Objective.Determine the prevalence of extended-spectrum β-lactamase (ESBL)–producing Enterobacteriaceae (ESBL-PE) contamination of food and colonization of food handlers in a hospital kitchen and compare retrieved ESBL-PE strains with patient isolates.Design.Cross-sectional study.Setting.A 2,200-bed tertiary care university hospital in Switzerland.Participants.Food handlers.Methods.Raw and prepared food samples were obtained from the hospital kitchen, with a comparator group from local supermarkets. Fecal samples collected from food handlers and selectively pre-enriched homogenized food samples were inoculated onto selective chromogenic media. Phenotypic confirmation of ESBL production was performed using the double disk method. Representative ESBL-PE were characterized using polymerase chain reaction (PCR) and sequencing for blaCTX-M, blaSHV, and blaTEM genes, and Escherichia coli strains were typed using phylotyping, repetitive element palindromic PCR, and multilocus sequence typing. Meat samples were screened for antibiotic residues using liquid chromatography time-of-flight mass spectrometry.Results.Sixty (92%) of the raw chicken samples were ESBL-PE positive, including 30 (86%) of the hospital samples and all supermarket samples. No egg, beef, rabbit, or cooked chicken samples were ESBL-PE positive. No antibiotic residues were detected. Six (6.5%) of 93 food handlers were ESBL-PE carriers. ESBL-PE strains from chicken meat more commonly possessed blaCTX-M-1 and blaCTX-M-2, whereas blaCTX-M-14and blaCTX-M-15 were predominant among strains of human origin. There was partial overlap in the sequence type of E. coli strains of chicken and human origin. No E. coli ST131 strains or blaCTX-M-15 genes were isolated from meat.Conclusions.Although there is significant ESBL-PE contamination of delivered chicken meat, current preventive strategies minimize risks to food handlers, hospital staff, and patients.

Latar belakang: Pemenuhan kebutuhan pangan asal ternak yang berasal dari unggas memiliki angka konsumsi yang cukup tinggi salah satunya yaitu ayam broiler. Untuk memenuhi tingginya permintaan terhadap ayam broiler pada pakandiberifeed additiveserta antibiotic growth promotor (AGP) dalam bentuk antibiotik untuk mempercepat pertumbuhan dan daya tahan tubuh. Pemanfaatan antibiotik yang tidak bijak akan menimbulkan residu antibiotik serta resistensi Escherichia coli penghasil Extended Spectrum β-lactamase (ESBL).Metode: Penelitian ini termasuk penelitian deskriptif. Data primer diperoleh dari hasil screeningtest. Data ditunjukkan dalam tabel dangambar yang dibahas secara deskriptif. Variabel yang diamati residu antibiotik tetrasiklin serta E. coli penghasil ESBL pada daging ayam broiler.Hasil: Temuan terhadap residu antibiotik tetrasiklin pada daging ayam broiler yaitu sebesar 7,14% dengan rerata zona hambat 12,13 mm, sedangkan hasil identifikasi E. coli penghasil ESBL pada daging ayam broiler diperoleh hasil sebesar 71,4%.Simpulan:Residu antibiotik tetrasiklin dalam daging ayam broiler masih tergolong aman dapat dikonsumsi namun, temuanE. coli penghasil ESBL pada daging ayam broiler menimbulkan masalah kesehatan, baik bagi kesehatan hewan maupun manusia, serta dapat menimbulkan resistensi terhadap antibiotik. ABSTRACT Title: Broiler Chicken Meat Sold at Purwokerto's Market Has Antibiotic Residues and Escherichia Coli That Produces EsblBackground: Poultry, of which broiler chickens are one example, is a food source with a relatively high consumption rate.Feed additives and antibiotic growth promoters (AGP) in the form of antibiotics in the ration are given to increase broilers’ growth and endurance to meet the high demand for them. It is important to note that unwise antibiotic use results in the buildup of antibiotic residues and resistance to the Extended Spectrum Beta-lactamase(ESBL)produced by Escherichia coli bacterium.Method: To collect primary data for this descriptive study, the screening test results were employed as a starting point. Next, the information was presented in the form of tables and figures.Result: According to this study, broiler chickens had a tetracycline residue of 7.14 percent, with an inhibitory zone mean of 12.13 mm, and 71.4 percent of ESBL-producing E. coli.Conclusion:According to the study, broiler chicken meat with tetracycline residue is still safe to consume. E. coli, which is known to develop an antibiotic-resistant strain of E. coli ESBL) can cause serious health problems in both humans and animals.

Sunan Ampel Religion Tourism area Surabaya is one of the most visited destinations for visitors and tourists that is surrounded by food traders carts, unfortunately food hygiene and sanitation practice among them is still poorly implemented that it could initiates food borne disease transmission. This study aims to analyze the personal hygiene conditions of food handlers, sanitation of merchant facilities and quality of feeders in the Sunan Ampel Religious Tourism Area, Surabaya. This research is a descriptive observational study with cross sectional design. The population in this study were 30 penyetan food traders and food samples in the form of sambal (chili paste) penyetan to be observed for the presence or absence of E. coli bacteria. The results of the study showed that the penyetan food handlers in the Sunan Ampel religious tourism area were at at age group of 20-25 years old for 14 subjects (46.67%). The subject respondents in majority are men with total of 21 subjects (70%). The education level of the respondents is high school for 10 respondents (33.33%). The highest proportion of work period is 1-5 years is for 14 respondents (46.66%). Overall, hygiene condition of the food handlers is 100% in high category. Sanitation conditions for food merchant facilities that meet the requirements are 28 stalls (93.33%). Laboratory test results found 2 samples of chili paste positive with E. Coli bacteria. This study concludes that the hygiene condition of food handlers in the Sunan Ampel Religious Tourism Area was nearly in accordance with hygiene and sanitation standard, however, further practice monitoring is required as respondents frequently did not use aprons, used jewelry and smoking while working. therefore, a risk of food contamination is still a significant threat of the area. The result of the sanitation assessment of public facilities within the area showed that 70% of them have been met with the requirement.

This study aimed to determine the occurrence of antibiotic resistance genes for β-lactamases; blaTEM and blaCTX-M in uropathogenic Escherichia coli isolates from urinary tract infection (UTI) suspected diabetic and nondiabetic patients. A hospital-based cross-sectional study was conducted in Kathmandu Model Hospital, Kathmandu, in association with the Department of Microbiology, GoldenGate International College, Kathmandu, Nepal, from June to December 2018. A total of 1,267 nonduplicate midstream urine specimens were obtained and processed immediately for isolation of uropathogens. The isolates were subjected to antibiotic susceptibility testing and extended spectrum β-lactamase (ESBL) confirmation. In addition, blaTEM and blaCTX-M genes were detected using specific primers. The overall prevalence of UTI was 17.2% (218/1,267), of which patients with diabetes were significantly more infected; 32.3% (31/96) as compared with nonpatients with diabetes, 15.9% (187/1,171). A total of 221 bacterial isolates were obtained from 218 culture-positive specimens in which E. coli was the most predominant; 67.9% (150/221). Forty-four percent (66/150) of the total E. coli was multidrug resistant and 37.3% (56/150) were ESBL producers. Among 56 isolates, 92.3% (12/13) were from patients with diabetes, and 83.0% (44/53) were from nondiabetics. Furthermore, 84.9% of the screened ESBL producers were confirmed to possess either single or both of blaTEM and blaCTX-M genes. The blaTEM and blaCTX-M genes were detected in 53.6% and 87.5% of the phenotypically ESBL confirmed E. coli, respectively. Higher rates of ESBL producing uropathogenic E. coli are associated among patients with diabetes causing an alarming situation for disease management. However, second-line drugs with broad antimicrobial properties are still found to be effective drugs for multidrug resistance strains.

Hygiene and sanitation practices in chicken influenced Eschericia coli bacterial contamination in food. A Study conducted by Sasmita and Juwita mentioned that there was positively E.coli content in chicken meat in supermarket. Thus, the purpose of this study was to assess the difference of E.Coli content in chicken meat and personal hygiene of food handler. It was an observational study with cross sectional approach. The population of this study included traditional market sellers and supermarkets. The sample of this study consisted of 14 samples of chicken meat in which 7 samples came from the South Keputran traditional market and other 7 samples came from Supermarket ‘X’. Moreover, 7 traditional market sellers and 2 supermarkets were involved to be examined. Variables of the study were E.coli content in chicken meat from Traditional Market of South Keputran and Supermarket ‘X’ and personal hygiene. Samples of chicken meat was done by accidental sampling. Data were collected through interview and observation, whereas the difference of E-Coli content was analyzed using statistical test. The results of this study indicated that one of chicken meat samples positively contained E.Coli bacteria, and no significant differences of the E.coli content were found on the chicken meat samples from both the Traditional Market of South Keputran and Supermarket ‘X’. The suggestion that can be given to Supermarket ‘X’ seller is to control and pay close attention to the sanitation process from suppliers to retails

Background: Opportunistic bacterial infections remain a serious morbidity among cancer patients. This study was aimed to determine the bacteriological and antibiotic profile of cancer patients admitted to the ICU of a tertiary care centre.Methods: Cross sectional study was done among cancer patients admitted in the Oncology neutropenic ICU during the period from August 2017 to July 2019. All patients admitted with a proven diagnosis of cancer for whom at least one bacterial culture was sent from any site were included in the study. Laboratory on culture reports were obtained from patient files and analysed.Results: A total of 278 samples from 256 patients (60±11.6 years) were analysed. Among the 111/278 positive cultures, 29 were blood samples and 1 was a pleural fluid sample. Gram negative organisms were 62.1% with Escherichia coli (25, 36.2%) as prevalent. Among the 37.8% gram positives, Staphylococcus aureus (18. 42.8%) was prevalent. Most of the E. coli strains showed highest resistance to ceftazidime (96%) and highest sensitivity to amikacin. The commonest gram-positive organism, Staphylococcus species were 100 % sensitive to vancomycin and linezolid and 100 % resistance to penicillin.  Conclusions: E. coli (gram negative) showed highest resistance to ceftazidime and sensitivity to amikacin. S. aureus (gram positive) was sensitive to vancomycin and linezolid and resistance to penicillin. An antibiogram for cancer patients helps the clinician to initiate an appropriate empirical antibiotic therapy to reduce mortality and morbidity.

Objectives: To determine incidence of steatosis in non-responder casesof chronic HCV. Study Design: Cross-sectional study. Setting: Tertiary Care Sanatorium inMedicine Department of LIAQUAT University Hospital Hyderabad/Jamshoro. Period: 26-9-2011to 25 August 2012. Material & Methods: Total 144 non-responders cases of Hepatitis C wereintegrated. Both gender, age 18 to 50 yrs, chronic HCV non-responder cases as well as cases fitfor hepatic biopsy were integrated in study. Grading of Inflammation was carried out as indicatedby histopathological measures. Results: Mean age was 48.5 + 5.2years. Uppermost prevalenceof chronic HCV disease was 65(45.13%) at 41 to 50 years age interval whereas 42(29.1%) caseswere noticed in 31 to 40 yrs age group and 25(17.3%) cases were observed in 20 to 30 yearsof young age group. In this study hepatic steatosis occurrence was observed in 103 (71.5%)cases and these were categories as: grade 1, grade 2, grade 3, as (<33%) 45/103(43.68%), (33to 66%) 35/103(33.98%), (>66%) 23/103(22.33%) respectively. Conclusion: Hepatic steatosisis a important risk factor the cases of HCV to decreased response to antiviral therapy and forprogression toward fibrosis as observed in cases related to metabolic steatosis.

Objectives: To determine the thrombocytopenia as marker of maternal sepsis and its related maternal morbidity and mortality at tertiary care Hospital. Study Design: Cross-sectional study. Setting: Gynaecological and Obstetrical department of Liaquat University Hospital, Hyderabad. Period: 6 months from March 2017 to August 2017. Material and Methods: All the pregnant females with diagnosis of sepsis were enrolled in the study. All the females with chronic hepatitis were excluded. After complete clinical examination every women underwent 5cc blood sample for complete blood picture. Thrombocytopenia was characterized as a platelet count below 150.000/mm3. Data regarding maternal mortality and maternal complications was filled in the proforma. Results: Total 120 septic mothers were included in the study, 70 patients had thrombocytopenia and 50 were with normal platelets. Most of the women 71.7% were with age groups of 20-30 years. Out of total women 65.0% were un-booked. According to the maternal morbidity, septic shock was most common 36.7%, multi-organ failure was in 08.3%, prolonged Hospital stay was in 16.7%, ICU admission occurred in 18.3% patients, while renal failure, respiratory failure, hepatic failure, coagulopathy and metabolic acidosis were found with percentage of 09.2%, 02.5%, 10.8%, 10.8% and 03.3% respectively. Mortality rate was found among 8.3% out of total cases. Almost all complications were higher among women with thrombocytopenia as compare to women with normal platelets level, while statistically p-value was quite insignificant. Mortality was significantly high among patients with thrombocytopenia p-value 0.032. Conclusion: It was concluded that thrombocytopenia is a good marker for adverse outcome among septic mothers. Maternal morbidity and mortality was higher among septic women with thrombocytopenia.

Canteen has an important role to provide of nutritious and safe food for students during school hours with relatively affordable prices. The results of Food and Drugs Monitoring (BPOM) inspection in 2013 by taking a sample of School Children Confectionary (PJAS) found that 80.79% of the samples confirmed as free from microbiological contamination. These results decrease in 2014, whereas PJAS samples confirmed as free from microbiological contamination is was 76.18%. This study aimed to analyze the implementation of personal hygiene food handlers in the cafeteria of SDN Model to figures Total Plate Count (TPC) in food. This was a descriptive study using cross sectional design. Samples were taken with total sampling where the sample size in this study was the total population of cafeteria booth at SDN Model, there are 5 booths canteen, 5 food handlers, and 5 food samples were taken from each canteen booth. All food samples were tested for bacteriological quality using Total Plate Count (TPC) parameter. The use of Personal Protective Equipment (PPE) such as mask, gloves, headgear, and apron is important to minimize contamination of microorganism from food handlers to food produced. In addition, the selection of ingredients, storage duration, and storage temperature are required to be considered. Suggestion for the cafeteria supervisor of SDN Model and food handler: for the cafeteria supervisor SDN Model to impose regulation and education for food handlers to use PPE during food processing. It is suggested to food handlers to get used to in using PPE during processing the food
