Abstract The study conducted in a rural sub-district, Matlab, Bangladesh among adolescent motherhood (10-19 years) in the International Centre for Diarrhoeal Disease Research, Bangladesh included total of 4,996 adolescent mothers in the analysis. Chi-square tests and binary logistic regression were used to document adolescent pregnancy trends and the differences in and causes of perinatal death. The fertility rate was 27 per 1000 adolescent mothers in ISA and 20 per 1000 adolescent mothers in GSA, during the 9 year of study period. The adjusted odds of an adolescent mother having a perinatal death in ISA, relative to GSA was 0.69 (95% CI: 0.52 - 0.91, p-value<0.05). Significant determinants of perinatal death among adolescent mothers were: maternal education, paternal education, mothers' age at first birth, asset score and distance from nearest facility. Dr. Rahman will raise the importance of addressing these determinants in designing health policy interventions targeting this specifically vulnerable group.

PurposeSixteen million adolescents give birth each year, constituting 11% of all births worldwide. Adverse adolescent pregnancy outcomes are well-documented. Available data on adolescent pregnancies have mainly relied on self-reported age and retrospective survey data, which might not capture adolescent births accurately. This paper reports on trends in adolescent pregnancy and associated adverse birth outcomes in Matlab, Bangladesh, using data from the Matlab Heath and Demographic System (HDSS) which precisely documents maternal age.Design/methodology/approachThe study was conducted in the rural subdistrict of Matlab in Bangladesh. HDSS data were used to examine trends in adolescent motherhood (10–19 years) in the International Centre for Diarrhoeal Disease Research, Bangladesh (icddr, b) service areas (ISA) and government service areas (GSA) between 2007 and 2015. A total of 4,996 adolescent mothers were included in the analysis. Chi-square testing and binary logistic regression were used to document adolescent pregnancy trends and the differences in and causes of perinatal death.FindingsThe fertility rate was 27 per 1000 adolescent mothers in ISA and 20 per 1000 adolescent mothers in GSA, during the 9 years of the study period. The adjusted odd of an adolescent mother having a perinatal death in ISA, relative to GSA was 0.69. Significant determinants of perinatal death among adolescent mothers included maternal education, paternal education, mother’s age at first birth, asset score and distance from the nearest health facility.Originality/valueThis paper documents the real trend of adolescent pregnancy by capturing the accurate age at pregnancy for the first time in Bangladesh.

Abstract Background Adolescent mothers (Girls aged 15-19) constitute 8% of annual global births, but account for 10% of annual maternal deaths. WHO recommended 4-8 Antenatal Care (ANC) visits, in addition to quality care and facility-based deliveries, are well-documented interventions to reduce maternal and child morbidity and mortality. Determinants of maternal and child health care in Bangladesh have received considerable attention, but less attention has been focused on adolescent mothers. This study explores the factors associated with 4 or more (4+) ANC visits and facility-based delivery among adolescent mothers in one rural area of Bangladesh, Methods This study uses Health and Demographic Surveillance System (HDSS) data. We conducted a comparative study on trends in 4+ ANC visits and facility-based deliveries among adolescent mothers (10-19 years) residing in an intervention area (icddr,b service area, ISA) against a comparison area (government service areas, GSA) of HDSS between 2007 and 2015. 4,996 adolescent mothers were included in the final analysis. Binary logistic regression was used to document the statistical difference on outcome indicators in the two study areas. Results Trends in 4+ANC visits and facility-based deliveries were higher in the ISA relative to the GSA. The adjusted odds of an adolescent mother accessing 4+ ANC visits in the GSA, relative to ISA was 0.57 (95% CI: 0.49 – 0.66, p-value <0.05); the adjusted odds of an adolescent mother accessing facility-based delivery in the ISA, relative to GSA was 6.63 (95% CI: 5.85 – 7.52, p-value <0.05). Increasing numbers of ANC visits were associated with increases in facility-based births in both the ISA and GSA. Conclusion This study documented that both 4+ ANC visits and facility delivery rates among adolescent mothers are much higher in the ISA than GSA. Increasing 4+ ANC visits and facility deliveries over the years, particularly in the ISA, coincide with programmatic efforts to improve the quality and availability of maternal and newborn health services. Learning from existing interventions in ISA and applying them to other areas will strengthen Bangladesh’s efforts to improve maternal and newborn health outcomes and achieve the sustainable development goal 3 (SDG 3).

Background: Child health and growth deteriorate as a result of inadequate quantities and frequencies of complementary feeding. Methods: A community-based cross-sectional study was conducted in urban field practice area of a tertiary medical college hospital among 280 children by systematic random sampling method. Children belonging to 12-23 months were included while sick child, children whose mother/guardian did not give consent were excluded. Results: The proportion of children who met the minimum dietary diversity (MDD) and minimum meal frequency (MMF) were 52.8 % (95% CI) and 52% (95% CI), respectively. Out of 280 children, 90 i.e. (32.1 %) children had achieved the recommended Minimum acceptable diet (MAD). Maternal education (Chi square value=11.2; CI 95%); socioeconomic status (Chi square value=57.4; CI 95%) were positively associated with dietary diversity. Higher percentage of male children (44.8 %) achieved minimum acceptable diet as compared to female children (20.5%); (chi square value=18.8, CI 95%). The percentage of children receiving MDD and MMF was more among children of higher socioeconomic class (p value 0.0001) and from nuclear family (p value=0.45). The birth order of the child was inversely related to the attainment of MAD. (p value=0.0001).Conclusions: Results of the study strongly suggest that poor dietary diversity and meal frequency is associated with numerous factors and their interplay is a multitude of combinations rather than a single cause.

Background: Teenage pregnancy is a common public health problem worldwide which is detrimental to the health of mother and child and has long been considered a high-risk situation. The risk of low birth weight (LBW) and preterm delivery is particularly high among teenagers. Methods: A comparative study was conducted in Nepalgunj Medical College Teaching Hospital, Kohalpur during the period of July 2015 to June 2016. The study was carried out to compare the immediate neonatal outcome and morbidity pattern in neonates of 50 adolescent and 50 adult mothers. Results: In the present study, 84% belonged to adolescent mother group (17-19 years) whereas 50% belonged to adult mother group (20-23 years). Illiteracy was seen more in adolescent mothers (62%) and most were from rural areas (68%). The common immediate neonatal outcome found in adolescent and adult mothers were preterm delivery (96% vs. 52%; p value 0.001), low birth weight (LBW) (70% vs. 38%; p value 0.001). The common neonatal morbidities seen significantly high in neonates of adolescent mothers, viz: Neonatal sepsis (NNS)(54% vs. 20% p value <0.02), apnea (30% vs. 14% P value <0.02), neonatal jaundice(NNJ)(44% vs. 30% p value <0.01) while the other morbidities found were birth asphyxia (20% vs. 14%; p value <0.1), Respiratory distress syndrome (RDS) (36% vs. 24% p value<0.1), anemia (16% vs. 8%; p value <0.1), seizure (10% vs. 8%; p value <0.1), meconium aspiration syndrome (MAS) (6% vs. 18%; p value<0.1) and intrauterine growth retardation(IUGR)(22% vs. 20%; p value <0.5). Similarly mortality was found to be more in neonates of adolescent mothers (14% vs. 8%; p value <0.1).Conclusion: Adolescent pregnant mothers are at risk of having poor neonatal outcome and morbidities like NNS, NNJ, RDS, apnea, IUGR, birth asphyxia, anemia and seizure. JNGMC,  Vol. 14 No. 2 December 2016, Page: 56-59

Background: Preterm birth (PTB) is a complex complication of pregnancy with multiple etiologies. This results in long term medical burdens to the families and health care system at large. The objective of the study was to determine the socio-demographic and obstetric characteristics that affect preterm delivery. Methods: The study was a prospective cohort study conducted at the Obstetrics and Gynecology Department of a tertiary health facility in Nigeria. Data for the study were retrieved from the ante-natal and delivery card of the women that delivered at the unit within the time of data collection. Data was analyzed using SPSS version 25 and variables were presented as frequencies, percentages, means, and standard deviation. Bivariate analysis was done using chi-square test. The level of significance was set at p value ≤ 0.05. Binary logistic regression was used to determine factors that predicted preterm delivery. Results: Majority of them were married 746(96.9%), Igbos 763(99.1) and Christians 766(99.5%). Most of the women were civil servants 429(55.7%), while about 31.3% of them were unemployed 241(31.3%). Majority of the women 484(62.9%) had tertiary education. About 48% of them delivered through caesarean section while 53.9% booked within 14-28weeks gestation. Delivering through caesarean section and gestational age at booking predicted preterm delivery. Un-booked mothers have about 7times odds of having PTB when compared to those that booked at ≥28 week’s gestation. Conclusion: The prevalence of PTB is still high in Nigeria. Booking status of the mother and C/S delivery were found to be strongly associated with preterm delivery. Key words: Enugu State, Nigeria, Preterm delivery, Prevalence, Tertiary health facility.

Introduction The global tobacco epidemic contributes to more than 8 million deaths annually. However, most tobacco control interventions have been driven by an emphasis on smoked tobacco. Globally and more so in Nigeria, less attention has been paid to the similarly harmful smokeless tobacco (SLT) whose use appeals to a different demography. We examined the prevalence, patterns of use and correlates of SLT in Nigerian adults to guide targeted control efforts. Methods We conducted a secondary analysis of the 2012 Global Adult Tobacco Survey (GATS) data. We obtained data on 9,765 non-institutionalised adults aged 15 years and older. Variables included current SLT use, sociodemographic characteristics and perceived harm of SLT use. We used Chi-square test to examine associations and binary logistic regression to assess predictors of current SLT use. All analyses were conducted with sample-weighted data. Results The prevalence of current SLT use was 1.9% of all adults. About 1.4% were daily users. The main types were snuff by nose (1.6%) and snuff by mouth (0.8%). There were higher odds of current SLT use for those in the South-East region (aOR = 13.99; 95% CI: 4.45–43.95), rural area residents (aOR = 1.56; 95% CI: 1.04–2.35), males (aOR = 4.43; 95% CI: 2.75–7.11), the 45–64 years age-group (aOR = 10.00; 95% CI: 4.12–24.29), those with no formal education (aOR = 2.67; 95% CI: 1.01–7.05), and those with no perception of harm from SLT use (aOR = 3.81, 95% CI: 2.61–5.56). Conclusion The prevalence of SLT use among Nigerian adults was low with clearly identified predictors. While a majority were aware of harm from SLT use, an unacceptably high proportion remain unaware. We recommended targeted interventions to increase awareness of the harmful effects of SLT use especially among residents of the South-East, those in rural areas, males, and individuals with no formal education. We also recommended a follow-up survey.

Background: Type of anesthesia during elective cesarean is very important and is chosen according to the decision of the specialist as well as the mother’s desire. This study aimed to determine the rate and associated factors of choosing general or regional (spinal and epidural) anesthesia among pregnant women who underwent elective cesarean in hospitals of Northern Iran in 2017. Methods: This descriptive-analytic study included the pregnant women referred to the hospitals in Sari with indications for both kinds of anesthesia. The reasons for choosing the type of anesthesia were asked and collected in a researcher-made checklist. The collected data were described and analyzed using SPSS software (version 24) through the Chi-square or Fisher's exact tests and Logistic regression. A p-value less than 0.05 was considered statistically significant. Results: Out of 384 pregnant women who participated in the study, 60% and 40% of the cases chose general and regional anesthesia, respectively. Fear of spinal cord damage (64.3%) and fear of observing and hearing in the operation room (53.3%) were the most reasons for the rejection of the regional methods of anesthesia. However, the fear of not waking up (54.3%) and being interested in seeing the baby during childbirth (40.7%) were the main reasons for choosing spinal anesthesia. Most of the women who had experienced regional anesthesia selected this procedure in the current operation (53%), while general anesthesia was chosen by women without previous history of regional anesthesia (62%). Conclusion: Although most of the pregnant women selected general anesthesia, the reasons for rejecting the spinal method were mainly non-scientific and could be managed with maternal education.
