Psychometric Properties of the Brazilian Version of the Nurses' Knowledge of High-Alert Medications Scale: A Pilot Study

Background and Purpose:High-alert medication is considered to be a medication that presents a high risk of causing significant patient harm when used erroneously and its consequences can be fatal. The Nurses' Knowledge of High-Alert Medication scale (NKHAM) is a tool available to evaluate the knowledge of nurses in practice about this issue.Aim:This pilot study aimed to measure the reliability and known-groups validity of the Brazilian version of the NKHAM.Methods:This pilot psychometric study was carried out at the Faculty of Nursing and University Hospital of the University of Campinas, São Paulo, Brazil. Forty nursing students and 44 registered nurses working in complex clinical or surgical settings completed a sociodemographic questionnaire and the Brazilian version of the NKHAM. The Kuder–Richardson 20 (KR-20) coefficient and Mann–Whitney test were used to establish reliability and known-groups validity. A significance level of ≤ 0.05 was adopted for all the analyses.Results:Analyses demonstrated preliminary acceptable reliability scores of 0.55 and 0.60 in domains A and B of NKHAM, respectively. A significant difference was found between the nursing students' and the registered nurses' knowledge of high-alert medications, demonstrating the scale's ability to discriminate between the two groups.Implications for Practice:Although this is pilot study, results suggest that the Brazilian version of the NKHAM might be a reliable and valid tool to measure nurses' knowledge of high-alert medications.

Aim: To evaluate the tendency, knowledge, awareness and behavior patterns of urology residents training at different institutions in Turkey about the use of fluoroscopy in operations. Methods: The 13-questioned survey prepared using “Google Forms©” as of 01.03.2021 was shared for four weeks in the “WhatsApp®” application group, which includes 279 urology residents studying with university hospitals and training and research hospitals in Turkey. One hundred and thirteen participants, who completed the questionnaire were included in the study. Results: Of the 113 urology residents included in the study, 56 (49.6%) were studying in university hospitals and 57 (50.4%) were in training and research hospitals. 67.3% of the residents stated that they never hesitated to participate in the operations which fluoroscopy was used. Additionally, the residents stated that, also 43.4% of the auxiliary healthcare staff frequently refrain from being involved in these cases(p <0.001). While 21 (37.5%) of the residents trained in the university hospital reported that they hesitated from these cases, this rate was found that 16 (28.2%) of the residents who were trained in the training and research hospitals and a significant difference was observed between two groups (p <0.016). Among residents, the rate of using radioprotective lead apron was 94.7%, and the rate of thyroid shield use was 98.2%. While the rate of using radiation protective glasses was 1.8%, it was learned that none of the residents used radioprotective gloves. Only 5.3% of the residents stated that they are trained in subjects about the harmful radiation effect. Conclusion: Urology residents in Turkey do not receive sufficient training on the harmful effects of fluoroscopy, which they frequently use in their daily practice. In addition, the residents whom training in university hospitals are more scared of the radiation exposure from fluoroscopy than their colleagues working in training and research hospitals.

Purpose: The aim of this study was to investigate the perceptions of intervention and levels of care required for maternal delivery between medical and nursing students. The differences in perceived practices were then considered in relation to health care approaches explored in either the medical or nursing professions. Methods: The number of individuals surveyed in this study included 98 (6th year) medical students and 78 (4th year) nursing students in the Oita University’s Faculty of Medicine. Out of the total 98 medical students recruited to participate in the study, 20 of them did not agree the survey. On the other hand, among the total 78 nursing students recruited, like their peers in medicine, 10 did not agree, whereby, only 68 nursing students were participated. As a result, the final rate of valid responses is 82.9% (78+68/98+78=146/176) as shown in (Table 1). The survey questionnaires were completed anonymously during the three months from August to October in 2014. The topics surveyed included the necessity or not of procedures used in delivery and types of care extended on delivery for mothers or their partners, or awareness for natural childbirth. The ethics committee of the university approved the study. Results: In the survey of medical care and treatment for delivery, the number of medical students who desired lithotomy position and the use of oxytocin under the direction of obstetricians were significantly larger than that of nursing students (Table 2. Question No.9, No. 13, P=0.0001). As for the importance of the quality of the delivery experience in relation to the decision of birth place (P=0.0001, Table 2, Question No 2, Table 7, Question No. 2), and comfortable labor position using a location other than the childbirth table (P=0.0001, Table 2, Question No. 4), the number of nursing students was significantly higher than that of medical students. In accordance with the effective and recommended guidelines in “Care in normal birth: A practical guide (WHO1996)”, the authors highlighted survey questions that included consideration of “Planning of childbirth together with a partner”. This survey approach also included “Information for choice of birth place”, Respect for decisions as to who should be accompanying on childbirth and “Comfortable labor position or favorite birth or delivery places other than the childbirth delivery table.” (1) The ratio of medical students significantly supports that birth plan should be decided with partner was higher when compared with nursing students (Table 7. P=0.017). (2) As for information for choice of birthplace, more nursing students significantly support that birth place should be consented as compared with medical students (Table 7. P=0.0001). (3) More nursing students significantly support the person accompanying on delivery (Table 7. P=0.048). (4) Both nursing students and medical students agree with “Comfortable labor position or favorite delivery or birth places other than the childbirth delivery table” (Table 7. P=0.006) Table 8 has shown no recommendation by WHO such as enema, shaving, preventive vascular access, conventional lithotomy position. No such difference could be seen between both the medical students and nursing students as in Table 4. Consequently, the medical students indicated that higher preference for more medical treatment including promotion of delivery due to oxytocin treatment and lithotomy position was necessary compared to the nursing students’ expression of such desirability of the approach. Conclusion: The study showed a significant difference in the perceived requirements for child birth or delivery between medical students and nursing students. The main differences are suspected to be due to differences in the educational programs and backgrounds which both the medical students or nursing students received while studying and training for their respective professions.

The aim of this study is to investigate the effects of participation in a 1-week summer camp on thehopelessness and self-esteem of the university students attending Sport Sciences Faculty. Participants were 36university students assigned to experiment group using a random procedure. Coopersmith Self-esteem and Beck Hopelessness Scales were completed at the beginning and end of the summer camp by designed the university. The obtained data were analysed in the SPSS 18.0 program and the significance level was taken as 0.05. The descriptive statistics, independent simple t test, paired simple t test and Pearson correlation were used for analyse the data in the study. According to the results of the research, no significant difference was observed in the comparison of the hopelessness and self-esteem levels between pre and post-test. In addition, there was a significant difference in the hopelessness level of male and female students but any significant difference was not observed in terms of self-esteem. There was a significant relationship between hopelessness and self-esteem pre and post-test. These result shows that a 1-week summer camp cannot change the hopelessness or self-esteem level. However, as the self-esteem rises, the rate of despair decreases whereas as the despair rises, the selfesteem decreases.

Objective Health education interventions during pregnancy can influence maternal oral health (OH), maternal OH-behaviors and children’s OH. Interventions that can be delivered at anytime and anywhere, for example mobile-health (mHealth) provides an opportunity to address challenges of health education and support activation of women in underserved and rural communities to modify their health behavior. This pilot study was undertaken as a part of a mHealth initiative to determine knowledge, attitudes, and behaviors related to pregnancy and ECC prevention among women attending obstetrics/gynecology (OB/GYN) practices at a large rurally-based clinic. Methods A cross-sectional survey study was voluntarily engaged by women (n = 191) aged 18 to 59 years attending OB/GYN visits, over a 3-week period from 12/2019 to 1/2020. Survey results were analyzed applying descriptive statistics, X2 and Fisher’s Exact tests. The significance level was set at P < .0001 for all analyses. Results Approximately half of respondents were between 18 and 29 years (53%), had a college degree (55%), and 100% reported cell phone use. Whereas 53% and 31%, respectively, indicated that they were “somewhat” or “very” sure of how to prevent ECC in their children, only 9% recognized evidence of early decay and 30% did not know the purpose of fluoride. Overall, only 27% of participants correctly answered the knowledge-based questions. Further, only 57% reported their provider explained things in a way that was easy to understand. Only 24% reported seeing a dentist during their current pregnancy. Conclusions Study results suggested potential gaps in knowledge and behaviors related to ECC prevention and provided baseline data to inform future interventions to improve ECC prevention practices. Notably, majority of participants used their cell phones for making medical/dental appointments and reported using their phones to look up health-related information. This demographic represents a potentially receptive target for mHealth approaches to improve understanding of oral health maintenance during pregnancy and ECC prevention.

ABSTRACT Purpose Verify the position of lips and tongue at rest in newborns with and without ankyloglossia. Methods Cross-sectional study, carried out with 130 newborns in University Hospital. Data collection was performed by the researcher and speech-language pathologists from the Hospital. Information on gestational age, sex, weight, height and days of life was collected. The position of the lips and tongue at rest was evaluated through visual inspection with the newborns asleep. After the newborns were awakened, Neonatal Screening of the validated Protocol for the evaluation of the lingual frenulum for infants was performed to detect the alteration of the lingual frenulum. The data obtained were described and submitted to statistical analysis using the Chi-Square test to verify the association between the position of the lips with the tongue and to compare the position of the lips and tongue with and without ankyloglossia. The Mann-Whitney test was used to verify the behavior of the variables the differed between newborns with and without ankyloglossia. The significance level of 5% was adopted. Results When comparing the data, a significant difference was found between: weight and height with and without ankyloglossia; position of lips and tongue. An association between the position of the tongue and lips with and without ankyloglossia was also found. Conclusion Newborns without alteration of the lingual frenulum have a tendency to remain with their lips closed and their tongue elevated during rest and newborns with ankyloglossia have a tendency to keep their lips parted and their tongue low during rest.

Background: Hospitals have constituted the limiting resource of the healthcare systems for the management of the COVID-19 pandemic. As the pandemic progressed, knowledge of the disease improved, and healthcare systems were expected to be more adapted to provide a more efficient response. The objective of this research was to compare the flow of COVID-19 patients in emergency rooms and hospital wards, between the pandemic's first and second waves at the University Hospital of Vall d’Hebron (Barcelona, Spain), and to compare the profiles, severity and mortality of COVID-19 patients between the two waves. Methods: A retrospective observational analysis of COVID-19 patients attending the hospital from February 24 to April 26, 2020 (first wave) and from July 24, 2020, to May 18, 2021 (second wave) was carried out. We analysed the data of the electronic medical records on patient demographics, comorbidity, severity, and mortality. Results: The daily number of COVID-19 patients entering the emergency rooms (ER) dropped by 65% during the second wave compared to the first wave. During the second wave, patients entering the ER were significantly younger (61 against 63 years old p<0.001) and less severely affected (39% against 48% with a triage level of resuscitation or emergency; p<0.001). ER mortality declined during the second wave (1% against 2%; p<0.000). The daily number of hospitalised COVID-19 patients dropped by 75% during the second wave. Those hospitalised during the second wave were more severely affected (20% against 10%; p<0.001) and were referred to the intensive care unit (ICU) more frequently (21% against 15%; p<0.001). Inpatient mortality showed no significant difference between the two waves. Conclusions: Changes in the flow, severity and mortality of COVID-19 patients entering this tertiary hospital during the two waves may reflect a better adaptation of the health care system and the improvement of knowledge on the disease.

Introduction. Needle stick injury (NSI) is an occupational health and safety issue. Nursing students are prone to NSI due to lack of experience with handling needles and sharps. Aim. To determine the level of knowledge about the prevention of NSI and examine the level of knowledge about the post-exposure procedure and the students’ understanding of NSI. Methods. The study was conducted in 2017 at the University of Applied Health Sciences in Zagreb. The participants were nursing students from all regions of Croatia. The data was collected using a questionnaire containing 17 questions specifically designed for this study. One question had three subquestions and one had five subquestions regarding the knowledge of how to react if a needle stick injury occurs. The participants also responded to a questionnaire on their socio-demographic data. Results. The study included 149 students. The results show that 16 students have experienced NSI. A statistically significant difference was observed among students who have finished a secondary medical school in the answers about post-NSI interventions and in answers to the question of whether the needle should be recapped. The respondents from medical schools answered correctly. A statistically significant difference was observed among students from non-medical secondary schools in the answers about education on post-exposition procedures and in the answers about necessary action following a needle stick injury. The respondents from non-medical secondary schools had higher scores. Conclusion. The results of this study can be used to establish appropriate education strategies, increase the awareness of needle stick injuries and minimize the occurrence of these injuries among nursing students in Croatia.

Abstract Purpose It is assumed that primary hyperparathyroidism (pHPT) in Multiple Endocrine Neoplasia (MEN) and lithium-associated pHPT (LIHPT) are associated with multiple gland disease (MGD), persistence and recurrence. The studies purpose was to determine frequencies, clinical presentation and outcome of sporadic pHPT (spHPT), LIHPT and pHPT in MEN. Additional main outcome measures were the rates of MGD and persistence/recurrence. Methods Retrospective analysis of medical records of 682 patients with pHPT who had attended the University Hospital of Marburg between 01–01–2004 and 30–06–2013. All patients were sent a questionnaire asking about their history of lithium medication. Results Out of 682 patients, 557 underwent primary surgery (532 spHPT, 5 LIHPT, 20 MEN), 38 redo-surgery (31 spHPT, 7 MEN), 55 were in follow-up due to previous surgery (16 spHPT, 1 LIHPT, 38 MEN) and 37 were not operated (33 spHPT, 1 LIHPT, 3 MEN). Primary surgeries were successful in 97.4%, revealed singular adenomas in 92.4%, double adenomas in 2.9% and MGD in 3.4% of the cases. Rates of MGD in MEN1 (82.35%) were significantly higher than in spHPT (3.8%), while there was no significant difference between LIHPT (20%) and spHPT. Rates of persistence/recurrence did not significantly differ due to type of surgery (bilateral/unilateral) or type of HPT (spHPT/LIHPT/MEN). Conclusions History of lithium medication is rare among pHPT patients. While MGD is common in MEN1, rates of MGD, persistence or recurrence in LIHPT were not significantly higher than in spHPT.

BackgroundThe American College of Surgeons Bleeding Control Basic (BCon) course aimed at teaching hemorrhage control techniques in the USA had not yet been taught in Spain. The purpose of this study is to assess its implementation among students and healthcare employees in the Lozano Blesa University Hospital of Zaragoza, a middle-sized Spanish city.MethodsThe study was conducted in a University Hospital and at the University of Zaragoza, scheduling four free B-Con sessions from 2017 to 2018. Two groups were identified as forming the population: healthcare employees and medical students. An anonymous questionnaire was completed at the end of the course regarding content, applicability, usefulness, relevance, and satisfaction. Study variables were ranked from 0 to 10: poor (when graded from 0 to 3), fair (4 to 6) and good (7 to 10). Results were compared between the groups, expressed in percentages and χ2 tested to analyze significant differences if any.ResultsAmong the 83 individuals who completed the course, 46 were medical students and 37 healthcare employees; 61% women and 39% men; aged 21 years to 52 years. Attendees evaluated satisfaction with the highest grade (84%), followed by usefulness (73%), applicability (70%), and relevance (66%). There was no variable graded as poor. The comparison of perceptions between groups did not reveal statistical differences based on a 0.05 significance level.ConclusionWe concluded that the B-Con course was valued as good for relevance, usefulness, applicability, and satisfaction by the majority of the studied population.Level of evidenceLevel III.

The objectives of this study were: firstly, to explore and describe the experiences of final year nursing students relating to how they experience their preparedness to fulfil the role of professional nurse; secondly, to explore and describe the experiences of novice professional nurses in the role of professional nurse; finally, to generate a model which will assist the final year nursing student to become a professional nurse. A theory-generative, qualitative, explorative, descriptive and contextual research design was utilized to reach the objectives of the study. Results indicated that final year nursing students experience a lack of confidence to take on the responsibilities of professional nursing. The results are displayed in table form and discussed in the article. This abstract forms part of a bigger study that addresses the professional maturity of the novice professional nurse for the practice of nursing.
