Relationship between tumour necrosis factor-alpha (TNFα) production and a specific multiple sclerosis (MS) associated TNF gene haplotype

Objective: To determine if monocyte TNFa production from patients homozygous for a specific MS associated TNF gene haplotype is different from that produced in patients either heterozygous for, or without this haplotype. Background: The balance between pro- and anti-inflammatory cytokines is important in the clinical outcome of inflammatory reactions. Levels of TNFa, a pro-inflammatory cytokine, is raised in MS as well as being found in acute and chronic MS lesions. A previous population based study in Northern Ireland with polymorphisms spanning the TNF gene region identified a conserved MS associated haplotype in relation to three markers (130: 118: 127 TNF d: a: b) for which 19 MS patients were homozygous. Methods: Venous blood collected in EDTA to give a concentration of 1073 M was drawn from 16 patients with the conserved MS associated haplotype, 19 patients heterozygous for the haplotype and 17 patients without the haplotype. Mononuclear cells were separated and cultured by standard techniques and levels of TNFa and of TNF binding proteins I and II were determined by commercial enzyme-linked immunosorbent assays. Results: There were no significant differences in TNFa production in the 3 h (P=0.28) or 24 h cultures (P=0.18) or following stimulation with interferon-g (P=0.17) between the group positive for the conserved haplotype and the group negative for this haplotype. There was also no significant difference when compared to the heterozygote group. No association was found between the MS associated haplotype and levels of either TNF binding protein. A greater proportion of patients with the conserved haplotype had a benign clinical course (P=0.06). Conclusion: We conclude that whilst a trend exists, we have found no significant association between peripheral TNFa production and a specific MS associated TNF haplotype in this population. Paradoxically this haplotype may also predict a more favourable clinical course.

Abstract The aim of this work was a correlation study and histopathological description of alterations associated with the presence of Leishmania infantumamastigote in the intestinal wall of dogs infected with canine visceral leishmaniasis (CVL). Three groups were used: G1 (n = 8), comprising naturally infected dogs with CVL with amastigotes of L. infantum in the small and large intestines; G2 (n = 9), infected dogs with CVL, without intestinal amastigotes; and G3 (n = 3), uninfected dogs. Histochemistry and immunohistochemistry methods were used for histopathology and amastigotes identification. 47.1% (8/17) of dogs from G1 group had amastigotes in the mucosa, submucosa and muscle layers of the small and large intestines and it was observed a prominent inflammatory reaction characterized by chronic infiltration of mononuclear cells: macrophages, lymphocytes and plasma cells. Comparison between the groups showed only a significant difference in relation to mucosal microscopic structural alterations in dogs from G1 in relation to G2 and G3. Parasite burden showed significant correlations with the microscopic alterations and clinical status of dogs in G1. By the conclusion, the inflammatory reactions caused by the parasites in the intestines might have contributed towards alterations in digestive processes, worsening the dogs’ clinical status of CVL.

The aim of our research was to evaluate the changes in levels of cytokines and redox state parameters in blood and isolated heart of rats subjected to different swimming protocols. Rats were divided into 3 groups: 1) controls, 2) moderately trained rats that during all 12 weeks swam 1 h/day, 5 days/week, and 3) overtrained rats that in 10th week swam twice, 11th week 3 times, and in 12th week 4 times a day for 1 h. After sacrificing, blood from jugular vein was collected, and the heart excised and perfused on a Langendorff apparatus. Samples of the coronary effluent were collected during coronary autoregulation. Levels of superoxide anion radical (O2-), hydrogen peroxide (H2O2), nitric oxide (NO) and thiobarbituric acid reactive substances (TBARS) were measured in plasma and coronary effluent, while reduced glutathione (GSH), activities of superoxide dismutase (SOD) and catalase (CAT) were measured in erythrocytes. Venous blood was also used for interleukin 6 (IL-6) and tumor necrosis factor alpha (TNF-α) determination. Moderate training protocol induced the decrease of TBARS in plasma, while both training protocols induced the decrease of O2- and H2O2 in coronary effluent. There was no significant difference in levels of cytokines between groups. The results of study add evidence about beneficial effects of moderate-intensity training on blood and cardiac redox state of rats, and furthermore, shows that exercising frequently, if the intensity stays within moderate range, may not have detrimental effects.

BACKGROUND:Tumour necrosis factor-alpha (TNF-α) is a major proinflammatory cytokine that is thought to be important in the pathogenesis of asthma. However, alterations in systemic regulation of this cytokine in asthma have not been examined in the context of corticosteroid therapy.OBJECTIVES:To examine the ability of peripheral blood mononuclear cells (PBMC) from three different groups of patients with asthma requiring varying amounts of inhaled corticosteroids (ICS) for clinical control, and to examine cells from age- and sex-matched nonasthmatic patients to produce TNF-α.DESIGN:All patients with asthma had a positive methacholine challenge test. 'High dose' ICS patients with asthma required ICS greater than or equal to 800 µg/day. 'Medium dose' patients with asthma were on less than or equal to 500 µg/day of ICS, whereas 'no ICS' patients with asthma had received no ICS for at least three months. Each patient with asthma was examined in parallel with an age- and sex-matched, nonasthmatic, nonatopic control subject. Cells were cultured (with or without the addition of potential stimulators phytohemagglutinin, lipopolysaccharide, formyl-methionine-leucine-phenylalanine or antihuman CD3), and TNF-αproduction was assessed by ELISA.MAIN RESULTS:PBMC from both high dose ICS (n=8) and no ICS (n=11) patients with asthma produced more than twice the amount of TNF-αthan cells from matched nonasthmatic control patients (P<0.01) when cultured alone or in the presence of each stimulus (P<0.05). In contrast, there was no significant difference in TNF-αproduction between medium dose ICS patients with asthma and control patients. A group of asymptomatic atopic patients (n=6) did not have an increased level of TNF-αproduction.CONCLUSIONS:Increases in TNF-αproduction within the PBMC compartment can be observed in both patients with asthma receiving high dose ICS and in a group of patients with mild asthma receiving no ICS therapy, but not in patients with asthma receiving a medium dose of ICS or atopic patients.

ABSTRACT We have previously demonstrated reactivation of latent human cytomegalovirus (HCMV) in myeloid lineage cells obtained from healthy donors. Virus was obtained from allogenically stimulated monocyte-derived macrophages (Allo-MDM), but not from macrophages differentiated by mitogenic stimulation (ConA-MDM). In the present study, the cellular and cytokine components essential for HCMV replication and reactivation were examined in Allo-MDM. The importance of both CD4+ and CD8+ T cells in the generation of HCMV-permissive Allo-MDM was demonstrated by negative selection or blocking experiments using antibodies directed against both HLA class I and HLA class II molecules. Interestingly, contact of monocytes with CD4 or CD8 T cells was not essential for reactivation of HCMV, since virus was observed in macrophages derived from CD14+monocytes stimulated by supernatants produced by allogeneic stimulation of peripheral blood mononuclear cells. Examination of the cytokines produced in Allo-MDM and ConA-MDM cultures indicated a significant difference in the kinetics of production and quantity of these factors. Further examination of the cytokines essential for the generation of HCMV-permissive Allo-MDM identified gamma interferon (IFN-γ) but not interleukin-1 or -2, tumor necrosis factor alpha, or granulocyte-macrophage colony-stimulating factor as critical components in the generation of these macrophages. In addition, although IFN-γ was crucial for reactivation of latent HCMV, addition of IFN-γ to unstimulated macrophage cultures was insufficient to reactivate virus. Thus, this study characterizes two distinct monocyte-derived cell types which can be distinguished by their ability to reactivate and support HCMV replication and identifies the critical importance of IFN-γ in the reactivation of HCMV.

Abstract BACKGROUND The 11q22.3 deletion (11q-) encompassing the ATM locus detected by fluorescence in situ hybridization (FISH) is present in up to 20% of CLL patients (pts), and is associated with an aggressive disease course and poor response to treatment (tx). However, there are few clinical studies that have specifically focused on pts with 11q-. Prognostic factors that may explain the variable outcome in this group have not been investigated, nor have survival outcomes in a population-based setting. Furthermore, few clinical studies have described practice patterns within the 11q- group. This study aims to improve our understanding of the prognosis, management and clinical course of CLL pts with 11q-. METHODS The BC Provincial CLL Database, which includes all pts who have undergone CLL FISH testing in a provincially validated laboratory since 2004, was used to identify all pts in BC (population 4.5 million) with a confirmed 11q- >10% found at any point during the disease course, with or without other detectable FISH abnormalities. Overall survival (OS), treatment-free survival (TFS) (time from diagnosis to first tx/death) and time to second-line treatment (tx) were the main outcomes of interest, and were investigated in relationship to age (<60 vs ≥60 years [yrs]), sex, Rai stage (3-4 vs 0-2), CD38 status, % 11q-, and presence of other recurrent FISH abnormalities (17p deletion [17p-], trisomy 12, 13q deletion, and IGH translocation). Multivariate analysis (MVA) was performed using Cox proportional hazard models with a backwards stepwise selection process to determine predictors of OS/TFS. RESULTS 125 out of 1044 (12%) pts were identified with 11q- detected at any point. Median age at CLL diagnosis was 61 yrs (range, 35-80). At a median follow-up of all patients of 6.3 yrs (range 0.5 - 26.2), median OS and TFS for the entire cohort are 12.6 yrs (standard error [SE] = 1.5 yrs) and 2.5 yrs (SE = 0.5 yrs), respectively. In MVA, advanced Rai stage (HR 3.7, 95% CI 1.28-10.8, P=.015), age ≥ 60 yrs (HR 2.2, 95% CI 1.2-4.3, P=.012) and male gender (HR 2.4, 95 % CI 1.5-5.3, P=.024) were significant predictors of OS. Clonal evolution (CE) to 11q- and/or other FISH abnormalities was documented in 12 pts. In 9/12 cases, 11q- developed after one or more tx courses whereas, in 3/12 cases, 11q- developed prior to tx initiation at median 5 yrs (range, 3.8-6.5) from initial FISH test. Sixty-nine pts had a documented 11q- pre-tx. At median follow-up of 5.0 yrs (range, 0.5 to 18.7), this group had median OS and TFS of 14.7 (SE =1.8) and 2.5 yrs (SE= 0.5), respectively (Fig 1). Pts with presence of 11q- and 17p- had a markedly worse prognosis compared to those without, with median OS 4.9 vs. 14.7 yrs (P< .001) and TFS of 0.2 vs. 2.5 yrs (P=0.31). In MVA, co-presence of 17p- (HR = 11.3, 95% CI 2.4-52.5, P=.02) and age ≥ 60 yrs (HR 3.8, 95% CI 0.83-17.1, P=.05) were adverse prognostic markers for OS. Age ≥ 60 yrs (HR =2.0, 95% CI 1.0-3.9, P=.03), presence of > 75% 11q- (vs. < 75 % 11q-) (HR = 1.8, 95%CI 1.0-2.3, P=.03) and advanced Rai stage (HR = 7.1, 95%CI 2.4-21.0, P=.01) were associated with shorter TFS. Of this group of 69, 52 pts (75%) received at least one tx course during follow-up. Thirty-three pts received fludarabine +/- rituximab (FR) as first line, 14 pts received alkylator based tx (FC +/- R in 8; CVP +/- R in 5; CHOP + R in 1) and 5 pts received chlorambucil-based +/- R. Pts treated with FR alone had median OS 12.8 yrs (SE=1.0), which was not statistically different from those treated with alkylators , where median OS was 6.8 yrs (SE=2.1), P=.35 (Fig 2). In MVA, receipt of fludarabine vs alkylator-based treatment was not associated with any significant difference in OS (P=.52) or time to 2nd tx (P=.65). CONCLUSION This study is to our knowledge the largest population-based report of CLL pts with 11q- and further enhances our understanding of the clinical course of such pts in a real world setting. Although median TFS of 11q- pts is short at 2.5 yrs, OS remains long at 12.6 yrs. The importance of combined 11q- and 17p- as adverse prognostic markers in CLL is highlighted. Most pts in BC with 11q- received FR as first line therapy. There is retrospective trial data suggesting that first line tx containing alkylators (ie. cyclophosphamide) can overcome the adverse prognostic features of 11q-. However, most of this trial data is from the pre-R era. Though limited in numbers, our study does not suggest that first line tx without alkylators is associated with negative outcomes. SummaryTo characterize the extent of early activation of the hemostatic system following angioplasty, we obtained blood samples from the involved coronary artery of 11 stable angina patients during the procedure and measured sensitive markers of thrombin formation (fibrino-peptide A, prothrombin fragment 1.2, and soluble fibrin) and of platelet activation ((3-thromboglobulin). Levels of hemostatic markers in venous blood obtained from 14 young individuals with low pretest probability for coronary artery disease were not significantly different from levels in venous blood or intracoronary samples obtained prior to angioplasty. Also, there was no translesional (proximal and distal to the lesion) gradient in any of the hemostatic markers before or after angioplasty in samples obtained between 18 and 21 min from the onset of the first balloon inflation. Furthermore, no significant difference was noted between angioplasty and postangioplasty intracoronary concentrations. We conclude that intracoronary hemostatic activation does not occur in the majority of patients during and immediately following coronary angioplasty when high doses of heparin and aspirin are administered.

Abstract: Routine blood examination is the earliest blood test or screening test to determine the diagnosis of an abnormality. Blood easily froze if it is outside the body and can be prevented by the addition of anticoagulants, one of which Ethylene Diamine Tetra Acetate (EDTA). Currently available vacuum tubes containing EDTA anticoagulants in the form of K2EDTA and K3EDTA. K3EDTA is usually a salt that has better stability than other EDTA salts because it shows a pH approaching a blood pH of about 6.4. The purpose of this research is to know the difference of erythrocyte index results include MCH, MCV and MCHC using K3EDTA anticoagulant with K2EDTA. This research is a cross sectional design. This study used venous blood samples mixed with K2EDTA anticoagulant and venous blood mixed with K3EDTA anticoagulants, each of 30 samples. Data were collected and analyzed using paired different test. Based on data analysis that has been done on MCH examination, p value <0,05 then there is a significant difference between samples with K3EDTA anticoagulant with K2EDTA to erythrocyte index value. Then on the examination of MCV and MCHC obtained p value <0.05 then there is no significant difference between samples with K3EDTA anticoagulant with K2EDTA to erythrocyte index value.Abstrak: Pemeriksaan darah rutin merupakan pemeriksaan darah yang paling awal atau screening test untuk mengetahui diagnosis suatu kelainan. Darah mudah membeku jika berada diluar tubuh dan bisa dicegah dengan penambahan antikoagulan, salah satunya Ethylene Diamine Tetra Acetate (EDTA). Dewasa ini telah tersedia tabung vakum yang sudah berisi antikoagulan EDTA dalam bentuk  K2EDTA dan  K3EDTA. K3EDTA  biasanya berupa garam yang mempunyai stabilitas yang lebih baik dari garam EDTA yang lain karena menunjukkan pH yang mendekati pH darah yaitu sekitar 6,4. Tujuan dari penelitian ini adalah untuk mengetahui perbedaan hasil indeks eritrosit meliputi MCH, MCV dan MCHC menggunakan antikoagulan K3EDTA dengan K2EDTA. Penelitian ini merupakan penelitian dengan desain cross sectional. Penelitian ini menggunakan sampel darah vena yang dicampur dengan antikoagulan K2EDTA dan darah vena yang dicampur dengan antikoagulan K3EDTA, masing-masing sebanyak 30 sampel. Data dikumpulkan dan dianalisis menggunakan uji beda berpasangan. Berdasarkan analisis data yang telah dilakukan pada pemeriksaan MCH didapatkan nilai p < 0,05 maka ada perbedaan yang signifikan antara sampel dengan antikoagulan K3EDTA dengan K2EDTA terhadap nilai indeks eritrosit. Kemudian pada pemeriksaan MCV dan MCHC didapatkan nilai p < 0,05 maka tidak ada perbedaan yang signifikan antara sampel dengan antikoagulan K3EDTA dengan K2EDTA terhadap nilai indeks eritrosit.
