Abstract Background Nursery farms that accept nursing and growing pre-weaned heifer calves from private dairy farms must work to prevent bovine respiratory disease (BRD). Knowledge of the BRD-associated risk factors related to calf management and calves’ condition will help to develop appropriate neonatal management practices at original farms and to identify calves at higher risk for BRD at nursery farms. In this study, the relationship between BRD and calf management practices (colostrum feeding, dam parity, serum total protein concentration at introduction (TP), body weight at introduction, introduction season, and daily average growth) was investigated using observational data from pre-weaned dairy calves introduced into a nursery farm in Hokkaido, Japan between 2014 and 2018 (n = 3185). Using additive Bayesian network (ABN) analysis, which is a multivariate statistical modelling approach, the direct and indirect associations between these factors were assessed. Results Colostrum feeding contributed to an increase in TP (correlation 1.02 [95 % CI, 0.94;1.10]), which was negatively associated with BRD directly (log odds ratio − 0.38 [− 0.46;−0.31]) and indirectly through increasing daily growth (correlation 0.12 [0.09;0.16]). Calves of multiparous dams had higher body weight at introduction (correlation 0.82 [0.74;0.89]), which indirectly reduced BRD risk through the increasing daily growth (correlation 0.17 [0.14;0.21]). Calves introduced during winter had the highest risk for BRD (log odds ratio 0.29 [0.15;0.44]), while those introduced in summer had the lowest risk (log odds ratio − 0.91 [− 1.06;−0.75]). The introduction season was also associated with BRD indirectly through dam parity, body weight at introduction, and daily growth. Conclusions The following calf management practices are recommended for preventing BRD in pre-weaned calves at nursery farms: (1) encouraging colostrum feeding to neonatal calves at their original farms; and (2) identifying calves with higher BRD risk, i.e., those without feeding colostrum, born to primiparous cattle, with low body weight at introduction, and/or introduced in winter, and paying intensive attention to the calves for rapid detection of BRD. ABN analysis applied enabled us to understand the complex inter-relationships between BRD incidence and the risk factors, which will help to reduce BRD incidence and to rear healthy calves at nursery farms.

Abstract Background The relationship between intraoperative low bispectral index (BIS) values and poor clinical outcomes has been controversial. Intraoperative hypotension is associated with postoperative complication. The purpose of this study was to investigate the influence of intraoperative low BIS values and hypotension on postoperative mortality in patients undergoing major abdominal surgery. Methods This retrospective study analyzed 1,862 cases of general anesthesia. We collected the cumulative time of BIS values below 20 and 40 as well as electroencephalographic suppression and documented the incidences in which these states were maintained for at least 5 minutes. Durations of intraoperative mean arterial pressures (MAP) less than 50 mmHg were also recorded. Multivariable logistic regression was used to evaluate the association between suspected risk factors and postoperative mortality. Results Ninety-day mortality and 180-day mortality were 1.5% and 3.2% respectively. The cumulative time in minutes for BIS values falling below 40 coupled with MAP falling below 50 mmHg was associated with 90-day mortality (odds ratio, 1.26; 95% confidence interval, 1.04-1.53; P = .019). We found no association between BIS related values and 180-day mortality. Conclusions Delicate adjustment of anesthetic depth is important to avoid excessive brain suppression and hypotension, which could be associated with postoperative mortality.

Background : In Indonesia, eclampsia in addition to bleeding and infection is still the main cause of maternal mortality and a high cause of perinatal mortality. Therefore, early diagnosis of preeclampsia, which is a preliminary level of eclampsia, and its handling need to be implemented immediately to reduce maternal and child mortality (Prawiroharjo, 2012). Method : The research design used was case control. Population is the whole object of research. The population in this study were all women who gave birth at Muhammad Sani Karimun Hospital, namely 218 people with the number of preeclampsia incidents of 30 people. The number of cases to be used is 2 (two) times the number of cases, namely 60 people. The analysis used was the Ods Ratio (OR), which is a measure of the association of exposure (risk factors) with the incidence of disease in the risk group (exposed to risk factors) compared to the incidence of disease in the group that is not at risk (not exposed to risk factors). Result : The results of the study, the value of the Prevalence Odds Ratio (POR) = 6,875 with a value of 95% Confidence Interval (CI) = 3,318-12,410. Congclusions : The conclusion of this research is that respondents with high parity are 6.8 times more likely to experience preeclampsia than respondents with little parity.

We investigated the relationship between mammary gland volume (MGV) of the breast as measured with three-dimensional chest computed tomography (CT) and breast cancer risk. Univariate analysis was used to assess the relationship between MGV and known risk factors in 427 healthy women. A case control study (97 cases and 194 controls) was conducted to assess breast cancer risk. MGV was significantly smaller for postmenopausal women than for premenopausal women, and was significantly larger for women with a family history of breast cancer than for women without. MGV, body mass index (BMI), and rate of family history of breast cancer were significantly higher among breast cancer patients than among healthy women, and number of deliveries was significantly lower among breast cancer patients. In postmenopausal women, age at menarche was significantly younger for breast cancer patients. MGV correlated well with breast cancer risk factors. The highest odds ratio was 4.9 for premenopausal women with the largest MGV. Regardless of menopausal status, the greater the MGV, the higher the odds ratio. Our results constitute the first reliable data on the relationship between MGV and breast cancer obtained through exact volume analysis.

AbstractAimsCannabis use is associated with a number of psychiatric disorders, however the causal nature of these associations has been difficult to establish. Mendelian randomization (MR) offers a way to infer causality between exposures with known genetic predictors (genome-wide significant single nucleotide polymorphisms (SNPs)) and outcomes of interest. MR has previously been applied to investigate the relationship between lifetime cannabis use (having ever used cannabis) and schizophrenia, depression, and attention-deficit / hyperactivity disorder (ADHD), but not bipolar disorder, representing a gap in the literature.MethodsWe conducted a two-sample bidirectional MR study on the relationship between bipolar disorder and lifetime cannabis use. Genetic instruments (SNPs) were obtained from the summary statistics of recent large genome-wide association studies (GWAS). We conducted a two-sample bidirectional MR study on the relationship between bipolar disorder and lifetime cannabis use, using inverse-variance weighted regression, weighted median regression and Egger regression.ResultsGenetic liability to bipolar disorder was significantly associated with an increased risk of lifetime cannabis use: scaled log-odds ratio (standard deviation) = 0.0174 (0.039); P-value = 0.00001. Genetic liability to lifetime cannabis use showed no association with the risk of bipolar disorder: scaled log-odds ratio (standard deviation) = 0.168 (0.180); P-value = 0.351. The sensitivity analyses showed no evidence for pleiotropic effects.ConclusionsThe present study finds evidence for a causal effect of liability to bipolar disorder on the risk of using cannabis at least once. No evidence was found for a causal effect of liability to cannabis use on the risk of bipolar disorder. These findings add important new knowledge to the understanding of the complex relationship between cannabis use and psychiatric disorders.

Frequent collapse accidents in tunnels are associated with many construction risk factors, and the interrelationship among these risk factors is complex and ambiguous. This study’s aim is to clarify the relationship among risk factors to reduce the tunnel collapse risk. A multicriteria decision-making method is proposed by combining interpretive structural modeling (ISM) and fuzzy Bayesian network (FBN). ISM is used to determine the hierarchical relationships among risk factors. FBN quantitatively analyzes the strength of the interaction among risk factors and conducts risk analysis. The ISM-FBN method contains three steps: (1) drawing the ISM-directed graph; (2) obtaining the probability of the FBN nodes; and (3) using GeNle to implement risk analysis. The proposed method is also used to assess the collapse risk and detect the critical factors in the Canglongxia Tunnel, China. This method’s tunnel collapse risk model can provide managers with clear risk information and better realize project management.

Aim: The aim of this study was to investigate the relationship between the degree of hydronephrosis and the presence of microscopic hematuria in patients that presented to the emergency department (ED) with ureteral stones. Methods: The records of patients who presented to our ED due to urolithiasis between January 2017 and December 2020 were retrospectively analyzed. Patients aged 18 years or older who underwent non-contrast computed tomography (CT abdomen/pelvis) and urinalysis (UA) and were diagnosed with ureteral stones were included in the study. Radiology reports were reviewed for stone size, localization, and degree of hydronephrosis. Patients with and without microscopic hematuria and the degree of hydronephrosis were compared. Results: A total of 476 patients were included, which consisted of 391 with microscopic hematuria and 85 without microscopic hematuria. The median stone size was 4.1 mm in the presence of microscopic hematuria and 5.5 mm in the absence of microscopic hematuria. Logistic regression analysis was performed to determine the factors associated with the development of hydronephrosis. Stone size [odds ratio (OR):2.15, 95% confidence interval (CI):1.12-4.16, p<0.001), presence of pyuria (OR: 2.58, 95%CI: 1.78-3.48, p<0.001), and absence of microscopic hematuria (OR: 1.31, 95%CI 1.04-2.89, p=0.017) were identified as risk factors for moderate and severe hydronephrosis. Conclusion: We consider that non-contrast CT imaging is necessary for the diagnosis and treatment of emergency cases in which microscopic hematuria is not detected in urinalysis since their stone size may be larger and degree of hydronephrosis may be more severe.

With the developing of high integrations in large scale systems, such as aircraft and other industrial systems, there are new challenges in safety analysis due to the complexity of the mission process and the more complicated coupling characteristic of multi-factors. Aiming at the evaluation of coupled factors as well as the risk of the mission, this paper proposes a combined technology based on the Decision Making Trial and Evaluation Laboratory (DEMATEL) model and the Bayesian network (BN). After identifying and classifying the risk factors from the perspectives of humans, machines, the environment, and management, the DEMATEL technique is adopted to assess their direct and/or indirect coupling relationships to determine the importance and causality of each factor; moreover, the relationship matrix in the DEMATEL model is used to generate the BN model, including its parameterization. The inverse reasoning theory is then implemented to derive the probability, and the risk of the coupled factors is evaluated by an assessment model integrating the probability and severity. Furthermore, the key risk factors are identified based on the risk radar diagram and the Pareto rule to support the preventive measurements. Finally, an application of the take-off process of aircraft is provided to demonstrate the proposed method.

Abstract Background Post-operative body weight loss (BWL) of patients who underwent esophagectomy is common. One month after esophagectomy, some patients lose weight remarkably, and others not. If preoperative or operative risk factors influencing body weight loss are identified, it will be possible that nutritional intervention is given to the patient having these risk factors. Methods In our department, the dietician stays in a surgical ward for nutritional management of patients. Ninety-two patients who underwent esophagectomy for esophageal cancer in 2016 were reviewed. Pre-operative and operative factors were correlated with post-operative BWL. The cut-off value of BWL 1 month after esophagectomy was defined with 75 percentile. Results The median body weight change was -4.8% (-18.5% ∼11.7%). The patients were divided into two groups; 24 patients with ≥ 7.3% of BWL (severe BWL group) and 68 with < 7.3% of BWL (mild BWL group). Patients’ characteristics or post-operative morbidities were not statistically different in these 2 groups. The incidence of patients with reconstruction using a colon or jejunum was higher in severe BWL group. Pre-operative body weight, body mass index, and the value of prealbumin was higher in severe BWL group. On postoperative 14 day, the patients in severe BWL group were likely to have lower energy intake per kilogram of pre-operative body weight. On multivariate analysis, high body mass index [odds ratio = 5.90; 95% confidence interval (CI) = 1.03–47.8; P = 0.046], upper location of tumor [odds ratio = 3.38; 95%, CI = 1.04–11.4; P = 0.043] were independently associated with severe BWL at 1 month after surgery. Conclusion High body mass index of ≥ 25, upper location of tumor were unfavorable risk factors for weight loss 1 month after esophagectomy for esophageal cancer.
