AimsThe main aim of this audit was to look at documentation in medication charts in an acute mixed inpatient unit in South Manchester. In addition, we also looked at completion of capacity assessment and consent to treatment forms as appropriate.BackgroundSafe prescription, administration and monitoring of medication is key to effective patient care. Due to the busy nature of inpatient hospital wards, errors do unfortunately occur both with the medications, and with the recording of their administration.We will use a data collection tool to collect data as per standards described in our local GMMH policy. The medication chart will be used as the standard, as this is the current chart that is in use in the Trust.MethodData were collected from 31 medication charts for inpatients admitted in the ward between the 5/12/19 to 18/12/19. We captured data from each page of the medication chart that required a record to be made by any staff, including details of prescribing, administration and pharmacist checks. Data were recorded as either Yes/No or NA (Not Applicable). Data were then summarised and analysed using MS excel.ResultOf the 31 patients, 22 (71%) had a capacity assessment form completed and 16 (52%) had a consent to treatment form completed. From the data analysis, it was clear that there are high rates of completion for the ‘essential’ parts of all prescriptions, including medicine name, dose, route and data. ‘Route’ was only recorded for 40% of prescriptions for depot medicines. Details of the administration of a medicine by a nurse was generally well-completed. For as required medications, all information relating to administration (date, time, dose and given by) were fully completed for 100% of prescriptions. For regular prescriptions however, the administration details were not as well-completed, where date of administration was recorded in 84% of prescriptions and signature in 29% of prescriptions. Unique patient identifiers are well-recorded on Page 1 of the prescription chart, though not maintained throughout the prescription chart. Nature of reaction to an allergy or sensitivity was only recorded in 6 of the 21 patients (29%).ConclusionOverall, there were good completion rates for the mandatory parts of the prescriptions. However improvements could be made for prescriptions as well as administration and pharmacy checks. The capacity assessment and consent to treatment forms could be improved upon too. We plan to put the recommendations and re-audit in 3-6 months’ time.

Parent's attention to children is still lacking, caused by several things including busyness, low educational background, lack of parent's direction of everything that has been achieved by the child, causing the child to become indifferent to learning. The problems that arise are how the influence of parenting parents on student learning discipline at MTs Al Hidayah Ciomas Panjalu.This study aims to determine the extent of the influence of parenting on student learning discipline in MTs Al Hidayah Ciomas Panjalu.The method used in this research is descriptive method with quantitative approach. The study population at MTs Al Hidayah was 162 students with a sample of 35 people. Sampling was done by purposive sampling technique. The data collection tool taken was observation and questionnaire as many as 35 respondents, after the data was collected with complete stages. The next step was data analysis using spermant rank coefficient (rs). Parental parenting values in Al Hidayah MTs were obtained at 39.4 at intervals of 38.2 - 40.3 with a fairly good classification. While the learning discipline value of students in Al Hidayah MTs was obtained at 37.97 at intervals of 35.22 - 38.44 with poor classification. Based on the testing of hypotheses that have been done, the results have a pretty good effect between parenting parents with student learning discipline in MTs Al Hidayah. This is obtained from rs = 0.59 with the Guilford scale at an interval of 0.41 - 0.60 with a fairly good classification. The pattern of Oreangtua Parenting affects the Discipline of Student Learning is 35% so 65% is influenced by other factors. The influence of these two variables, namely Parenting Parents Against Student Learning Discipline is a significant correlation. This is evidenced by t_count = 4.20> t_table = 1.69236 so that Ha is accepted and Ho is rejected. Thus Parenting Parents have a pretty good effect on Student Learning Discipline.

This study aims to see whether there is a relationship between promotion and decision making learners in following the tutoring in BBC Limbanang Kecamatan Suliki Kabupaten Lima Puluh Kota. This type of research is correlational. The population in this study is the citizens learn BBC learning guidance. Sampling technique in this research is stratified random sampling from entire population. Data collection techniques used are questionnaires, while the data collection tool questionnaire. Technique of data analysis by using formula of percentage and product moment. From the results of the study showed that: promotion done by the guidance of learning in BBC less good so that decision to follow guidance study in BBC considered less good also by learners.

 This research is motivated by the number of teenagers  in South Bungus Village shows behavior that is less good, which is suspected because of the way early married parents that has not seen the emotional stability in educational communication and behave in front of teenagers. This study aims to reveal the educational communication early married parents with teenagers, well behavior in early married parents, and the relationship between educational communication with teenagers well behavior in early married parents. This study used correlational research with population of all teenagers of early married parents in South Bungus  Padang City by using Area Random Sampling technique. Data collection techniques used questionnaires, data collection tool questionnaire with percentage data analysis using Product Moment formula to see the relationship of both variables. The results showed that educational communication of early married parents applied to teenagers is categorized low that can be seen from the highest score found in the scores rarely, the behavior of adolescent propriety in early married parents categorized low seen from the highest score is on the score rarely, and there is a significant relationship between educational communication with teenagers well behavior in early married parents in South Bungus Padang City. Suggestions for this research is to parents who marry early to be better communicate at the time of educating children so that they can behave well and politely later.Keywords: Educational Communication, Teenagers well Behavior

The present research aimed to identify and compare playfulness levels of individuals aged 10-14 years engaging in individual and team sports. The sample consisted of 356 participants. We employed a correlational design and used the “Playfulness Scale for 10-14-years-old Children” developed by Hazar as the data collection tool. In data analysis, we utilized descriptive statistics, Independent Samples T-test, One-Way Analysis of Variance (ANOVA), and Tukey HSD and LSD tests for within-group comparisons. We computed the internal consistency coefficient of the scale to be .84. The results suggested that all participants showed high playfulness levels (97.26 ± 16.02) and got the highest scores on the “Social Adaptation” sub-scale (40.50 ± 6.67) while obtaining the lowest scores on the “Game Passion” sub-scale (21.02 ± 7.00). Overall, we determined that the participants significantly differed in playfulness by age and branch, but it was not the case by gender.

The condition of drug abuse in Indonesia is very worrying. According to the records of the National Narcotics Agency (BNN), Indonesian citizens who have been victims of abuse of illicit goods have reached more than 4 million. This paper will review the maslahah murshalah of Islamic law against narcotics, psychotropic substances and addictive substances (NAPZA), their abuse and prevention in the province of West Java. This research is an analytic descriptive type with a qualitative approach. The researcher took the setting in West Java. The research techniques using observation techniques, interviews and documentation as a data collection tool. Data analysis uses data reduction, data presentation and conclusion drawing. The findings of this study are that, the level of compliance of narcotics victims in undergoing medical and social rehabilitation, for children who use narcotics is still not in accordance with the existing legislation, namely where the existing regulations should be rehabilitated for children who abuse drugs, but this provision is not maximal applied, more children were convicted than rehabilitated.

Background: Large prospective institutional data provide the opportunity to conduct level II and III studies using robust methodologies and adequately powered sample-sizes, while circumventing limitations of retrospective databases. We aimed to validate a prospective data collection tool, the Orthopaedic Minimal Data Set Episode of Care (OME), implemented at a tertiary North American health care system for distal radial fracture (DRF) open reduction and internal fixation (ORIF). Methods: The first 100 DRF ORIFs performed after OME inception (February 2015) were selected for this validation study. A blinded review of the operative notes and charts was performed, and extracted data of 75 perioperative DRF ORIF procedure variables were compared with OME collected data for agreement. Outcomes included completion rates and agreement measures in OME versus electronic medical record (EMR)-based control datasets. Data counts were evaluated using raw percentages and McNemar tests. Cohen (κ) and concordance correlation coefficient analyzed categorical and numerical variable agreement, respectively. Results: Overall, OME demonstrated superior completion and agreement parameters versus EMR-based retrospective review. Nine data points (12.0%) demonstrated significantly higher completion rates within the OME dataset ( P < .05, each), and 88% (66/75) of captured variables demonstrated similar completion rates. Up to 80.0% (60/75) of variables either demonstrated an agreement proportion of ≥0.90 or were solely reported in the OME. Of 33 variables eligible for agreement analyses, 36.4% (12/33) demonstrated almost perfect agreement (κ > 0.80), and 63.6% (21/33) exhibited almost perfect or substantial agreement (κ > 0.60). Conclusions: The OME is a valid and accurate prospective data collection tool for DRF ORIF that is reliably able to match or supersede traditional retrospective chart review. Future investigations could use this tool for large-scale analyses investigating peri/intraoperative DRF ORIF variables.

ABSTRACTThis research is based by the problems encountered in TK Negeri Pembina Tasikmalaya City associated with the improvement of locomotor child. Ideally the implementation of improved locomotor child movement skills is done through various acitivities, methods and media that can stimulate the locomotor childs movement skills but in practice in the field with media limitations and strategies. The ability of teachers in choosing a metdhod leads to a lack of learning practices related to locomotor locomotion skills. This study was designed to see the effect of using the game of obstacles against the improvement of locomotor skill of the green group B children in the TK Negeri Pembina Tasikmalaya City after the use of game of engklek obstacle. This research uses classroom action research methods (PTK) model kemmiss and taggart which is dones collaboration with teachers and principals. The object of the study is locomotor skill using the game of engklek obstacle. Data collection tool used is using a form of observation made by researchers and documented, while the data analysis techniques using descriptive qualitative dan quantitative. Based on the action done as much as 3 cycles show the user of game engklek obstacles affect the increase of locomotor childs movement skills. of the study showed that the use of the game engklek obstacles can improve the locomotor skills of children group B Green TK Negeri Pembina Tasikmalaya City. ABSTRAKPenelitian ini dilatarbelakangi oleh permasalahan yang ditemui di TK Negeri Pembina Kota Tasikmalaya berkaitan dengan peningkatan keterampilan gerak lokomotor anak. Idealnya pelaksanaan peningkatan keterampilan gerak lokomotor anak dilakukan melalui berbagai kegiatan, metode, dan media yang dapat merangsang keterampilan gerak lokomotor anak. Namun pada praktiknya di lapangan dengan keterbatasan media dan strategi, kemampuan guru dalam memilih metode menyebabkan kurangnya praktik pembelajaran yang berkaitan dengan keterampilan gerak lokomotor. Penelitian ini dirancang untuk melihat pengaruh penggunaan permainan engklek rintangan terhadap peningkatan keterampilan gerak lokomotor anak kelompok B Hijau di TK Negeri Pembina Kota Tasikmalaya, setelah penggunaan permainan engklek rintangan diberikan. Penelitian ini menggunakan metode Penelitian Tindakan Kelas (PTK) menggunakan model kemmis dan taggart yang dilakukan bekerja sama dengan guru. Objek penelitian adalah keterampilan gerak lokomotor menggunakan permainan engklek rintangan. Alat pengumpulan data yang digunakan yaitu menggunakan berupa lembar observasi yang dibuat oleh peneliti dan dokumentasi, sedangkan teknik analisis data menggunakan deskriptif kualitatif dan kuantitatif. Berdasarkan tindakan yang dilakukan sebanyak 3 siklus dengan, menunjukan penggunaan permainan engklek rintangan, berpengaruh terhadap meningkatnya keterampilan gerak lokomotor anak.). Kendala yang diperoleh selama penelitian adalah mengkondisikan anak, namun peneliti telah berhasil mengatasinya. Hasil penelitian menunjukkan bahwa penggunaan permainan engklek rintangan dapat meningkatkan keterampilan gerak lokomotor anak kelompok B Hijau TK Negeri Pembina Kota Tasikmalaya.

This research is motivated by the high learning result of santri that can be seen from the achievement of studying santri tahfidz Al-Quran at Al Hidayah Mosque of Solok City. This condition is influenced because of the discipline of students who are very good in following the teaching and learning process. The purpose of this study is to look at the discipline of santri from: 1) discipline of time, 2) discipline in obedience and obedience, 3) discipline in responsibility. The population is twenty-one persons and a sample of twenty-one people is assigned based on the sampling technique of the census. The data collecting technique uses questionnaire of data collection tool. Data analysis techniques use percentage calculations. The results showed that the discipline of santri was very good in terms of 1) time discipline, 2) discipline in obedience and compliance, 3) discipline in responsibility. In conclusion santri have a very good discipline. The discipline of the students affects the improvement of the students' learning outcomes and their learning achievement. This research advice for santri in order to improve self-discipline in daily life.Keywords: Discipline, TPA

This research is motivated by the low level of stimulus given by parents to children around the Tabing Indah Housing Market, Pasar Baru, Bayang District, Pesisir Selatan Regency, this is presumably because parents' understanding of children's fine motor development is relatively low. This study aims to: 1) describe parents' understanding of fine motor development; 2) describe the provision of stimulus to children; and 3) looking at the relationship between parents' understanding of fine motor development and providing stimulus to children around the Tabing Indah Housing Market, Bayang District, Pesisir Selatan Regency. This type of research is quantitative research with correlational descriptive technique. The population in this study were parents of children around Tabing Indah Housing, Bayang District, Pesisir Selatan Regency as many as 20 people and a sample of 14 people. The data collection technique is in the form of a questionnaire with a data collection tool in the form of a questionnaire. The data analysis technique used the Spearman rho proportion and correlation formula. The results showed that: (1) parents' understanding of fine motor development was low, (2) giving children a low stimulus, (3) there was a significant relationship between parents' understanding of fine motor development and providing stimulus to children around Tabing. It is recommended that parents increase knowledge about the effect of stimulation on children's fine motor development so that their development is maximized.

This research is motivated by, the scout member in SMA Adabiah Padang has good discipline, allegedly caused by the leadership of scout leader. The purpose of this study is to describe the leadership of scout leader which includes: the leadership of the coach in moving the scout leader's leadership in guiding the scout members, and the leadership of the builder in influencing the scout members. This type of research is descriptive quantitative. The population in the study amounted to 230 people. Sampling technique in this research is random sampling technique. The number of samples in this study is 46 members of Boy Scouts on extracurricular scout activities. Data collection techniques used are questionnaires, while the data collection tool uses a list of statements. Data analysis techniques using the formula percentage. The results showed that the leadership of scout extracurricular activities is very good, it can be seen from: (1) the leadership of the supervisor in the move, (2) the leadership of the supervisor in directing, (3) the leadership of the supervisor in influencing. It is suggested to the principal to increase the leadership of scout leader, to the coach in order to maintain the leadership system, to the scout members in order to apply it in everyday life.
