Nurses anaesthetists’ versus patients’ assessment of anxieties in an ambulatory surgery setting

Background Failure to assess patients’ anxiety perioperatively by means of a validated instrument makes the assessment arbitrary. Studies are lacking about how well nurse anaesthetists estimate patients’ preoperative worries. Purpose To compare the nurse anaesthetists’ estimations of patients’ preoperative anxieties with the patients’ own assessment of their anxieties. Design Quantitative prospective design. Methods Eighty-five pairs of patients and nurse anaesthetists in two ambulatory surgery units in a university hospital in Sweden were included. Patients’ perioperative anxieties were graded using the Numeric Visual Analogue Anxiety Scale. Results The nurse anaesthetist overestimated the patients’ level of preoperative anxiety in 53% of patients and underestimated patients’ anxieties in 31% of the patients. Consensus was seen in 16% of the pairs. In fifty-six pairs (65%), the difference between the estimation of level of patients’ anxiety according to Numeric Visual Analogue Anxiety Scale was between −3 (overestimation) and +3 levels (underestimation). Median levels of anxiety were estimated as 3 within the patient group and 4 among the nurse anaesthetists. Conclusions Systematic assessment of patients’ level of anxiety could lead to identifying patients with severe anxiety levels and to offer more individualised treatment. The patients’ own estimation must form the basis for the discussion and treatment.

Summary: To construct a Spanish version of the Kuwait University Anxiety Scale (S-KUAS), the Arabic and English versions of the KUAS have been separately translated into Spanish. To check the comparability in terms of meaning, the two Spanish preliminary translations were thoroughly scrutinized vis-à-vis both the Arabic and English forms by several experts. Bilingual subjects served to explore the cross-language equivalence of the English and Spanish versions of the KUAS. The correlation between the total scores on both versions was .93, and the t value was .30 (n.s.), denoting good similarity. The Alphas and 4-week test-retest reliabilities were greater than .84, while the criterion-related validity was .70 against scores on the trait subscale of the STAI. These findings denote good reliability and validity of the S-KUAS. Factor analysis yielded three high-loaded factors of Behavioral/Subjective, Cognitive/Affective, and Somatic Anxiety, equivalent to the original Arabic version. Female (n = 210) undergraduates attained significantly higher mean scores than their male (n = 102) counterparts. For the combined group of males and females, the correlation between the total score on the S-KUAS and age was -.17 (p < .01). By and large, the findings of the present study provide evidence of the utility of the S-KUAS in assessing trait anxiety levels in the Spanish undergraduate context.

Introduction: The aim of this study was to determine the prevalence of exposure to hepatitis A by means of serologic markers in chronic hepatitis B patients, with the secondary aim of finding the best prevention method for hepatitis A infection in susceptible groups of our setting. Methods: During the period between 2016 and 2017, we recruited 403 hepatitis B patients aged more than 14 years and regularly attending the infectious diseases clinic at a referral university hospital, Tehran, Iran. A blood sample was collected from all the patients and tested for hepatitis A IgG. The data was analyzed by SPSS v.19. Results: Although none of the patients had previously received hepatitis A vaccine, the results for serologic level of hepatitis A IgG, demonstrated positive results in 379 (94%) cases. The mean age of patients with negative and positive IgG was 29.17 and 42.46 years, respectively; the difference was statistically significant (P≤0.001). The majority of seronegative patients were young adults aged < 25 years and 25 to 35 years (P <0.001). Conclusion: Seroprevalence of hepatitis A in chronic HBV patients in Iran is high. As HBV infected patients younger than 35 years could be seronagative for HAV infection, evaluation of these patients for HAV infection and vaccination of seronegative patients would be a reasonable approach.

The study was carried out to examine the situation of university students from one month after the beginning of a very strict confinement process in Spain during the COVID-19 pandemic. Students responded to a survey which included the 7-item Generalized Anxiety Disorder Scale (GAD-7) together with other questions relating to their general well-being from the European Quality of Life Survey (EQLS). A total of 198 university students answered the web-based survey. The questionnaire was generated using Microsoft Forms and was explained and distributed online. The results indicated that around 18.7% of students were suffering from severe anxiety and 70.2% were suffering either mild or moderate anxiety at this point of the strict confinement process. The findings show that when emotional well-being (quality of sleep, the perception of feeling fear, death of a relative) is reduced and material well-being is negatively affected (income level) anxiety levels are increased. On the other hand, the results show that having good interpersonal relationships with family members and taking care of personal development (routines and habits that make them feel good) help reduce anxiety levels. The female students in the sample also suffered higher levels of anxiety than males during strict confinement.

Background: Anxiety is a feeling felt by respondents regarding disturbing mental tension, which as a general reaction with the inability to overcome a problem or there is no sense of security, which later leads to a physiological and psychological change. The level of family anxiety can be assessed by using a questionnaire with each respondent's characteristics and level of anxiety can be divided into several parts, namely: normal anxiety levels, mild anxiety levels, moderate anxiety levels, severe anxiety levels and panic. Objective: Knowing the description of the level of family anxiety in caring for patients with schizophrenia at the Mental Poliklinik UPTD. Rumah Sakit Jiwa Provinsi Bali and the characteristics of families caring for patients with schizophrenia based on education, age, and type of work. Methods: The research method that used in this study was descriptive research. This study used a cross sectional study, which was a study in which the subjects were evaluated only once and the measurement of each only one variable at the same time that using sampling consecutive sampling technique. The sample used was 32 people consisting of family members of hallucinations patients. Results: Based on research conducted at the Poliklinik UPTD. Rumah Sakit Jiwa Provinsi Bali toward 32 family members who was caring for patients by schizophrenia, most family members did not experience anxiety or anxiety in the normal range which was 29 family members of 32 family members and 3 families who experienced mild anxiety levels. Conclusion: That most family members who treated patients by schizophrenia at the Poliklinik UPTD. Rumah sakit Jiwa Provinsi Bali does not experience anxiety or anxiety still within normal limits. Latar Belakang: Kecemasan adalah suatu perasaan yang dirasakan oleh responden mengenai ketegangan mental yang menggelisahkan, dimana sebagai reaksi umum dengan ketidakmampuan dalam mengatasi suatu masalah atau tidak ada rasa aman, yang nantinya menimbulkan suatu perubahan fisiologis dan psikologis. Keluarga yang memiliki anggota keluarga dengan  gangguan jiwa dapat mengalami kecemasan Tingkat kecemasan keluarga dengan skizofrenia yaitu cemas ringan (80,49%) dan 8 keluarga mengalami cemas sedang (19,51%). Tujuan: Mengetahui gambaran tingkat kecemasan keluarga dalam merawat pasien dengan skizofrenia di Poliklinik Jiwa UPTD Rumah Sakit Jiwa Provinsi Bali dan karakteristik keluarga yang merawat pasien dengan skizofrenia berdasarkan pendidikan, umur, dan jenis pekerjaan. Metode: Metode penelitian yang digunakan dalam penelitian ini adalah penelitian bersifat deskriptif. Penelitian ini menggunakan penelitian cross sectional yaitu suatu penelitian dimana subjek diobsevasi hanya satu kali saja dan pengukurannya masing-masing satu variabel pada waktu yang sama yang menggunakan teknik sampling consecutive sampling. Sampel yang digunakan berjumlah 32 orang terdiri dari anggota keluarga pasien Halusinasi. Hasil: Berdasarkan penelitian yang dilakukan di Poliklinik Jiwa UPTD. Rumah Sakit Jiwa Provinsi Bali terhadap 32 anggota keluarga yang merawat pasien dengan skizofrenia, sebagian besar anggota keluarga tidak mengalami kecemasan atau kecemasan masih dalam batas normal jumlah 29 anggota keluarga dari 32 anggota keluarga dan 3 keluarga yang mengalami tingkat kecemasan ringan. Kesimpulan: Sebagian besar anggota keluarga yang merawat pasien dengan skizofrenia di Poliklinik Jiwa UPTD. Rumah Sakit Jiwa Provinsi Bali tidak mengalami kecemasan atau kecemasan masih dalam batas normal.

<strong><em></em></strong><em>Menarche is the first menstruation that usually occurs in the age range of 10-16 years or in early adolescence in the middle of puberty before entering reproduction (Proverawati and Misaroh, 2009). Menarche is the first mestruasi blood expenditure, the arrival of menarche can cause positive or negative reactions, a positive reaction if the individual is able to appreciate, understand and accept menarche as a sign of maturity that is owned by a woman otherwise a negative reaction will cause anxiety. Anxiety is a condition where an individual or group experiences anxiety (judgment or opinion) and autonomic nervous system activity in responding to threats that are unclear, non specific. The purpose of this research is to determine the level of anxiety of adolescents facing menarche in  IT Khalisaturrahmi Binjai, mild anxiety level, moderate anxiety level, severe anxiety level. The type of research used is descriptive. The population in this study amounted to 32 respondents. The sample in this study amounted to 24 respondents with consecutive sampling techniques. The results showed that respondents experienced mild anxiety levels of 12 people (50%), 8 people (33%) moderate anxiety and 4 people (17%) with severe anxiety. From the results of the study it can be concluded that the level of anxiety of adolescents facing menarche in IT Middle School Khalisaturrahmi Binjai majority in the category of mild anxiety levels, amounting to 12 respondents or (50%). Abstract Objective: Carpal tunnel syndrome (CTS) is the most common peripheral nerve entrapment neuropathy and is seen in 3 % of the general population. The aim to this study was examine the relationship between the symptoms of cleaning compulsion with using Moudsley obsessive compulsive inventory ( MOCI ) and patients with CTS. Methods: Sixty patients with CTS and 60 healty controls were evaluated in Bezmialem Vakif University Faculty of Medicine Neurosurgery Clinic. Neurological examination, Electrophysiological findings, the presence of neuropathic pain evaluated by the Leeds assessment of neuropathic symptoms and signs (LANSS) , the symptoms of obsessive- compulsive disorder using with MOCI and anxiety levels assessed by the Beck Anxiety Inventory (BAI) were evaluated. Results: No statistically significant relationship was observed between anxiety levels and CTS (p › 0.005). No statistically significant relationship was found MOCI rumination, control and doubting subscale scores (p › 0.005). Patient group had higher MOCI cleaning subscale scores than the control group and the difference was statistically significant (p=0.008). Patient group had higher MOCI slowness subscale scores than the control group and the difference was statistically significant (p=0.004). Conclusion: Increased wrist movement in people that have the symptoms of compulsive cleaning is associated with the risk of CTS. Our study show that a reduction of high level of wrist flexion-extansion movements may decrease the new CTS cases.

Objective: Sexual dysfunction is a side effect of methadone maintenance therapy (MMT). Opium Tincture (OT) has been used as a maintenance treatment. This study aimed to determine and compare the trend of sexual function and its related factors during treatment with both drugs. Method: An observational study was designed to measure the blood tests including free and total testosterone, prolactin, and sex hormone-binding globulin and a battery of questionnaires, including demographics and drug use history, in 42 and 53 patients entering MMT and OT treatment before and 1 and 3 months after the treatment. Results: Significant changes in testosterone levels were observed in the MMT but not the OT group. The difference between the two groups was not significant. Neither between nor within changes in the sexual function and premature ejaculation scores were significant (P =0.370& 0.698). Anxiety levels were significantly different (P= 0.001) within and between groups. There was a considerable difference in the trend of depression changes in the OT group, but not different in MMT group and between the two groups. Conclusion: No difference was found between MMT and OT effects on sexual function variables. The decrease in Testosterone during the three months of MMT, was not associated with diminished sexual function. In the MMT group, anxiety levels diminished during treatment. It seems that decreased testosterone in the MMT group was compensated by improved anxiety. Gonadotropin levels may not be the sole determinant in sexual activity, and complex interaction of mood and anxiety, agonist levels, and gonadotropins are involved.

Background: This cross- sectional observational study was carried out with an aim to look for microsatellite instability (MSI) status in colorectal carcinoma and their association with different histomorphological patterns and biological behavior of colorectal carcinoma.Methods: This cross-sectional observational study was done in the Department of Pathology, Bangabandhu Sheikh Mujib Medical University Hospital (BSMMU), Dhaka, Bangladesh during September 2014 to October 2015. A total of 39 surgically resected sample of colorectal carcinoma were included. Consent from each patient was taken. The samples were histopathologically evaluated according to the standard protocol. The statistical analyses were done using Statistical packages for social sciences (SPSS 15) for Windows.Results: A total of 39 cases of colorectal carcinoma were included in this study. Majority of the patients (55.5%) was in 6th decade in MSI and 29.1% were MSI absent group. The mean age was found 47.67±10.97 years in present group and 47.84±14.26 years in absent group. The difference was not statistically significant (p>0.05). TNM stage with MSI was observed. The mean CEA level was 100.74±103.66 and 60.43±91.72. The mean Hb was 9.72±1.99 % and 9.92±2.17, the range was 7.2-12.2 and 4.6-13.4 among the groups. The mean difference was not statistically significant (p>0.05). Ulcerated was 3 (33.3%) and 19 (64.5%). Stage 3 tumor was 4 (44.4%) and 16 (51.6%). Grade 2 tumor was 5 (55.6%) and 17 (58.0%).Conclusions: For the first time in Bangladesh, this study was undertaken to evaluate the microsatellite instability (MSI) status in colorectal cancer tissue and their association with different histomorphological patterns of colorectal carcinoma.

IntroductionFalls of the elderly to a degree been associated with poor mental health, poor social support and poor physical health.ObjectivesTo investigate the falls of elderly people in relation to their mental and physical healthy.AimsTo compare the effects of falls in the elderly in the areas of mental and physical health.MethodsThe current study used purposive sampling compromised from 48 people that visited the emergency department at the Patras University Hospital in 2016. The inclusion criterion for participation was age (> 65 years). Data was collected using WHO's questionnaire, the WHOQUOL-BREF. Finally, data was analyzed using the test t test for independent samples.ResultsThe sample constituted by 39.6% of male and 61.4% of female. The average age of the sample was M = 75.89 years. In relation to mental health, the average of the elderly with a history of falls found M = 57.26 (SD = ± 22.87), while the other was found M = 74.45 (SD = ± 15.81). The difference between the two groups was statistically significant (P < 0.05), while physical health although again the first group found to have a smaller average (M = 56.65, SD = ± 22.13) relative to the second group (M = 63.78, SD = ± 12.59) no statistical difference was observed.DiscussionsThese results demonstrates that falls beyond the physical damage that are immediately visible can as well create significant issues in the psychological state of the elderly exacerbating anxiety, fear and social isolation, which has been associated with depression event.Disclosure of interestThe authors have not supplied their declaration of competing interest.
