evaluate differences in uteroplacental blood flow and pregnancy outcome in women with idiopathic recurrent spontaneous miscarriage (IRSM) following administration of micronized vaginal progesterone versus oral dydrogesteron. Materials and Methods: This prospective, randomized-controlled study comprised 90 pregnant women who came to outpatient clinic of obstetrics  .All women had a singleton pregnancy with active cardiac pulsations at gestational age between 5-8 weeks Pregnant women in the study group were randomly distributed into: Group {A}: 30 pregnant women received 10 mg of oral dydrogesterone (Duphaston; Abbott Company) twice daily.Group {B}: 30 pregnant women received 200 mg micronized vaginal progesterone (Prontogest) twice-daily. Control group:30 pregnant women without history of recurrent miscarriage served as controls and they received folic acid as placebo. Results: comparing the Doppler indices before progesterone supplementation, the mean resistance index (RI) was statistically significant less in the control group compared with both study groups (A&B) (P=0.012, P=0.005 respectively) .Moreover, pulsatility index (PI) was statistically significant less in the control group compared with both study groups (A&B) (P=0.026, P=0.05 respectively) .Paralleled to that, the S/D ratio was statistically significant less in control group compared with both group A &B (P=0.43, & P=0.019respectively) .In addition, the mean PSV was significantly higher in control group  compared to group B (P=0.047) and was higher in control group than group A with nearly significant P value. Conclusion: Considerable improvement in uteroplacental blood flow parameters of pregnant women with IRSM is evident with progesterone supplementation.

Objective To determine the role of anti-annexin V antibodies (a-A5) as an etiologic factor in recurrent pregnancy failure. Study design Prospective observational study. Material and methods The study included ninety first trimester pregnant women who had a history of unexplained recurrent miscarriage (group I) with ninety well-matched pregnant women with a history of normal reproductive outcome allocated as control group (GII) and another ninety nonpregnant women (GIII). Sera from all women controls were analyzed for anti-annexin antibody measured by Elisa. Results The mean value of a-A5 was 11.37 ± 6.78, 7.7 ± 1.40 and 6.20 ± 0.95 ng/ml in groups I, II and III respectively. There was a significant increase in the mean value a-A5 among women with a history of recurrent miscarriage, compared with controls. The mean value was 13.92 ± 2.42 ng/ml among patients with unfavourable outcome, compared with a corresponding value of 6.95 V 0.58 ng/ml among women with favourable outcome. The receiver operator characteristic curve revealed that the cutoff value of a-A5 was 8.61 ng/ml. Conclusion This study emphasizes the relationship between anti-annexin V antibodies and recurrent miscarriage.

Background: The prevalence of anemia among pregnant women was still high in Indonesia, especially in Bengkulu. Consuming papaya is considered as one of the solutions to increase hemoglobin and hematocrit level.Objective: This study aims to examine the effect of consuming papaya on the level of hemoglobin and hematocrit in pregnant women.Methods: This study employed a true experiment with randomized pretest and posttest design with control group. There were 42 respondents recruited in this study using simple random sampling. Randomization was performed to divide the samples into two groups, namely 21 respondents in the treatment group and 21 respondents in the control group. A total of 110 grams of papaya was given to the intervention group every day for 14 days.  Data were analyzed using dependent t-test and independent t-test.Result: There was a significant effect of consuming papaya on the hemoglobin and hematocrit level with p-value 0.000 (< .05). The mean difference between two groups showed that hemoglobin level (control group 10.010 gr/dL; intervention group 10.838 gr/dL) and hematocrit level (control group 27.43 %; intervention group 30.10 %).Conclusion: Consuming papaya has a significant effect on changes in hemoglobin and hematocrit levels. It is suggested that consuming papaya should be one of alternative treatment for midwives to prevent anemia in pregnant women.

Background: Thyroid diseases have a strong predominance in woman of childbearing age. Pregnancy may be associated with thyroid dysfunction. The aim of the present study was to assess the serum FT3, FT4 and TSH levels in pregnant women. Methods: This cross-sectional analytical study was done in the Department of Physiology of Dhaka Medical College, Dhaka, from July 2006 to June 2007. Total 50 apparently healthy women of low socioeconomic class, age ranged from 18-40 years were selected from the Outpatient Department of Urban Primary Health Care Project (UPHCP) at Mirpur, Dhaka. Out of them 30 pregnant women of different trimester were taken as study group (group A) and 20 age matched non pregnant women were taken as control (group B). Serum FT3, FT4 and TSH levels were parameters in both groups. Statistical analysis was done by the SPSS version 12.0. Results: The mean serum FT3 levels were 6.36±1.16 pmol/L and 6.381.36 pmol/L in group A (Study) and group B (control) respectively. The mean serum FT4 levels were 20.25±4.77 pmol/L and 19.39±8.17 pmol/L in group A (Study) and group B (control) respectively. The mean serum TSH levels were 0.96±0.96 mIu/L and 1.27±0.86 mIu/L in group A (Study) and group B (control) respectively. The difference was not significant (p>0.05) between group A and B. Conclusion: From the results obtained in the present study, it may be concluded that there is no change of serum FT3, FT4 and TSH level in pregnancy. This was a hospital based case control study. The study included patients attending in outdoor and indoor Department of Obstetrics and Gynaecology of Rajshahi Medical College, Hospital since July 2014 to June 2016. The aim of this study was to determine thyroid antibodies is a risk factor for recurrent pregnancy loss. Sixty seven pregnant or non pregnant women with history of recurrent miscarriage during 1st trimester were selected as case group and another sixty seven pregnant women who reached their 12 weeks uneventfully with no history of recurrent miscarriage were selected as control group. Patients with other cause of recurrent miscarriage such as metabolic or endocrinologic disorders, genital organ anomaly, uterine fibroid were excluded from the study. Thyroid function test and thyroid antibodies (FT4, TSH and TPO-Ab) were measured for the two groups. The result of this study showed that the percentage of positive TPO-Ab in target and control group is 67.16% and 5.95% respectively. The study observed that the mean serum concentrations of FT4 in the control subject was significantly higher than the mean of the target group (p-value <0.05). The TSH concentration was increased in miscarriage women with positive antibodies compared with the concentration of TSH in the control group with positive antibodies. The conclusions are that there is a deficiency in thyroid hormones or thyroid’s functional capacity is unable to meet the extra demands of pregnancy which may be one of the causes of recurrent miscarriage. Moreover positive thyroid antibodies pregnant women can reach term and have babies when the concentration of TSH is low during the first trimester, so the risk of miscarriage could be high in positive thyroid antibodies.TAJ 2017; 30(1): 49-55

AbstractThe aim of the study is to compare the efficacy of flecainide, beta-blockers, sotalol, and verapamil in children with frequent PVCs, with or without asymptomatic VT. Frequent premature ventricular complexes (PVCs) and asymptomatic ventricular tachycardia (VT) in children with structurally normal hearts require anti-arrhythmic drug (AAD) therapy depending on the severity of symptoms or ventricular dysfunction; however, data on efficacy in children are scarce. Both symptomatic and asymptomatic children (≥ 1 year and < 18 years of age) with a PVC burden of 5% or more, with or without asymptomatic runs of VT, who had consecutive Holter recordings, were included in this retrospective multi-center study. The groups of patients receiving AAD therapy were compared to an untreated control group. A medication episode was defined as a timeframe in which the highest dosage at a fixed level of a single drug was used in a patient. A total of 35 children and 46 medication episodes were included, with an overall change in PVC burden on Holter of -4.4 percentage points, compared to -4.2 in the control group of 14 patients. The mean reduction in PVC burden was only significant in patients receiving flecainide (− 13.8 percentage points; N = 10; p = 0.032), compared to the control group and other groups receiving beta-blockers (− 1.7 percentage points; N = 18), sotalol (+ 1.0 percentage points; N = 7), or verapamil (− 3.9 percentage points; N = 11). The efficacy of anti-arrhythmic drug therapy on frequent PVCs or asymptomatic VTs in children is very limited. Only flecainide appears to be effective in lowering the PVC burden.

SummaryObjective: The aim of the study was to investigate the use of somatic cell count (SCC) and aerobic bacterial counts as possible tools to choose animals for selective dry cow treatment (DCT) on cow level. Blanket antibiotic DCT is an approved and common method in Germany aimed at increasing the cure rate of existing intramammary infections (IMI) at the time of drying off and decreasing the risk of new infections during the dry period. However, not all animals benefit equally from an antibiotic DCT. Moreover, dairies, consumers and politicians demand a reduction in the use of antibiotics in farm animals. Several criteria for identifying animals which would benefit from antibiotic DCT have been described in the literature. Material and methods: Animals chosen for this investigation were divided into three study groups: Aerobic Count Petrifilm® group A (153 cows), SCC group S (160 cows) and control group C (169 cows). All quarters were treated with an internal teat sealant (ITS) no matter which group the cows belonged to. Results: Group C, in which all cows received an antibiotic DCT, achieved the best results referred to udder health, particularly regarding bacteriological dry period cure. However, the results of the two study groups A and S, in which the animals were dried off selectively, were only marginally worse. A significant difference regarding bacterial dry period cure was revealed between group S and group C (p = 0.039), whereas no significant difference concerning new IMI risk could be detected between the three study groups. Compared to a blanket antibiotic DCT of all cows in the corresponding study group, a reduction of the usage of antibiotics of 23.5 % (A) and 55 % (S) was noticed, respectively. Furthermore, it could be shown that the lactation number as well as the microorganisms present at the time of drying off influence udder health. Conclusion: Selective DCT is an alternative method to the widespread blanket antibiotic DCT. However, it has to be mentioned that selective DCT involves the described marginal decline in udder health and the additional amount of time, which might arise from the procedure of choosing the right animals and performing the applied rapid test methods.

The present study evaluated the effect of whole egg consumption on the liver, testes, cauda epididymal sperm reserve and lipid profile of male rats. These evaluations were carried out on adult twenty (20) male albino rats, which were randomly selected into four groups of 5 rats each, designated groups A, B, C and D. Group A was the control group and received only equivalent volume of distilled water, while groups B, C and D received 0.25mg/kg, 0.5mg/kg; and 1.0mg/kg body weight of the quail egg respectively. Standard procedures were carried out in the tissue processing, cauda epididymal sperm reserve and in lipid profile determinations. On days 35 and 49, the mean serum total cholesterol value of group D was significantly lower than that of the control group. On day 35, the mean serum low density lipoprotein and high density lipoprotein (LDL and HDL cholesterol) values of all the treatment groups were significantly lower and higher than that of the control group, respectively. However, on days 49 and 63, the mean serum very low density lipoprotein (VLDL cholesterol) and triglyceride values of all the treatment groups were significantly higher than that of the control group. A significant increase in cadual epididymal sperm count (CESR) was recorded on day 63 at the mid and high doses. No obvious pathological lesions were observed in the histomorphology of the testes and liver when compared to the control. Therefore, whole quail egg consumption caused an increase in serum triglyceride and very low density lipoprotein concentration, and also improved fertility. In other words, prolonged consumption of quail egg should be done with caution as it may predispose one to cardiovascular disease.

This study was done to assess the hypercoagulability and the risk of thromboembolism in women taking oral contraceptive pill for prolonged period of time. This cross sectional study was done in the department of Physiology, Dhaka Medical College, Dhaka from Jan 2012 to Dec 2012. Ninety female subjects with the age range from 25-45 years, were taken as a study population. Among them, 60 women taking oral contraceptives for prolonged period of time ( > 1 years) were included for the study group and age matched 30 women of OCP nonusers were taken as a control. Study subjects were divided into two groups according to their duration of oral pill use: group BI ( 1 to 5 years users) were 30 women and group B2 ( >5 to 10 years users) were 30 women. Prothrombin time and activated partial thromboplastin time were estimated in all groups. Statistical analysis was done by unpaired Student's ? t' test and Pearson's Correlation Coefficient test. In this study, the mean (*SD) PT levels in group B1 & B2 were shortened than that of group A which were statistically highly significant (P < 0.001). Within the study groups, PT levels were positively correlated (r=+0.027) with the group B1 and negatively correlated (r= -0.163) with the group B2. But both the relationships were statistically non significant. The mean (+SD) AM' level in group B1 was shortened than that of group A but the result was not statistically significant. The mean (*SD) AM' level in group B2 was shortened than that of group A but the result was statistically highly significant (P<0.001). Within the study groups, APPT levels were negatively correlated with the group B1 (r= -0.268) and also group B2 (r= -0.122). But both the relationships were statistically non significant. My present study revealed that prolonged duration of OCP use ( at least for 5 years) increases the risk of hypercoagulable state and thromboembolism in women.

Objective: Spontaneous abortion is the most common complication of early pregnancy, affecting up to 20% of recognized pregnancies. Kisspeptin is predominantly released by placental syncytiotrophoblasts, and regulates their placental invasion into the uterine matrices. We aimed to establish an association of serum kisspeptin levels with pregnancy outcomes during the early gestational stage of the first trimester. Method: In this prospective study, 90 pregnant women in their 7 to 8 6/7 gestational weeks were classified into three groups: (i) The control group, consisting of healthy pregnant women (n=30), (ii) the threatened abortion group (n=30), and (iii) the spontaneous abortion group (n=30). The maternal serum samples were analyzed for complete blood count parameters and kisspeptin levels. Results: There was no statistical difference regarding body mass index (BMI) and gestational age (p=0.370). Regarding detailed obstetric notations, including gravida, parity, abortion, and living children, socioeconomic levels, and employment rates, all study groups were comparable (p>0.05, for all). No significant association was found regarding the biochemical parameters of complete blood count, including neutrophil, lymphocyte, and platelet concentrations, as well as neutrophil-to-lymphocyte ratios (NLR) and platelet-to-lymphocyte ratios (PLR) (p>0.05, for all). The median serum kisspeptin levels of the study groups did not differ between the groups (p=0.153). Correlation analysis revealed no significant relationship between serum kisspeptin levels and other study parameters in any study groups (p>0.05, for all) Conclusions: We found no statistically significant relationship between serum kisspeptin concentrations and pregnancy outcomes in the early gestational stage of the first trimester, and serum kisspeptin concentrations did not seem to be a reliable marker to distinguish abortion status from viable pregnancy
