The importance of allergic diseases is due to their increasing prevalence in both developed and developing countries. The prevalence of allergic diseases in most countries of the world is 10–30% among urban and rural population. In the regions of Russia, the prevalence of allergic diseases reaches 19–40% among the adult population and over 27% among children and adolescents. The article deals with the main aspects of the pathogenesis of allergic rhinitis as the most common allergic pathology. The features of antihistamine therapy with modern drugs of the 2nd generation and their influence on the links of the inflammatory process are considered. The data of studies of the effectiveness and safety of the original drug bilastine. Given the use cases bilastine in clinical practice.

Abstract Background The identification of existing rehabilitation interventions and related evidence represents a crucial step along the development of the World Health Organization’s (WHO) Package of Interventions for Rehabilitation (PIR). The methods for such identification have been developed by the WHO Rehabilitation Programme and Cochrane Rehabilitation under the guidance of the WHO’s Guideline Review Committee secretariat. The aim of this paper is to report on the results of the systematic search for clinical practice guidelines (CPGs) relevant to the rehabilitation of adults with fractures and to present the current state of evidence available from the identified CPGs. Methods This paper is part of the Best Evidence for Rehabilitation (be4rehab) series, developed according to the methodology presented in the World Health Organization’s (WHO) Package of Interventions for Rehabilitation (PIR) introductory paper. It is a systematic review of existing CPGs on fractures in adult population published from 2009 to 2019. Results We identified 23 relevant CPGs after title and abstract screening. According to inclusion/exclusion criteria, we selected 13 CPGs. After checking for quality, publication time, multiprofessionality, and comprehensiveness, we finally included five CPGs dealing with rehabilitative management of fractures in adult population, two CPGs addressing treatment of distal radius fracture and three the treatment of femoral/hip fracture. Conclusion The selected CPGs on management of distal radius and femoral/hip fracture include few recommendations regarding rehabilitation, with overall low to very low quality of evidence and weak/conditional strength of recommendation. Moreover, several gaps in specific rehabilitative topics occur. Further high-quality trials are required to upgrade the quality of the available evidence. Level of evidence Level 1.

Allergic rhinitis is characterized by a significant prevalence among the adult population. Patients with allergic diseases have a higher sensitivity to pathogens of acute respiratory infection of the upper respiratory tract (ARVI). Inflammation of the mucous membrane of the nasal cavity and paranasal sinuses in such patients is characterized by more pronounced symptoms and greater resistance to therapy. An observational study was conducted at the Department of Otorhinolaryngology of the Moscow State Medical University named after A. I. Evdokimov, during which the effect of modern antihistamines on the severity of nasal and non-nasal symptoms of inflammation was evaluated in patients who were diagnosed with the development of acute respiratory viral infections against the background of seasonal exacerbation of allergic rhinitis. The data of 50 patients divided into two groups were analyzed. In the first group (n = 25), benzhydrylpiperazinylbutylmethylxanthine succinate (Theoritin) was included in the therapy regimen, in the second – cetirizine preparations (n = 5). During the follow-up, each patient underwent a clinical assessment of the severity of nasal and non-nasal symptoms three times (1st, 7th and 14th days of follow-up), and the severity of nasal obstruction was assessed using anterior active rhinomanometry. According to the data obtained, there was a pronounced positive dynamics in both groups, while the clinical efficacy was confirmed by instrumental evaluation data. However, by the 7th day of treatment, the positive dynamics was more pronounced in the group of patients receiving theoritin as part of therapy. The use of modern antihistamines in patients comorbid for allergic rhinitis and acute respiratory diseases has a significant therapeutic effect. Theoritin provides more pronounced therapeutic effects in the early stages, which makes its further study and use of the drug for the treatment of acute respiratory viral infections against the background of allergic rhinitis promising.

The significance of the issues of allergic diseases is not in doubt. It is difficult to find a person who has not suffered at least some Allergy symptom, not everyone develops diseases, but most people face Allergy symptoms. The prevalence of various allergic diseases is increasing every year, currently reaching 30% of the population, and the world health organization predicts it will reach 50% in the next few decades. The most common nosologies are allergic rhinitis, bronchial asthma, atopic dermatitis, urticaria, but there are many other conditions of hypersensitivity. This article addresses the issues of inflammatory reactions in General and specifically allergic inflammation, discusses the main aspects of the pathogenesis of allergic rhinitis and urticaria, and questions of drug therapy for these diseases. Most people are sure that they can take anti-allergic medications for allergies, and most often they are referring to antihistamines. Unfortunately, even some medical specialists lack an understanding of the specifics of antihistamine therapy. some people still believe that there are three or four generations of antihistamines, and patients use drugs not as prescribed by doctors, but on the recommendation of friends or pharmacists. Irrational use of antihistamines leads to a decrease in effectiveness, and it is not uncommon to discredit specific drugs and groups of drugs in General. This article demonstrates the results of various European and Asian studies on the effectiveness and safety of antihistamines and bilastin in particular, as well as their own experience of using antihistamines in practice.

According to the statistics data about 40% of the population of the world suffer from allergic diseases. A variety of the factors participate in activation and formation of allergic illnesses, assume the use of a wide spectrum of medical products for correction and changes arising at an allergic inflammation. It is necessary to note that a basis of treatment of these diseases is preventive maintenance. We will stop in detail at the group of preparations of cromoglicate acid, which is widely used in clinical practice for prevention of exacebrations and treatment of allergic diseases. Some features of cromons such as a high profile ofsafety, good tolerance, possibility of long using in pediatrics and during pregnancy, low system bioavailability and economic advisability will be discussed.

Levocetirizine is an effective H1-blocker of histamine receptors in complex treatment of various allergic diseases. The article presents the literature data and own observational data on clinical efficacy and safety of levocetirizine in the treatment of allergic diseases such as allergic rhinitis, bronchial asthma, allergic dermatoses.

Allergic rhinitis is one of the most common human diseases and causes a deterioration of the quality of life. The prevalence of allergic rhinitis in the Russian Federation and in the world has increased significantly. The main pharmacological group in the treatment of allergic rhinitis is the 2nd generation antihistamines with high affinity for H1-receptors. The article presents an overview of the new representative of the abovementioned group of drugs - the drug bilastine (Nixar®).

This paper examines the trajectory of growth in the Global South. Before the 1500s all countries were roughly at the same level of development, but from the 1500s Western countries started to grow faster than the rest of the world and PPP GDP per capita by 1950 in the US, the richest Western nation, was nearly 5 times higher than the world average and 2 times higher than in Western Europe. Since 1950 this ratio stabilized - not only Western Europe and Japan improved their relative standing in per capita income versus the US, but also East Asia, South Asia and some developing countries in other regions started to bridge the gap with the West. After nearly half of the millennium of growing economic divergence, the world seems to have entered the era of convergence. The factors behind these trends are analyzed; implications for the future and possible scenarios are considered.

This paper analyzes the problem of technological backwardness of economy. In many mostly developing countries their economies use obsolete technologies. This can create the illusion that this or that business is prosperous. At the level of international competition, however, it is obvious that these types of firms do not have any chance for success. Retroeconomics as a theory of technological backwardness and its detrimental effect upon a country’s economy is considered in the paper. The role of the government is very important for overcoming the effects of retroeconomy. The phenomenon of retroeconomy is already quite deep-rooted throughout the world and it is essential to consolidate the attention of economists and politicians on this threat.

Dengue is unquestionably one of the biggest health concerns in India and for many other developing countries. Unfortunately, many people have lost their lives because of it. Every year, approximately 390 million dengue infections occur around the world among which 500,000 people are seriously infected and 25,000 people have died annually. Many factors could cause dengue such as temperature, humidity, precipitation, inadequate public health, and many others. In this paper, we are proposing a method to perform predictive analytics on dengue’s dataset using KNN: a machine-learning algorithm. This analysis would help in the prediction of future cases and we could save the lives of many.

The incorporation of a trade-labour (standards) linkage into the multilateral trade regime of the World Trade Organisation (WTO) has been persistently opposed by developing countries, including those in Africa, on the grounds that it has the potential to weaken their competitive advantage. For that reason, low levels of compliance with core labour standards have been viewed as acceptable by African countries. However, with the impact of WTO agreements growing increasingly broader and deeper for the weaker and vulnerable economies of developing countries, the jurisprudence developed by the WTO Panels and Appellate Body regarding a trade-environment/public health linkage has the potential to address the concerns of developing countries regarding the potential negative effects of a trade-labour linkage. This article argues that the pertinent WTO Panel and Appellate Body decisions could advance the prospects of establishing a linkage of global trade participation to labour standards without any harm befalling developing countries.
