<b><i>Background:</i></b> The World Health Organization (WHO) classifies malnutrition as the biggest threat to public health worldwide, and this condition is observed in 20–60% of hospitalized patients. Malnutrition is a state of the body in which due to insufficient supply or incorrect absorption of essential nutrients, the body composition changes and the body’s functions are impaired. Malnutrition is associated not only with reduced body mass index but also with obesity. <b><i>Summary:</i></b> Obesity is defined as a paradoxical state of malnutrition, which despite excessive energy consumption is associated with a shortage of individual microelements. Deficiency or lack of homeostasis of essential micronutrients can significantly affect daily performance, intellectual and emotional state, but also the physical state of the body. Food deficiency can also contribute to further weight gain or the development of other metabolic diseases. Micronutrient deficiency may include not only incorrect dietary choices and insufficient access to nutrient-rich foods but also changes in the absorption, distribution or excretion of nutrients, and altered micronutrient metabolism resulting from systemic inflammation caused by obesity. An effective therapy method recommended for people with morbid obesity is bariatric surgery aimed at both weight loss and improving quality of life. Unfortunately, the effects of these treatments are often medium- and long-term complications associated with micronutrient deficiency as a result of reduced consumption or absorption. Therefore, the use of bariatric surgery in patients with extreme obesity can affect the metabolism of microelements and increase the risk of nutritional deficiencies. <b><i>Key Messages:</i></b> Studies by many authors indicate a higher incidence of food deficiency among people with excessive body weight, than in people with normal body weight of the same age and same sex. Monitoring the concentration of minerals and vitamins in blood serum is a good practice in the treatment of obesity. The proper nutritional status of the body affects not only the state of health but also the effectiveness of therapy. The aim of the review was to present the issue of malnutrition in the context of obesity.

Background: G protein-coupled bile acid receptor (TGR5) is involved in a number of metabolic diseases. The aim of this study was to identify the role of TGR5 after Roux-en-Y gastric bypass (GBP). Methods: Wild type and TGR5 knockout mice (tgr5-/-) were fed a high-fat diet (HFD) to establish the obesity model. GBP was performed. The changes in body weight and food intake were measured. The levels of TGR5 and peptide YY (PYY) were evaluated by RT-PCR, Western blot, and ELISA. Moreover, the L-cells were separated from wild type and tgr5-/- mice. The levels of PYY in L-cells were evaluated by ELISA. Results: The body weights were significantly decreased after GBP in wild type mice (p<0.05), but not tgr5-/- mice (p>0.05). Food intake was reduced after GBP in wild type mice, but also not significantly affected in tgr5-/- mice (p>0.05). The levels of PYY were significantly increased after GBP compared with the sham group (p<0.05); however, in tgr5-/- mice the expression of PYY was not significantly affected (p>0.05). After INT-777 stimulation in L-cells obtained from murine intestines, the levels of PYY were significantly increased in L-cells tgr5+/+ (p<0.05). Conclusion: Our study suggests that GBP up-regulated the expression of TGR5 in murine intestines, and increased the levels of PYY, which further reduced food intake and decreased the body weight.

Background: The Anatomical Therapeutic Chemical Classification / Defined Daily Dose (ATC/DDD) system recommended by World Health Organization is accepted worldwide as the standard method of quantification of drug consumption. However, owing to individual variation in body weight, the ATC/DDD system cannot be used for comparison across paediatric population. Objective: This study aimed to develop a novel metric system for standard quantification of antibiotic consumption in paediatric population. Method: The standard unit of drug quantification in adult population is DDD/100 patient days (PD). We conceived a new unit of DDD/1000 kg-days (KD) where KD is the product of the body weight and length of hospital stay of an individual patient. We simulated the quantification and comparison of drugs in a computer model of five virtual paediatric hospitals (H1 to H5, n=100, 200, 100, 100, 100 respectively). We re-applied the metric system on two, real world, hospital-based, time cohorts (TC) (TC18, n=38 and TC19, n=47) of 2 weeks each, in two consecutive years. Results: The body weights (mean±SD) in H1-H5 were 5.7±3.0, 5.7±2.8, 25.3±8.5, 20.6±11.7 and 19.8±11.4 kg, respectively. The antibiotic consumption in terms of DDD/100 PD and DDD/1000 KD in the five hospitals was 1.26, 1.20, 5.52, 4.41 and 2.00, and 2.24, 2.14, 2.22, 2.17 and 1.06 respectively. In TC18 and TC19, the mean body weight, DDD/100 PD and DDD/1000 KD were 12.24±13.17, 30.93, 20.34 and 19.51±12.28, 11.99, 6.23, respectively. Conclusion: DDD/1000 kg-days is a potential standard unit for drug quantification in paediatric population independent of weight distribution and size of the study sample. The universal application and comparison across diverse samples can generate useful information for resource allocation, anti-microbial stewardship, disease burden and drug use, and can help in taking policy decisions to improve healthcare delivery in the paediatric population.

AbstractIntroduction:According to the World Health Organization obesity it's the result of an abnormal or excessive body fat accumulation, which presents a high risk for the health. Bariatric surgery appears as an alternative to the conventional treatment for the morbid obese individuals. However, this type of intervention causes changes in the anatomy and physiology of the gastrointestinal tract, which may lead to the development of nutritional deficiencies in patients, in particular anemia.Aim:To evaluate micronutrient deficiencies in patients submitted to bariatric surgery in preoperative and postoperative periods, in particular iron, ferritin, and B12 vitamin.Methods:In this longitudinal study, we evaluated, retrospectively and prospectively, patients who attended the nutrition appointment at a central hospital. We completed a preexisting database containing anthropometric and biochemical data, adding biochemical data, at various periods: pre at 6th, 12th, 18th, 24th, 30th and 36th months post-surgery.Results:from the 121 patients submitted to bariatric surgery, 79,3% were female. The prevalence or iron deficiency reached 15%, ferritin deficiency reached almost 10% and B12 vitamin deficiency reached 18%. Furthermore there were deficiencies in other nutrients, e.g., vitamin D, magnesium and zinc. There was more than 85% adhesion to take the multivitamin supplementation and frequent use of specific supplementation.Conclusion:The prevalence of nutritional deficiencies is high, with a tendency to persist over time even with use of multivitamin supplementation, leading to the need for complementary supplementation, in particular to prevent the risk of anemia. Hence, periodic and long term monitoring is fundamental.

Rats maintaining reduced body weights after lesions of the lateral hypothalamus (LH; LH rats) are characterized by smaller body protein masses. Two experiments were conducted to determine whether this reduced protein mass is actively defended. In the first, it was found that LH rats induced to overeat and restore body weight to the level of nonlesioned controls markedly increased their body fat without significantly increasing body protein. That is, LH rats at normal body weights were notably obese. In the second experiment, body protein losses produced by food restriction in LH rats were both relatively small and proportionally the same as those seen in similarly restricted nonlesioned controls. These observations demonstrate that LH rats retain the capacity for preserving body protein when challenged by either under- or overnutrition. The apparently irreversible reduction in the body protein mass thus appears to be the result of a specific lean tissue downregulation induced by LH damage.

The relatively new field of bariatric surgery is increasingly used as an option to address issues linked to obesity. Practice nurses must understand how they can manage patients who have undergone these procedures Obesity is steadily rising, with 64% of adults in England in 2017 being classified as overweight or obese. Obesity is strongly linked to metabolic diseases such as cardiovascular disease, type 2 diabetes mellitus (T2DM), non-alcoholic fatty liver disease (NAFLD), polycystic ovary syndrome (PCOS), musculoskeletal and respiratory disorders, cancer as well as psychological illness ( World Health Organization (WHO), 2018 ). Therefore every opportunity should be taken to support patients to lose weight and maintain weight loss. Practice nurses are in a unique and pivotal position to guide patients in a variety of options to assist in their weight loss. Bariatric surgery in England is relatively new, with its usage having increased dramatically in the last 10 years. Practice nurses require knowledge of bariatric surgery to undertake their role effectively. This article provides information to assist practice nurses in providing care and support to patients who have had a surgical intervention. This paper also highlights signs and symptoms of physical and psychological complications including pregnancy and excess skin and recommended action and advice including nutritional monitoring and supplementation.

Obesity is a modern disease and the cause is quite complicated. This study explores the effects of aerobic and resistance exercise on weight loss and their relationship with the PGC-1α/Irisin/UCP-1 signaling pathway. First, we selected 52 obese students for aerobic and resistance exercise. Second, we established an obesity rat model, and then subjected them to 12 weeks of aerobic and resistance exercise. The body weights and blood lipid contents of all the subjects were measured, and mRNA and protein expressions were determined via real-time PCR and western blot. In obese students, aerobic exercise and the combination of aerobic and resistance exercise significantly reduced body weight and blood lipid levels. In obese model rats, aerobic and resistance exercise significantly reduced the body weight, blood lipid levels, and increased the PGC-1, Irisin, and UCP-1 levels. Furthermore, the combined effects of aerobic and resistance exercise were similar to those of prolonged aerobic exercise in both human subjects and model rats. The mechanism of weight loss via aerobic and resistance exercise may be related to the PGC-1α/Irisin/UCP-1 pathway, and the combination of aerobic and resistance exercise may be a more suitable weight loss option for obese patients.

  Children under five are a group that is prone to malnutrition because they are still experiencing a growth cycle, a development that requires more nutrients than other age groups. The impact of nutritional deficiencies; low body resistance, the body is susceptible to infectious diseases. The purpose of this study was to analyze the effectiveness of giving modified dietetic skim and cotton sheet oil (MODISCO) and Formula-75 milk to increase body weight of malnourished children in Puskesmas Waigete, Sikka Regency. The type of research used is Quasy experimental pretest and posttest design. The population of this research was 181 underweight children with malnutrition. The sampling technique used purposive sampling. The sample of this study was 24 underweight children with malnutrition, divided into 2 (two), namely group A was given MODISCO intervention and group B was given Formula-75 milk. The intervention was given once a day for 1 month at a dose of 150 kcal / kgbb / day for MODISCO and 1000 ml per day for 75 formula milk. Data analysis used Paired t-Test and Independent Samples Test. Paired T-Test analysis, groups A and B obtained P-value (0.000) <0.05, it was concluded that there was an increase in body weight after being given MODISCO and Formula-75 milk. Based on the analysis of the Independent Samples Test, groups A and B obtained a P-value (0.876)> 0.05, it was concluded that there was no difference in body weight for under-nourished toddlers who were given MODISCO and Formula-75 milk. There is no difference in body weight of malnourished underweight children in group A and group B, so it is concluded that MODISCO and Formula-75 milk can increase the weight of malnourished children underweight. The results of this study are an additional intervention to increase the weight of children under malnutrition.

The beneficial effects of the Mas/Ang-(1-7) pathway prompted us to develop novel Ang-(1-7) analogues and ligands for Mas. In the present study, we evaluated the cardiometabolic effects of a pharmacological formulation developed by including the Mas agonist A-1317 in hydroxypropyl β-cyclodextrin (HPβCD). The inclusion compound was given orally (10 μg/Kg body weight) to Spontaneously hypertensive rat (SHR) and fructose-fed rats. Mean arterial pressure (MAP) and heart rate (HR) were monitored for 5 hours after administration of a single dose of A-1317-HPβCD in conscious SHR. Seven-weeks-old male Sprague-Dawley rats were fed with either normal rat chow (CTL) or the same diet plus 10% fructose in the drinking water (FFR). For the last 4 wk of a 9-wk period of each diet, a subgroup of each group of animals was treated daily with the oral A-1317 (CTL-A or FFR-A) or with vehicle (CTL-V or FFR-V). Rats were subjected to oral glucose tolerance test (2 g/Kg body weight) concomitantly with the evaluation of plasma insulin levels. A-1317 reduced MAP with the maximum change occurring after 4 hours of administration (Δ=-23±2mmHg). There was no significant effect of A-1317 on HR of SHR. Once a day administration of A-1317 ameliorated all metabolic conditions altered by fructose-feed, including the glucose tolerance with less release of insulin and the decreased in the basal insulinemia. However no change in glycemia was observed. Regarding the lipidic metabolism, there was a decrease in the hepatic and serum tryacilglicerol levels (CTL-V=51±3; CTL-A=44±4; FFR-V=74±6; FFR-A=45±5 mg/dl serum levels), the body weight gain and the epididymal and mesenteric adipose tissue mass. Moreover hepatic and serum cholesterol levels were surprisingly diminished in both treated groups. These data suggest that A-1317 inclusion compound is an innovative therapeutic tool for treatment the cardiovascular and metabolic diseases.

Assessment of toxicities of DA.AMLODEPON HVD hard capsule on experimental animals. The acute toxicity of DA.AMLODEPON HVD was assessed on Swiss mice according to World Health Organization Guidance, and LD50 determination according to the method of Litchfield – Wilcoxon. The sub-chronic toxicity study of DA.AMLODEPON HVD at two doses (0.42 g/kg/day and 1.26g/kg/day) was conducted in rats for four consecutive weeks. After administration, general conditions and the body weight of rats were evaluated. Blood samples were collected for analyzing serum parameters before treatment (T0), second week (T1), and fourth week (T2). Histopathological analysis of livers and kidneys was observed at the end of the experiment. The results revealed that mice were taken up to a maximum dose of 39.15 g/kg with no symptoms of acute toxicity, LD50 of DA.AMLODEPON HVD has not been determined. The sub-chronic toxicity study at two doses did not change the body weight of rats, general conditions. The parameters for structures and functions of livers and kidneys and microscopic of the livers and kidneys are in a normal range during the study period.

Nerunjil kudineer/Neermulli Kudinner is an official Siddha polyherbal formulation used for the various ailments related to the kidney. Cisplatin is an alkylating agent used in chemotherapy for the treatment of various cancers. Its use is limited due to their nephrotoxicity. In this study nephroprotective effect of Nerunjil kudineer (NK) on cisplatin-induced nephrotoxicity in Wistar albino Rats was studied. Male Wistar albino rats were used for this study. Animals were divided into 5 groups (n=6) as control, Cisplatin control (Single dose 7 mg/Kg i.p on the 7th day), Cisplatin with Cystone (p.o), NK200 and NK400 mg/Kg (p.o) for 10 days. At the end of the study, animals were weighed and sacrificed to estimate the relative kidney weight, serum creatinine and Urea. Kidney tissue was estimated for oxidant-antioxidant parameters (MDA, GSH & SOD) and histology study was carried out. Cisplatin reduced body weight and increased kidney weight significantly (P<0.0001). It deprived the renal function by elevating serum creatinine & urea, MDA and reduction in endogenous antioxidants GSH and SOD significantly (P<0.0001). Cisplatin group exhibited focal tubular necrosis and congested blood vessels in histology study. The standard drug Cystone and NK400 significantly increased the body weight, reduced the kidney weight, normalized the kidney function parameters (Serum Cr and Urea), bolstered antioxidant status and showed a trend towards the recovery of histological alterations. NK showed a dose-dependent activity and higher dose, NK400mg /Kg possessed strong nephroprotective activity, which may be due to the efficient antioxidant potential, which reduces lipid peroxidation and oxidative stress induced by cisplatin. The results strongly suggest that Nerunjil kudineer is an effective nephroprotective drug against Cisplatin induced nephrotoxicity.
