Background: An important step in accelerometer data analysis is the classification of continuous, 24-hour data into sleep, wake, and non-wear time. We compared classification times and physical activity metrics across different data processing and classification methods.Methods: Participants (n = 576) from the Finnish Retirement and Aging Study (FIREA) wore an accelerometer on their non-dominant wrist for seven days and nights and filled in daily logs with sleep and waking times. Accelerometer data were first classified as sleep or wake time by log, and Tudor-Locke, Tracy, and ActiGraph algorithms. Then, wake periods were classified as wear or non-wear by log, Choi algorithm, and wear sensor. We compared time classification (sleep, wake, and wake wear time) as well as physical activity measures (total activity volume and sedentary time) across these classification methods.Results:M(SD) nightly sleep time was 467 (49) minutes by log and 419 (88), 522 (86), and 453 (74) minutes by Tudor-Locke, Tracy, and ActiGraph algorithms, respectively. Wake wear time did not differ substantially when comparing Choi algorithm and the log. The wear sensor did not work properly in about 29% of the participants. Daily sedentary time varied by 8–81 minutes after excluding sleep by different methods and by 1–18 minutes after excluding non-wear time by different methods. Total activity volume did not substantially differ across the methods.Conclusion: The differences in wear and sedentary time were larger than differences in total activity volume. Methods for defining sleep periods had larger impact on outcomes than methods for defining wear time.

Abstract Background Reliable estimates of habitual sleep, physical activity, and sedentary time are essential to investigate the associations between these behaviours and health outcomes. While the number of days needed and hours/day for estimates of physical activity and sedentary time are generally known, the criteria for sleep estimates are more uncertain. The objective of this study was to identify the number of nights needed to obtain reliable estimates of habitual sleep behaviour using the GENEActiv wrist worn accelerometer. The number of days to obtain reliable estimate of physical activity was also examined. Methods Data was used from a two-year longitudinal study. Children wore an accelerometer for up to 8 days 24 h/day across three timepoints. The sample included 2,745 children (51 % girls) between the ages of 7-12-years-old (mean = 9.8 years, SD = 1.1 year) with valid accelerometer data from any timepoint. Reliability estimates were calculated for sleep duration, sleep efficiency, sleep onset, wake time, time in bed, light physical activity, moderate physical activity, moderate-to-vigorous physical activity, vigorous physical activity, and sedentary time. Results Intraclass correlations and the Spearman Brown prophecy formula were used to determine the nights and days needed for reliable estimates. We found that between 3 and 5 nights were needed to achieve acceptable reliability (ICC = 0.7) in sleep outcomes, while physical activity and sedentary time outcomes required between 3 and 4 days. Conclusions To obtain reliable estimates, researchers should consider these minimum criteria when designing their studies and prepare strategies to ensure sufficient wear time compliance.

Objectives: Prior literature suggests that sedentary behavior may represent a risk factor that is independent of physical activity. The mechanism for this association remains poorly understood. We hypothesized that sedentary behavior might be inversely associated with cardiorespiratory fitness, independent of moderate and vigorous activity. Methods: We included 2,223 participants (ages 12-49 years, 47% female) without known heart disease who had both cardiovascular fitness testing and at least one valid day of accelerometer data from NHANES 2003-2004. From accelerometer data, we quantified bouts of exercise (at least 8 of 10 minutes above previously defined thresholds corresponding to moderate- or vigorous-intensity activity) as mean minutes of activity bouts per day for each participant. Sedentary time was defined as <100 counts per minute of wear time in mean minutes per day. VO2 max estimates were derived from a sub-maximal exercise treadmill test using measured heart rate responses to known levels of exercise workloads. Multivariable-adjusted linear regression analyses were performed with fitness as the dependent variable. Models were stratified by gender and adjusted for age, BMI, mean wear time and included both sedentary and moderate-vigorous activity time. Results: We observed that moderate and vigorous exercises levels were positively correlated to VO2 max (R=0.237, p<0.0001). Sedentary time was found to be inversely associated with VO2 max (R=-0.07, p=0.002). With each additional minute of moderate-vigorous activity, VO2 max increased by 0.05 ml/kg/min (p=0.001, men) and 0.08 ml/kg/min (p=0.004, women). For each additional minute of sedentary time, VO2 decreased by 0.01 ml/kg/min in both men (p=0.026) and women (p=0.001). Conclusions: After adjustment for moderate and vigorous activity, sedentary behavior appears to have a minor, inverse association with fitness. These findings suggest that the risk related to sedentary behavior might be mediated, in part, through lower fitness levels.

AbstractPrevious research in children and adolescents with congenital heart defects presents contradictory findings concerning their physical activity (PA) level, due to methodological limitations in the PA assessment. The aim of the present cross-sectional study was to compare PA in children and adolescents treated for valvular aortic stenosis with healthy controls using an improved accelerometer method. Seven-day accelerometer data were collected from the hip in a national Swedish sample of 46 patients 6–18 years old treated for valvular aortic stenosis and 44 healthy controls matched for age, gender, geography, and measurement period. Sports participation was self-reported. Accelerometer data were processed with the new improved Frequency Extended Method and with the traditional ActiGraph method for comparison. A high-resolution PA intensity spectrum was investigated as well as traditional crude PA intensity categories. Children treated for aortic stenosis had a pattern of less PA in the highest intensity spectra and had more sedentary time, while the adolescent patients tended to be less physically active in higher intensities overall and with less sedentary time, compared to the controls. These patterns were evident using the Frequency Extended Method with the detailed PA intensity spectrum, but not to the same degree using the ActiGraph method and traditional crude PA intensity categories. Patients reported less sports participation than their controls in both age-groups. Specific differences in PA patterns were revealed using the Frequency Extended Method with the high-resolution PA intensity spectrum in Swedish children and adolescents treated for valvular aortic stenosis.

Abstract Purpose: The aim of this cross-sectional study was to describe and analyze accelerometer-measured sedentary time and physical activity (PA) among adolescents in a multicultural area characterized by low socioeconomic status (SES). Method: Seventh-graders (n=114 (girls n=66), mean age: 12.8±0.5 y) were recruited from three schools in a multicultural area of the city of Gothenburg, Sweden. Sedentary time and PA were measured with ActiGraph™ accelerometers. Result: Of total wear-time, 70 (±6)% was sedentary, with girls being more sedentary than boys. Girls had less light PA (LPA) and moderate-to-vigorous PA (MVPA) than boys. Similar patterns were shown during in-school and out-of-school hours. During wear-time, 53% had a mean of ≥60 min of MVPA per day, but only 6% of the girls and 24% of the boys were sufficiently physically active every day. Girls had more sedentary bouts of ≥10 min and fewer MVPA bouts of ≥5 min per day than boys. Those who participated in organized sports spent a mean of 15 more minutes of MVPA per day compared to those who did not. No association was observed between body mass index (BMI) and sedentary time and PA. Conclusion: Only a few adolescents from a Swedish multicultural area characterized by low SES met the PA recommendations every day, and girls were more sedentary and less physically active than boys. Adolescents involved in organized sports had more of MVPA per day than their non-involved peers. Sedentary time and PA were not related to BMI.

This study investigated the association between ambient air quality and sedentary time in Chinese adults. The participants were 3270 Chinese users (2021 men and 1249 women) of wrist-worn activity trackers. The data of participants’ daily activities were collected from July 2015 to October 2015. A novel algorithm based on raw accelerometer data was employed to determine sedentary time. Personal data, including sex, age, weight and height, were self-reported by the participants. Data of air quality, ambient temperature and weather were collected from the data released by the China National Environmental Monitoring Centre and the China Central Meteorological Observatory and matched in accordance with the Global Positioning System and time information. Multilevel regression analyses were conducted to investigate the association between air quality and sedentary time and adjusted for gender, age, region, body mass index, weather, temperature, weekday/weekend and monitored wake time per day. Better air quality index levels and lower concentrations of fine particulate matter were significantly associated with approximately 20 and 45 min reduction in sedentary time, respectively. Poor air quality appears to be an independent factor associated with prolonged sedentary time in Chinese adults.

Background The aim of this study was to validate the Total Activity Measure, a brief questionnaire, to measure physical activity in an older adult population with heart disease. Methods Two versions of the Total Activity Measure were administered twice, 7 days apart. The Total Activity Measure 1 asked respondents for the frequency and average duration of bouts of physical activity at three different intensity levels per week, whereas the Total Activity Measure 2 asked respondents for the total time spent in activity at each activity level per week. Questionnaire accuracy was studied in 62 men and 15 women aged 47-84 years, by repeatability and comparison of both administrations of the Total Activity Measure 1 and Total Activity Measure 2 with 7-day RT3 accelerometer data. Results Seventy-three adults (58 men, 15 women) were used for all statistical analyses. Intraclass correlation coefficients for the Total Activity Measure 1 and Total Activity Measure 2 total activity scores (metabolic equivalent per minute) were r = 0.73 (95% confidence intervals, 0.56-0.83) and r = 0.82 (95% confidence intervals, 0.71-0.88), respectively. Correlations between the Total Activity Measure 1 and RT3 accelerometer for total activity score (metabolic equivalent per minute) were significant, r = 0.26 at time 1 and r = 0.27 at time 2 for moderate intensity activities. Correlations between the Total Activity Measure 2 and RT3 accelerometer for total activity score (metabolic equivalent per minute) were also significant, r = 0.38 at time 1 and r = 0.36 at time 2, r = 0.31 at time 2 for strenuous intensity activities and r = 0.29 at time 1 and r = 0.25 at time 2 for moderate intensity activities. Participants overestimated the amount of physical activity on both questionnaires as compared with the RT3 accelerometer. Conclusions The Total Activity Measure 2 was reasonably accurate in assessing total and moderate intensity activity over a 7-day period and demonstrated good test-retest reliability. The Total Activity Measure 1 was less accurate. The Total Activity Measure 2 is a suitable measure of total or moderate intensity physical activity for surveys and audits in an adult cardiac population. Purpose:To compare physical activity measures and their associations with insulin sensitivity, β-cell function and body mass index (BMI) between Iraqi immigrants and native Swedes.Methods:A cross-sectional study of 493 Iraqis (58% men) and 469 Swedes (54% men) aged 30 to 75 years living in the city of Malmö, Sweden. Accelerometry was used for physical activity measures (sedentary time, breaks in sedentary time, moderate and vigorous physical activity, total counts). Insulin sensitivity index and oral disposal index were determined from an oral glucose tolerance test and BMI by body weight and height.Results:Iraqi men were less physically active than Swedish men, while the physical activity was more similar in the women. BMI was a strong predictor of insulin sensitivity and β-cell function and frequently associated with the physical activity measures. BMI modified the associations of insulin sensitivity and β-cell function with the physical activity measures to such extent that only VPA and total counts show direct associations with insulin sensitivity in addition to the indirect associations via BMI. Iraqi women demonstrated weaker associations compared with Swedish women.Conclusions:Physical activity and performed at vigorous intensity may be important mainly for the insulin sensitivity in Iraqi immigrants and native Swedes.

Objectives: To characterize and quantify differences in accelerometer-measured physical activity and sedentary time between children with concussion (within the first month of injury) and 1:1 matched healthy controls. Methods: Secondary analysis of accelerometer data collected on 60 children with concussion and 60 healthy controls matched for age, sex, and season of accelerometer wear. Daily and hourly sedentary time, light physical activity (LPA), moderate physical activity (MPA), and vigorous physical activity (VPA) were compared between groups per independent samples t-tests. Results: Children with concussion (12.74 ± 2.85 years, 31 females) were significantly more sedentary than controls (12.43 ± 2.71 years, 31 females; mean difference [MD], 38.3 minutes/day, p=0.006), and spent less time performing LPA (MD, -19.5 minutes/day, p=0.008), MPA (MD, -9.8 minutes/day, p<0.001), and VPA(MD, -12.0 minutes/day, p<0.001); hour-by-hour analyses showed that these differences were observed from 8:00AM to 9:00PM. Sex-specific analyses identified that girls with concussion were less active and more sedentary than both boys with concussion (MD, 50.8 minutes/day; p=0.010) and healthy girls (MD, 51.1 minutes/day; p<0.010). Days post-injury significantly predicted MPA (β=0.071, p=0.032) and VPA (β=0.095, p=0.004), but not LPA or sedentariness in children with concussion. Conclusion: Clinical management should continue to advise against prolonged rest following pediatric concussion, given the activity debt observed within the first-month of injury. Currently, clinical management of concussion is shifting towards prescribing a single bout of daily sub-maximal aerobic exercise. Interventions aimed at reducing overall sedentary time and increasing habitual physical activity in pediatric concussion also warrant study.

2016 has been an exciting year for research in physical activity, inactivity and health. Recognition of the importance of all physical behaviors (physical activity, sedentary time and sleep) across the 24-hr day continues to grow. Notable advances have included: applications of recent methodological innovations that account for the codependence of the behaviors in the finite 24-hr period showing that the balance of these behaviors is associated with health; methodological innovations focusing on the classification of behaviors and/or quantification of the 24-hr diurnal activity pattern; and a series of systematic reviews that helped provide the evidence base for the release of the innovative 24-hr movement guidelines earlier this year. This commentary focuses on just two of these papers: the first by Goldsmith and colleagues who demonstrate a new statistical method that exploits the time series nature of accelerometer data facilitating new insights into time-specific determinants of children’s activity patterns and associations with health; the second by Tremblay and colleagues who describe the evidence base for associations between each physical behavior and children’s health, the emerging evidence base for associations between the balance of behaviors and health, and development of the world’s first 24-hr movement guidelines.

Abstract Context Time spent in moderate-to-vigorous physical activity (MVPA), but not in sedentary behavior (SB), is related to cardiometabolic risk among non-Hispanic white youth. Objective Examine associations of SB and MVPA with cardiometabolic risk factors among Hispanic/Latino youth. Design Cross-sectional analysis. Setting Four US communities. Participants Hispanic/Latino youth (N = 1,426) ages 8 to 16 years. Measurements Associations of MVPA and SB, measured using 7-day accelerometer data (independent variables), with markers of glucose and lipid metabolism, inflammation, and endothelial function (dependent variables), were assessed in multivariable linear regression models while adjusting for sociodemographic characteristics and accelerometer wear time. Additional models controlled for obesity measures. Results SB comprised a mean (SD) of 75% (13%) of accelerometer wear time; mean (SD) time of MVPA was 35 min/d (22 min/d). Deleterious levels of high-density lipoprotein–cholesterol (HDL-C), triglycerides, insulin resistance, C-reactive protein, and plasminogen activator inhibitor-1 were associated with lower levels of MVPA and higher levels of SB (all P &lt; 0.05). Associations of MVPA with log-transformed triglyceride concentrations (β per 15-min/d increment, −0.039; SE, 0.018; P = 0.037) and SB with HDL-C (β per 30-min/d increment, −0.63; SE, 0.26; P = 0.018), but not those with other markers, remained significant after adjusting for MVPA or SB and further adjustment for body mass index and waist circumference. Higher SB tertiles were associated with lower soluble receptor for advanced glycation end products in fully adjusted models (P for trend = 0.037). Conclusions Physiological precursors of diabetes and cardiovascular disease were associated with MVPA and SB among US Hispanic/Latino youth, a group that bears a disproportionate burden of metabolic disorders.
