About the effectiveness of cell technologies in extensive soft tissue defects plasty

Abstract. Fetal fibroblast culture transplantation results were evaluated in the treatment of 18 burn victims. Comparison group consisted of 18 burn patients received medical care without cellular technologies utilization. The main comparison parameters in the study groups: the timing of the first stage of autodermoplasty; the number of autodermoplasties during the treatment; hospitalization duration; the newly formed epidermis area estimation. Fetal fibroblast culture transplantation in burn patients with extensive skin defects was performed on average 14,883,56 days after the injury. The timing of the first stage of autodermoplasty did not differ in the main and control groups, not exceeding an average of 19,122,01 days (p=0,48). An average of 2,710,67 surgeries using cell technologies performed in patients of the study group. The use of fetal fibroblasts culture in patients with extensive skin defects reduces the need for autodermoplasty by 1,6 times due to the granulation tissue formation and the epidermal growth beginning 7 days after and complete epidermal formation 14 days after transplantation. Regenerative medicine technologies utilization in patients with extensive skin lesions is possible and appropriate. Due to the fetal fibroblasts culture transplantation a kind of temporary biological coating is formed in the wound. It accelerates the wound healing process phase change from exudation to proliferation and the preparation of skin defects for autodermoplasty, expanding the possibilities of effective patients treatment.

The rate of failed skin graft in Dr. Soetomo General Academic Hospital Surabaya was around 26% of the cases, and it became a concern because skin graft was the main procedure to close large burn wounds. Many problems might affect this event; one of them was electrolyte imbalance. Hypernatremia was found in major burn patients. This condition disrupted the wound healing process of skin graft. This cross-sectional study evaluated patients with burns admitted to the Burn Centre of Dr. Soetomo Hospital between January 2014 and December 2018. 143 subjects participated in this study. Hypernatremia was found in 16% of all subjects (23 subjects), 19% with hypernatremia (28 subjects), and the majority of it, 65% with normonatremia (92 subjects). This study found that the risk of failed skin graft was higher on hypernatremia than normonatremia subjects. This risk was higher if the skin graft procedure took more than 10%.

BACKGROUND: Hyaluronic acid (HA) was explored as a powerful positive biocompatible material that participates in numerous biological processes related to morphogenesis and tissue healing. AIM: In this context, we elevated in this pilot study the effect of HA on soft tissue healing and bone repair of dental sockets (DS) as well as the postoperative pain. MATERIALS AND METHODS: Ten individuals of both genders (80% females and 20% males) with age range 18–44 years, were included who were diagnosed with two non-restorable teeth (total sockets n = 20). A split-mouth study design was performed where the DS of each patient were divided into Group A (study group): Topical oral HA gel (Gengigel®) was applied into sockets while Group B (control group): Sockets were left untreated. Study variables were evaluated including socket length, socket healing scores (healing index), and post-operative pain in both groups at day 1, 5, and 10 for each patient. RESULTS: There were no statistically significant differences between the control and study groups regarding the reduction of the socket length and postoperative complications but results revealed that HA enhances and fasten the healing capacity. CONCLUSION: HA can be used as adjunctive treatment to improve the wound healing process.

Backgroud: Burns are one of the most dangerous injuries and the main cause of disability to death. Burns can be interpreted as one of the serious problems in society and are recorded to cause around 265,000 deaths each year. Honey can maintain the wound condition to keep it moist and in high viscosity to prevent infection. Method: This study was conducted to determine the effect of honey on the wound healing process in burn patients. This study uses a literature review method. The population in this study is a journal with a background in the health sector with 10 international journals on burns with the qualifications of 9 SCIMAGO indexed journals and 1 SINTA indexed journal published from 2015 to 2020. Results: Burns that treated in the form of pure honey can generate good results and heal in an average of 20 days. The healing process will be better, if the wound given a mixture of other ingredients which also have a low acidity such as garlic, chitosan and beeswax and olive oil. Honey has antimicrobial properties and a low pH level. The microbe most sensitive to honey is Staphylococcus aureus. Conclusion: The conclusion from this study, honey can be used as an alternative treatment for burns.

Background : From collective data from 2014 – 2018 from Burn Centre in Dr. Soetomo Hospital, the rate of failed skin graft was 26%. This high number became a concerned because skin graft was the main procedure to close large burn wound. This event is affected by many problems, one of them is electrolyte imbalance. Hypernatremia was seen in major burn patients that causes the disruption of wound healing process of skin graft. Methods: A cross sectional study of patients with burns admitted to the Burn Centre of Dr. Soetomo Hospital between January 2014 and December 2018 were evaluated. Results : 143 subjects were involved in this study. 44 subjects are female (31%) and 99 subjects are male (69%). 66% of the burn injuries are caused by fire (93 subjects), 14% caused by electricity (20 subjects), 8% caused by scald (12 subjects), 6% caused by boiled oil (8 subjects), 4% caused by chemical agent (6 subjects) and 2% caused by blast injury (1 subject). 77% of all subjects (110 subjects) underwent split thickness skin graft less than 10% while 23% (33 subjects) underwent the procedure 10% or more than it. Hypernatremia is found in 16% of all subjects (23 subjects), 19% with hyponatremia (28 subjects) and majority of it, 65%, with normonatremia (92 subjects). From the study, it was found that the risk of failed skin graft was higher on hypernatremia subjects than normonatremia subjects, the relative risk was 6,06 to fail. This number was higher if the skin graft procedure took more than 10%. But, it was found the risk was higher on hyponatremia subjects than the rest of subjects, with the relative risk was 7,75 to fail. Conclusions: Hypernatremia caused high risk of failed skin graft on major burn patients

Abstract Defective cell migration causes delayed wound healing (WH) and chronic skin lesions. Autologous micrograft (AMG) therapies have recently emerged as a new effective and affordable treatment able to improve wound healing capacity. However, the precise molecular mechanism through which AMG exhibits its beneficial effects remains unrevealed. Herein we show that AMG improves skin re-epithelialization by accelerating the migration of fibroblasts and keratinocytes. More specifically, AMG-treated wounds showed improvement of indispensable events associated with successful wound healing such as granulation tissue formation, organized collagen content, and newly formed blood vessels. We demonstrate that AMG is enriched with a pool of WH-associated growth factors that may provide the starting signal for a faster endogenous wound healing response. This work links the increased cell migration rate to the activation of the extracellular signal-regulated kinase (ERK) signaling pathway, which is followed by an increase in matrix metalloproteinase expression and their extracellular enzymatic activity. Overall we reveal the AMG-mediated wound healing transcriptional signature and shed light on the AMG molecular mechanism supporting its potential to trigger a highly improved wound healing process. In this way, we present a framework for future improvements in AMG therapy for skin tissue regeneration applications.

Moreover, AMG-treated wounds showed increased granulation tissue formation and organized collagen content. Overall, we shed light on AMG molecular mechanism supporting its potential to trigger a highly improved wound healing process.

Background: Sucralfate is a basic aluminum salt of sucrose octasulphate which was orally taken for prevention and treatment of several gastrointestinal diseases. This study primarily aims to analyze whether sucralfate accelerates wound healing process in burn patients. The incidence of infection & relieve in pain in burn patients was also compared.Methods: This is an observational study carried out in the Department of General Surgery, Hamidia Hospital Bhopal on 50 patients divided into group 1 (sucralfate)and group 2 [a-sucralfate; b-silver sulfadiazine (SSD)]. Demographics, history, physical, and systemic examinations of the patients were recorded.Results: It was observed that sucralfate augments the formation of granulation tissue (in 6-17 days) as compared to SSD (14-22 days). It was noticed that faster healing by re-epithelialization was present in sucralfate group (11-22 days) as compared with SSD group (15-30 days). By the end of 3rd week 50-75% of wound was healed in sucralfate group as compared with 35-50% in SSD group. Incidence of secondary infection was less when topical sucralfate was used (group 1 = 25%; group 2a = 16.6%; group 2b = 66.66%). There was a marked relief in pain and discomfort after sucralfate application as compared to SSD.Conclusions: Using topical sucralfate expedite the burn wound healing process, significantly decreases pain with no local or systemic adverse reactions to the topical application therefore it can be used as an adjunctive or alternative agent in the future. However, multicentric trials with larger sample size are needed to insure the concept.

In this work, a one-shot process for the simultaneous foaming of polycaprolactone (PCL) and impregnation of mesoglycan (MSG) into the porous structure was successfully attempted. Supercritical carbon dioxide plays the role of the foaming agent with respect to PCL and of the solvent with respect to MSG. The main objective is to produce an innovative topical device for application on skin lesions, promoting prolonged pro-resolving effects. The obtained device offers a protective barrier to ensure a favorable and sterilized environment for the wound healing process. The impregnation kinetics revealed that a pressure of 17 MPa, a temperature of 35 °C, and a time of impregnation of 24 h assured a proper foaming of PCL in addition to the impregnation of the maximum amount of MSG; i.e., 0.22 mgMSG/mgPCL. After a preliminary study conducted on PCL granules used as brought, the MSG impregnation was performed at the optimized process conditions also on a PCL film, produced by compression molding, with the final goal of producing medical patches. Comparing the dissolution profiles in phosphate buffered saline solution (PBS) of pure MSG and MSG impregnated on foamed PCL, it was demonstrated that the release of MSG was significantly prolonged up to 70 times. Next, we performed functional assays of in vitro wound healing, cell invasion, and angiogenesis to evaluate the biological effects of the PCL-derived MSG. Interestingly, we found the ability of this composite system to promote the activation of human keratinocytes, fibroblasts, and endothelial cells, as the main actors of tissue regeneration, confirming what we previously showed for the MSG alone.

Dermatitis is a term used to describe inflammation and damage of the layers of the skin. The causative agents are bacterial, fungal, mold, and parasitic infestations. Dermatitis has become problems in various regions in Indonesia and some other countries. Dermatitis often causes an unpleasant and disgusting feeling for the owner. Skin damage occurs to the dermis, thus showing clinical symptoms of hair loss, itching, redness of the skin, skin ulcers, and a bad smell of the animals. This study aims to determine the effect of giving honey from Trigona bees to improve the skin layer structure of patients with dermatitis in dogs and to see the development of skin lesions macroscopically. Research method: 10 dogs diagnosed with complex dermatitis were given fresh honey of 5 ml/day/dog. The treatment is given orally once a day for 35 days. Clinical observation of the wound healing process on the skin was observed every week for 5 weeks, and histopathological observations of the skin were carried out on the 0, 7, and 21 days. The results showed that giving fresh honey was able to repair skin damage with dermatitis lesions. It could be seen from the decrease in symptoms of erythema and the start of hair growth at the site of alopecia lesions. Histologically there was a decrease in the number of inflammatory cells (p<0.05) in the administration of honey. We concluded that Trigona's honey could be used as an herbal ingredient for treating dermatitis in dogs

Hailey–Hailey disease (HHD) is a rare, chronic and recurrent blistering disorder, characterized by erosions occurring primarily in intertriginous regions and histologically by suprabasal acantholysis. Mutation of the Golgi Ca2+-ATPase ATP2C1 has been identified as having a causative role in Hailey–Hailey disease. HHD-derived keratinocytes have increased oxidative-stress that is associated with impaired proliferation and differentiation. Additionally, HHD is characterized by skin lesions that do not heal and by recurrent skin infections, indicating that HHD keratinocytes might not respond well to challenges such as wounding or infection. Hypochlorous acid has been demonstrated in vitro and in vivo to possess properties that rescue both oxidative stress and altered wound repair process. Thus, we investigated the potential effects of a stabilized form of hypochlorous acid (APR-TD012) in an in vitro model of HHD. We found that treatment of ATP2C1-defective keratinocytes with APR-TD012 contributed to upregulation of Nrf2 (nuclear factor (erythroid-derived 2)-like 2). Additionally, APR TD012-treatment restored the defective proliferative capability of siATP2C1-treated keratinocytes. We also found that the APR-TD012 treatment might support wound healing process, due to its ability to modulate the expression of wound healing associated cytokines. These observations suggested that the APR-TD012 might be a potential therapeutic agent for HHD-lesions.
