Objective: To explore the clinical influence of the changes of glycosylated hemoglobin level of patients with diabetes on hypertension and coronary heart disease. Methods: One hundred and ninety-six patients between February 2015 and December 2016 were divided into a control group (96 non-diabetic patients) and an observation group (100 patients with diabetes) with or without diabetes. The biochemical indexes of patients in the two groups were compared. Moreover patients in the observation groups were divided into subgroups according to the presence of hypertension and coronary heart disease, and the level of HbA1c was compared between different subgroups. Results: The levels of total cholesterol (TC), triglyceride (TG) and low-density lipoprotein cholesterol (LDL-C) of patients in the two groups had no significant differences (P>0.05). However, the level of high-density lipoprotein cholesterol (HDL-C) of patients in the observation group was significantly lower than that of the control group (P<0.05). The Systolic Blood Pressure (SBP) and diastolic blood pressure (DBP), fasting plasma glucose (FPG), fasting insulin (FINS) and levels of high-sensitivity C-reactive protein (hs-CRP) and HbA1c of patients in the observation group were apparently higher than those of the control group (P<0.05). The level of HbA1c of patients with hypertension was significantly higher than that without hypertension (P<0.05). The level of HbA1c of patients with coronary heart disease was apparently higher than that without coronary heart disease (P<0.05). The Pearson correlation analysis results demonstrated that the level of HbA1c of patients in the diabetes group was in a positive correlation with SBP, DBP and level of hs-CRP (P<0.05). Conclusion: The level of HbA1c of patients with diabetes was in a positive correlation with blood pressure and level of hs-CRP. The level of HbA1c can effectively predict the occurrence of hypertension and coronary heart disease. Detecting level of glycosylated hemoglobin is of important significance in screening patients with hypertension and coronary heart disease. How to cite this:Wei F. There is a log-linear increase in the risk of coronary heart disease with elevation of levels of blood pressure. Allowing for the phenomenon of regression dilution bias, this corresponds to around a 20 to 25% increase in risk for each 5 to 6 mm Hg elevation in usual diastolic blood pressure. In diabetic subjects, a similar relationship occurs, but of somewhat lesser degree. Recent overviews of therapy suggest that in nondiabetics, reducing blood pressure reverses around 50% of the excess coronary heart disease risk, but this has not yet been conclusively shown in patients with diabetes. The reduction in risk with therapy is a prerequisite to defining the antecedent as a causal influence on outcome, but it is as likely that the incomplete reversibility of excess risk represents other pathways of connection between hypertension and coronary heart disease as a consequent of iatrogenic effects of current treatments. Several alternative mechanisms are outlined, and the suggestion is made that only in the context of randomized controlled studies could the possible benefits on coronary heart disease of agents influencing such mechanisms be assessed.

【Abstract】Objective: Toanalyze the efficacy of combined drug therapy for elderly patients with coronary heart disease and hypertension.METHODS:Sixty-six elderly patients with coronary heart disease and hypertension were enrolled from December 2017 to November 2018. They were randomly divided into two groups, 33 patients in each group. Patients in the experimental group received nifedipine. In combination with enalapril, patients enrolled in the control group received nifedipine monotherapy.RESULTS:Compared with the control group, the total effective rate, Serum Nitric Oxide (Serum NO) after treatment, CRP after treatment, HCY after treatment, and blood pressure after treatment were significantly improved (P<0.05). Serum NO and treatment before treatment in the 2 groups. There was no significant difference in pre-CRP, pre-treatment HCY, pre-treatment blood pressure, and adverse reactions during treatment (P>0.05).Conclusion: Theelderly patients with coronary heart disease and hypertension are treated with nifedipine and enalapril.

Angina pectoris (AP) with coronary heart disease (CHD) is one of the common cardiovascular diseases in clinical practice, which can be classified as “chest paralysis” in Chinese medicine according to its symptoms, and it is described in many ancient documents. Ancient Chinese medicine believes that the main pathogenesis of the disease is poor blood flow leading to paralysis of the heart and veins, so it is often treated by activating blood and removing blood stasis. In this study, 120 patients with AP of CHD of Qi stagnation and blood stasis type were randomly divided into the observation (n = 60) and the control group (n = 60). In the control group, basic care, conventional treatment, and unselected copper acupuncture scraping were used, while in the observation group, copper acupuncture scraping was performed at the right time of the heart meridian (11 : 00–13 : 00) on the basis of the control group, and all patients received the treatment for a total duration of 4 weeks. We collected data on the traditional Chinese medical (TCM) syndrome score, frequency and duration of angina attacks, nitroglycerin dosage, inflammatory factor levels, and hematological indices pretreatment and posttreatment in both groups. Patients’ adverse effects during treatment were recorded, and the clinical efficacy and ECG efficacy in both groups were evaluated after 4 weeks. We used SPSS.20 statistical software to statistically analyze the above data, and the results showed that the clinical efficacy and ECG efficacy of the observation group were significantly higher than the control group posttreatment. After treatment, the TCM symptom score, angina attack frequency, attack duration and nitroglycerin dosage, serum interleukin-8 (IL-8), hypersensitive C-reactive protein (hs-CRP), and tumor necrosis factor-α (TNF-α) levels, whole blood viscosity (WBV), plasma viscosity (PV), fibrinogen (FIB), and hematocrit (Hct) were significantly lower in both groups compared with those posttreatment. And the observation group showed a greater decrease when compared with the control group. The results also showed that the overall incidence of adverse reactions was lower in both groups during the treatment period. The above results indicate that while ensuring high safety, the copper stone based on theory of midnight-noon ebb-flow can more effectively improve the symptoms and inflammatory response of the body and reduce the viscosity of the blood in AP with CHD of Qi stagnation and blood stasis, and it has better therapeutic effects.

In this study, dictionary learning and expectation maximization reconstruction (DLEM) was combined to denoise 64-slice spiral CT images, and results of coronary angiography (CAG) were used as standard to evaluate its clinical value in diagnosing coronary artery diseases. 120 patients with coronary heart disease (CHD) confirmed by CAG examination were retrospectively selected as the research subjects. According to the random number table method, the patients were divided into two groups: the control group was diagnosed by conventional 64-slice spiral CT images, and the observation group was diagnosed by 64-slice spiral CT images based on the DLEM algorithm, with 60 cases in both groups. With CAG examination results as the standard, the diagnostic effects of the two CT examination methods were compared. The results showed that when the number of iterations of maximum likelihood expectation maximization (MLEM) algorithm reached 50, the root mean square error (RMSE) and peak signal to noise ratio (PSNR) values were similar to the results obtained by the DLEM algorithm under a number of iterations of 10 when the RMSE and PSNR values were 18.9121 dB and 74.9911 dB, respectively. In the observation group, 28.33% (17/60) images were of grade 4 or above before processing; after processing, it was 70% (42/60), significantly higher than the proportion of high image quality before processing. The overall diagnostic consistency, sensitivity, specificity, and accuracy (88.33%, 86.67%, 80%, and 85%) of the observation group were better than those in the control group (60.46%, 62.5%, 58.33%, and 61.66%). In conclusion, the DLEM algorithm has good denoising effect on 64-slice spiral CT images, which significantly improves the accuracy in the diagnosis of coronary artery stenosis and has good clinical diagnostic value and is worth promoting.

Objective. To investigate the characteristics of intestinal flora in patients with gastric cancer complicated by coronary heart disease and heart failure and the guiding value of probiotics intervention for clinical treatment. Methods. (1) One hundred and sixty-eight gastric cancer patients with complications of coronary heart disease and heart failure from August 2017 to December 2020 were selected as the observation group. A total of 125 patients with coronary heart disease treated at the same time were selected as control group 1, and 89 healthy subjects were selected as control group 2. Fecal samples were retained to extract the total RNA, and high-throughput sequencing was applied to complete the analysis of microbial diversity and structure differences, so as to obtain the biological species information of the specimens. (2) Patients in the observation group were randomly divided into two equal groups of 84 patients, namely, group A and group B. Group A was treated with conventional methods, and group B was combined with probiotics intervention on the basis of group A; then, the differences in the intestinal mucosal barrier between the two groups were compared. Results. The Chao, ACE, and Simpson index in the observation group were lower than those in control group 1 ( P < 0.05 ), and the Shannon index was higher than that in control group 1 ( P < 0.05 ). The Chao, ACE, and Shannon index in control group 1 were lower than those in control group 2 ( P < 0.05 ), whereas the Simpson index was higher than in control group 2 ( P < 0.05 ). The abundance of Bacteroidetes in the observation group was lower than that in control group 1 and control group 2 ( P < 0.05 ). The abundance of Firmicutes was higher than that of control group 1 and control group 2 ( P < 0.05 ). Four weeks after treatment, the levels of ET, D-lactic acid, and PCT in the group B were (0.10 ± 0.01), (3.99 ± 0.32), and (0.41 ± 0.10), respectively, which were lower than those in group A (0.19 ± 0.03), (4.51 ± 0.46), and (0.81 0.13). Conclusion. Gastric cancer patients with complications of coronary heart disease and heart failure are associated with intestinal flora disorder, which may be involved in the occurrence and development of the disease. Probiotics intervention is helpful to repair the intestinal mucosal barrier in patients, which is worthy of popularization and application.

Background: The incidence of ischemic heart disease/ myocardial infarction is rapidly increasing in India. However, the traditional risk factors alone could not explain this excess of Coronary Heart Disease (CHD). So, we are in need of a tool to assess the severity and prognosis of these acute coronary syndromes. Lipoprotein (a) [Lp(a)] and High Sensitive C-Reactive Protein (hs-CRP) have been recognised as independent risk factors for CHD in many retrospective case control studies. As the data shows inconsistency in the prediction of risk by Lp(a) and hs-CRP, the study is carried out to compare and correlate the levels of Lp(a) and hs-CRP in coronary heart disease patients with controls.Methods: An observational case control study was conducted at Maharaja’s Institute of Medical Sciences, Nellimarla, with 120 participants. 80 admitted with CHD were categorised as type 2 diabetic and non-diabetic. Remaining 40 participants were age matched controls, who have attended the OP for general health check-up. Samples collected from the participants were analysed for Lp(a), hs-CRP and HbA1c.Results: Lp(a) levels were significantly elevated in CHD patients with diabetes (69.2±27.5) and non-significant in CHD patients without diabetes (50.4±24.3) as compared to their controls (36.6±22.5). There was significant correlation and elevation of hs-CRP in CHD patients with diabetes (6.0±2.6) and without diabetes (3.7±2.0) as compared to their controls (0.7±0.4).Conclusions: The present study shows a lack of association of Lp(a) levels in CHD patients with and without diabetes. A strong correlation of the inflammatory marker, hs-CRP was observed between the CHD patients with and without diabetes and even as compared to their controls. It may be concluded that hs-CRP is a better and independent marker than Lp(a) in patients with CHD.

Objective. To investigate the correlation between changes in serum RBP4, hs-CRP, and IL-27 levels and rosuvastatin in the treatment of coronary heart disease (CHD). Methods. One hundred and twenty patients with CHD admitted in our hospital were selected as the research object, including 60 patients with acute coronary syndrome as the ACS group, and 60 patients with stable angina as the SA group. Another 60 patients without CHD who were examined in our hospital at the same time were included in the non-CHD group. The patients with CHD were further divided into the control group (CG) (n = 42, with routine treatment) and the study group (SG) (n = 78, with routine treatment and rosuvastatin) to measure serum RBP4, hs-CRP, and IL-27 levels and analyze the correlation between each index and rosuvastatin in the treatment of CHD. Results. After retrospective analysis, no significant difference was found among the ACS group, the SA group, and the non-CHD group ( P  > 0.05). As for serum RBP4, hs-CRP, and IL-27 levels, ACS group > SA group > non-CHD group, with obvious differences among groups ( P  < 0.05). After Spearman correlation analysis, a positive correlation was observed between Gensini score and serum RBP4, hs-CRP, and IL-27 levels in patients with CHD ( P  < 0.05). After treatment, serum RBP4, hs-CRP, and IL-27 levels were gradually reduced. At 4 weeks after treatment, serum RBP4, hs-CRP, and IL-27 levels of the CG and the SG were decreased conspicuously, and compared with the control, each index of the SG was obviously lower ( P  < 0.05). Conclusion. Serum RBP4, hs-CRP, and IL-27 play an important role in the occurrence and development of CHD, with a positive correlation to the Gensini score, which can indicate the severity of cardiovascular disease to a certain extent. Meanwhile, rosuvastatin can remarkably reduce serum RBP4, hs-CRP, and IL-27 levels, which is of significance for prognosis.

AbstractObjective: To observe the effect of Danhong injection combined with magnesium polarized solution and nitrate on coronary heart disease.Methods: A total of 58 patients who had been diagnosed with coronary heart disease who participated in the voluntary study were selected and divided into observation group and control group. The number of patients in the two groups was 29, and the control group was mainly injected with nitroglycerin. The observation group was mainly treated with Danhong injection combined with magnesium polarized solution and nitrate ester. The main comparisons was the difference of clinical treatment effect, total cholesterol before and after treatment between the two groups of patients under different therapeutic drugs and the differences in endothelin were recorded.Results: The observation group was superior to the control group in terms of clinical treatment effect and total cholesterol and endothelin after treatment.Conclusion: Danhong injection combined with magnesium polarized solution and nitrate ester has a good effect in the treatment of coronary heart disease, which can be promoted and developed.

Abstract Purpose Time of drug administration may significantly influence its effect. The aim of the present study was to investigate the effect of ASA (administrated in the morning or in the evening) on the anti-hypertensive effect and diurnal blood pressure profile in the high-risk group of cardiovascular patients. Methods All patients (n = 114) had been diagnosed with coronary heart disease and arterial hypertension prior to the enrolment and had been treated with 75 mg per day of ASA in the morning. The patients were randomly assigned to one of the two study groups receiving 75 mg of ASA per day in a single antiplatelet therapy for 3 months in the morning (n = 58) or in the evening (n = 56). The control group (n = 61) consisted of patients with arterial hypertension but without coronary heart disease, not receiving ASA. In all the patients, during each visit, clinical blood pressure (BP) and ambulatory blood pressure measurements (ABPM) were performed. Results There was a significant reduction in 24-h BP and blood pressure at night in the ASA group evening group compared with the ASA morning group and the control group. Conclusions The present study demonstrated that compared with the use of ASA in the morning, its administration in the evening may lead to favourable drop in the ABPM and an improvement of the diurnal profile in the high-risk group of cardiovascular patients who are not naïve to ASA.
