Abstract Background Over the last few decades tick-borne diseases have emerged as an increasingly significant public health problem. The distribution and prevalence of zoonotic pathogens infecting ixodid ticks from Western Europe have been extensively examined. However, data on ticks and tick-borne pathogens in Eastern Europe, particularly Ukraine are scarce. The objective of the current study was, therefore, to investigate the prevalence rates of Anaplasma phagocytophilum , the Anaplasmataceae family, Rickettsia spp., Babesia spp., Bartonella spp., and Borrelia burgdorferi sensu lato in engorged and questing ixodid ticks collected from five administrative oblasts (regions) of Ukraine, namely Chernivtsi, Khmelnytskyi, Kyiv, Ternopil, and Vinnytsia.Methods The ticks were collected from both wild and domestic animals and from vegetation. Of 524 ixodid ticks collected, 3, 99, and 422 ticks were identified as Ixodes hexagonus , Ixodes ricinus, and Dermacentor reticulatus , respectively. DNA samples individually extracted from 168 questing and 354 engorged adult ticks were subjected to pathogen-specific PCR analyses.Results The mean prevalence rates in all I. ricinus and D. reticulatus ticks were, respectively: 10.3% (10/97) and 2.8% (12/422) for A. phagocytophilum ; 69.1% (67/97) and 52.1% (220/422) for the Anaplasmataceae family; 24.7% (24/97) and 27.7% (117/422) for Rickettsia spp.; 3.1% (3/97) and 1.4% (6/422) for Babesia spp.; and 9.3% (9/97) and 4.7% (20/422) for Bartonella spp. Overall, between the five cities, there was no significant difference in the prevalence rates of any of the pathogens for the respective ticks ( p >0.05). The prevalence of B. burgdorferi s.l. in the engorged and questing I. ricinus ticks varied from 26.7% and 0% to 44.4% and 14.3%, respectively, with no statistical significance identified between the five cities ( p >0.05).Conclusions In addition to reporting the updated data for Kyiv and Ternopil, this study is the first to provide the prevalence rates of the tick-borne pathogens for Chernivtsi, Khmelnytskyi, and Vinnytsia. This investigation is also the first to detect Ca. N. mikurensis in ixodid ticks from Ukraine. These new data will be useful for medical and veterinary practitioners as well as public health officials when diagnosing infections and when implementing measures to combat tick-borne diseases in Ukraine.

Abstract Background Pancreatitis is a critical public health problem, and the burden of pancreatitis is increasing. We report the rates and trends of the prevalence, incidence, and years lived with disability (YLDs) for pancreatitis at the global, regional, and national levels in 195 countries and territories from 1990 to 2017, stratified by sex, age, and sociodemographic index (SDI). Methods Data on pancreatitis were available from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2017. Numbers and age-standardized prevalence, incidence, and YLDs’ rates per 100,000 population were estimated through a systematic analysis of modeled data from the 2017 GBD study. Both acute and chronic pancreatitis are being modeled separately in the GBD 2017; however, our data show acute and chronic pancreatitis together. Estimates were reported with uncertainty intervals (UIs). Results Globally, in 2017, the age-standardized rates were 76.2 (95% UIs 68.9 to 83.4), 20.6 (19.2 to 22.1), and 4.5 (2.3 to 7.6) per 100,000 population for the point prevalence, incidence, and YLDs, respectively. From 1990 to 2017, the percent changes in the age-standardized prevalence and YLDs rates increased, whereas the age-standardized incidence rate decreased. The global prevalence increased with age up to 60–64 years and 44–49 years in females and males, respectively, and then decreased, with no significant difference between females and males. The global prevalence rate increased with age, peaking in the 95+ age group, with no difference between sexes. Generally, positive correlation between age-standardized YLDs and SDIs at the regional and national levels was observed. Slovakia (297.7 [273.4 to 325.3]), Belgium (274.3 [242.6 to 306.5]), and Poland (266.7 [248.2 to 284.4]) had the highest age-standardized prevalence rates in 2017. Taiwan (Province of China) (104.2% [94.8 to 115.2%]), Maldives (72.4% [66.5 to 79.2%]), and Iceland (64.8% [57.2 to 72.9%]) had the largest increases in age-standardized prevalence rates from 1990 to 2017. Conclusions Pancreatitis is a major public health issue worldwide. The age-standardized prevalence and YLDs rates increased, but the age-standardized incidence rate decreased from 1990 to 2017. Improving the quality of pancreatitis health data in all regions and countries is strongly recommended for better monitoring the burden of pancreatitis.

Introduction: Toxoplasmosis is a zoonosis that represents a serious public health problem, caused by Toxoplasma gondii, which affects 20-90% of the world human population [1,2]. It is a serious problem especially when considering the congenital transmission due to congenital sequels. Treatment with highly diluted substances is one of the alternative/complementary medicines most employed in the world [3,4]. The current ethical rules regarding the number of animals used in animal experimental protocols with the use of more conservative statistical methods [5] can not enhance the biological effects of highly diluted substances observed by the experience of the researcher. Aim: To evaluate the minimum number of animals per group to achieve a significant difference among the groups of animals treated with biotherapic T. gondii and infected with the protozoan regarding the number of cysts observed in the brain. Material and methods: A blind randomized controlled trial was performed using eleven Swiss male mice, aged 57 days, divided into two groups: BIOT-200DH - treated with biotherapic (n=6) and CONTROL - treated with hydroalcoholic solution 7% (n=7).The animals of the group BIOT-200DH were treated for 3 consecutive days in a single dose 0.1ml/dose/day. The animals of BIOT – 200DH group were orally infected with 20 cysts of ME49-T. gondii. The animals of the control group were treated with cereal alcohol 7% (n=7) for 3 consecutive days and then were infected with 20 cysts of ME49 -T. gondii orally. The biotherapic 200DH T. gondii was prepared with homogenized mouse brain, with 20 cysts of T. gondii / 100μL according to the Brazilian Homeopathic Pharmacopoeia [6] in laminar flow. After 60 days post-infection the animals were killed in a chamber saturated with halothane, the brains were homogenized and resuspended in 1 ml of saline solution. Cysts were counted in 25 ml of this suspension, covered with a 24x24 mm coverglass, examined in its full length. This study was approved by the Ethics Committee for animal experimentation of the UEM - Protocol 036/2009. The data were compared using the tests Mann Whitney and Bootstrap [7] with the statistical software BioStat 5.0. Results and discussion: There was no significant difference when analyzed with the Mann-Whitney, even multiplying the "n" ten times (p=0.0618). The number of cysts observed in BIOT 200DH group was 4.5 ± 3.3 and 12.8 ± 9.7 in the CONTROL group. Table 1 shows the results obtained using the bootstrap analysis for each data changed from 2n until 2n+5, and their respective p-values. With the inclusion of more elements in the different groups, tested one by one, randomly, increasing gradually the samples, we observed the sample size needed to statistically confirm the results seen experimentally. Using 17 mice in group BIOT 200DH and 19 in the CONTROL group we have already observed statistical significance. This result suggests that experiments involving highly diluted substances and infection of mice with T. gondii should work with experimental groups with 17 animals at least. Despite the current and relevant ethical discussions about the number of animals used for experimental procedures the number of animals involved in each experiment must meet the characteristics of each item to be studied. In the case of experiments involving highly diluted substances, experimental animal models are still rudimentary and the biological effects observed appear to be also individualized, as described in literature for homeopathy [8]. The fact that the statistical significance was achieved by increasing the sample observed in this trial, tell us about a rare event, with a strong individual behavior, difficult to demonstrate in a result set, treated simply with a comparison of means or medians. Conclusion: Bootstrap seems to be an interesting methodology for the analysis of data obtained from experiments with highly diluted substances. Experiments involving highly diluted substances and infection of mice with T. gondii should be better work with experimental groups using 17 animals at least.

Ixodes ricinus is the most common tick in Europe, distributed almost throughout the continent, except the northern regions. Dermacentor reticulatus is a widespread species of tick in Europe and Western Asia. The study included monitoring the distribution density and seasonal activity of ticks D. reticulatus and I. ricinus in the natural environment during 2018-2019. A total of 1592 D. reticulatus ticks and 712 I. ricinus ticks were collected in Ternopil, Ivano-Frankivsk and Lviv oblasts. There was a tendency to increasing of the number of ticks of both species in 2019 compared to 2018. The largest number of ticks was collected in autumn 2019, during the autumn peak of activity, which amounted to 710 adult ticks. Thus, the average density of ticks of both species collected in the Ternopil region was 40 ticks/1000 m2, in Ivano-Frankivsk – 32 and 45 in Lviv regions in 2018 and 62, 46 and 63 ticks/1000 m2 in 2019, respectively. In the spring of 2018, almost twice as many adult ticks of D. reticulatus were detected as in the fall in all areas. With regard to I. ricinus ticks, we did not observe a statistically significant difference in seasonal activity. In the spring and autumn, two peaks of ticks’ activity were registered in all regions and the average number of ticks in the spring in the meadows was 20 ticks/1000 m2, and on the wooded area – 39 ticks/1000 m2, and in autumn – 17 and 41 ticks/ 1000 m2 respectively. During both years and in all three oblasts, females prevailed over males during all the period. Although the density of ticks of both species in all areas was quite high, but the ticks were unevenly distributed. The largest number of D. reticulatus ticks was found in the Lviv region in 2019, which ranged from 46 to 119 ticks/1000 m2. Low and medium density of adult ticks D. reticulatus and I. ricinus, from 11 to 77 ticks/1000 m2 was observed in Ivano-Frankivsk region in 2018. Throughout the study period, the seasonal activity of D. reticulatus and I. ricinus ticks was significantly affected by air temperature, which fluctuated during collection of ticks.

SUMMARYThe first reported New Zealand-acquired case of murine typhus occurred near Auckland in 1989. Since then, 72 locally acquired cases have been recorded from northern New Zealand. By 2008, on the basis of the timing and distribution of cases, it appeared that murine typhus was escalating and spreading southwards. To explore the presence ofRickettsia typhiin the Waikato region, we conducted a seroprevalence study, using indirect immunofluorescence, Western blot, and cross-adsorption assays of blood donor samples. Of 950 human sera from Waikato, 12 (1·3%) hadR. typhiantibodies. The seroprevalence forR. typhiwas slightly higher in northern Waikato (1·4%) compared to the south (1·2%; no significant difference,χ2P= 0·768 atP< 0·05). Our results extend the reported southern range ofR. typhiby 140 km and indicate it is endemic in Waikato. Evidence of pastRickettsia felisinfections was also detected in six sera. Globally,R. felisis an emerging disease of concern and this pathogen should also be considered when locally acquired rickettsiosis is suspected. If public health interventions are to be implemented to reduce the risk of rickettsioses as a significant public health problem, improvements in rickettsial diagnostics and surveillance will be necessary.

Background: Diabetes mellitus is a critical public health problem, and its prevalence in Indonesia remains high. Diabetes mellitus may cause complications, one of which is neuropathy that can impair foot sensitivity. This requires a treatment by doing diabetic foot exercises using sponges and paper.Objective: To examine the effectiveness of diabetic foot exercise using sponges and newspapers on foot sensitivity in patients with diabetes mellitus.Methods: This is a quasi-experimental study with pretest posttest with a control group research design, which was conducted at Public Health Center Depok III, Sleman Regency, Yogyakarta, Indonesia. An accidental sampling technique was used to select participants, with a total sample of 108 respondents consisting of 36 respondents in a control group, 36 respondents in a sponge group, and 36 respondents in a newspaper group. Data were analyzed using Wilcoxon and Mann-Whitney test.Results: Among the three groups, only those who received foot exercises using sponges and newspapers had a significant effect on foot sensitivity (p <.05). However, there was no significant difference on the effect of foot exercise on foot sensitivity between sponges and newspapers group (p >.05).Conclusion: The use of sponges and newspapers in foot exercise could significantly improve foot sensitivity in patients with type 2 diabetes mellitus. It is therefore recommended for nurses to provide the foot exercise as a part of nursing practice in both hospitals and community health centers.

Hemoglobinopathies are hereditary disorders of the hemoglobin molecule with a high prevalence worldwide. Brazil has a prevalence of 0.1 to 0.3% of newborns with sickle cell anemia and 20.0 to 25.0% of heterozygous alpha2 thalassemia among African Brazilians. In the present study, we investigated the presence of variant hemoglobins and alpha2(3.7 Kb) and alpha2(4.2 Kb) thalassemia in newborns from Salvador, Bahia, Brazil. Samples of umbilical cord blood from a total of 590 newborns were analyzed, of which 57 (9.8%) were FAS; 36 (6.5%) FAC; one (0.2%) SF; and five (0.9%) FSC. One hundred fourteen (22.2%) newborns had alpha2(3.7 Kb) thalassemia, of whom 101 (19.7%) were heterozygous and 13 (2.5%) homozygous, showing statistical significance for hematological data between newborns with normal alpha genes and alpha2(3.7 Kb) thalassemia carriers. The alpha2(4.2 Kb) thalassemia was not found. Frequencies found in the present study confirm that hemoglobinopathies are a public health problem in Brazil, emphasizing the need for neonatal screening and genetic counseling programs.

Background and purpose: Scorpionism is a public health problem in Darmian City.  This study was conducted to determine the biodiversity and faunistic of scorpions in order to discover the health preventive features of scorpion sting among the residents of these regions.Methods: A nightly and daily actively searching on distribution and biodiversity of scorpions was carried during April 2015 to March 2016 in Darmian, Iran.Results: A total of 685 scorpions including five species belonging to buthidae family were identified. The highest abundance was observed in warmer months, and Mesobuthus eupeus (54.5%) was the most abundant species. The researchers found a positive correlation between abundance and temperature and a negative correlation between abundance and humidity. The scorpions were observed with more nocturnal activity and were well-adapted with more abundance to the plain, rural, and outdoors environments. Regarding constancy index, all species except A. crassicauda were classified as constant species (C=100). There was no significant difference in the richness of scorpions in different time collections (day or night), geographical areas (plain or mountainous), regions collected (urban or rural), and places (indoors or outdoors). The Shannon and dominant indices were found to be not significantly different between mentioned sites except in time collection.Conclusions: Species composition of scorpions revealed that scorpions were constantly active throughout the year which can play an important role in sting events. So, familiarity of healthcare experts and indigenous people with existing species and their seasonal activities can largely reduce the risk of scorpion stings.

Background and Objective: Human visceral leishmaniasis is endemic in Natal, northeast of Brazil, where the domestic dog is an important parasite reservoir in the infectious cycle of Leishmania spp. In this study, was evaluated the antileishmanial IgG antibody and epidemiological factors related to canine visceral leishmaniasis (CVL). Methods: Sera samples obtained by venipuncture of 1,426 dogs living in areas of human visceral leishmaniasis occurrence were tested for detection of IgG anti-leishmania antibodies with Immunofluorescence Antibody Assay (IFA) and Enzyme Linked Immunosorbent Assay (ELISA). Chi-square (x2) and Odds Ratio (OR) were calculated. Differences were considered statistically significant at p≤0.05. RESULTS: The overall seroprevalence was 10.30% (147/1,426); prevalence increased when the samples were found to be positive at least for one technique ([417/1,426] 29.3%). The high percentage of seroprevalence was observed in Nova República (15.35%), Nova Natal I (12.7%) and Lagoa Azul (11.4%) neighborhoods. In the Planalto, Soledade and Brasil Novo, the infection rates ranged from 7.5 at 8.06%. (p=0.00051). There was neither statistically significant difference between leishmanial infection nor clinical signs of disease (p=0.84; OR=1.0 [0.41; 2.3]), sex (p=0.78, OR=0.94 [0.66; 1.28]); and breed (p=0.92; OR=1.0 [0.65; 1.54]) were observed. Conclusion: The RESULTS suggest that CVL is widely distributed in Natal, may be increasingly urbanizing and will spread through neighborhoods but not endemic, resulting in a serious public health problem, emphasizing the need for epidemiological studies to a greater understanding of the distribution of canine leishmaniasis in these specific areas and contribute proactively to the public health policies.

Introducción: la infección por el virus del Zika es un problema de salud pública por las complicaciones neonatales de moderada y grave severidad. Objetivo: establecer las complicaciones en los neonatos de madres diagnosticadas con Zika. Metodología: investigación cuantitativa, no experimental, descriptiva, con una muestra de 82 neonatos de madres con infección de virus del Zika, datos obtenidos del departamento de estadísticas del Distrito 09D01, durante el periodo enero 2017 a enero 2018, procesados por el software SPSS Estatistics 24.0 para windows, con una distribución numérica y porcentual, con la prueba de T de Student se estableció la significancia estadística (p<0.05). Resultados: En los 82 neonatos de madres infectadas con el virus Zika 52 casos (63,4 %) nacieron por cesárea y 30 casos (36,59 %) por parto vaginal, 76 casos (92,68%) nacieron vivos y 6 casos (7,3%) mortinatos, con igual proporción de neonatos pre-términos y a términos, microcefalia en 13 casos (15,88%), seguido de hiperbilirrubinemia en 5 casos (6,10%) e hipotiroidismo en 3 casos (3,66%), con una diferencia significativa (p<0,05) en la microcefalia en relación a las otras complicaciones. Conclusiones: Se evidenció un predominio en el tipo de parto por cesáreas, aunque diversos consensos internacionales establecen que la infección por el virus Zika o la microcefalia no son indicaciones, por lo que la cesárea se realizará solo por motivos obstétricos exclusivamente. En los neonatos existió igual proporción en la edad gestacional y se evidenciaron complicaciones como microcefalia, hiperbilirrubinemia e hipotiroidismo predominando la microcefalia en relación a las otras complicaciones.  Palabras Clave: Zika; neonatos; gestantes, malformaciones, microcefalia, mortalidad. ABSTRACT Introduction: Zika virus infection is a public health problem due to moderate and severe neonatal complications. Objective: to establish the complications in the neonates of mothers diagnosed with Zika. Methodology: quantitative, non-experimental, descriptive research, with a sample of 82 neonates of mothers with Zika virus infection, data obtained from the statistics department of District 09D01, during the period January 2017 to January 2018, processed by the software SPSS Statistics 24.0 for windows, with a numerical and percentage distribution, with the Student's t test statistical significance was established (p<0.05). Results: In the 82 neonates of mothers infected with the Zika virus, 52 cases (63.4 %) were born by cesarean section and 30 cases (36.59%) by vaginal delivery, 76 cases (92.68%) were born alive and 6 cases (7.3%) were stillborn, with the same proportion of preterm and full term neonates, microcephaly in 13 cases (15.88%), followed by hyperbilirubinemia in 5 cases (6.10%) and hypothyroidism in 3 cases (3.66%), with a significant difference (p<0.05) in microcephaly in relation to the other complications. Conclusions: There was evidence of a predominance in the type of cesarean delivery, although various international consensuses made that infection by the Zika virus or microcephaly are not indications, so that cesarean section is performed only for obstetric reasons exclusively. In neonates there was an equal proportion in gestational age and complications such as microcephaly, hyperbilirubinemia and hypothyroidism were evidenced, with microcephaly predominating in relation to the other complications Keywords: Zika; neonates; pregnant; malformations; microcephaly; the mortality.

Tobacco use is a multi- dimensional and a worldwide problem with an alarming increase in its incidences. Adolescence continuing use of tobacco is major public health problem in India. The present study was attempted to study the personality traits of tobacco users (TU) than tobacco non-users (TNU) students. The purposive sampling method was adopted to select the sample. The sample consisted of 100 subject with tobacco users (n=50) and non-users (n=50) were selected from different schools and colleges of Jamnagar city of Gujarat state. The tools used were Eysenck’s Personality Inventory, gujarati version. Student-T test was used to analyses the data. The present study revealed that there is significant difference between student tobacco users and non-users on Neuroticism and Extraversion dimensions of personality. Student tobacco users score higher on both dimensions of personality than non-users. Conclusion: when tobacco prevention and intervention program are developed, these personality traits should be consider in addition to physiological aspects.
