In order to investigate the impact of holistic care on line coagulation and safety in hemodialysis and to address limitations of the conventional ultrasound flow vector imaging (VFM) technique, which requires proprietary software to acquire raw Doppler and scatter tracking data, a combined deep-learning-enabled holistic care approach to line coagulation in hemodialysis is proposed. First, velocity along the direction of the sound beam, which is provided by the color Doppler echocardiogram, is obtained as the radial velocity component using a velocity scale. Moreover, the left ventricular wall contour is automatically identified using a U-Net network and the left ventricular wall velocity is calculated as the boundary condition of continuity equation by a retrained PWC-Net model. Likewise, the velocity component of each blood mass in the vertical direction of the sound beam is obtained by solving the continuity equation (i.e. tangential velocity component). Finally, the velocity vector map of cardiac flow field was synthesized and visualized in the flow diagram. For this purpose, sixty patients admitted to receive hemodialysis from February 2019 to June 2020 were randomly divided into two groups of 30 patients where the control group implemented conventional care and the study group implemented all-round care on the basis of conventional care. The nursing effects of both groups were compared. Incidence of pipeline coagulation and complications in the study group were lower than those in the control group and the difference was statistically significant ( P < 0.05 ). The nursing detail score, nursing attitude score, nursing professionalism score, and total satisfaction score in the study group were higher than those in the control group and the difference was statistically significant ( P < 0.05 ). Applying all-round nursing in hemodialysis can effectively reduce the incidence of line coagulation complications and improve the safety of hemodialysis, as well as improve patients’ satisfaction with nursing care.

BACKGROUND Mobile apps are becoming increasingly relevant to health care. Apps have been used to improve symptoms, quality of life, and adherence for oral drugs in patients with cancers, pregnancy, or chronic diseases, and the results were satisfying . OBJECTIVE This study aims to develop an information platform with the help of a mobile app and then evaluate whether information platform-based nursing can improve patient’s drug compliance and reduce the incidence of VTE in patients with hip fractures. METHODS We retrospectively analyzed hip fracture patients performed with traditional prevention and intervention of VTE (control group) between January 2008 and November 2012, and prospectively analyzed hip fracture patients conducted with nursing intervention based on the information platform (study group) between January 2016 and September 2017. The information platform can be divided into medical and nursing care end and the patient’s end. Based on the information platform, we could implement risk assessments, monitoring management and early warnings, preventions and treatments, health educations, follow-up and other aspects of nursing interventions for patients. We compared basic characteristics, outcomes including drug compliance, VTE occurrence, and mean length of hospitalization between the two groups. Besides, a subgroup analysis was performed in the study group according to different drug compliances. RESULTS Regarding baseline data, patients in the study group had more morbidities than those in the control group (P<0.05). The difference of drug compliance between the two groups was statistically significant (P<0.001): 64.7% of the patients in the control group had poor drug compliance and only 6.1% patients had poor drug compliance in the study group. In terms of VTE, 126 patients (10.7%) in the control group had VTE, while only 35 patients (7.1%) in the study group had VTE, and the difference was statistically significant (P=0.024). Moreover, the average length of hospitalization in the study group was also significantly lower than that in the control group (10.4 d vs. 13.7 d, P=0.000). Subgroup analysis of the study group showed that the incidence of VTE in patients with non-compliance, partial compliance, and good compliance was 56.7%, 5.8% and 2.8%, respectively (P=0.000). CONCLUSIONS Poor drug compliance leads to higher VTE occurrence. The information platform-based nursing can effectively improve the compliance of patients with hip fracture and thus significantly reduce the incidence of VTE.

Abstract Septic cardiomyopathy remains a difficult medical problem to manage in critically ill patients. With all currently available therapeutic options, the mortality rate in these patients remains high. Our study included 29 patients diagnosed clinically with sepsis. A control group was used to compare the results. In all patients, p53 expression was assessed in cardiac tissue obtained from these patients and a statistical correlation was made with clinical data. The different expression rates of p53 do not correlate with patient’s age, having appropriate means in years, but with an increasing tendency with increasing expression (p=0.2110). The pulmonary infections are responsible for the majority of the septic state in the study group (over 55%). The difference between the infection sites is statistically significant (p<0.0001).

Background: The hamstring tendons are commonly harvested for anterior cruciate ligament (ACL) reconstruction with detachment of the tibial insertion. Retaining the insertion may help to preserve vascularity and viability of the graft and bypass the stages of avascular necrosis and revascularization, which might be beneficial to graft maturity. Purpose: To investigate and compare graft maturity by magnetic resonance imaging (MRI) after ACL reconstruction with preservation or detachment of hamstring tendon tibial insertion at up to 2 years. Study Design: Randomized controlled trial; Level of evidence, 2. Methods: Forty-five patients (age range, 18-45 years) undergoing isolated ACL reconstruction with hamstring tendon were enrolled and randomized to 2 groups. The tibial insertion of the hamstring tendon was preserved in the study group (n = 21) and detached in the control group (n = 24). Patients had follow-up at 3, 6, 12, and 24 months, which consisted of the following: (1) clinical examination and (2) MRI evaluation of graft signal intensity based on signal/noise quotient (SNQ) values. Finally, 18 patients in the study group and 19 in the control group received full follow-up evaluation (ie, at all 4 time points). Results: All knees acquired full range of motion at 24 months without significant laxity. At each time point, the KT-1000 arthrometer revealed no significant difference between groups; the clinical scores significantly improved in both groups, although the difference between groups was not significant. In the control group, the SNQ value increased from 3 months, peaked at 6 months, and then decreased (3 months, 21.4 ± 12.7; 6 months, 25.6 ± 12; 12 months, 18.3 ± 7.7; 24 months, 15.3 ± 6.3). However, the insertion-preserved graft in the study group maintained relatively lower and unchanged signal intensity throughout all time points (3 months, 15.0 ± 11.2; 6 months, 14.9 ± 6.3; 12 months, 12.6 ± 7.0; 24 months, 14.6 ± 7.0). Between groups, there was no significant difference at 3 or 24 months ( P = .11 and .75, respectively), while the SNQ values were significantly lower in the study group versus the control group at 6 and 12 months ( P = .002 and .02, respectively). Conclusion: The insertion-detached hamstring tendon grafts underwent a significantly increasing change in signal intensity during the first 2 years after ACL reconstruction, while the insertion-preserved grafts kept a relatively lower and unchanged signal intensity. The difference was most significant at 6 and 12 months postoperatively.

To analyze the clinical effect of nursing cooperation in transsphenoidal approach microscopic hypophysectomy. From January 2017 to January 2020, 80 patients who underwent transsphenoidal microscopic hypophysectomy in our hospital were selected to participate in the analysis and study. They were divided into two groups according to the randomized allocation, namely the observation group and the control group. Among them, 40 patients in the observation group and 40 patients in the control group were given routine nursing care for the patients in the control group, and comprehensive nursing intervention was adopted for the patients in the observation group, and the overall nursing effect of the two groups of patients was compared. After taking different nursing methods, the condition of patients in both groups was effectively controlled, and the effective rate of patients in the observation group with comprehensive nursing intervention was significantly better than that of patients in the control group with conventional nursing methods, and the difference had certain statistical significance (P < 0.05); The satisfaction degree of patients in the study group was significantly better than that of patients in the control group, and the difference was statistically significant (P < 0.05). The degree of negative emotions of patients in the study group was significantly better than that of patients in the control group after receiving comprehensive nursing intervention, and the difference was statistically significant (P < 0.05), and the difference in the incidence of adverse events between the two groups was not statistically significant (P > 0.05); The scores of each index of SF-36 questionnaire of patients in both groups were higher than those before nursing, and the scores of each index of patients in observation group were higher than those of patients in control group, and the difference was statistically significant (P > 0.05). With adequate preoperative preparation and mastery of the use of mechanical equipment, comprehensive nursing intervention can effectively improve the treatment effect of patients, make patients more satisfied with the nursing work, and can soothe patients’ negative psychological mood, eliminate panic, improve patients' life confidence, enhance intraoperative cooperation, and ensure that the operation can be completed smoothly.

Introduction: Sodium-glucose Cotransporter-2 (SGLT-2) inhibitors are drugs with profound interest because of their beneficial effect on multiple organ systems, including the cardiovascular and renal systems. Their interplay with these systems could prove beneficial in managing complex conditions like heart failure with preserved ejection fraction. We sought to compare cardiac diastolic function in patients on SGLT-2 inhibitors with those on metformin. Methods: Data were collected retrospectively after approval from our institutional review board. The study group included patients on SGLT-2 inhibitors (n=75), and the control group had patients on metformin (n=82), primarily for diabetes mellitus. Patients with ejection fraction less than 50% were excluded from the study. Data collected included age, systolic blood pressure (SBP), left ventricular (LV) ejection fraction, LV and left atrial dimensions, and diastolic parameters. Mean values, along with standard deviation, were calculated. Multivariate regression analysis for diastolic function was performed while adjusting for confounders, including age, body mass index, and SBP. Results: Study group showed better diastolic function as evidenced by significantly higher e’ medial (6.8 v/s 6.16 cm/s; p = 0.03) and e’ lateral (9.8 v/s 7.9 cm/s; p=0.008) when compared to the control group. E/e’, a marker of pulmonary capillary wedge pressure, was lower in the study group (12.87 v/s 14.36; p=0.08); however, it did not reach statistical significance. LA volume index was significantly lower in the study group, suggestive of favorable hemodynamic status (26.5 v/s 29.6 mL/m 2 ; p=0.03). SBP was significantly lower in the study group (127 v/s 135 mm Hg; p=0.01). Multivariate regression model demonstrated SBP as a significant confounder that contributed to e’ (average) difference between the two groups. Conclusions: SGLT-2 inhibitors favored better diastolic measurements when compared with metformin alone. The mechanism may be attributed to its BP-lowering effect. A larger prospective study is required to determine the role of SGLT-2 inhibitors.

Background: Planned induction of labor is an established part of modern obstetrics and is used as a definite form of treatment where continuation of pregnancy would be detrimental to the health of mother or fetus. The objective of this study was to evaluate the effect of mifepristone in pre-induction cervical ripening and labor induction. Methods: A total of 200 pregnant women at term with Bishop Score 4 or less were selected for this prospective randomized placebo-controlled study. The sample was equally divided into study group to receive 200 mg of mifepristone and control group to receive placebo orally for 2 days. Bishop score was assessed at every 24 hours interval till patient entered in spontaneous labor or 72 hours after 1st dose. Women who did not enter labor spontaneously, labor induction was planned with per vaginal insertion of prostaglandin (PG) E2 analogue, Dinoprostone gel 2.5 mg or PGE1 analogue Tab. Misoprostol 25 µg.Results: Ninety-six subjects in the study group and eighty-one in the control achieved successful ripening of cervix and the difference was statistically significant. Sixty-eight of study group and thirty-nine of placebo group entered in spontaneous active labor within 72 hours. Requirement of oxytocin as adjuvant treatment was significantly lower in the study group. Nineteen women of study group and fifteen of control group delivered within 24 hours, and eighty-one of study group and sixty-two of placebo delivered in 48 hours. The mean induction delivery interval was 35.53±13.67 hours in the study group, whereas it was significantly prolonged in the placebo group 50.49±20.92 hours. Eighty-two subjects of study group and seventy-eight of the control group delivered vaginally, the differences were statistically not significant. Conclusions: Mifepristone was found to be an effective agent for cervical priming prior to labor induction in women at term and significantly reduces the induction delivery interval compared with placebo.

AbstractObjectiveTo evaluate the prevalence of severe acute respiratory syndrome coronavirus-2 infection in patients presenting with epistaxis to a tertiary otolaryngology unit.MethodsA prospective study was conducted of 40 consecutive patients presenting with epistaxis referred to our tertiary otolaryngology unit. A group of 40 age-matched controls were also included. All patients underwent real-time reverse transcriptase polymerase chain reaction testing for severe acute respiratory syndrome coronavirus-2. Symptoms of fever, cough and anosmia were noted in the study group.ResultsThe mean age was 66.5 ± 22.4 years in the study group. There were 22 males (55 per cent) and 18 females (45 per cent). The mean age in the control group was 66.3 ± 22.4 years (p = 0.935). There were six positive cases for severe acute respiratory syndrome coronavirus-2 (15 per cent) in the epistaxis group and one case (2.5 per cent) in the control group. The difference was statistically significant (p = 0.05).ConclusionEpistaxis may represent a presenting symptom of severe acute respiratory syndrome coronavirus-2 infection. This may serve as a useful additional criterion for screening patients.
