Intestinal obstruction is an unusual complication of pregnancy, but may be encountered more frequently in the future. Two patients with such a complication are described. Relief of the obstruction was achieved by surgical intervention in one patient and by delivery of a term baby in the other. The management of intestinal obstruction in late pregnancy is discussed.

The main objective of health science is to provide better health to every human being. Indian system of medicine commonly known as Ayurveda has a holistic approach towards the disease and provides treatment without affecting the other parts of the body. Similarly, it is effective in preventing an individual from getting diseased in the future. In Ayurveda, diseases can be classified into two basic categories such as -ailments treated or managed by Shodhan therapy and the ailments treated by surgical intervention. Further, it’s suggested that in conditions where surgery is indicated, one can try Shodhan or Panchkarma therapies before performing surgery or/ if the patient is not fit for surgery or not willing to undergo a surgical procedure. Similarly, Acharya has specified that physician should not advocate Surgery in diseases which can be treated by Shodhan and Shaman therapies (conservative management). Panchakarma is a combination of five procedures of purification- Vamana (Emesis), Virechana (Purgation), Niroohavasti (Decoction enema), Nasya (instillation of medicine through nostrils), and Anuvasanavasti (Oil enema). These procedures aim at plucking away the deep-rooted imbalances in the body.

Pregnancy shearing has been shown to increase lamb birthweights. However, results have been variable between studies. It has been suggested that the birthweight response to pregnancy shearing is greatest in conditions in which the unshorn dam gives birth to lambs of low birthweight. This study was designed to examine the birthweight response under 2 differing maternal treatments (one designed to restrict fetal growth and the other designed not to limit fetal growth), with the aim of explaining the variation observed in pregnancy shearing studies. Mixed aged Romney cross ewes were either shorn during mid pregnancy (n = 55) or left unshorn (n = 50). At shearing, ewes were split into either a ‘maintenance’ group (64 kg at shearing and fed to permit total ewe liveweight gain equivalent to expected conceptus growth during mid- to late-pregnancy) (n = 52) or a ‘low’ group (54 kg at shearing and fed to permit total ewe liveweight to increase at half the expected conceptus growth during mid- to late-pregnancy) (n = 53), respectively. Mid-pregnancy shearing was found to increase the birthweights of singletons (5.6 v. 4.9 kg, P < 0.05) but not twins, and of lambs born to maintenance group ewes (by 0.6 kg, P < 0.05), but not low group ewes. Additionally, mid-pregnancy shearing had no effect on the thermoregulatory capacity of twin-born lambs. When the results of this study are considered with those of previous New Zealand pregnancy shearing studies, they suggest that there are 2 criteria (and not one, as previously suggested) that must be met to achieve a response to pregnancy shearing. Firstly, the dam must have the potential to respond [i.e. have been destined to give birth to an otherwise lightweight lamb(s)], and secondly, the ewe must have the means to respond (i.e. an adequate level of maternal reserves and/or level of nutrition to partition towards additional fetal growth).

Background. Intestinal obstruction in pregnancy is uncommon. The condition is associated with significant maternal and fetal mortality. The delay in diagnosis is due to nonspecific symptoms and a disinclination to carry out radiologic investigations in pregnancy.Case. A 39-year-old lady at 32 weeks of gestation presented with abdominal pain and nausea. Her symptoms worsened during admission. A computed tomography (CT) scan showed dilated small bowel loops suggestive of intestinal obstruction. She eventually underwent a laparotomy as conservative measures failed.Conclusion. A high index of clinical suspicion is required to diagnose intestinal obstruction in pregnancy. Prompt diagnosis should be made and the appropriate treatment instituted. Surgical intervention should be performed if necessary as further delay only results in increased morbidity and mortality.

A significant proportion of patients with intestinal obstruction will be evaluated with a CT scan of the abdomen. This study presents a group of 97 patients diagnosed with mechanical obstruction or ileus on CT scan over a 16-month period at a community based teaching hospital and follows the further management of these patients. Our study shows that 43.3 per cent of patients with mechanical obstruction, diagnosed by CT scan, eventually needed surgical treatment. On the other hand, even when CT indicates ileus, 20 per cent of these patients may still require surgical intervention.

In the XIV century. centripetal tendencies began to appear in the Moscow principality. Inside the Russian church, several areas were distinguished. Part of the clergy supported the specificobar form. The other understood the need for transformations in society. As a result, this led to a split in the Russian church in the 15th century for "non-possessors" and "Josephites". The former linked the fate of the future with the ideology of hesychasm and its moral transformation, while the latter sought support in alliance with a strong secular power.

This paper explores Arnold Schoenberg’s curious ambivalence towards Haydn. Schoenberg recognized Haydn as an important figure in the German serious music tradition, but never closely examined or clearly articulated Haydn’s influence and import on his own musical style and ethos, as he did with many other major composers. This paper argues that Schoenberg failed to explicitly recognize Haydn as a major influence because he saw Haydn as he saw himself, namely as a somewhat ungainly, paradoxical figure, with one foot in the past and one in the future. In his voluminous writings on music, Haydn is mentioned by Schoenberg far less frequently than Bach, Mozart, or Beethoven, and his music appears rarely as examples in Schoenberg’s theoretical texts. When Schoenberg does talk about Haydn’s music, he invokes — with tacit negativity — its accessibility, counterpoising it with more recondite music, such as Beethoven’s, or his own. On the other hand, Schoenberg also praises Haydn for his complex, irregular phrasing and harmonic exploration. Haydn thus appears in Schoenberg’s writings as a figure invested with ambivalence: a key member of the First Viennese triumvirate, but at the same time he is curiously phantasmal, and is accorded a peripheral place in Schoenberg’s version of the canon and his own musical genealogy.

Buying and selling is a way to transfer land rights according to the provisions in Article 37 paragraph (1) of Government Regulation Number 24 of 1997 concerning Land Registration which must include the deed of the Land Deed Making Official to register the right of land rights (behind the name) to the Land Office to create legal certainty and minimize the risks that occur in the future. However, in everyday life there is still a lot of buying and selling land that is not based on the laws and regulations that apply, namely only by using receipts and trust in each other. This is certainly very detrimental to both parties in the transfer of rights (behind the name), especially if the other party is not known to exist like the Case in Decision Number 42 / Pdt.G / 2010 / PN.Mtp

The future of medicine belongs to immunology and alergology. I tried to not be too wide in description, but on the other hand to mention the most important concepts of alergology to make access to these diseases more understandable, logical and more useful for our patients, that without complex pathophysiology and mechanism of immune reaction,we gain some basic insight into immunological principles. The name allergy to medicine was introduced by Pirquet in 1906, and is of Greek origin (allos-other + ergon-act; different reaction), essentially representing the reaction of an organism to a substance that has already been in contact with it, and manifested as a specific response thatmanifests as either a heightened reaction, a hypersensitivity, or as a reduced reaction immunity. Synonyms for hypersensitivity are: altered reactivity, reaction, hypersensitivity. The word sensitization comes from the Latin (sensibilitas, atis, f.), which means sensibility, sensitivity, and has retained that meaning in medical vocabulary, while in immunology and allergology this term implies the creation of hypersensitivity to an antigen. Antigen comes from the Greek words, anti-anti + genos-genus, the opposite, anti-substance substance that causes the body to produce antibodies.

The AICPA has taken the position that accreditation of CPAs in specific areas of practice is an important aspect of repositioning the CPA profession for the future. The AICPA currently offers two designations exclusively to CPAs, one of which is the Personal Financial Specialist (PFS) designation. However, the issue of accrediting CPAs by granting official AICPA designations is a complex and highly debated issue with opposing sides having compelling arguments supporting their positions. CPAs and other professionals specializing in personal financial planning have opportunities to obtain designations other than the PFS. This paper examines the relative value of these alternative options for financial planners. Specifically, the research was designed to examine the differential effects of alternative financial-planning accreditations on users' perceptions. These perceptions relate to various professional attributes of a financial planner such as their knowledge and expertise, objectivity, and level of trust and ethics possessed. In addition, these perceptions relate to fees charged and the influence that the designation has on the public's choice of a financial planner. Our results indicate that the CPA designation used in conjunction with the PFS designation is generally perceived to signal a higher level of professional attributes than the other designations examined in the study. In addition, a CPA with a PFS designation has a significantly greater influence on the public's choice of a financial planner than do the other designations. These results suggest that important benefits may accrue to CPAs from holding the PFS specialty accreditation.
