The obesity pandemia linked metabolic syndrome (MS) connected with it acquires not only medical, but also increasing social importance. This state has unfavorable action on human organism and is the serious diseases risk factor. Special importance MS available in the women of childbearing age. There are described some literature data about the special features of MS pathogenesis in women and the possibilities of its correction with sibutramine and by the combined medication Reduxin® (sibutramine + cellulose microcrystalline). Our own data of the experience of treatment by the Reduxin® 53 women in reproductive age with MS are cited, opsomenorrhea and with a formation of polycystic ovarian syndrome. Patients took the medication during 12 weeks. Mean body weight loss was 13% and 27 patients had weight loss more than 5% at the end the period of observation. Circle of waist was also the decreased on the average on 6±2 cm. 43 women had a normalization of rhythm, duration and intensity of menstrual hemorrhages. Remaining patients it was required the designation of gestagen in the cyclic regime. An improvement of the blood lipid profile was detect: the average value of atherogenic index triglycerides/ high density lipoproteins cholesterol was improved to 35,8%. The maximum decrease of the cardiovascular disease risk factors was marked in the patients, who had weight loss more than 10%.

Overweight and obesity are growing problems both in Canada and around the world. Obesity is associated with a number of chronic diseases including type 2 diabetes and CVD, which puts a tremendous burden on the health care systems in place. The present study sought to investigate whether there were differences in the effectiveness of three low-fat, hypo- and isoenergetic diets differing in protein:carbohydrate ratio, low protein (LP, 1 g protein:4 g carbohydrate), normal protein (NP, 1 g protein:2 g carbohydrate) or high protein (HP, 1 g protein:1 g carbohydrate), on weight loss and markers of the metabolic syndrome (MetS) in overweight women. Subjects were randomly assigned to receive one of three intervention diets, all of which included a 60 min exercise programme three times/week for 12 weeks. Of the total subjects, fifty-four overweight and obese local women with MetS risk factors completed the study. All groups had similar improvements in body weight, insulin sensitivity, lipid profile, blood pressure and fitness. Subjects reported that the NP diet was easier to comply with and achieved better improvements in body fat, waist circumference and waist:hip ratio, and preservation of lean mass compared with the other two diets. In conclusion, energy restriction and exercise both facilitate weight loss in overweight and obese subjects and reduce symptoms of the MetS. A diet with a 1:2 protein:carbohydrate ratio promoted better improvements than either the LP or HP diets, and may be superior in reducing long-term chronic disease risk in this population.

Abstract Objectives The objective of this study is to investigate the cardiovascular risk factors associated with metabolic syndrome (MetS), which is increasingly becoming prevalent in childhood obesity. Methods A total of 113 patients, 76 of whom were between the ages of 10 and 17 (mean age: 14.5 ± 1.8 years) and diagnosed with obesity (30 non-MetS and 46 MetS using IDF) and 37 of whom constituted the control group, participated in the study. Echocardiographic examination and atherogenicity parameters (Atherogenic index of plasma [AIP: logTG/HDL], total cholesterol/HDL, and TG/HDL ratio and non-HDL) were evaluated. Results The most common component accompanying obese MetS was found to be hypertension and low HDL. While obesity duration, body mass index (BMI), blood pressure, fasting insulin, insulin resistance, atherogenicity parameters were determined to be significantly higher in the obese-MetS group. Echocardiography showed that while the thickness, volume, and diameter of LV end-diastolic wall, left ventricular mass (LVM), LVM index (LVMI g/m2) and relative wall thickness (RWT) were significantly high in the MetS group, however, mitral E/A ratio was significantly lower (p<0.05). Change in LV geometry consistent with concentric remodeling (increased RWT, normal LVMI) was visible in obese groups. LVM were positively significantly related to BMI, waist circumference, insulin resistance, blood pressure, LDL level, and negative to mitral E/A ratio. In the obese-MetS group, LVMI was positively correlated to office systolic BP, left atrium end-diastolic volume/index. Conclusions LVMI and atherogenicity parameters that were found to be significantly higher in obese MetS exhibit increased cardiovascular risk in childhood.

Introduction: Few studies have examined behavioral weight loss interventions with respect to change in cardiovascular disease risk factors in young adults (aged 18 to 35 years). Hypothesis: We tested the hypothesis that a 6 month behavioral weight loss intervention resulted in significant improvements in selective cardiovascular disease risk factors in young adults. Methods: Data are presented as median [25 th , 75 th percentiles]. 470 participants (age: 30.9 [27.8, 33.7] years); BMI: 31.2 [28.4, 34.3] kg/m 2 ) were enrolled in a 6 month behavioral weight loss intervention that included weekly group sessions and prescribed an energy restricted diet and moderate-to-vigorous physical activity. Assessments included weight using a standardized protocol, resting blood pressure, and fasting lipids, glucose, and insulin. Statistical significance of change was according to tests of symmetry or the Wilcoxon matched pairs signed ranks test. Results: The primary outcome (weight) was available for 424 of the 470 participants (90.2%). Weight significantly decreased (-7.8 kg [-12.2, -3.7]) (p<0.0001). Systolic (-4.0 mmHg [-8.5, 0.5] and diastolic blood pressure (-3.0 mmHg [-6.5, 1.0]) decreased (p<0.0001). Total cholesterol (-13 mg/dl [-28.0, 2.0]), LDL cholesterol (-9.5 mg/dl [-21.7, 2.0]), triglycerides (-8.5 mg/dl [-44.0, 9.0]), glucose (-4.0 mg/dl [-8.0, 1.0]), and insulin (-2.6 mIU/L [-5.9, 0.7]) decreased (p<0.0001, n=416). There was not a significant change in HDL cholesterol (p=0.72). Conclusions: In conclusion, after 6 months, weight loss was observed in young adults assigned to this behavioral intervention that focused on physical activity and diet modification. They tended to also have improved cardiovascular disease risk factors. This may demonstrate an approach to reducing cardiovascular disease risk in young adults. Abstract Background: Swimming and other aquatic fitness are important aerobic exercises that have been proposed as an effective nonpharmacological approach in the management of type 2 diabetes (T2DM), hyperlipidemia, and hypertension (HTN). The current study aimed to assess the effect of long-term swimming sessions on glycemic and lipidemic parameters, body composition, and hemodynamic responses for patients with metabolic risk factors. Methods: Forty participants from both genders with T2DM and HTN (aged 52.4±5.5 yrs) agreed to take part in this quasi-experimental study and were divided into two groups. The first group included the participants who performed long-term swimming sessions and the second group served as the reference. The first group exercised for 2 h, 3 times/week in 29-33 ◦C swimming pool for 16 weeks. While the reference group did not participate in any kind of exercise and advised to keep on with their normal lifestyle. All the obtained metabolic syndrome risk factors data were analyzed using a paired t-test which was applied to separately determine the differences between pre- and post-tests for both genders and groups, and the percentage of change (Δ %) was computed. Independent t-test was applied to determine the differences in the post-tests (Exp. vs Ref) in men patients as well as for women separately. Results: The results showed that there were statistically significant differences at p ≤ 0.05 between pre- and post- exercise concerning Total Cholesterol (TC), High-Density Lipoproteins (HDL), Low-Density Lipoproteins (LDL), Triglycerides (TG), glycemic parameters, systolic and diastolic blood pressures, body mass index (BMI) and fat mass percent in favor of posttests in the experimental group for both genders. Whereas, no significant differences were found at p≤0.05 between pre- and post-tests for all studied variables in the reference group for both genders. Significant differences were found at p≤0.05 on the post-tests in favor of the experimental for both genders.Conclusion: Findings of the current study suggested that the regular 16 weeks of the conducted swimming sessions could be considered as nonpharmacological approaches in the management of T2DM and HTN.

Objectives: To evaluate the effect of extreme weight loss programs on circulating metabolites and their relationship with cardiometabolic health in children with metabolic syndrome.Methods: This study was a quasi-experimental design with a pretest and post-test. Thirty children with metabolic syndrome and aged 10–17years were recruited to an extreme weight loss program (i.e., exercise combined with diet control). The primary outcomes included plasma metabolites, body composition, and cardiometabolic risk factors. A total of 324 metabolites were quantitatively detected by an ultra-performance liquid chromatography coupled to tandem mass spectrometry system, and the variable importance in the projection (VIP) value of each metabolite was calculated by the orthogonal projection to latent structures discriminant analysis. The fold change (FC) and p value of each metabolite were used to screen differential metabolites with the following values: VIP&gt;1, p value&lt;0.05, and |log2FC|&gt;0.25. Pathway enrichment and correlation analyses between metabolites and cardiometabolic risk factors were also performed.Result: A large effect size was observed, presenting a weight loss of −8.9kg (Cohen’s d=1.00, p&lt;0.001), body mass index reduction of −3.3kg/m2 (Cohen’s d=1.47, p&lt;0.001), and body fat percent reduction of −4.1 (%) (Cohen’s d=1.22, p&lt;0.001) after the intervention. Similar improvements were found in total cholesterol (Cohen’s d=2.65, p&lt;0.001), triglycerides (Cohen’s d=2.59, p&lt;0.001), low-density lipoprotein cholesterol (Cohen’s d=2.81, p&lt;0.001), glucose metabolism, and blood pressure. A total of 59 metabolites were changed after the intervention (e.g., aminoacyl-tRNA biosynthesis, glycine, serine, and threonine metabolism; nitrogen metabolism, tricarboxylic acid cycle, and phenylalanine, tyrosine, and tryptophan biosynthesis). The changes in metabolites (e.g., amino acids, fatty acids, organic acids, and carnitine) were related to lipid metabolism improvement (p&lt;0.05). Organic acids and carnitines were associated with changes in the body composition (p&lt;0.05).Conclusion: Exercise combined with dietary control improved the body composition and cardiometabolic health in children with metabolic syndrome, and these changes may be related to plasma metabolites.

Non-communicable diseases (NSDs) are responsible for two-thirds of all deaths globally, whereas cardiovascular disease (CVD) alone counts for nearly half of them. To reduce the impact of CVD, targeting modifiable risk factors comprised in metabolic syndrome (e.g., waist circumference, lipid profile, blood pressure, and blood glucose) is of great importance. Beneficial effects of fish consumption on CVD has been revealed over the past decades, and some studies suggest that fish consumption may have a protective role in preventing metabolic syndrome. Fish contains a variety of nutrients that may contribute to health benefits. This review examines current recommendations for fish intake as a source of various nutrients (proteins, n-3 fatty acids, vitamin D, iodine, selenium, and taurine), and their effects on metabolic syndrome and the CVD risk factors. Fatty fish is recommended due to its high levels of n-3 fatty acids, however lean fish also contains nutrients that may be beneficial in the prevention of CVD.
