Abstract. The industrial competition requires companies to produce high-quality products, but the phenomenon is that companies still produce products that do not meet specifications and are returned by customers. This is the basis for conducting research aimed at analyzing the implementation of quality control, knowing the main defects of the product, and making improvements to reduce or eliminate problems during the production process using the Six Sigma approach with steps: Define, Measure, Analyze, Improve and Control. The research sample is the result of the final product inspection in April - September 2019. The results of the study indicate that the implementation of Quality control has been carried out but not maximally, with the root causes of the main defects being method, material, and machine. CTO in this research are: foreign body contamination, leak packaging, product temperature exceeds the standard, over drip, and thick fat. After improving, there was an increase in sigma level from 3.48 (performance baseline) to 3.83. Control is carried out to maintain consistency of corrective actions by regular briefings to operators, increasing GMP training, 5S, and product knowledge, standardizing SOP changes, adding necessary tools, and controlling preventive maintenance implementation. Abstract This study aims to determine and identify the causes of defects in the production process of PT. Triteguh Manunggal Sejati and know the level of sigma level. This research uses the six sigma method with the DMAIC approach as a quality control tool, which includes the Define, Measure, Analyze, Improve and Control stages. Based on this study the results obtained are the level of sigma level at PT.Triteguh Manunggal Sejati is 4.96, which means that in the stage of sigma level the company has not reached the level of six sigma levels because in the production process at PT.Triteguh Manunggal Sejati still has product defects in the production process not yet achieved zero defect. The causes of product defects are based on cause and effect diagrams namely lid / seal defects are leaky lid, broken lid, and tilted lid. Six Sigma is one of the latest quality improvement initiatives that have gained popularity and recognition in many sectors worldwide. It is expected that Six Sigma approach will also be adopted in the tourism sector, ensuring high service quality. This study aimed at applying a Six Sigma model to improve the service process of a five-star hotel located in the city centre. A total of 357 samples with sufficient data quality were taken from 386 questionnaire forms presented to the hotel customers. The data obtained were analyzed with Six Sigma methods and tools. The five steps, which are the expansion of DMAIC methodology, consisting of define, measure, analyze, improve and control, were followed from among these methods. The current situation of the hotel was defined in the first phase; sigma level about the service quality was calculated in measuring phase; the measurements were interpreted, and proposed solutions for the problems regarding improvement activities were presented in the analyzing phase; and lastly, controls to be carried out to ensure the continuity of these improvements were explained. One of the aims of the study is to examine with the Servqual model, whether there is a difference between the quality of service expected and perceived by customers who purchase services from an accommodation facility. A questionnaire based on the Servqual model, which consists of tangibles, reliability, empathy, responsiveness and assurance dimensions, was designed and used as a data collection tool. According to the results obtained, it was found that the expectations of the customers regarding hotel services in general, could not be met. The sigma score of the hotel was calculated as 2,01. In the study, tangibles and reliability dimensions were found to be the lowest level of satisfaction among hotel customers.

Quality control becomes an important key for companies in suppressing the number of defective produced products. Six Sigma is a quality control method that aims to minimize defective products to the lowest point or achieve operational performance with a sigma value of 6 with only yielding 3.4 defective products of 1 million product. Stages of Six Sigma method starts from the DMAIC (Define, Measure, Analyze, Improve and Control) stages that help the company in improving quality and continuous improvement. Based on the results of research on baby clothes products, data in March 2018 the percentage of defective products produced reached 1.4% exceeding 1% tolerance limit, with a Sigma value of 4.14 meaning a possible defect product of 4033.39 opportunities per million products. In the pareto diagram there were 5 types of CTQ (Critical to Quality) such as oblique obras, blobor screen printing, there is a fabric / head cloth code on the final product, hollow fabric / thin fabric fiber, and dirty cloth. The factors caused quality problems such as Manpower, Materials, Environtment, and Machine. Suggestion for consideration of company improvement was continuous improvement on every existing quality problem like in Manpower factor namely improving comprehension, awareness of employees in producing quality product and improve employee's accuracy, Strength Quality Control and give break time. Materials by making the method of cutting the fabric head, the Machine by scheduling machine maintenance and the provision of needle containers at each employees desk sewing and better environtment by installing exhaust fan and renovating the production room.

As technology advances at this time, players in business are aware of the importance of product quality in the increasingly fierce competition in the industrial world due to the emergence of many similar companies. Therefore, companies must be able to compete to meet customer desires and try to retain customers. To maintain customers and their marketing areas, companies must have high competitiveness in order to survive by prioritizing quality improvement, increasing efficiency and increasing productivity to improve quality because by increasing quality, products can be accepted among consumers so that company goals can be fulfilled. Therefore, the company must carry out effective quality control which will result in high productivity, lower overall cost of making goods and the factors that cause production failure to be minimized. To improve quality, use the six sigma method, DMAIC and seven tools so that it can be known the cause of the damage and what actions are taken so that there needs to be a controversy to stabilize the processes of the production process so that we can know what percentage of damage and what factors cause damage, therefore there must be measurements and recommendations for improvement and control to reduce the causes From the analysis, it was found that the dent cup was 20.36%, the lid was 21.36% less dense, the lid was damaged in the finished product 18.72%, the cup was 19.28% less thick, the packaging was flexible 20.55%Seiring kemajuan teknologi pada saat ini pelaku di bisnis menyadari akan pentingnya kualitas produk dalam persaingan dunia industri yang semakin ketat karena banyak bermunculan perusahaan-perusahaan sejenis. Oleh sebab itu perusahaan harus dapat bersaing untuk memenuhi keinginan  pelanggan dan berusaha dapat mempertahankan pelanggan. Untuk mempertahankan pelangan dan wilayah pemasaranya perusahaan-perusahaan harus mempunyai daya saing yang tinggi untuk dapat bertahan dengan mengutamakan peningkatan mutu, peningkatan efisiensi dan peningkatan produktivitas untuk meningkatkan kualitas karena dengan peningkatan kualitas, produk dapat diterima di kalangan konsumen sehingga tujuan perusahaan dapat terpenuhi. Maka dari itu perusahaan harus melakukan pengendalian kualitas yang efektif akan menghasilkan produktivitas yang tinggi, biaya pembuatan barang keseluruhan yang lebih  rendah serta  faktor-faktor yang menyebabkan kegagalan produksi akan dapat ditekan sekecil mungkin. Untuk meningkatkan kualitas mengunakan metode six sigma, DMAIC dan seven tools agar dapat diketahui penyebab  kerusakan  dan  tindakan  apa  saja  yang dilakukan sehingga perlu ada kontror untuk menstabilkan  peoses proses produksi sehinga dapat di ketahui berapa persen  kerusakan dan faktor-faktor apa saja yang menyebabkan  kerusakan maka dari itu harus ada pengukuran dan  rekomendasi perbaikan serta melakukan kontrol untuk mengurangi penyebab kerusakan. Dari hasil analisis  di ketahui cup  penyok 20,36%, lid kurang  rapat  21,36%, lid  rusak  pada produk jadi 18,72%,cup kurang tebal 19,28 %kemasan lentur 20,55%

Abstrak. Dunia industri yang semakin berkembang akan mengakibatkan banyaknya persaingan. Perus-ahaan yang dapat bersaing adalah perusahaan yang dapat menjaga  kualitas produknya dengan baik sehingga dapat memenuhi kepuasan konsumen. Maka dari itu diperlukan pengendalian kualitas. Penelitian ini difokuskan pada penurunan cacat pada produk batu bata merah di CV. Ghatan Fatahillah dengan metode Six Sigma-DMAIC (Define, Measure, Analyze, Improve, dan Control). Dari hasil penelitian didapatkan DPMO sebesar 36.212 dengan nilai sigma 3,29. Jenis cacat yang paling sering terjadi pada batu bata merah yaitu pecah/patah yaitu sebesar 4.327 atau 59,81% dari total keseluruhan produk yang cacat.  Hasil dari tahap analyze dengan fishbone diagram, ditetapkan penyebab dari pecah/cacat, yaitu: pekerja kurang paham standar kualitas, tidak adanya pemeriksaan produk, pekerja kurang paham prosedur kerja, kinerja mesin tidak stabil, pekerja kurang teliti,  jumlah penggilingan tidak menentu, dan komposisi batu bata tidak menentu. Maka perlu dilakukan usulan perbaikan agar dapat mengurangi jumlah produk yang cacat pada batu bata merah. Kata kunci: DMAIC, DPMO, Pengendalian Kualitas, Six Sigma Abstract. The growing industrial world will result in a lot of competition. Companies that can compete are companies that can maintain the quality of their products well so that they can meet customer satisfaction. Therefore, quality control is needed. This research is focused on reducing defects in red brick products at CV. Ghatan Fatahillah with the Six Sigma-DMAIC method (Define, Measure, Analyze, Improve, and Control). From the research results obtained DPMO of 36,212 with a sigma value of 3.29. The type of defect that most often occurs in red bricks is broken / broken, which is 4,327 or 59.81% of the total defective products. The results of the Analyze stage with the fishbone diagram showed that the causes of breakage / defects were determined, namely: workers do not understand quality standards, lack of product inspection, workers do not understand work procedures, unstable machine performance, workers are not careful, the number of mills is erratic, and composition erratic bricks. Indonesia has several creative economy sectors that rapidly developed and potentially can be improved to 7% annually. One of them is textile industry specifically bag industry. This industry’s development could enhance the production of commodities and emerge other SMEs to initiate the similar field. Therefore, to raise its profit, customer’s satisfaction must be elevated by minimizing defected products. In order to support it, an improvement on production process is a must. This research employs Six Sigma methods integration with Fuzzy Analytical Hierarchy Process (Fuzzy AHP)-Failure Mode and Effect Anlysis (FMEA). Six Sigma is designated to analyze the level of products’ defect through Define, Measure, Analyze, Improve, and Control (DMAIC) stages and assess the factors that cause the defects by using Fuzzy AHP decision support system and FMEA in risk analysis on defect. The result of the research shows the shortage of defects that demonstrated by DPMO indicator and Sigma level. It is identified that the number of DPMO was being reduced from 20003.75 to 11185.73. While Six Sigma level demonstrated the enhancement from 3.61 to 3.86. Those suggest that the improvement solution provides has significant effect on the defects decreasing at the bag convection industry.

This study focused on quality improvement on ceramic tiles production process at PT Arwana Citramulia. This study used data defects for 12 months (May 2016 – April 2017) and only focus on one type of defect and it is chop corner. Six sigma with DMAIC (define, measure, analyze, improve, and control) approach was used to improve the process. Each step of DMAIC was conducted to carefully analyze and keep the process precisely. The ceramic tiles production process contains a number of 4375 products defects in million opportunities (DPMO), with sigma level of 4.13. In the improve step of DMAIC, FMEA form was used to propose some recommendations in order to improve the process, some of that that are provision of lubricant periodically by the operator, polishing on the surface of the liner to clean and clear, examination and maintenance periodically. The main factor to achieve business success in the era of globalization is quality. Quality control is key in maintaining customer loyalty. In the company's production process is still found the resulting product does not comply with the specified specifications. This study aims to determine the value of sigma and factors causing damage to the production process of bagging section For companies with quality control is expected to achieve corporate goals. In this study, the methods used is Six Sigma with define, measure, analyze, improve. Six Sigma result is measurement of company performance baseline at measurement stage that is company at condition 5,1 sigma with DPMO equal to 162,4532. The factors causing the disability of sugar packing are lack of research and skill of the operator, instability of conveyor speeds and jet engine position, machine hygiene condition, lack of weighing machine, and ineffective treatment, and control method.

The Six Sigma approach has received a lot of attention in various industrial sectors from the manufacturing industry to the service industry. More specific knowledge about Six Sigma has grown rapidly. Much of the training and research on Six Sigma is carried out in various industries and university. This literature review related to Six Sigma purpose to provide an overview of Six Sigma implementation in the manufacturing industries.  The Six Sigma approach that has been introduced and implemented for a long time is DMAIC (Define, Measure, Analyze, Improve, and Control). This paper discusses the published literature related to Six Sigma ranging from 2015 to 2020. This paper involves the study review of 50 papers related to the implementation of Six Sigma of known database search including Elsevier, Science Direct, Emerald Insight and Google Scholars. This literature review contains results from a variety of different perspectives. The perspective includes the focus of the industry, the focus of the number of distribution by country, the focus of the year of publication and the focus of the number of publishers. In fact, this is useful for all types of manufacturing industries to find solutions to problems. The paper also provides advantages for researchers next to add to the literature.
