Correlation Between Intestinal Microflora and Gastrointestinal Dysfunction in Elderly Patients with Sepsis

Multiple organ dysfunction syndrome (MODS) is a key factor that leads to death in elderly patients with sepsis. Therefore, early prevention and treatment of gastrointestinal dysfunction (GIDF) in elderly patients with sepsis is an important measure to prevent MODS occurrence. This research explores the correlation between intestinal microflora and GIDF in elderly patients with sepsis and provides ideas for the prevention and treatment of GIDF in elderly patients with sepsis. In this study, 152 patients with sepsis (122 patients with sepsis and GIDF) treated in the Third Affiliated Hospital of Yunnan University of Chinese Medicine from January to September 2019 were selected as the sepsis group and 100 elderly who had normal physical examination results were selected as the control group. The common intestinal microflora of the two groups was compared. Patients with sepsis and GIDF were treated as the GIDF group and the other patients with sepsis were treated as the non-GIDF group. The common intestinal microflora, gastrointestinal indicators, serum inflammatory factors, and immune function indices were compared between the two groups. Correlation analysis of the observed indices with statistical significance was carried out. The results showed 152 patients with sepsis and 122 patients with sepsis and GIDF; thus, the incidence of sepsis with GIDF was 80.26%. The total average score of sepsis with GIDF was 3.61±0.09. There was no statistically significant difference in GIDF scores of patients ages 65–75 and > 75 years old. The number of Bifidobacterium and Lactobacillus in elderly patients with sepsis was lower and the number of Escherichia coli was higher than in the control group. In elderly patients with sepsis, the number of Bifidobacterium and Lactobacillus in the GIDF group was lower and the number of E. coli was higher than in the non-GIDF group. White blood cell (WBC) count, procalcitonin (PCT), C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), gastrin (GAS), and diamine oxidase (DAO) in GIDF patients were higher and motilin (MOT), CIT (CIT), CD4+, and CD8+ were lower than in the non-GIDF group. WBC count, PCT, CRP, TNF-α, GAS, and DAO were negatively correlated with the number of Bifidobacterium and Lactobacillus but positively correlated with E. coli. MOT, CIT, CD4+, and CD8+ were positively correlated with the number of Bifidobacterium and Lactobacillus but negatively correlated with E. coli. There was a negative correlation between Bifidobacterium and Lactobacillus and GIDF score and a positive correlation between E. coli and GIDF score. Therefore, the change in the intestinal microflora in elderly patients with sepsis is related to GIDF.

Abstract Background and purpose Although participation in cardiac rehabilitation (CR) improves quality of life (QoL), participation in CR, especially among elderly, is limited. We conducted this study to assess whether mobile home-based CR (mCR) increases QoL in elderly (≥65 years old) patients with coronary artery disease (CAD) or a valvular intervention who decline participation in conventional CR. Methods It is designed as a randomised multi-centre study with two parallel arms. Randomisation assigned patients either to mCR or a control group. mCR Consisted of six months of home-based CR with telemonitoring and coaching. Control-group patients did not receive any form of CR throughout the study period. Quality of life was measured with the SF-36v2 questionnaire at 0, 6 and 12 months. Results A total of 179 patients were included in this study (90 control, 89 mCR). A flowchart of the trial is presented in Figure 1. Patients were predominantly male (81.1%). Baseline characteristics can be found in Table 1. Patients using mCR improved on physical QoL after 6 (p=0.026) and 12 (p=0.008) months. There was no difference on mental QoL for both groups (mCR 6 months p=0.563, 12 months p=0.945; control 6 months p=0.589, 12 months p=0.542). No difference existed in QoL between the mCR and control group (physical: 6 months p=0.070, 12 months p=0.150; mental: 6 months p=0.355, 12 months p=0.625). Conclusion Although there is no significant difference in QoL between the control and mCR group, mCR increases physical QoL after 6 and 12 months in elderly patients who decline participation in conventional CR. Therefore E-Health tools should be considered as an alternative for conventional CR when (elderly) patients decline to participate in conventional CR. Figure 1. 【Abstract】Objective: Toanalyze the efficacy of combined drug therapy for elderly patients with coronary heart disease and hypertension.METHODS:Sixty-six elderly patients with coronary heart disease and hypertension were enrolled from December 2017 to November 2018. They were randomly divided into two groups, 33 patients in each group. Patients in the experimental group received nifedipine. In combination with enalapril, patients enrolled in the control group received nifedipine monotherapy.RESULTS:Compared with the control group, the total effective rate, Serum Nitric Oxide (Serum NO) after treatment, CRP after treatment, HCY after treatment, and blood pressure after treatment were significantly improved (P<0.05). Serum NO and treatment before treatment in the 2 groups. There was no significant difference in pre-CRP, pre-treatment HCY, pre-treatment blood pressure, and adverse reactions during treatment (P>0.05).Conclusion: Theelderly patients with coronary heart disease and hypertension are treated with nifedipine and enalapril.

Gnotobiotic growing rats harbouring either a whole human faecal flora or single human strains ofEscherichia coli(EM0) orBacteroides vulgatus(BV8H1) were fed for 7 weeks on semi-synthetic diets in which the protein source was either soya-bean meal (SM) or rape-seed meal (RM). For each bacterial status the RM-diet group was compared with the control group fed on the SM diet. The association of human faecal flora with the RM diet was responsible for reduced feed intake and reduced weight gain, an enlargement of the liver and thyroid and a decrease in both thyroxine and triiodothyronine plasma levels. The association of theB. vulgatusBV8H1 strain with the RM diet reproduced all these effects, except that triiodothyronine plasma levels were not significantly modified. Rats inoculated with theE. coliEM0 strain and fed on the RM diet exhibited a goitre and lowered thyroxine and triiodothyronine plasma levels. These results show that the human intestinal microflora may be involved in glucosinolate metabolism when cruciferous vegetables are consumed by man. The specificity of the symptoms observed according to the rat bacterial status supports the hypothesis that bacteria yield specific toxic glucosinolate derivatives according to their enzymic potential.

Introduction: Power chains is the common device used in fixed orthodontic treatment. Force decay in power chains is a problem that can affect the teeth movement due to a continuous force cannot be maintained. Force decay in power chain can be affected by the use of alcohol-containing mouthwash or alcohol-free mouthwash. The objective of this study was to determine the effect of alcoholic-containing mouthwash, alcohol-free mouthwash, and artificial saliva towards the power chains force decay. Methods: This research was an experimental analytic laboratory in-vitro with the comparative approach. The sample in this study were 40 power chain, short A (SA); 40 power chain, short B (SB); 40 power chain, long A (LA); and 40 power chain, long B (LB), which divided into 5 groups that were consecutively exposed to the artificial saliva as control group; Minosep® with 0.1% of chlorhexidine gluconate; Listerine® with 0% of alcohol; Hexadol® with 9% of alcohol; and Listerine® with 26.9% of alcohol. The measurement of force decay was performed with digital force gauge on day 0, 1, 14, and 28. The statistical analysis was done by using the Wilcoxon and Mann-Whitney tests. Result: There was a significant difference (p<0.05) between the force decay of power chain that exposed to the alcohol-containing mouthwash, alcohol-free mouthwash, and the artificial saliva. Conclusion: The force decay level of power chain that exposed to an alcohol-containing mouthwash was higher compared to the force decay level of power chain that exposed to an alcohol-free mouthwash and an artificial saliva.

Purpose: In this study we examined the ability of selenium and vitamin E to prevent sepsis-induced changes in lung tissue. Methods: Fifty rats were divided into five groups: Group 1: Control group; Group 2: Sepsis group. In this group only cecal ligation and perforation (CLP) was performed. Group 3: Selenium group. An intraperitoneal dose of 100 µg selenium was given for the first two days followed by a daily dose of 40 µg for the next five days. CLP was performed the following day. Group 4: Selenium and vitamin E group. In addition to selenium, vitamin E was given intramuscularly in a dose of 250 mg/kg/day for seven days. CLP was performed the following day. Group 5: Vitamin E group. Vitamin E was given intramuscularly in a dose of 250 mg/kg/day for seven days. CLP was performed the following day. Results: There were significant differences between Group 2 and all other groups in terms of blood gas values (pH, pCO2, SaO2), and leukocyte, C-reactive protein (CRP) and glutathione peroxidase levels (p < 0.005). There was no statistically significant difference between groups 3, 4 and 5 in terms of histopathological changes in lung tissue (p > 0.05), but all groups were significantly different compared with Group 2 (p < 0.05). Conclusion: Sepsis-induced lung tissue damage can be reduced or prevented by pre-treatment with of selenium and/or vitamin E in a rat model.

Background Elderly patients with rib fractures carry a high morbidity rate, particularly due to pulmonary complications as decreased respiratory efforts ensue secondary to pain. Risk of bleeds in the elderly on anticoagulant therapy is high. The effort to reduce narcotic use in patients is now a health care priority. We propose that the use of paravertebral analgesia (PVA) pumps is an alternative pain control method with less risk and easy placement. Methods Two hundred and seventy-nine patients were admitted with multiple fractured ribs to the Trauma Center of Community Hospital and treated with the application of continuous PVA via a pump (72 patients). Pain scores were recorded before and after the initiation of the pump. These patients were compared with a group of the remaining 207 patients who received intravenous narcotics only. Results The mean change from baseline in pain scores for all patients was 1.43 (SD = 2.75). The mean change in pain for the treatment group was 1.93 (SD = 2.60), and the change in pain for the control group was 1.24 (SD = 2.79). Change in pain differed between groups (1.3 vs 1.8; P = .01) although it was a small difference. After adjusting for age, gender, Injury Severity Score, Glasgow Coma Scale, number of fractures, and comorbid conditions, there was no significant difference in pain post-procedure (odds ratio = 0.75; P = 0.39) with an effect size of 30% and total sample size of 279. Conclusion The PVA pump using bupivacaine is an effective safe and alternative method for managing elderly patients with rib fractures eliminating the serious side effects associated with narcotics.

<b><i>Background:</i></b> Patients with maintenance hemodialysis (MHD) generally have a microinflammatory state. The aim of this study was to investigate the effects of hemodialysis (HD) combined with hemoperfusion (HP) on microinflammatory state in elderly patients with MHD. <b><i>Methods:</i></b> One hundred and fifty elderly patients with MHD were randomly divided into the control group and the observation group. The control group received simple HD treatment, and the observation group received combined HD + HP treatment on the basis of the control group. After 6 months of continuous treatment, the patients were evaluated to compare the quality of life, inflammation, adverse reactions, and nutritional indicators in the 2 groups before and after treatment. <b><i>Results:</i></b> There was no significant difference in the quality of life between the 2 groups before treatment. After treatment, the scores of psychological aspects, physiological aspects, social aspects, environmental aspects, and independent ability in the observation group were higher than those in the control group, with statistical significance (<i>p</i> &#x3c; 0.05). There was no statistical significance in the level of inflammation between 2 groups before treatment. After treatment, the levels of hs-CRP, Hcy, IL-6, and TNF-α in the observation group were significantly lower than those in the control group, with statistical significance (<i>p</i> &#x3c; 0.05). The incidence of dry mouth, skin reaction, neuritis, and subcutaneous tissue fibrosis in the observation group was lower than that in the control group, with statistical significance (<i>p</i> &#x3c; 0.05). There was no statistical significance in nutritional level indexes between 2 groups before treatment (<i>p</i><sub>1</sub> &#x3e; 0.05). After treatment, the levels of hemoglobin, total protein, albumin, and transferrin in the observation group were significantly higher than those in the control group, with statistical significance (<i>p</i> &#x3c; 0.05). <b><i>Conclusion:</i></b> The clinical effect of HD combined with HP in elderly MHD patients is significant, which can effectively reduce the incidence of adverse reactions and inflammation in the patients and improve the quality of life and nutritional indicators of the patients.

Animals (122 mice) were infected each with eighty cercariae of S. mansoni and subsequently challenged intravenously eight weeks later with the following gram-negative organisms. S. typhi, E. coli, Klebsiella-enterobacter species, Proteus mirabilis and Pseudomonas aeruginosa. Enumeration of bacteria in the liver, spleen and blood and S. mansoni from the portal sistem was performed from one to four weeks later in infected animals. A significant difference between infection produced by S. typhi and other gram negative organisms was observed: S. typhi persisted longer in the spleen and liver and could be recovered from S. mansoni worms up to three weeks following bacterial infection. Other gram negative bacteria disappeared from S. mansoni worms after two weeks of initial challenge. Additional animals (51 mice) infected with S. mansoni were given S. typhi, E. coli or sterile saline. After two weeks, animals were sacrificed and the recovery rate of worms from the portal system, and the mesenteric and hepatic oogram were determined. in animals infected with E. coli a significant decrease in the number of worms was observed compared to the saline control group; thirty worms were recovered in the control group compared to two worms in e. coli infected animals. In addition, the patterns of oviposition was significantly different in these latter animals suggesting complete inhibition of this process. Following S. typhi infection the difference in recovery of worms and pattern of oviposition was minimal. These findings suggest a difference in the interaction of various gram negative bacteria and S. mansoni and are consistent with the clinical observation of prolonged salmonella bacteremia in patients with schistosomiasis.

Background:Sacral fractures are a source of pain leading to loss of autonomy in elderly patients. Sacroplasty may be an effective alternative of conservative medical treatment.Objectives:To evaluate the short-term analgesic effect of sacroplasty compared to conservative treatment in patients with osteoporotic sacral fracturesMethods:This is a retrospective study of cases of osteoporotic sacral fractures treated with sacroplasty, compared with cases treated with conservative medical procedure over the same period. Outcome was evaluated by pain (Visual analogic scale) short-term (one month) evolution and side effects occurrence.Results:From January 2009 to June 2019, eleven patients were treated with sacroplasty for osteoporotic fractures at the Besançon University Hospital Centre. These were compared to 12 patients with osteoporotic sacral fracture with exclusive medical management, as a control group. The two groups were similar in age, gender and pain level at baseline. The median VAS was 7/10 in both groups at baseline, In the sacroplasty group, a significant decrease of pain was observed over the two first weeks, with a tendency remaining at day 30. There were no significant differences in the conservative treatment group at one week (p=0.2), fourteen days (p=0.6) and thirty days (p=0.7) compared to basal assessment.When comparing the sacroplasty group and the conservative treatment group, no differences were noted at baseline between the two groups, there was a significant difference between the two groups the following day (p=0.001), one week (p=0.003) and day 15 (p=0.01) after the intervention. However, there was no significant difference between the two groups at day 30, but only a trend (p=0.08).Regarding analgesic treatments, 30% of patients in the sacroplasty group could reduce their analgesics between the time they entered and left hospital. None of the patients in the control group were able to reduce their analgesic treatment over this period. In addition, half of the patients in the sacroplasty group were successful in returning home compared to only one-third of the patients in the conservative treatment group.Conclusion:In this study, sacroplasty was associated with an early and significant pain relief compared to conservative management in patients with osteoporotic sacral fracture. Abstract The effect of feed mixture supplementation with MgCl2 on alimentary tract microflora, haematological markers, and selected parameters of reproductive performance of sows was investigated. The animals were divided into control group (K) and two experimental groups (E1 and E2). The sows from the experimental groups were receiving 1 g of MgCl2 6H20/100 kg b.w./day, which constituted 120 mg of pure magnesium, administered in a small portion of a feed mixture. The sows from group E1 were receiving MgCl2 every day for 30 d, whereas sows from group E2 for 60 d. A significant decrease in the count of E. coli, Providencia sp., and Proteus sp. was noted in faeces of the swine of both experimental groups. The study has also demonstrated a reduced number of lymphocytes and an increased number of granulocytes in blood of the sows receiving MgCl2. Feeding of pregnant sows with the addition of magnesium salt decreased the mortality rate of newborn piglets, increased the survival rate of piglets until 21 d of their life, and increased body weight values of litters originating from experimental sows, compared to the litters of control sows.
