ABSTRACT In areas where Plasmodium falciparum is endemic, pregnant women are at increased risk for malaria, and this risk is greatest during the first pregnancy. The placenta sequesters parasites that are able to cytoadhere to chondroitin sulfate A (CSA), a molecule expressed by the placental syncytiotrophoblast, while parasites from a nonpregnant host do not bind to CSA. Cytoadherence is mediated by the expression of variants of the P. falciparum-erythrocyte membrane protein 1 family. Each member of this molecule family induces antibodies that specifically agglutinate infected erythrocytes and inhibit their cytoadherence ability. We investigated whether the higher susceptibility of primigravidae was related to the lack of immune response towards CSA-binding parasites. In a cross-sectional study, primigravidae delivering with a noninfected placenta were less likely to have antibodies agglutinating CSA-binding parasites than multigravidae (P < 0.01). In contrast, parasites from nonpregnant hosts were as likely to be recognized by the sera from women of various parities. In a longitudinal study, at 6 months of pregnancy, antibodies against CSA-binding parasites were present in 31.8% of primigravidae and in 76.9% of secundigravidae (P = 0.02). The antibodies against CSA-binding parasites inhibited the cytoadherence of a CSA-adherent parasite strain to the human placental trophoblast. Our data support the idea that the higher susceptibility of primiparae is related to a lack of a specific immune response to placental parasites.

Background: Obesity is considered а noteworthy public health issue in both developed & developing countries. Among the 1.5 billion overweight individuals worldwide, 300 million of them were obese women. In the general, the prevalence of maternal obesity has increased 60% in the previous two decades with nearly 1 in 3 women now entering pregnancy obese. Also, the periodontаl disease has been observed to be prevalent in pregnant women with the prevalence ranging from 20% to more than 50%, especially economically disadvantaged women.Aim: explore the relation between pre-pregnant overweight and obesity with periodontal disease during pregnancy.Subjects & Methods: cross-sectional study among 400 pregnant women were booked in the high-risk obstetric departments and the antenatal outpatient clinics at governmental general hospitals in El-Fayoum City and governmental university hospital in El-Mansoura city.Results: The mean age of pregnant women was 29.9 ± 6.2 with increase the prevalence of periodontal disease in pregnant women (83.5%). Statistically significant correlation was found between prenatal weight and periodontаl disease during pregnancy (p ≤ 0.0001) with increasing the prevalence of periodontal disease in prenatal obese women (53.2%) and over weight (39.7%) were observed in women who were in their 3rd trimester (р = 0.011). Increase prevalence of periodontal with poor oral hygiene and sedentary activity.Conclusion: increased pre-pregnancy obesity & overweight are positively correlated with periodontal disease prevalence among pregnant women, and Pregnancy itself may also be associated with аn increased risk of periodontal disease.Recommendations: Activating the role of the maternity and community health nurse in branches of Obstetrics and antenatal clinics to enhance pregnant women's knowledge regarding oral health risks of obesity & overweight.

Abstract Anemia is a global public health problem affecting women worldwide, and reproductive-age women are at increased risk. We conducted a population-based cross-sectional study analyzing the prevalence of overall anemia and anemia according to severity in Chinese pre-pregnant women to update current knowledge on anemia epidemiology. Based on the National Free Preconception Check-up Projects supported by the Chinese government, 5,679,782 women participating in this project in 2017 were included in this study. The cyanmethemoglobin method was applied to assess hemoglobin concentrations. Univariate and multivariate logistic regressions were applied for associated factors. The prevalence of anemia among Chinese pre-pregnant women was 21.64% (mild anemia: 14.10%, moderate anemia: 7.17%, severe anemia: 0.37%). The prevalence of overall and severe anemia was the highest in Tibet and the lowest in Beijing among 31 provinces. Women’s age, region, ethnic origin, educational level, occupation, and pregnancy history were all correlated with anemia. Women with B blood type (aOR=0.89), higher BMIs (overweight: aOR=0.84; obesity: aOR=0.70), and alcohol consumption (aOR=0.69) were less likely to have anemia, while those with rhesus negative blood type (aOR=1.10), history of anemia (aOR=2.60), older age at menarche (aOR=1.19), heavy menstrual blood loss (aOR=1.39), longer menstrual period (aOR=1.09) and shorter menstrual cycle (aOR=1.08) were more likely to suffer from anemia. Meat or egg eaters were not significantly associated with severe anemia. Anemia is of moderate public health significance among Chinese pre-pregnant women. Interventions should be considered to prevent anemia to the greatest extent possible to avoid potential harm in this population.

Abstract: Currently more than 2 billion people infected with worms. High prevalence is found primarily in industrialized countries (developing countries). In Indonesia worms is a public health problem after malnutrition. Worms often associated with deficiency anemia espencially iron deficiency. World Health Organization (WHO) reported that the prevelence of pregnant women who experienced approximately 35-75% iron deficiency and increased along with increasing gestational age. Burden of disease and morbidity from worm disease is transmitted through the ground is influenced by the highly complex immune response, TNF immune response against bacteria, fungal invasion, viruses and parasites. The research is analytic survey doing by cross sectional study. After that data collected were processed using SPSS. All of the population are pregnant women in 6 working areas of North Bolaang Mongondow district health center and the population sampel section is taken by purposive sampling. By using the chi square test at the 95 % confidence level of this study showed there is a correlation worm infection with TNF α in pregnant women and there is no corelation worm infection with hemoglobin levels in pregnant women. No correlation between worm infections and hemoglobin levels and there is correlation between worm infection with TNF α in pregnant women in North Bolaang Mongondow regency.Keywords: worm infection, TNF α, and Hb.Abstrak: Saat ini lebih dari 2 miliar penduduk dunia terinfeksi cacing. Prevalensi yang tinggi ditemukan terutama di negara-negara non industri (negara yang sedang berkembang). Di Indonesia cacingan merupakan masalah kesehatan masyarakat setelah malnutrisi. Penyakit cacingan sering dihubungkan dengan kejadian anemia, terutama defisiensi besi. Badan kesehatan dunia World Health Organization (WHO) melaporkan bahwa prevalensi ibu-ibu hamil yang mengalami defisiensi besi sekitar 35-75% serta semakin meningkat seiring bertambahnya usia kehamilan. Beban penyakit dan angka kesakitan dari penyakit cacing yang ditularkan lewat tanah ini dipengaruhi oleh respon imun yang sangat kompleks. TNF α sebagai respon kekebalan terhadap bakteri, invasi jamur, virus dan parasit. Penelitian dilakuakan bersifat survei analitik dengan rancangan cross sectional study. Selanjtunya data yang terkumpul diolah dengan menggunakan bantuan komputer program SPSS. Populasi adalah semua ibu hamil yang ada di wilayah kerja 6 puskesmas kabupaten Bolaang Mongondow Utara dan sampel di ambil dengan cara purposive sampling. Dengan menggunakan uji Chi Square pada tingkat kepercayaan 95% Penelitian ini menunjukan ada hubungan infeksi cacing dengan TNF α pada ibu hamil dan tidak ada hubungan infeksi cacing dengan kadar hemoglobin pada ibu hamil. Tidak ada hubungan antara infeksi cacing dengan kadar hemoglobin dan ada hubungan infeksi cacing dengan TNF α pada ibu hamil di Kabupaten Bolaang Mongondow Utara.Kata kunci: infeksi cacing, TNF α, dan kadar Hb.

ObjectiveTo determine the rates of asymptomatic viral carriage and seroprevalence of SARS-CoV-2 antibodies in healthcare workers.DesignA cross-sectional study of asymptomatic healthcare workers undertaken on 24/25 April 2020.SettingUniversity Hospitals Birmingham NHS Foundation Trust (UHBFT), UK.Participants545 asymptomatic healthcare workers were recruited while at work. Participants were invited to participate via the UHBFT social media. Exclusion criteria included current symptoms consistent with COVID-19. No potential participants were excluded.InterventionParticipants volunteered a nasopharyngeal swab and a venous blood sample that were tested for SARS-CoV-2 RNA and anti-SARS-CoV-2 spike glycoprotein antibodies, respectively. Results were interpreted in the context of prior illnesses and the hospital departments in which participants worked.Main outcome measureProportion of participants demonstrating infection and positive SARS-CoV-2 serology.ResultsThe point prevalence of SARS-CoV-2 viral carriage was 2.4% (n=13/545). The overall seroprevalence of SARS-CoV-2 antibodies was 24.4% (n=126/516). Participants who reported prior symptomatic illness had higher seroprevalence (37.5% vs 17.1%, χ2=21.1034, p<0.0001) and quantitatively greater antibody responses than those who had remained asymptomatic. Seroprevalence was greatest among those working in housekeeping (34.5%), acute medicine (33.3%) and general internal medicine (30.3%), with lower rates observed in participants working in intensive care (14.8%). BAME (Black, Asian and minority ethnic) ethnicity was associated with a significantly increased risk of seropositivity (OR: 1.92, 95% CI 1.14 to 3.23, p=0.01). Working on the intensive care unit was associated with a significantly lower risk of seropositivity compared with working in other areas of the hospital (OR: 0.28, 95% CI 0.09 to 0.78, p=0.02).Conclusions and relevanceWe identify differences in the occupational risk of exposure to SARS-CoV-2 between hospital departments and confirm asymptomatic seroconversion occurs in healthcare workers. Further investigation of these observations is required to inform future infection control and occupational health practices.

Background: No specific physical activity guidelines are available for women in multiple pregnancy. Aim of the study was to assess the knowledge and experience of women regarding physical activity during their latest twin pregnancy. Methods: A cross-sectional study including women after a twin delivery was conducted in Poland. A questionnaire was distributed in 2018 via web pages and Facebook groups designed for pregnant women. Results: 652 women filled out the questionnaire completely. Only 25% of women performed any physical exercises during twin gestation. The frequency of preterm delivery was similar in physically active and non-active participants. 35% of the respondents claimed to have gained information on proper activity from obstetricians during antenatal counselling while 11% claimed to be unable to identify the reliable sources of information. 7% of women admitted to feel discriminated by social opinion on exercising during a twin pregnancy. Conclusions: The population of women with a twin gestation is not sufficiently physically active and is often discouraged from performing exercises during gestation. Therefore, it is crucial to inform obstetricians to recommend active lifestyle during a twin gestation and to provide reliable information on physical activity to pregnant women. Further research on this topic is necessary in order for obstetric providers to counsel women on appropriate exercise with a twin pregnancy.
