BackgroundDiarrhoeal disease remains the second leading cause of death in children under five years, being associated with about 525,000 deaths every year. The most common pathogens worldwide are Shigella spp/EIEC, rotavirus, adenovirus 40/41, ST-ETEC and Cryptosporidium spp. Public health interventions rely on estimates of pathogen-specific burden for prioritisation. Sadly, comprehensive data on the aetiology of diarrhoea in children is lacking for Gabon. This study aimed to identify the spectrum of pathogens found in Lambaréné, Gabon and provide baseline data on their prevalence, needed for implementation of effective control measures.MethodsA cross-sectional study was conducted at Albert Schweitzer and Georges Rawiri Regional hospitals in Lambaréné from February 2017 to February 2018. A consecutive sample of children under 5 year old with diarrhoea or a history of diarrhoea within the previous three days were prospectively studied. A single stool sample was collected from each study participant and processed using commercial rapid immunoassays to detect antigens of rotavirus, adenovirus, and Cryptosporidium spp. Multiplex PCR was used for Cryptosporidium spp., Giardia lamblia and Cyclospora cayetanensis detection, and characterisation of E. coli strains.ResultsOut of 188 participants who provided stool samples, one or more pathogens could be detected in 34.6% of the cases. The most prevalent parasites were Giardia lamblia (14.9%), Cryptosporidium spp. (11.7%), and Cyclospora cayetanensis (2.7%). Enteric viruses also were identified in these children: 10.6% and 1.6% of rotavirus and adenovirus, respectively. Multiple pathogens were detected in 5.3% of samples.ConclusionThis analysis of the causes of diarrhoea in children under 5 years of age in our setting showed three main pathogens: Giardia lamblia, Cryptosporidium spp. and rotavirus. Our study confirms major agents of acute diarrhoeal diseases in children, highlights research needs (Cryptosporidium) and supports the introduction of new tools such as the implementation of the rotavirus vaccine in the national immunisation programme.

ABSTRACT Background: Pneumonia is still causing the most deaths among children in developing countries. This disesase often occurs in children under 5 years of age. Nutritional status is a factor that is closely related to infectious diseases such as pneumonia. This study aimed to examine the correlation between nutritional status and pneumonia among 6-59 months years old in Tangerang, Banten. Subject and Methods: A cross-sectional study was conducted at Pakuhaji Community Health Center, Tangerang, Banten, from January to February 2018. A total of 29 children under five were enrolled in this study. The dependent variable was pneumonia. The independent variable was nutritional status. The data were collected from direct measurement of the children under five and questtioner. The data were analyzed by Chi-square. Results: As many as 16 children under five (55.17%) had pneumonia, 7 children under five (24.13%) were malnutrition, 3 children under five (10.34%) were short, and 5 children under five (7.24%) were thin. Nutritional status based on weight for age and weight for height had differences in nutritional status with the incidence of pneumonia among children under five, and they were statistically significant (p< 0.001). Conclusion: Nutritional status is associate with the incidence of pneumonia among 6-59 months years old (children under five) in Tangerang, Banten. BACKGROUND Excessive screen time is detrimental to the child’s health. However, screen time situation among Malaysian children is poorly understood. OBJECTIVE This study aims to identify the prevalence and determinants of screen time among children under five years old using the latest WHO guidelines. METHODS A cross sectional design was used to randomly select 489 children from nine government health clinics. Total screen time and factors were assessed using validated self-administered questionnaires and analyzed using multiple logistic regression. RESULTS Results show an overall prevalence of 91.4% with a median of 3.00 hours (IQR: 1.36-5.04). Majority of children watched television (66%), followed by mobile phones (30%) and computers (4%). The determinants of screen time were Malay ethnicity, (AOR 3.56, 95% CI: 1.65-7.68), parental age of 30 years or more (AOR 3.12, 95% CI: 1.58-6.16), parental screen time exceeding 2 hours a day (AOR 2.42, 95% CI: 1.24-4.73), parent’s moderate self-efficacy to influence child’s physical activity (AOR 2.29, 95% CI: 1.01-5.20) and parent’s positive perception on influence of screen time on child’s cognitive well-being (AOR 1.15, 95% CI:1.01-1.32). CONCLUSIONS Parents played an important role in determining their child’s screen time. Future interventions that focus on the parents may ensure age appropriate screen time for their children.

Introduction: Anaemia is a pathologic deficiency in oxygen-carrying haemoglobin in red blood cells which may be the result of genetic diseases, infections and deficiency of several nutrients. Nepal is one of the developing countries where anaemia is one of the most serious public health problems. So, a study was planned to evaluate the prevalence of anaemia among children under five years in tertiary care hospital of Nepal. Methods: A cross-sectional study was carried out from August 2011 to January 2012 using a structured questionnaire interview and observation. The children were divided into three groups: group I (0-1 years), group II (>1-3 years), group III (>3-5 years). Anaemia was assessed using  haemoglobin measurement in gram in decilitre on Sysmex KX -21 (automated haematology analyzer). The severity of anaemia were grouped as follows: severe anaemia, < 7.0 g/dL; moderate anaemia, 7.0 to 8.9 g/dL; and mild anaemia, 9.0 to 10.9 g/dL as per WHO classification. Stool test was also carried out to assess worm infestation. Data were analysed using SPSS 11.5. Results: There were 208 children among which 52.9 % were male while 47.10 % were female.  The overall prevalence of anaemia was found to be 49.5% of which 43.3% had mild, 15.8% had moderate and 0.5% had severe anaemia. Out of 20 children in group I, 70 % were anaemic. Among the 94 each in group II and group III, children suffering from anaemia were 51% and 43% respectively. The stool investigations showed that 5.3 % children suffered from worm infestations.   Conclusions: prevalence of anaemia was seen in 49.5% of children below five years age group and the diet and worm infestations didn’t affect it.doi:  http://dx.doi.org/10.3126/mjsbh.v13i1.12998

Diarrhea is still a public health problem in the world and the mostly occur in children under five years. At the moment the number of diarrhea-caused mortality was 3.8 per 1000 per year by 3.2 episodes per year in children under five years. This research aims to analyze the association between hand-washing habits and toilet use with the incidence of diarrhea in children under five years. This is an observational analytic with cross sectional study. Population of the research was a children's mother took her children to the public health center of Sekardangan Sidoarjo Subdistrict. The number of samples taken as many as 58 mother of children under five years. Data collection was done with interviews to parents of children and the observations of the toilets. Technique of data analysis using chi-square test or Fisher's exact test. On the results of the research there is a significant association the habit of hand washing (p = 0.006) and toilet use(p = 0.014) with the incidenceof diarrhea in children under five years. Conclusions of the research is the habit of hand washing and toilet use had a significant association with the incidenceof diarrhea in children under five years. Advice that can be given to the mother of a children under five years is to wash the hands before and after defecating and before preparing food for children. Keywords: diarrhea, hand washing, toilet use, observational, children under five years

BACKGROUND Globally, there is an increasing prevalence of excessive screen time exposure among young children, including Malaysia. Parents are advised to limit this exposure but there are barriers for many of them to follow this recommendation. To date, there is a lack of studies on the factors that cause these parental barriers. OBJECTIVE This study aimed to determine the parental barrier towards the reduction of excessive child’s screen time and its predictors among parents of children under five years old in Petaling District, Selangor. METHODS A cross-sectional study was conducted from April 2019 to June 2020 among 789 parent-child dyads attending child health clinics in Petaling District, Selangor. Validated self-administered questionnaires were used to capture information on sociodemographic factors, parental influences, child-related factors, environmental factors, and parental barriers. Stratified sampling with probability proportionate to size was employed. Data were analysed with IBM SPSS version 25. Descriptive analysis and bivariate analysis were performed before multiple linear regression was used to identify the predictors of parental barriers. RESULTS The overall mean score of parental barriers was 3.51 ± 0.83, indicating that the average numbers of barriers experienced by parents were more than three. The multivariate analysis showed that the predictors of parental barriers included monthly household income (adjusted β: -0.031, 95% CI: -0.048 to -0.015), parents who worked in public sectors (adjusted β: 0.178, 95% CI: 0.063 to 0.292), positive parental attitude on screens (adjusted β: 0.684, 95% CI: 0.576 to 0.793), parents’ low self-efficacy to influence child’s physical activity (adjusted β: -0.318, 95% CI: -0.432 to -0.204), and child’s screen time (adjusted β: 0.042, 95% CI: 0.024 to 0.061). CONCLUSIONS The strongest predictor of the parental barriers to reduce excessive child’s screen time was the positive parental attitude on screen time. Thus, future intervention strategies should aim to foster correct parental attitudes towards screen time activities among young children.

Abstract Background: Wasting is a consequence of food insecurity, inappropriate dietary practices, and inadequate caring and feeding practices. The present study assessed association between wasting and household food insecurity among under five years old children, along with other socio-demographic characteristics.Methods: This study is a secondary analysis of the Nepal Demographic and Health Survey 2016. The survey is cross-sectional in design with use of standardized tools. The sampling frame used is an updated version of the frame from the 2011 National Population and Housing Census. The participants were children under five years of age (n= 2414). Logistic regression was carried out to identify the odds of being wasted for children belonging to different levels of food insecure households using odds ratio and 95 percent confidence intervals. Results: The prevalence of wasting increased with the level of food insecurity, from mild (9.4%) to moderate (10.8%) and to severe (11.3%). The highest proportions of wasted children were in Province 2 (14.3%), from rural areas (10.1%), born to mothers with no education (12.4%) and from a richer quintile (11.3%). Children belonging to severe food insecure households had 1.36 (95%CI 0.72-2.57) adjusted odds of being wasted and those belonging to mild food insecure and moderately food insecure households had 1.13 (95%CI 0.65-1.97) and 1.36 (95%CI 0.72-2.57) odds of being wasted respectively. Province 1 (AOR 2.06, 95%CI 1.01-4.19) and Province 2 (AOR 2.45, 95%CI 1.22-4.95) were significantly associated with wasting.Conclusion: Considering the increment in childhood wasting as per level of food insecurity, an integrated intervention should be developed in Nepal that, 1. addresses improving knowledge and behavior of community people with respect to diet and nutrition; 2. reduce the problem of food insecurity through agricultural interventions.

Abstract Background: Diarrhea is responsible for 525,000 children under-five deaths and 1.7 billion cases globally and is the second leading cause of death among children under-five every year. It is a major public health problem in low income countries like Ethiopia. The main aim of this study was to assess the prevalence of diarrhea and associated risk factors among children under-five in Debre Berhan Town, Ethiopia. Methods: A community-based cross-sectional study was conducted in 420 parent or caretaker/children pairs in Debre Berhan town between 13–18 April 2018. A multi-stage sampling strategy was used to select the study participants. Data were collected using pre-tested and structured questionnaires. Data were entered in Epi-info computer software version 3.5.1 and exported to SPSS Window Version-16 for analysis. Adjusted odds ratio with 95% confidence intervals were used to assess the level of significance. Results: The two week prevalence of diarrhea among children under-five was 16.4% (69/351). Children aged 7-11 months (adjusted odds ratio (AOR): 4.2, 95% confidence interval (CI): 1.2 - 15.3), being the second-born child (AOR: 3.9, 95%CI: 1.8 - 8.5), not vaccinated against rotavirus (AOR: 10.3, 95%CI: 3.2 - 91.3) and feeding children by hand (AOR: 2.5, 95%CI: 1.1 - 6.1) were significant predictors of diarrhea. Conclusions: This study revealed that the two weeks period prevalence of diarrhea among children under-five years was 16.4%. Education programs on the importance of vaccination against rotavirus, increasing breast feeding frequency with complementary food after six months and the critical points of hand washing are recommended.

 Background: Family conscious nutrition (Kadarzi) is an Indonesia government program that aims to address the nutrition problem. The family expected to be able to independently giving the best nutrition to improve health. Each Kadarzi indicator has an important role to resolve and prevent the occurrence of utrition problem as indicator of vitamin A supplementation is used to resolve vitamin A deficiency.  Nutrition problems can be seen by using nutritional status. Children under five years are age-prone to experiencenutritional problem such as underweight, stunting, and wasting. Objective: The purpose of this study was to analyze the relationship between the application of Kadarzi behavior with the nutritional status of children under five (WAZ, HAZ, and WHZ). Method: The research was an analytic obsevation research, using cross sectional design. The sample consist of 72 toddlers in Salakkembang Village, Kalidawir Subdistrict, Tulungagung Regency. The data were collected by using weight and height measurement, also Kadarzi bahavior questionnaire. The data were analyzed using Fisher Exact test. Result: The results of the study showed that there was a correlation between Kadarzi behavior with nutitional status of toddlers based on WAZ (p=0.010), and HAZ (p=0.000) but not with WHZ (p=0.368). Conclusion: The better apllication of Kadarzi behavior, the better the nutritional status of toddlers WAZ and HAZ. Mothers should apply Kadarzi behavior to prevent toddlers from nutritional problems. ABSTRAK Latar belakang: Keluarga Sadar Gizi (Kadarzi) merupakan program pemerintah Indonesia yang bertujuan mengatasi masalah gizi. Keluarga diharapkan dapat secara mandiri mewujudkan keadaan gizi yang sebaik-baiknya untuk meningkatkan kesehatan. Setiap indikator Kadarzi memiliki peran penting untuk mengatasi dan mencegah terjadinya masalah gizi seperti indikator pemberian suplemen vitamin A digunakan untuk mengatasi kekurangan vitamin A. Masalah gizi dapat dilihat salah satunya dengan menggunakan status gizi. Balita merupakan usia yang rawan mengalami masalah gizi seperti underweight, stunting, dan wasting.Tujuan: Tujuan dari penelitian ini adalah menganalisis hubungan antara penerapan perilaku Kadarzi dengan status gizi balita (BB/U, TB/U, dan BB/TB).Metode: Penelitian ini merupakan penelitian observasinol analitik, dengan desain penelitian cross sectional. Sampel terdiri dari 72 balita di Desa Salakkembang, Kecamatan Kalidawir, Kabupaten Tulungagung. Pengumpulan data menggunakan penimbangan berat badan, tinggi badan, dan kuesioner perilaku Kadarzi. Data dianalisis menggunakan Fisher Exact test. Hasil: Hasil dari penelitian menunjukkan terhadap hubungan antara perilaku Kadarzi dengan status gizi balita BB/U (p=0,010) dan TB/U (p=0,000) namun tidak dengan BB/TB (p=0,368).Kesimpulan: Semakin baik penerapan perilaku Kadarzi maka semakin baik status gizi balita BB/U dan TB/U. Ibu dan seluruh anggota keluarga seharusnya menerapkan perilaku Kadarzi agar balita terhindar dari masalah gizi.

Background: One of the efforts made in overcoming malnutrition in the toddler age group is by implementing a Recovery Supplementary Feeding program.Objectives: The purpose of this study was to analyze the differences in nutritional status of children based on weight for height before and after the Supplementary Food Recovery and differences in nutritional status of children after Supplementary Food Recovery and when they had not received Supplementary Food Recovery in the Simomulyo Community Health Center work area.Methods: This study was observational study using case-control design. A total of 38 toddlers with history of obtaining Supplemental Feeding Recovery (PMT) Program in the working area of Simomulyo Health Center were randomly selected. Nutritional status of toddlers is obtained using anthropometric methods of body weight and height / body length. Statistically tested using t-test paired with a significance level of <0.05.Results: After not getting Supplemental Feeding Recovery, it was found that 2.6% of children under five severely wasting, 34.2% wasting, and 63.2% had normal nutritional status. There was a decrease in the percentage of infants with normal nutritional status compared to after completing PMT Recovery program from 68.4% to 63.2%. There were no significant differences in nutritional status of children before and after the program (p=0.585). There was no difference between the nutritional status of children under five after the program and when they had not received the program (p=0.430)Conclusions: There was no difference in nutritional status of children (weight-for-height) before and after Supplemental Feeding Recovery Program.ABSTRAK  Latar Belakang: Upaya yang dilakukan dalam mengatasi kekurangan gizi pada kelompok usia balita adalah program Pemberian Makanan Tambahan (PMT) Pemulihan.Tujuan: Penelitian ini dilakukan bertujuan untuk menganalisis adanya perbedaan pada status gizi balita berasarkan BB/TB sebelum dan setelah Pemberian Makanan Tambahan (PMT) Pemulihan serta perbedaan status gizi balita setelah PMT Pemulihan dan saat sudah tidak mendapat PMT Pemulihan di wilayah kerja Puskesmas Simomulyo.Metode: Penelitian ini adalah peneitian observasional dengan desain penelitan cross sectional. Lokasi penelitian adalah di wilayah kerja Puskemas Simomulyo, Surabaya. Sebanyak 38 balita dengan riwayat mendapat PMT Pemulihan dipilih secara acak menggunakan metode simple random sampling. Status gizi balita didapat menggunakan metode antropometri berat badan dan tinggi/panjang badan. Indeks BB/TB digunakan dalam penilaian status gizi dengan alasan sasaran utama PMT Pemulihan adalah balita dengan status gizi kurus.  Uji secara statistik dilakukan menggunakan uji t berpasangan dengan tingkat signifikansi <0,05.Hasil: Setelah 3 bulan mendapat PMT Pemulihan ada peningkatan persentase balita dengan status gizi normal dari 65,8% menjadi 68,4%. Setelah tidak mendapat PMT Pemulihan ada penurunan persentase balita dengan status gizi normal menjadi 63,2%. Tidak ada perbedaan yang bermakna status gizi balita berasarkan BB/TB sebelum dan setelah PMT Pemulihan (p=0,585). Tidak ada perbedaan pada status gizi dapat disebabkan oleh konsumsi PMT yang belum optimal. Begitu juga diketahui tidak ada perbedaan yang bermakana status gizi balita setelah PMT Pemulihan dan saat sudah tidak mendapat PMT Pemulihan (p=0,430).Kesimpulan: Tidak ada perbedaan pada status gizi balita dengan indeks antropometri BB/TB saat sebelum PMT Pemulihan dan setelah PMT Pemulihan.

Since posyandu was build the in kelurahan Balai Ahad it is nothing posyandu able to reach up the national target in visiting posyandu. In the posyandu sayang ibu found were only 24% of children under five years old who come to posyandu meanwhile national target were 80%. The aim of this research was to know about factors that are related with mothers motivation in taking children under five years old to posyandu. The design of research is correlation that used cross sectional. A number of population were 30 people and sample 30 people. The samples this research are all mother who have mother of children under five years and old in Pincuran Tujuh and for taking of data finished by use questionnaires and testing statistic 0,05. The result of this research were 70% high education and 30% low education, 60% good knowledge about posyandu and 40% bad knowledge, there are 46,67% positive of posyandu and 53,3% have negative of posyandu, there are 60% to get motivation from family and there are 40% not able to get motivations from family. Respondents who are motivated in taking children under five year old to posyandu were 6 responden (20%) and 24 respondent (80%) no not motivated. Based on result of questionnaires are got that 5 respondent (22,73) have high education in taking children under five year old to posyandu, 6 respondent (33,33%) have good knowledge, are motivated in taking children under five year old to posyandu, 4 respondents (22,22%) have positive thinking, motivated in taking chidren under five year old to posyandu, 5 respondent (22,78%) get motivation from family and motivated in taking children under five year old to posyandu. Based on the statistical testing of statistic 0,05, got the meaningful relation with mother’s motivation in taking children under five year old to posyandu is variable education. From the result of this research, it’s necessary to provide intensively about health promotion to motivate mothers to take children under five year old to posyandu
