Abstract With increasing air pollution, the association between green space exposure and health outcomes is a global health concern. The relationship between air pollution and the survival of patients with chronic kidney disease (CKD) who are exposed to residential greenness is yet to be elucidated. This study aimed to determine this relationship in Seoul between 2002 and 2015. A time-varying survival analysis was conducted to investigate the association between long-term exposure to air pollutants and mortality in 29,602 patients with CKD living in residential environments with small and large green infrastructure. The low and high index groups were defined using continuous and percentile thresholds of the satellite data—the derived average Normalized Difference Vegetation Index within 250 m and 1,250 m of residence, respectively. During the observation, 3,863 (14%) deaths occurred. The effect of air pollution exposure on mortality was worse in the low index group than in the high index group. Particularly, exposure to SO2 was associated with increased mortality risk, regardless of the greenness threshold. Consistent results were observed in co-pollutant models. High greenery exposure significantly reduced the risk of air pollution related mortality. Our results emphasize the need for creating environmental infrastructures that include green spaces.

IntroductionChronic kidney disease is a serious and a frequent disease associated with a high risk of morbi-mortality. Although several risk factors have already been well addressed, mostly diabetes and hypertension, many remain underappreciated, such as chronic exposure to air pollution.Methods and analysisWe will search EMBASE, PubMed, Web of Science, Cochrane Library and CINAHL database, from inception to 31 March 2020, for relevant records using a combination of keywords related to the type of exposure (ozone, carbon monoxide, nitrogen oxides and dioxide, sulfur dioxide, PM2.5, PMcoarse and PM10) and to the type of outcome (chronic kidney disease, end-stage renal/kidney disease, kidney failure, proteinuria/albuminuria, renal function, renal transplant, kidney graft, kidney transplant failure, nephrotic syndrome and kidney cancer). The review will be reported according to the guidelines of the Meta-analysis Of Observational Studies in Epidemiology. Two independent reviewers will select studies without design or language restrictions, using original data and investigating the association between exposure to one or more of the prespecified air pollutants and subsequent risk of renal outcomes. Using random-effects meta-analyses, we will present pooled summary statistics (HR, OR or beta-coefficients with their respective 95% CI) associated with a standardised increase in each pollutant level. The results will be presented by air pollutant and outcome. Heterogeneity will be assessed using the χ2 test on Cochran’s Q statistic and quantified by calculating I2. The Egger’s test and visual inspection of funnel plots will be used to assess publication bias.Ethics and disseminationSince primary data are not collected in this study, ethical approval is not required. This review is expected to provide relevant data on the associations between various air pollutants’ exposure and renal outcomes. The final report will be published in an international peer-reviewed journal.PROSPERO registration numberCRD42020187956.

Air pollution is a leading public health concern. In addition, poor populations have been reported as showing increased exposure to such pollution. The current study thus aimed to evaluate the socioeconomic status of the population exposed to vehicle-related air pollution in the city of São Paulo, Brazil. The study used data from the 2010 Census on head-of-household’s mean monthly income and the percentage of households connected to the sewage system. Exposure to air pollutants was estimated according to traffic density in the census tract plus a 200m surrounding buffer. The relationship between exposure and socioeconomic variables was analyzed by the Kruskal-Wallis test. Exposure increased with increasing socioeconomic status (p < 0.001). The population with the highest socioeconomic status lives in the most polluted areas of the city. However, place of residence alone is not capable of measuring exposure. The study suggests that future epidemiological studies include other indicators of vulnerability.

The outbreak of COVID-19 has created a serious public health concern worldwide. Although, most of the regions around the globe have been affected by COVID-19 infections; some regions are more badly affected in terms of infections and fatality rates than others. The exact reasons for such variations are not clear yet. This review discussed the possible effects of air pollution on COVID-19 infections and mortality based on some recent evidence. The findings of most studies reviewed here demonstrate that both short-term and long-term exposure to air pollution especially PM2.5 and nitrogen dioxide (NO2) may contribute significantly to higher rates of COVID-19 infections and mortalities with a lesser extent also PM10. A significant correlation has been found between air pollution and COVID-19 infections and mortality in some countries in the world. The available data also indicate that exposure to air pollution may influence COVID-19 transmission. Moreover, exposure to air pollution may increase vulnerability and have harmful effects on the prognosis of patients affected by COVID-19 infections. Further research should be conducted considering some potential confounders such as age and pre-existing medical conditions along with exposure to NO2, PM2.5 and other air pollutants to confirm their detrimental effects on mortalities from COVID-19.

Abstract Background Numerous studies have shown that exposure to air pollution, especially particulate matter (PM) with a diameter <2.5 μm (PM2.5), was associated with various diseases. We tried to determine the impact of PM2.5 and other weather factors on acute lung edema in patients with Stage 5 nondialysis chronic kidney disease (CKD Stage 5-ND). Methods In total, 317 CKD Stage 5-ND (estimated glomerular filtration rate 6.79 ± 4.56 mL/min) patients residing in central Taiwan who developed acute lung edema and initiated long-term dialysis were included in this case-crossover study. Pearson’s correlation test was used to examine the relationship of acute lung edema cases with PM2.5 levels and ambient temperature separately. Results The average PM2.5 level within the 7-day period correlated with acute lung edema incidence in the fall [adjusted odds ratio (OR) 3.23, P = 0.047] and winter (adjusted OR 1.99, P < 0.001). In winter, even a 3-day exposure to PM2.5 was associated with increased risk (adjusted OR 1.55, P < 0.001). The average temperatures within 3 days in spring and summer were correlated positively with the risk (adjusted OR 2.77 P < 0.001 and adjusted OR 2.72, P < 0.001, respectively). In the fall and winter, temperatures were correlated negatively with the risk (adjusted OR 0.36, P < 0.001 and adjusted OR 0.54, P < 0.001, respectively). Conclusions A high PM2.5 level was associated with an increased risk of acute lung edema. High ambient temperature in hot seasons and low ambient temperature in cold seasons were also associated with increased risk. It is essential to educate these patients to avoid areas with severe air pollution and extreme ambient temperature.

The public green spaces in Baghdad city have an obvious shortage and doesn’t fulfill and meet the needs of the population and the required open space criteria, moreover these green spaces are disjointed, disconnected and does not function as a unified system, that is why it suffers from neglect which contributes to deterioration of the city’s environment and increasing air pollution.     Human beings depended on nature into providing clean air and good health, with the growth of the population and the urban areas, there has been negative influence on the natural environmental system. A new term lately appeared “Green Infrastructure”especially in the developed countries (USA, UK and other countries in Europe), as one form of solutions to conserve the natural environmental system of green and open spaces. Consequently this thesis depended on this new green space approach to solve the green spaces problem in Baghdad city and suggested new sites in different locations in the city to be a new green spaces connected to the existing green spaces in Baghdad. This will increase the public green space areas, accessibility to these areas and social cohesion, it will also contribute to the improvement of the city’s environment by reducing air pollution and reducing air temperature especially during the long summer season. Other economic benefits could be achieved by planning for a connected network of green spaces in Baghdad city as well.

AbstractChronic kidney disease is a major public health concern that affects millions of people globally. Alterations in gut microbiota composition have been observed in patients with chronic kidney disease. Nevertheless, the correlation between the gut microbiota and disease severity has not been investigated. In this study, we performed shot-gun metagenomics sequencing and identified several taxonomic and functional signatures associated with disease severity in patients with chronic kidney disease. We noted that 19 microbial genera were significantly associated with the severity of chronic kidney disease. The butyrate-producing bacteria were reduced in patients with advanced stages of chronic kidney diseases. In addition, functional metagenomics showed that two-component systems, metabolic activity and regulation of co-factor were significantly associated with the disease severity. Our study provides valuable information for the development of microbiota-oriented therapeutic strategies for chronic kidney disease.

Background: Chronic kidney disease (CKD) is an increasing health concern worldwide. In addition to systemic changes, kidney disease patients also present with oral complications. The objectives of the study were to assess and compare the oral health status in patients with CKD (with diabetes and without diabetes) undergoing dialysis and not undergoing dialysis aged between (30-70) years and controls in Mangalore, South India.Methods: A descriptive cross-sectional study was conducted among 160 adults with various types of kidney disease and controls, attending a tertiary care hospital in Mangalore. Oral health status was assessed using WHO oral health assessment form for adults 2013. Oral hygiene was assessed by using simplified oral hygiene index and gingival status was assessed by using modified gingival index.Results: Number of decayed teeth was more among controls, and number of missing teeth was more among CKD patients. Diabetic CKD patients who were not on dialysis had highest number of teeth with periodontal pocket and more sextants with loss of attachment. Non-diabetic CKD patients who were on dialysis had more gingival bleeding and gingival inflammation. Multinomial logistic regression analysis showed that CKD patients have more odds of having poor oral health than controls.Conclusions: CKD patients had poor oral hygiene, gingival and periodontal status. In contrast, dental caries was significantly lower in CKD patients than the control group. Good oral health condition mitigates the risk of infection, contributing to a better quality of life among CKD patients.

Background: Acute kidney injury (AKI) remains a major global public health concern due to its high morbidity and mortality. The progression from AKI to chronic kidney disease (CKD) makes it a scientific problem to be solved. However, it is with lack of effective treatments.Summary: Both innate and adaptive immune systems participate in the inflammatory process during AKI, and excessive or dysregulated immune responses play a pathogenic role in renal fibrosis, which is an important hallmark of CKD. Studies on the pathogenesis of AKI and CKD have clarified that renal injury induces the production of various chemokines by renal parenchyma cells or resident immune cells, which recruits multiple-subtype lymphocytes in circulation. Some infiltrated lymphocytes exacerbate injury by proinflammatory cytokine production, cytotoxicity, and interaction with renal resident cells, which constructs the inflammatory environment and induces further injury, even death of renal parenchyma cells. Others promote tissue repair by producing protective cytokines. In this review, we outline the diversity of these lymphocytes and their mechanisms to regulate the whole pathogenic stages of AKI and CKD; discuss the chronological responses and the plasticity of lymphocytes related to AKI and CKD progression; and introduce the potential therapies targeting lymphocytes of AKI and CKD, including the interventions of chemokines, cytokines, and lymphocyte frequency regulation in vivo, adaptive transfer of ex-expanded lymphocytes, and the treatments of gut microbiota or metabolite regulations based on gut-kidney axis.Key Message: In the process of AKI and CKD, T helper (Th) cells, innate, and innate-like lymphocytes exert mainly pathogenic roles, while double-negative T (DNT) cells and regulatory T cells (Tregs) are confirmed to be protective. Understanding the mechanisms by which lymphocytes mediate renal injury and renal fibrosis is necessary to promote the development of specific therapeutic strategies to protect from AKI and prevent the progression of CKD.
