Abstract Objective Literature revealed that oral health status and awareness about oral hygiene measures of Narikuravar tribes were very poor. There was also an increased prevalence of tobacco usage among this population. Considering this, incidence and prevalence of oral precancerous and cancerous lesions are expected to be high in this population. Surprisingly, the literature search revealed a lack of data on the awareness, knowledge, and prevalence of oral cancer and precancer in this group. The primary objective of this survey is to assess knowledge, awareness and attitude toward oral precancer and cancer among the Narikuravar tribal population in Pondicherry state. Materials and Methods This questionnaire survey is a part of the Model for Oral Cancer Eradication project conducted among the Narikuravar population in Pondicherry, which is funded by Indian Council of Medical Research (ICMR). A total of 153 participants were recruited for the survey. A prevalidated questionnaire was used for the survey. Results The results indicated that 47% of the participants were aware of what is oral precancer and cancer. As much as 62% knew that chewing tobacco causes oral cancer and 44% thought that the growth of tissue in the mouth could be a sign of oral cancer. Almost 100% did not think that the presence of a red lesion, white lesion, or limitation of mouth opening could be signs of cancer or precancer. Only 16% thought that oral cancer is preventable. Conclusion Awareness about oral cancer and precancer is inadequate among the Narikuravar population of Pondicherry. Majority of the population is not aware of the risks, signs, or treatment options of oral precancer and cancer. The National Programme for Prevention and Control of Cancer, Diabetes, Cardiovascular Diseases and Stroke (NPCDCS) identified the people’s knowledge gaps, and improving awareness is one of the strategies of the program.

Oral Cancer (OC) is a serious and growing problem which constitutes a huge burden on people in more and less economically developed countries alike. The scenario is clearly depicted from the increase in the expected number of new cases in the US diagnosed with OC from 49,670 people in 2016, to 49,750 cases in 2017. The situation is even more alarming in India, with 75,000 to 80,000 new cases being reported every year, thus making it the OC capital of the world. Leukoplakia, erythroplakia, oral lichen planus, oral submucous fibrosis, discoid lupus erythmatosus, hereditary disorders such as dyskeratosis congenital and epidermolisys bullosa are highlighted by WHO expert working group as the predisposing factors increasing the risk of OC. Consumption of tobacco and alcohol, genetic factors, and human papilloma virus are assigned as the factors contributing to the aetiology of OC. On the other hand, pathogenesis of OC involves not only apoptosis but also pain, inflammation and oxidative stress. Inspite of current treatment options (surgery, radiotherapy, and chemotherapy), OC is often associated with recurrence and formation of secondary primary tumours resulting in poor overall survival rates (∼50%). The intervention of nano technology-based drug delivery systems as therapeutics for cancers is often viewed as a cutting edge for technologists. Though ample literature on the usefulness of nano-coutured cancer therapeutics, rarely any product is in pipeline. Yet, despite all the hype about nanotechnology, there are few ongoing trials. This review discusses the current and future trends of nano-based drug delivery for the treatment of OC.

Introduction: Patients’ decisions to undergo major surgery such as orthognathic treatment are not just about how the decision is made but what influences the decision. Objectives: The primary objective of the study was to identify the key processes involved in patients’ experience of decision making for orthognathic treatment. Methods: This study reports some of the findings of a larger grounded theory study. Data were collected through face-to-face interviews of patients who were seen for orthognathic treatment at a teaching hospital in the United Kingdom. Twenty-two participants were recruited (age range 18–66 y), of whom 12 (male = 2, female = 10) were 6 to 8 wk postsurgery, 6 (male = 2, female = 4) were in the decision-making stage, and 4 (male = 0, female = 4) were 1 to 2 y postsurgery. Additional data were also collected from online blogs and forums on jaw surgery. The data analysis stages of grounded theory methodology were undertaken, including open and selective coding. Results: The study identified the central role of dental care professionals (DCPs) in several underlying processes associated with decision making, including legitimating, mediating, scheduling, projecting, and supporting patients’ decisions. Six categories were related to key aspects of decision making. These were awareness about their underlying dentofacial problems and treatment options available, the information available about the treatment, the temporality of when surgery would be undertaken, the motivations and expectation of patients, social support, and fear of the surgery, hospitalization, and potentially disliking their new face. Conclusion: The decision-making process for orthognathic treatment is complex, multifactorial, and heavily influenced by the role of DCPs in patient care. Understanding the magnitude of this role will enable DCPs to more clearly participate in improving patients’ decision-making process. The findings of this study can inform future quantitative studies. Knowledge Transfer Statement: The results of this study can be used both for informing clinical practice around enabling decision making for orthognathic treatment and also for designing future research. The findings can better inform clinicians about the importance of their role in the patients’ decision-making process for orthognathic treatment and the means to improve the patient experience. It is suggested that further research could be conducted to measure some of the key constructs identified within our grounded theory and assess how these change during the treatment process.

The growing popularity of inexpensive IoT (Internet of Things) sensor networks makes their uncertainty an important aspect of their adoption. The uncertainty determines their fitness for purpose, their perceived quality and the usefulness of information they provide. Nevertheless, neither the theory nor the industrial practice of uncertainty offer a coherent answer on how to address uncertainty of networks of this type and their components. The primary objective of this paper is to facilitate the discussion of what progress should be made regarding the theory and the practice of uncertainty of IoT sensor networks to satisfy current needs. This paper provides a structured overview of uncertainty, specifically focusing on IoT sensor networks. It positions IoT sensor networks as contrasted with professional measurement and control networks and presents their conceptual sociotechnical reference model. The reference model advises on the taxonomy of uncertainty proposed in this paper that demonstrates semantic differences between various views on uncertainty. This model also allows for identifying key challenges that should be addressed to improve the theory and practice of uncertainty in IoT sensor networks.

Abstract Objectives The aim of the present study is to evaluate the role of immunoglobulins (IgA, IgG, and IgM) and circulating immune complexes (CIC) as tumor marker in oral cancer and precancer patients. Materials and Methods The present study was performed on 45 individuals subdivided into three groups, that is, oral precancer, oral cancer and healthy individuals, and levels of immunoglobulins, and CIC was estimated by turbidometry and ELISA method. Results In the present study, the mean serum IgA levels in oral precancer were 161.00 ( ±  118.02) mg/dL, oral cancers were 270.67 ( ±  171.44) mg/dL, and controls were 133.73 ( ±  101.31) mg/dL. Mean serum levels of IgG in oral precancer were 1,430.87 ( ±  316) mg/dL, oral cancers were 1,234.27 ( ±  365.42) mg/dL, and controls were 593.87 ( ±  323.06) mg/dL. Conclusion We found that the levels of serum IgG and IgA were elevated consistently in precancer and cancer group, and Serum IgM levels were increased only in precancer. Also, significant increase in serum CIC levels were seen in oral precancer and cancer group on comparison with control.

This work presents an exploratory development of J and CTOD estimation procedures for welded fracture specimens under bending based upon plastic eta factors and plastic rotation factors. The techniques considered include: i) estimating J and CTOD from plastic work and ii) estimating CTOD from the plastic rotational factor. The primary objective is to gain additional understanding on the effect of weld strength mismatch on estimation techniques to determine J and CTOD fracture parameters for a wide range of a/W-ratios and mismatch levels. Very detailed non-linear finite element analyses for plane-strain models of SE(B) fracture specimens with center cracked, square groove welds provide the evolution of load with increased load-line displacement and crack mouth opening displacement which are required for the estimation procedure. The results show that levels of weld strength mismatch within the range ±20% mismatch do not affect significantly J and CTOD estimation expressions applicable to homogeneous materials, particularly for deeply cracked fracture specimens. The present analyses, when taken together with previous studies, provide a fairly extensive body of results which serve to determine parameters J and CTOD for different materials using bend specimens with varying geometries and mismatch levels.

Cancer represents one of the leading causes or mortality worldwide, oral cancer accounting for almost 9% of deaths, early diagnosis playing a crucial role. Salivary biomarkers analysis is proving to be an alternative diagnosis method. Oral cancer biomarkers can be compounds that play role in every aspect of malignancy from triggering factors to markers of progression, inflammation or invasiveness. There are numerous genomic markers, ranging from well known ones such as p16, p21, p27 and p53 genes, cyclin D1, EGFR gene (epidermal growth factor receptor), C-kit gene (KIT proto-oncogene, receptor tyrosine kinase), bcl-6(B-cell lymphoma 6 protein gene) to least studied ones such as OXSR1(oxidative stress-responsive kinase-1gene). Proteomic markers range from inflammatory factors such as interleukins IL-8 and Il-6, transcription factors such as FOXO3 (forkhead box O3) protein and S100B protein, matrix metalloproteinases (MMP) involved in extracellular matrix degradation and their inhibitors (TIMP - tissue inhibitor of metalloproteinases), specific proliferation markers such as Ki-67 protein and many more. Developing saliva based oral cancer screening and prognosis tests may lead to better treatment options.

Diabetes mellitus is a significant threat to public health. It is estimated that more than 1.5 million people with diabetes live in the Islamic Republic of Iran. We report on the preliminary results of the national programme for the prevention & control of type 2 diabetes which began in 1996. The pilot project has so far been instituted in 17 provinces. Of 595 717 people aged 30 years and over, 247 518 were classed as at risk and 3.6% had diabetes, 4.3% of women and 2.6% of men. Diabetes prevalence varied from 1.3% in rural areas to 14.5% in large cities. Early detection and control strategies are aimed at diminishing the heavy burden of diabetes

Chronic heart failure (HF) frequently causes progressive decline in kidney function, known as cardiorenal syndrome-2 (CRS2). Current treatment options for CRS2 remain unacceptably limited. Trimethylamine-N-oxide (TMAO), a metabolite of gut microbiota, has recently been implicated in the pathogenesis of both HF and chronic kidney disease. Here we examined whether circulating TMAO is elevated in CRS2 and if so, whether attenuation of circulating TMAO would ameliorate the progression of CRS2. Sprague-Dawley rats underwent surgery for myocardial infarction (MI) or sham (week 0) followed by subtotal (5/6) nephrectomy (STNx) or sham at week 4 to induce CRS2 or control. At week 6, MI + STNx rats and control rats received vehicle or 1.0% 3,3-Dimethyl-1-butanol (DMB, a TMAO inhibitor) treatment for 8 weeks. Compared with control rats, MI + STNx rats exhibited elevated serum TMAO at week 6, which was increased further at week 14 but was attenuated by DMB treatment. MI + STNx rats showed cardiac dysfunction as assessed by echocardiography and renal dysfunction as evidenced by increased serum creatinine and urinary kidney injury molecule-1 and decreased creatinine clearance at week 6. The cardiac and renal dysfunction in MI + STNx rats was exacerbated at week 14 but was prevented by DMB treatment. Molecular and histological studies revealed myocyte hypertrophy and increases in interstitial myocardial fibrosis and gene expression of pro-hypertrophic and pro-fibrotic markers in both heart and kidney at week 14, which were accompanied by elevated gene expression of proinflammatory cytokines. The changes in molecular and histological parameters observed in MI + STNx rats were significantly reduced by DMB treatment. These findings suggest that rats with CRS2 have elevated circulating TMAO, which is associated with the exacerbation of cardiac and renal dysfunction. Attenuation of circulating TMAO can ameliorate cardiac and renal injury and prevents the progression of CRS2.

SUMMARYThe majority of the 30–100 million people infected withStrongyloides stercoralis, a soil transmitted intestinal nematode, have subclinical (or asymptomatic) infections. These infections are commonly chronic and longstanding because of the autoinfective process associated with its unique life cycle. A change in immune status can increase parasite numbers, leading to hyperinfection syndrome, dissemination, and death if unrecognized. Corticosteroid use and HTLV-1 infection are most commonly associated with the hyperinfection syndrome.Strongyloidesadult parasites reside in the small intestine and induce immune responses both local and systemic that remain poorly characterized. Definitive diagnosis ofS. stercoralisinfection is based on stool examinations for larvae, but newer diagnostics – including new immunoassays and molecular tests – will assume primacy in the next few years. Although good treatment options exist for infection and control of this infection might be possible,S. stercoralisremains largely neglected.

Development of psychiatric diseases such as posttraumatic stress disorder (PTSD) invokes, as with most complex diseases, both genetic and environmental factors. The era of genome-wide high throughput technologies has sparked the initiation of genotype screenings in large cohorts of diseased and control individuals, but had limited success in identification of disease causing genetic variants. It has become evident that these efforts at the genomic level need to be complemented with endeavours in elucidating the proteome, transcriptome and epigenetic profiles. Epigenetics is attractive in particular because there is accumulating evidence that the lasting impact of adverse life events is reflected in certain covalent modifications of the chromatin.In this review, we outline the characteristics of PTSD as a stress-related disease and survey recent developments revealing epigenetic aspects of stress-related disorders in general. There is also increasing direct evidence for gene programming and epigenetic components in PTSD. Finally, we discuss treatment options in the light of recent discoveries of epigenetic mechanisms of psychotropic drugs.
